20대 성인 남성의 흡연에 따른 구강건강관련 특성 비교연구 (A comparative study on recognition of oral health among 20s male adults according to smoking status)

  • The purpose of this study was to establish an effective strategy for smoking cessation programs of adult by examining the relationship between smoking and oral characteristics in 166 men in their 20s in Ulsan and Busan. As a result, there was a difference in dental fear (χ2=4.72, p<0.01), tooth stain (χ2=10.22, p<0.01) and others perception of bad breath (χ2=7.35, p<0.01). Smokers had 2.03 times more dental fear, 3.26 times more tooth stain, and 2.51 times more other perception of bad breath higher than non smokers. Therefore, effective smoking cessation education should be continuously operated in order to prevent 20-year-old men from leading to lifetime smoking.

Evaluation of Head and Neck Cancer Awareness and Screening Status in Jeddah, Saudi Arabia

  • Background: Head and neck cancer (HNC) is one of the deadliest cancers worldwide. Early detection is a key determinant of HNC prognosis. Hence, raising awareness of this disease may improve survival rates. The present study evaluated the level of awareness and screening status for HNC in the general population of Jeddah, Saudi Arabia. Materials and Methods: An e-questionnaire was distributed during our HNC awareness campaign at the Red Sea Mall in Jeddah. The questions assessed HNC knowledge and screening status among participants. Results: Of the 112 respondents, 68% indicated that they had no knowledge of HNC. Social media was the major source of information (39%) for respondents. The majority (40%) believed that it was the joint responsibility of dentists, dental hygienists, and general physicians to screen for HNC; 82% had never been screened. Conclusions: Knowledge and awareness of HNC must be increased in the general population and among dental health professionals.

일부 만성 정신질환자의 구강건강 상태 (Oral Health Status of Some Patients with Chronic Mental Illness in Korea)

  • 본 연구는 최근에 크게 증가하고 있는 정신질환자의 구강건강 문제에 주목하여 그 실태를 파악하고자 시행하였다. 강원도 소재의 정신보건센터 한 곳의 주간 프로그램 참여대상자(23명)와 정신병원 한 곳의 입원 대상자(69명) 전체 92명을 편의집락추출하여 구강건강상태와 치주건강상태, 구강위생상태를 사정하였으며 수집된 자료를 비교 분석한 결과 다음과 같은 결론을 얻었다. 1. 상실치아(MT)는 병원 대상자(6.42)가 센터 대상자(1.83)보다 더 높았으나, 충전치아(FT)는 센터 대상자(4.78)가 병원 대상자(2.52)보다 높았다(p<0.05). DMFT index는 정신질환자(12.80)가 전국표본(6.22)에 비해 더 높았고, 연령이 높을수록, 학력이 낮을수록, 의료급여 대상자의 구강건강 상태가 더 나빴다(p<0.05). 2. 구강위생상태는 PHP index 3.41로 자가구강건강관리가 잘 이루어지지 않는 편이였다(p<0.05). 3. 치주건강 상태는 병원 대상자(81.7%)가 센터 대상자(47.6%)에 비해 치주요양필요자율이 더 높았으며, 특히 40대와 60대, 학력이 낮을수록, 가족이 없을수록 더 나빴다(p<0.05). 치주요양불필요자율(CPITN0)은 병원 대상자(18.3%)가 전국표본(27.4%)에 비해 낮았으며, 치주조기병치료필요자율(CPITN3)은 병원 대상자(13.3%)가 전국표본(5.7%)보다 더 높았다. 4. 정신질환자의 구강건강상태를 개선하기 위하여 치과위생사는 정신질환자의 구강건강 상태 증진을 위한 노력을 해야 하며, 이에 앞서 해당 집단 안에서 연령과 학력, 결혼유무, 가족 형태, 경제적인 상태 등을 선행조사할 필요가 있다. 또한 정신질환 종류와 심도별 인지능력을 고려하여 교육매체와 교육 프로그램을 구분하여 고안해야 한다. 한편 치과계에서는 정신질환자의 구강건강상태를 개선하기 위한 정책적인 방안을 마련하는 데에 점진적인 노력을 기울여야 하며, 주기적인 구강건강관리실태를 파악할 필요가 있다. 5. 본 연구는 접근이 어려운 정신질환자를 대상자로 선정하여 전반적인 구강건강상태를 측정하였다는 점에 그 의의가 있으나, 표본을 단일모집단으로 선정하였다는 점에 한계가 있다.

