  • 본 증례에서는 MAS 환자의 치과적인 특성에 관하여 논하였다. 본 환아는 현재 특히 하악 우측골의 팽융과 안면 비대칭 소견을 보이고 있었으며 치아의 교모, 변위, 맹출 지연, 맹출 경로 이상 등의 특징을 보이고 있었다. MAS 환자들에게서는 이러한 증상뿐 아니라 앞서 언급되었던 치아 우식에 이환될 위험성을 높이는 다른 여러 증상들이 발현될 수 있다. 따라서 환아에게 해당 증상이 발생할 수 있음을 보호자에게 설명하였고, 주기적인 관찰을 시행하고 있다. 결론적으로 MAS 환자에서 가장 중요한 것은 정기적인 검진이며, 이러한 검진 과정에서 임상적인 증상들이 나타난다면 치과의사의 교정치료나 충치치료 등의 적절한 개입이 필요하고 환자에게도 교육의 중요성이 더욱 강조된다. 장기적인 검진은 본 환아의 병소에 대한 이해를 높일 것이고 치료 결과의 질을 높여줄 것이라고 판단된다. 소아의 파노라마 방사선 사진 판독 시 치아와 주변 골조직의 주의 깊은 관찰이 동반된다면 이러한 질환의 조기 진단에 도움이 될 것이다.

청소년의 알레르기비염이 구내염에 미치는 영향 (Impact of allergic rhinitis on stomatitis in Korean adolescents)

  • Objectives: This study aimed to determine whether adolescents with allergic rhinitis are at an increased risk of stomatitis and to ascertain possible sex-specific differences. Methods: The data for this cross-sectional study was obtained from the 18th Korea Youth Risk Behavior Web-based Survey (2022). Of the 56,213 students surveyed, 51,850 (boys 26,397 and girls 25,453) were included in the final analyses. Multivariate logistic regression was performed to identify the impact of allergic rhinitis on the risk of stomatitis among Korean adolescents. Results: In Model II, which was adjusted for allergic disease, boys with allergic rhinitis had a 1.53-fold (adjusted odds ratio [aOR]=1.53, 95% confidence interval [CI]=1.40-1.67), and girls had a 1.35-fold (aOR=1.35, CI=1.25-1.46) elevated risk of stomatitis than in those without. In Model III, after adjusting for all covariates, boys with allergic rhinitis had a 1.48-fold (aOR=1.48, CI=1.36-1.62) elevated risk of stomatitis than in those without, and girls had a 1.32-fold (aOR=1.32, CI=1.22-1.43) elevated risk, which remained statistically significant after adjusting for covariates. Therefore, allergic rhinitis in adolescents exerts a detrimental effect on the risk of experiencing stomatitis symptoms; the risk was higher in boys than in girls. Conclusions: These findings suggest that oral health care attention and intervention are needed for adolescents with allergic rhinitis.

골격성 III급 부정교합자의 임상적 특성에 따른 심리상태 (Psychological state of the patients according to skeletal class III malocclusion symptoms)

  • Objectives: The purpose of the study is to investigate the psychological state of the patients according to skeletal class III malocclusion symptoms. Methods: The subjects were 200 skeletal class III malocclusion patients. The questionnaire consisted of general characteristics, clinical manifestation, and T score of Korean version of self-rated Symptom Checklist-90-Revision modified by Jae-hwan Kim. The data were analyzed using SAS version 9.2 and t-test, ANOVA, and ANCOVA were used. Clinical manifestation included subjective recognition and radiological analysis. The subjective recognition of the patients consisted of self-satisfaction of the appearance, phonation, mastication, and temporomandibular joint pain. T score consisted of somatization, obsessive-compulsive, interpersonal sensitivity, depression, anxiety, hostility, phobic anxiety, paranoid ideation, and psychoticism. Results: The most serious factor in 9 scales was the severe TMJ pain by ANCOVA. Male patients had a higher T score in phobic anxiety, psyochoticism, somatization and depression than female patients. Those having low appearance satisfaction had the problem in interpersonal sensitivity. Obsessive-compulsive symptoms were conspicuous in phonation difficulty and temporomandibular pain. Conclusions: The patients with skeletal class III malocclusion have more satisfaction with appearance, pronunciation, and phonation than those with skeletal class III malocclusion and overjet. Proper dental treatment will improve the communication and quality of life.

