고등학생의 구강건강 관련 삶의 질이 학교생활 만족도에 미치는 영향 (Effect of Oral Health-Related Quality of Life on School Life Satisfaction in High School Students)

  • 이번 연구는 고등학생의 구강건강 관련 삶의 질을 측정하여 학교생활 만족도와 상관관계를 알아보고, 청소년의 구강건강 관련 삶의 질이 학교생활 만족도에 미치는 영향을 분석하기 위해 실시하였다. 2015년 11월 30일부터 12월 7일까지 대구에 위치한 1개 고등학교 학생을 대상으로 편의표본추출법에 의한 자기기입 방식으로 설문조사를 실시하여 다음과 같은 결과를 얻었다. 고등학생의 구강건강 관련 삶의 질 중 기능적 삶이 3.51로 가장 높았으며, 학교생활 만족도 중에서는 대인관계 만족도가 3.75로 가장 높았다. 그리고 구강건강 관련 삶의 질에 유의한 영향을 주는 일반적 특성은 학업성적, 학업 스트레스(p<0.05)였으며, 학교생활 만족도에 유의한 영향을 주는 일반적 특성은 학업성적(p<0.001)이었다. 구강건강 관련 삶의 질은 학교생활 만족도의 하위요인 중 대인관계, 교육환경, 사회적 지지와 정(+)의 상관관계를 보였으며, 학교생활 만족도에 영향을 주는 요인으로는 학업성적과 구강건강 안녕 삶의 질이 유의한 영향을 나타내었다. 이상의 연구결과를 종합해보면, 고등학생의 구강건강 관련 삶의 질은 청소년의 학교생활 만족도와 관련되는 다양한 요인에 영향을 미치고 있다는 것을 알 수 있다. 따라서 청소년들이 올바른 구강건강 관리습관을 형성할 수 있도록 정기적인 구강보건교육 프로그램을 개발하여 효과적으로 실행할 수 있는 제도를 마련해야 하며, 학생 및 학교, 그리고 관련 부서에서 구강건강의 중요성을 인식하고 예방 및 조기치료, 구강건강 증진을 위해 노력해야 한다.

The Effect of Depression and Cognitive Function on the Geriatric Oral Health Assessment Index in the Elderly

  • Background: Depression and cognitive function have a positive effect on the improvement of quality of life and extension of lifespan in the elderly. In addition, it appears as a major factor influencing oral health status. Therefore, this study looked at the relationship between the Geriatric Oral Health Assessment Index (GOHAI), depression, and cognitive function in the elderly using the Korea Longitudinal Study of Aging. Methods: In this study, 4,535 elderly people aged 65 years and over were targeted using the 7th data of the 2018 Korea Longitudinal Study of Aging. A t-test and ANOVA analysis were performed to compare GOHAI, depression, and cognitive function by group. In addition, hierarchical multiple linear regression was performed to understand the effect of the elderly's perceived depression scale and cognitive ability on GOHAI. Results: As a result of adding the depression scale and cognitive function variables to Model 2, the explanatory power was 22%. Educational level, marital status, private health insurance subscription, average monthly allowance, subjective health status, use of dentures, smoking status, economic activity, depression scale, and cognitive function were found to have significant influence (p<0.05). In addition, when controlled and viewed with all factors, depression and cognitive function were found to have an effect on oral health-related quality of life. Conclusion: The findings indicate that depression and cognitive function are associated with oral health-related quality of life in the Korean elderly. As the age increases, the quality of life declines due to depression and cognitive function problems, in addition to oral discomfort, eating disorders, and physical discomfort.