울산광역시 장애인 구강건강증진을 위한 자원봉사 중심의 지속적 치과진료사업의 사례 (Oral health promotion of the disabled by consistent voluntary dental care services)

  • The purpose of this study was to evaluate the oral health promotion of the disabled persons by voluntary dental services in Ulsan Metropolitan City. Two dentists taken a calibration training for national oral health survey examined the oral health status of 473 disabled persons from two special schools for the disabled children and adolescents, a residential facility and a gymnasium for the disabled persons in 2009-2010. The surveyed disabled persons in the age range was from 7 to 74 years old. Voluntary dentists, oral hygienists and other civilian volunteers had supplied with the oral health care services to the disabled persons at dental clinics of special schools for the disabled children and adolescents, and a dental clinic supported from Nam-Gu Public Health Center in Ulsan Metropolitan City since 1997. The obtained data from these surveys were analyzed with the SPSS statistical package. Among subjects aged 12-14 years, subjects with decayed, missing and filled teeth (DMFT) in permanent dentition was 46.9%; subjects with untreated decayed teeth, 17.2%. The number of decayed, missing and filled teeth in permanent dentition was 1.36. The proportion of decayed components of DMFT score was 28.00%; proportion of missing components of DMFT score, 1.43%; proportion of filled components of DMFT score, 70.57%. The proportion of filled components of DMFT score among disabled persons of all age group in Ulsan were evaluated to be a similar level to non-disabled citizens in Ulsan from 2010 Korean National Survey. The oral health care programs for disabled persons by voluntary services of dental professionals and other civilians are evaluated to be effective for the oral health promotion of disabled persons in Ulsan.

Bitter Taste, Rising New Functions and Significance of Extra-oral Expressions

  • Taste is closely related to intake of food. Taste perception is also influenced by type of food ingested, and nutrition and health status. Bitter taste plays an important role in the survival of human and animals to avoid probable toxic and harmful substances. Vertebrate animals recognize bitter taste through type 2 taste receptors (T2Rs). Several T2Rs have been expressed extra-oral such as the gastrointestinal tract, respiratory tract, urogenital tract, brain and immune cells, and parts of their functions are being revealed. This review will discuss physiological roles of T2Rs in relation to innate immunity, secretion and smooth muscle contraction expressed in extra-oral cells and tissues, and we summarize relationships between polymorphisms in T2Rs and general or oral diseases. It is not a coincidence that animals pay much genetic costs for taste and smell during evolution.

치매환자의 구강관리가 삼킴기능과 구강관련 삶의 질에 미치는 영향 (Effect of Oral Health Care on Swallowing Function and Swallowing-Quality of Life in Patients with Dementia)

  • 본 연구는 장기 요양시설에 거주하는 치매환자들을 대상으로 구강관리 실태를 평가하고 이것이 삼킴 기능과 구강건강관련 삶의 질에 미치는 영향을 연구하였다. 광주, 전남에 위치한 장기요양기관에 입소한 치매환자 60명을 대상으로 구강관리, 삼킴곤란 임상척도와 구강건강관련 삶의 질을 측정하였다. 결과는 첫째, 장기요양시설을 이용하는 치매환자의 구강관리가 삼킴기능과 구강건강관련 삶의 질에 미치는 영향은 구강관리 점수에 따라 삼킴기능과 구강건강관련 삶의 질에 각각 유의한 차이를 나타내었다(p<.05)(p<.01). 둘째, 치매환자의 구강관리, 삼킴기능과 구강 건강관련 삶의 질의 관계를 알아본 결과, 치매환자의 구강관리는 삼킴기능(r=.405, p<.01), 구강건강관련 삶의 질(r=.462, p<.01) 그리고 삼킴기능은 구강건강관련 삶의 질(r=.351, p<.01)과 유의한 상관관계를 나타내었다. 따라서, 장기 요양시설 이용자의 구강관리는 삼킴 기능과 구강건강관련 삶의 질에 매우 밀접한 관련을 나타내었다.