내원경로 분석을 통한 전문과목으로서의 구강내과의 역할 (The Analysis of Patients in Oral Medicine and the Evaluation of Oral Medicine as a Special Field)

  • 전문의는 일반의(General Practitioner)와는 차별된 전문성을 가지고 증상의 난이도가 높은 환자를 처리하는 능력이 요구되는 진료과목의 필요성을 기반으로 하고 있다. 또한 전문의제도는 국민의료적인 측면에서 임상 각 분야에 있어 단일과목을 전공하는 의사를 양성하여 그들로 하여금 의료 전 과목에 관한 기본적 이론과 실기를 교육받은 일반의사의 능력에서 벗어나는 진료기능을 의료전달체계에서 담당하도록 하여 국민의료의 향상을 기하는 데 있다. 한편, 구강내과는 악안면 통증, 연조직 질환, 법의치과분야, 구강진단분야를 다루는 전문과목으로서 그 전문성이 매우 중요한데, 구강내과학적 측면에서 비추어 볼 때, 현대화 사회로 갈수록 현대인들은 대도시 중심의 생활환경의 변화와 함께 일상적 스트레스가 증가함에 따라 측두하악장애, 구강내 연조직 질환, 삼차신경통과 같은 신경병변 등의 유병율이 증가하고 있으며, 이에 대한 환자들의 의존도가 높아지고 있는 추세이다. 이에 본 연구는 2004년 8월부터 2005년 4월까지 연세대학교 치과대학병원 구강내과 외래에 내원한 초진환자 3,707명을 대상으로 하여, 의뢰율 및 내원경로를 중심으로 분석한 결과 다음과 같은 결론을 얻었다. 1. 구강내과에 의뢰된 전문 진료분야로는 구강안면통증, 구강 연조직 질환, 법의치과분야, 구강진단분야로 나타났다. 2. 구강내과에 의뢰된 환자의 비율은 58.51%로 과반수를 넘는 환자들이 의뢰되었다. 3. 의뢰환자 중 의뢰기관의 영역분석을 시행한 결과, 치과에서 의뢰된 경우가 83.23%로 가장 많았고 의과 및 한의과 영역에서 의뢰된 경우는 16.78%를 보였다. 4. 자의내원한 환자 중 인터넷 및 매스미디어에서 구강내과 전문분야에 대한 사전 검색 및 주변의 권유에 의해 내원한 환자들이 응답자의 30.52%를 차지하였다. 이상의 결과를 종합해 볼 때, 구강내과는 대부분 의뢰된 환자를 진료하고 있으며, 치과영역 뿐 아니라 의과영역에서의 의뢰율이 높은 것으로 보아 치과영역에서는 일반의들이 치료할 수 없는 수준의 진료영역을 담당하고 있는 것으로 볼 수 있으며 의과학 분야에서는 구강내과학적 전문성을 요하는 질환을 구강내과와 협진하에 해결하려는 것으로 사료되어 전문과목으로서 구강내과학의 역할과 비중이 매우 높음을 확인할 수 있었고, 이는 향후 전문치의제도 인력수급 및 이의 기반이 되는 전공의의 정원책정에서 시장적 접근 및 규범적 접근의 기초자료로 활용되어야 할 것이다.

노인요양시설 거주 노인을 위한 한국형 근거기반 연하장애 간호 프로토콜 개발 (Development of Evidence-based Dysphagia Nursing Care Protocol for Nursing Home Residents)

  • Purpose: The purpose of this study was to develop an evidence-based dysphagia nursing care protocol for nursing home (NH) residents in Korea. Methods: The protocol was developed based on international guidelines and literatures. After testing content validity by experts, the protocol was applied to the intervention group (n=35) for 4 weeks at one NH in December, 2011, whereas the control group (n=34) received routine care. Results: The protocol was composed of one-page algorithm and detailed guidelines. Algorithm pathway was organized in 3 parts, including screening dysphagia risk, grouping by dysphagia risk level, and assigning nursing care into each group. The nursing care included positioning, oral care, meal time care, diet modification, providing exercise and maneuver, and checking dysphagia signs and symptoms. The experts verified the content validity. Protocol was revised to fit NH practice after the participant observation. Clinical validity was established upon evaluating usefulness, appropriateness, and convenience of the protocol by NH nurses. Dysphagia risk of the intervention group was significantly decreased. Conclusion: Developed protocol will improve the quality of dysphagia care in nursing homes as it can serve as a consistent and integrated standard for nursing care of residents with dysphagia.

A Pediatric Case of Long-term Untreated Distal Renal Tubular Acidosis

  • Distal renal tubular acidosis (dRTA) is a rare renal tubular disorder characterized by normal anion gap metabolic acidosis, hypokalemia, and high urine pH. It can be inherited or acquired. In untreated pediatric patients with dRTA, rickets and growth retardation are common. We report the case of a 12-year-old Lao girl who presented with typical clinical features of dRTA with severe bone deformities that developed after a bed-ridden state due to a bicycle accident at the age of 8 years. Initial laboratory tests revealed metabolic acidosis with a normal anion gap, hypokalemia, and alkali urine. Renal ultrasonography revealed bilateral medullary nephrocalcinosis. Whole exome sequencing revealed no pathogenic mutations. After treatment with oral alkali, potassium, and vitamin D, she could walk and run. Later, she underwent corrective orthopedic surgeries for bony deformities. Thus, in pediatric dRTA patients, despite severe symptoms remaining untreated, accurate diagnosis and proper management can improve quality of life.