스트레스 지각과 구강건강의 관련성 (The relationship between stress perception and oral health status)

  • Objectives : The aim of this study was to investigate the factors related to stress perception. Methods : A self-reported questionnaire was filled out by 109 people living in Gimpo city from January 16 to April 15, 2013. The data were analyzed using SPSS window ver. 19.0(IBM CO., Armonk, NY, USA) for t-test, one-way ANOVA, and multiple linear regression analysis. Results : Men and group of 59 years old tended to have higher stress perception than women and those who over 60 years old, but the results were not statistically significant. Stress perception was higher among the group with systemic dryness and oral dryness(each${\geq}1$) and the group reporting high difficulty in daily living due to oral problem(${\geq}2$)(p<0.05). Especially, the factor that was most correlated to stress perception was systemic dryness(${\beta}$=0.347) followed by problem of daily living for oral problem(${\beta}$=0.278, p<0.01). Conclusions : Systemic dryness and oral problem can increase the level of stress perception, mouth dryness and oral problems are a part of stress responses. Therefore, the regular health checkup must be done and early treatment to reduce stress perception as well as negative aspects caused by stress. It will contribute to the enhancement of public health and life of quality.

일부 한국 노인 구강건강 관련 삶의 질 평가를 위한 핵심 음식 선택 (Key Food Selection for Assessement of Oral Health Related Quality of Life among Some Korean Elderly)

  • 본 연구는 구강건강과 구강건강 관련 삶의 질을 파악할 수 있는 핵심 음식 문항을 선택하기 위해 대전 동구보건소 노인 불소도포 스케일링 사업에 참가한 503명의 설문조사 결과를 이용하였다. Kim 등에 의한 저작능력평가 한국음식 30종류 문항 중에서 한국 음식 섭취 빈도, 내적 일치도, 상관분석, 의사결정나무, 주성분분석, 요인분석 등을 통해서 13개의 문항으로 축약하였으며 원문항과 핵심 문항을 성별, 연령대, 치아수, OHIP-14에 따른 분석을 시행한 결과 원문항과 핵심 문항은 유사한 결과를 나타냄을 보여주었다. 구강건강과 구강건강 관련 삶의 질을 파악할 수 있는 13개의 핵심 문항은 단단한 음식군으로 단감, 땅콩, 깍두기, 캐러멜 4문항, 중강도 음식군으로 삼겹살, 찹쌀떡, 배추김치, 사과, 참외, 삶은 닭으로 6문항, 연한 음식군으로 생선조림, 귤, 국수 3문항이 선정되었으며, 본 문항을 구강건강관련 삶의 질 설문조사와 함께 이용할 수 있을 것으로 생각된다.

Dental Fear Survey 척도를 이용한 치과위생사들의 치과공포감 및 관련요인 (Dental fear and related factors of dental hygienists using analysis on dental fear survey)

  • Objectives: The purpose of the study is to investigate the dental fear and related factors of dental hygienists using dental fear survey. Methods: A self-reported questionnaire was completed by 279 dental hygienists from January 15 to March 28, 2015. The study instrument was dental fear survey. Data were analyzed using SPSS 19.0 program by t-test, one way ANOVA and stepwise multiple regression analysis. The questionnaire included general characteristics of the subjects, subjective oral health status, use of dental care services, subjective oral health status, and recent dental treatment. Results: Alcohol consumption was the most influential factor to the dental fear. The subjective status of oral health and direct pain sense also contributed to the dental fear. Conclusions: To reduce the dental fear, it is important to have high confidence toward the dental treatment performance in the dental hygienists.

한국 노인의 객관적·주관적 저작능력 평가에 영향을 미치는 요인 (Factors affecting objective and subjective masticatory ability assessment of Korean elderly people)