한국 성인의 치주치료요구와 구강건강행동의 관련성 (The association between periodontal treatment needs and oral health behaviors in Korean adults)

  • 본 연구는 본 연구는 지역사회치주치료요구지수(CPITN) 각 단계의 치주치료요구와 구강건강행동의 관련성을 확인하고자 하였다. 제7기 국민건강영양조사 자료를 이용하여 19-79세 성인 11,002명을 대상으로 다변량 로지스틱 회귀분석을 실시하였다. 치실과 치간칫솔과 같은 치간관리용품을 사용하는 경우 CPITN1(OR=0.709, p<0.001), CPITN2(OR=0.672, p<0.001), CPITN3(OR=0.783, p<0.05)과 치주염(OR=0.726, p<0.001) 위험을 유의하게 감소시켰다. 1일 3회 이상의 칫솔질은 CPITN1(OR=0.755, p<0.01)과 CPITN2(OR=0.811, p<0.05)의 위험을 유의하게 감소시켰으며, 치과검진은 CPITN1(OR=0.661, p<0.001), CPITN2(OR=0.621, p<0.001)와 치주염(OR=0.818, p<0.001)의 OR을 감소시켰다. CPITN의 각 단계 및 치주염 유무에 따라 구강건강행동의 효과는 차이가 있었다. 따라서 구강보건교육을 할 때에는 치간관리용품 사용 교육을 필수로 포함하고, 대상자의 치주건강상태에 따라 구강건강행동을 선택하여 교육하는 것이 도움이 될 것이다.

다문화 이주여성의 구강건강상태에 영향을 미치는 요인 (Factors influencing oral health status of immigrant women in multi-cultural families)

  • Objectives: This paper aimed to contribute to better oral disease prevention and practice of health behavior for immigrant women in multi-cultural families, to define missing and filled permanent teeth index of immigrant women, data from the 6th Korea National Health and Nutrition Examination Survey from the Korea Centers for Disease Control and Prevention was used. Methods: For the immigrant women to be subjects, they needed to be born overseas, had acquired Korean citizenship as a married immigrant women, and the estimate of the number of subjects was 133,093 women. For analyzing data, SPSS 21statistical program was used. We used covariance analysis (ANCOVA) andgeneral linear models for finding the relation with the missing and filled permanent teeth index. The significance level was 0.05. Results: DMFT-index of immigrant women was 7.33 points. $R^2$ was 0.416; and increased with age, and $R^2$ was 0.126 points higher (p<0.01). In household income, 'lower' was 5.933 points lower than 'upper' (p<0.05), and in toothbrushing after lunch, 'yes' was 3.598 points lower than 'no' (p<0.01). In preventive treatment, 'yes' was 4.301 points lower than 'no' (p<0.05). Conclusions: The result of this paper is as follows: for maintaining oral health of immigrant women, we think that the government needs to develop an oral health policy and a customized education system suited to immigrant women for preventive management of dental disease in immigrant women. In addition, basic data will be provided for public dental health programs based on the result of the study.