Natural Products as Sources of Novel Drug Candidates for the Pharmacological Management of Osteoarthritis: A Narrative Review

  • Osteoarthritis is a chronic degenerative articular disorder. Formation of bone spurs, synovial inflammation, loss of cartilage, and underlying bone restructuring have been reported to be the main pathologic characteristics of osteoarthritis symptoms. The onset and progression of osteoarthritis are attributed to various inflammatory cytokines in joint tissues and fluids that are produced by chondrocytes and/or interact with chondrocytes, as well as to low-grade inflammation in intra-articular tissues. Disruption of the equilibrium between the synthesis and degradation of the cartilage of the joint is the major cause of osteoarthritis. Hence, developing a promising pharmacological tool to restore the equilibrium between the synthesis and degradation of osteoarthritic joint cartilage can be a useful strategy for effectively managing osteoarthritis. In this review, we provide an overview of the research results pertaining to the search for a novel candidate agent for osteoarthritis management via restoration of the equilibrium between cartilage synthesis and degradation. We especially focused on investigations of medicinal plants and natural products derived from them to shed light on the potential pharmacotherapy of osteoarthritis.

  • Coronavirus is an enveloped virus with positive-sense single-stranded RNA. Coronavirus infection in humans mainly affects the upper respiratory tract and to a lesser extent the gastrointestinal tract. Clinical symptoms of coronavirus infections can range from relatively mild (similar to the common cold) to severe (bronchitis, pneumonia, and renal involvement). The disease caused by the 2019 novel coronavirus (2019-nCoV) was called Covid-19 by the World Health Organization in February 2020. Face-to-face communication and consistent exposure to body fluids such as blood and saliva predispose dental care workers at serious risk for 2019-nCoV infection. As demonstrated by the recent coronavirus outbreak, information is not enough. During dental practice, blood and saliva can be scattered. Accordingly, dental practice can be a potential risk for dental staff, and there is a high risk of cross-infection. This article addresses all information collected to date on the virus, in accordance with the guidelines of international health care institutions, and provides a comprehensive protocol for managing possible exposure to patients or those suspected of having coronavirus.

한국 성인의 저작문제와 우울증의 연관성 (Association between chewing problems and depression in Korean adults)

  • Objectives: To examine the association between chewing problems and depression in a representative sample of Korean adults. Methods: This study included 4,887 participants aged 19 years or older. Chewing problems were self-reported. Depression was measured using the PHQ-9 scale, which scored the frequency of symptoms over the last two weeks. Logistic regression analysis was used to estimate the odds ratio (OR) and 95% confidence intervals (CI). Results: There was a significant positive correlation between chewing problems and depression. Compared to those without chewing problems, participants with chewing problems had a 1.62 times higher risk of mild depression (OR=1.62; 95% CI:1.29-2.05), a 2.27 times higher risk of moderate depression (OR=2.27; 95% CI:1.52-3.38), and a 6.15 times higher risk of severe depression (OR=6.15; 95% CI:2.31-16.37). Conclusions: Results of this study indicate a significant association between chewing problems and depression. In addition, chewing problems were found to have a stronger relationship with higher severity levels of depression.

요양시설 거주 뇌졸중 노인을 위한 연하 훈련 프로그램의 효과 (Effects of Swallowing Training Program for Nursing Home Residents with Stroke)

  • Purpose: This study aimed to verify the effects of a swallowing training program on swallowing function and depression for nursing home residents with dysphagia after stroke. Methods: This is a quasi-experimental study with non-equivalent control group pre-post test design. The program (oro-facial muscle strengthening exercises, swallowing exercises, expiratory muscle strengthening exercises, and brain stimulation exercises) applied to the experimental group three times per week for eight weeks, 40-45 minutes for each intervention. The final data from 42 people (21 experimental and 21 control) were analyzed by SPSS/WIN 25.0 using descriptive statistics. 𝜒2 test, t-test, Wilcoxon rank sum test, and Friedman test. Results: The experimental group was significantly improved than control group in oro-facial muscle strength, swallowing symptoms (Z=-2.22, p=.026), and oral intake function level (Z=-2.00, p=.046). However, there was no significant difference between two groups in depression. Conclusion: This study is meaningful in that it reorganized and mediated a swallowing training program as a safe, non-invasive exercise that nurses can implement in a nursing facility with limited medical personnel and it could be easily followed by older adults.