  • Objectives: The purpose of the study was to investigate factors affecting the assessment of objective and subjective masticatory ability in the elderly, and to evaluate masticatory ability assessment more accurately. Methods: A total of 112 participants were recruited after oral examination in senior citizen welfare facilities. The participants' masticatory ability was evaluated objectively (Mixing ability index; MAI), and subjectively (Key food intake ability; KFIA). Participants' general characteristics and oral health-related variables were also recorded. Based on masticatory ability assessment, participants were classified as either high or low. IBM SPSS Statistics Ver.23.0 was used for all analyses, including descriptive statistics, Chi-square test, Mann-Whitney U test, Spearman rank correlation, and Logistic regression analysis. Results: Higher masticatory ability was positively correlated with higher scores on MAI and KFIA. Additionally, there was a significant positive correlation between MAI and KFIA. When analyzing factors affecting objective and subjective masticatory ability assessments, Functional tooth units (FTUs) were revealed as a related factor. In subjective masticatory ability assessment, oral moisture, difficulty in chewing, and the Geriatric Oral Health Assessment Index (GOHAI) were also influential factors. Conclusions: In order to accurately assess masticatory ability, it is necessary to use both objective and subjective measures. Additionally, to improve the masticatory ability in the elderly, treatment should be provided to improve overall oral health and satisfaction.

구강암의 위험요인 분석을 위한 환자-대조군 연구 (RISK FACTORS FOR ORAL CANCER ; A CASE-CONTROL STUDY)

  • The purpose of this study was to investigate the relationship between oral cancer and such factors as smoking and drinking pattern, oral health status, dietary intake pattern, socio-economic status. Oral cancer patients and other disease patients who visited Yonsei University Dental Hospital from May to September in 2000 were selected as the study subjects. The numbers of cases and controls were 41, 108, respectively. Two groups were matched with age and sex for case control study. Oral examination and questionnaires survey was performed by the dentist. To assess the strength of associations between oral cancer and other variables, chisquare tests were performed. The results were as follows : 1. The durations of smoking and alcohol drinking were not related significantly with oral cancer. But the doses of smoking and alcohol intake increased the risk of oral cancer significantly(OR=2.52, 4.11, p<0.05). 2. Denture wearing, the number of missing teeth and spicy and salty food, coffee, tea and fresh fruit intake frequency did not significantly increase the risk of oral cancer. But low education level, residency in rural area increased risk of oral cancer significantly(p<0.01).

지방공무원의 직무스트레스가 구강건강 삶의 질에 미치는 영향 (The effects of occupational stress on oral health impact profile (OHIP) in local government workers)

  • Objectives : This study looked at the relationship between occupational stress and the Oral Health Impact Profile (OHIP), to evaluate the effect of occupational stress-related factors. Methods : Data was obtained from a cross-sectional survey of 260 local officials in Gangwondo. The research comprised three questions relating to subjective oral symptoms, an occupational stress measurement tool and an oral health impact factor which was composed of questions. The data was analysed using t-test, one-way ANOVA, Pearson correlation and path analysis in Amos. Results : Occupational stress had a positive correlation to drinking frequency, Oral symptoms had a negative correlation. And drinking frequency, smoking amount and occupational stress had a positive correlation to oral symptoms. It denoted that drinking frequency, occupational stress and oral symptoms had a negative correlation to OHIP. The path model had an excellent goodness of fit (p=0.07, namely p>0.05). Five 'goodness-of-fit indices' of the model were all above 0.9: GFI=0.987, AGFI=0.952, NFI=0.902, IFI=0.939, CFI=0.934), and its RMSEA was 0.045. Occupational stress and oral symptoms had a firsthand impact on OHIP. In addition, it affected OHIP through the parameters of oral symptoms. Occupational stress exercised a firsthand influence on drinking frequency, drinking frequency exercised a firsthand influence on smoking amount. Smoking amount had a firsthand impact on oral symptoms. Conclusions: Oral health education programs for the development of an improved oral hygiene environment through reduction in drinking and smoking also need to focus on relieving stress by improving workplace culture. In addition, due to good communication is required to reduce occupational stress caused by interpersonal conflict.