T-scan III® system을 이용한 일부지역 노인의 교합양상과 인지기능의 관련성 (Evaluation of relationship between cognitive function and occlusal status in elderly individuals using the T-scan III® system)

  • Objectives: The aim of this study was to investigate the relationship between cognitive function and occlusal status in elderly individuals. Methods: A total of 162 individuals aged 65 years and older, who attended the senior citizen center in Daegu city, were included after consent for participation in the study was obtained. The Korean version of the Mini-Mental State Examination (MMSE-K) was used to evaluate the level of cognitive function. Occlusal status was determined using the T-scan $III^{(R)}$ system. All collected data were analyzed by ${\chi}^2$ test, t-test, one way ANOVA, and linear regression analysis using SPSS version 23.0 for Windows. Results: There was a significant relationship between the cognitive function and the use of dentures. Individuals with posterior occlusal status had higher MMSE-K scores than those with anterior occlusal status. There was a positive correlation between the cognitive function and posterior occlusal force. Conclusions: These results suggest that occlusal status was associated with cognitive function in the elderly individuals. Active national policies to improve occlusal condition in the elderly population are needed.

흡연이 치아건강에 미치는 영향에 관한 연구 - 대구지역 치기공과 학생을 중심으로 - (Research about influence on the teeth health by the smoking - research intended for students of dental technology in Daegu -)

  • This survey study was conducted on dental technician school students in April 2008 to investigate the effects of smoking on oral health. 110 male and 39 female students were surveyed and cross analysis was performed to examine the relationship between oral health status and smoking-related and oral health maintenance characteristics of smoking and non-smoking group. T-test and one-way ANAVA was used to analyze the average difference verification of two independent samples(smoking group and non-smoking group) and followings are the results. In general characteristics, 74 male students(74.7%) and 25 female(25.3%) students smoked and the percentage of male students were higher in smoking group. Regarding class grade, 2nd-year students(37 students, 37.4%) and 1st-year students(18 students, 36.0%) took the highest percentage of smoking and non-smoking group, respectively. Concerning the experience of drinking, smoking group consumed more alcoholic beverages(94 students, 94.9%) With respect to the frequency of between-meal consumption, respondents who eat between-meal once or twice smoked more. Respecting preferences of sweet food, respondents who enjoyed sweet food smoked more. Concerning smoking characteristics, many of smoking group had been smoked fo a long time(two to five years) and it was worrisome. About the amounts of smoking, 50 respondents(52.6%) of smoking group smoked 20 cigarettes a day with the highest proportion. With regard to the awareness of harmfulness, 70 respondents(70.7%) considered smoking as very harmful and showed that smoking group were aware of its harmfulness but it was habitualized. Respecting smokers in family members, father was more common as smokers(58 respondents, 58.6%) in family and showed that the influence of father. Main reasons of poor periodontal health were alchoholic consumption, smoking, and oral parafunctional habit(57 respondents, 56.7%) in smoking group and neglect of oral hygiene(21 respondents, 42.0%) in non-smoking group. With regard to the scaling, 80 respondents of smoking group(80.8%) didn't received scaling and showed that more oral health education was required. Regarding the periodontal status, non-smoking group was $1.26{\pm}0.44$, respondents who received scaling was $1.43{\pm}0.50$, respondents who regularly floss was $1.50{\pm}0.52$, respondents who brush more than three times a day was $1.38{\pm}0.49$ and had better periodontal health. Bleeding during tooth brushing was more frequent in smoking group ($1.51{\pm}0.70$). Regarding mouth order, non-smoking group was $2.34{\pm}0.62$, after scaling was $2.02{\pm}0.76$, patients who regularly floss was $1.50{\pm}0.52$, patients who brush more than three times a day was $1.81{\pm}0.87$. Concerning the experience of dental caries treatment, smoking group was $1.20{\pm}0.40$, patients who do not floss was $1.30{\pm}0.46$, patients who brush once a day was $1.29{\pm}0.45$. With regard to dental prosthesis, non-smoking group was $3.78{\pm}1.62$, patients who received scaling was $1.43{\pm}0.50$, patients who regularly floss was $1.40{\pm}0.51$, patients who brush more than three times a day was $1.24{\pm}0.43$. From these results, oral health education and smoking has correlation and more oral health education needs to be carried out to educate students with non-health related majors about harmfulness of smoking on oral health. Also more study are required.