산업간호사를 대상으로 한 구강보건교육의 효과 평가 (Effects of Oral Health Education for Occupational Health Nurses)

  • 이번 연구에서는 근로자들의 구강건강증진 및 구강질환 예방을 도모하고, 구강건강에 대한 관심을 증가시키며, 근로자 구강보건사업의 지지 및 참여를 높이고자 산업간호사를 대상으로 한 구강보건교육 프로그램을 개발 및 수행하여 교육 전 후의 구강보건지식 및 태도수준을 비교하여 평가함으로써 근로자 구강보건교육 프로그램 개발을 위한 기초 자료로 활용하고자 하였다. 산업간호사를 대상으로 구강보건교육을 실시한 후, 교육 전 후 자가인지 구강건강수준 및 구강건강관리법에 대한 인지 및 권장수준의 변화를 비교 분석한 결과는 다음과 같다. 산업간호사를 대상으로 한 구강보건교육의 시행 후, 자가인지 구강건강수준과 구강건강관심도는 교육 시행 전에 비해 모두 유의하게 증가하였다. 치실 혹은 치간솔 사용, 법정 구강검진을 포함한 정기검진, 정기적인 치면세마를 포함한 치주병 예방법에 대한 인지수준이 교육 전에 비해 모두 유의하게 향상되었고, 구강질병 예방 및 구강건강증진을 위한 구강건강관리법에 대한 권장수준도 교육 전에 비해 매우 유의하게 향상되었다. 이와 연관된 요인을 확인해 본 결과, 구강건강관심도가 낮을수록 치주병 예방법별 인지수준과 구강건강관리법별 권장수준의 향상 정도가 모든 항목에서 크게 나타났다. 산업간호사의 근로자 구강건강증진 프로그램에 대한 의견을 듣고자 설문조사를 실시한 결과, 근로자를 위한 구강건강증진 프로그램을 수행하는 데 필요한 항목으로는 구강보건교육 전후 모두 '산업간호사에 대한 구강보건교육'이 1순위로 나타났다. 그러므로, 근로자들의 구강건강증진을 도모하기 위하여 산업구강보건의 중요한 협력자인 산업간호사를 대상으로 하는 지속적인 구강보건교육 시행이 필요하고, 이러한 교육을 통해 산업간호사의 구강보건지식 및 태도가 향상된다면, 앞으로 근로자 구강보건사업을 계획하고 수행하는 데 있어 산업간호사의 지지와 참여를 효율적으로 이끌어낼 수 있을 것이라 생각된다.

Symptoms of Oral Mucosal Diseases and Vocational Preference Inventory

  • Purpose: The purpose of this study was to assess the association of vocational interest and personality with oral mucosal diseases. Methods: Three hundred and fifty eight college students in Gyeonggi-do completed Vocational Preference Inventory L form and a questionnaire and collected data were analyzed by R program. Results: The prevalence of symptoms of oral mucosal diseases showed no significant difference among six vocational personality types. Compared to subjects with good or fair general health status, a significantly increased percentage of subjects with bad general health status showed herpetic stomatitis (p<0.01), oral malodor (p<0.01), and glossodynia (p<0.0001). Prevalence of taste disturbance increased significantly as the score of emotional instability (${\beta}=0.0438$, p=0.0082), anxiety (${\beta}=0.038$, p=0.0174), angry hostility (${\beta}=0.0398$, p=0.0061), depression (${\beta}=0.0443$, p=0.0035), and impulsiveness (${\beta}=0.0358$, p=0.0186) increased. Subjects who strongly felt oral malodor revealed significantly higher mean scales of scores of anxiety and angry hostility than subjects who did not feel oral malodor (p<0.05). Subjects who strongly felt oral malodor manifested significantly higher mean scales of scores of anxiety than subjects who slightly felt oral malodor (p<0.05). Conclusions: Taste disturbance was affected by emotional instability, anxiety, angry hostility, depression, and impulsiveness. Oral malodor was related to anxiety and angry hostility. Therefore, psychological aspects of taste disturbance and oral malodor could be evaluated by Vocational Preference Inventory L form.
