Bioequivalence of Cefaclor Monohydrate 250mg Capsules Using an Improved HPLC Analytical Method (개선된 HPLC분석법을 이용한 세파클러 모노하이드레이트 250 mg 캡슐의 생물학적동등성)

  • A bioequivalence study of CKD $Cefaclor^{(R)}$ capsule (Chong Kun Dang Pharm Co., Ltd) to $Ceclor^{(R)}$ capsule (Lilly Korea Co., Ltd.) was conducted according to the guidelines of Korea Food and Drug Administration (KFDA). Twenty four healthy male Korean volunteers received each medicine at the cefaclor dose of 250 mg in a $2{\times}2$ crossover study. There was a one-week washout period between the doses. An improved high-performance liquid chromatorgraphy (HPLC) analytical method with UV detection was used to determine plasma cefaclor concentration in human volunteers for 8 hr after oral drug administration. The area under the plasma concentration-time curve from time zero to 8 hr ($AUC_{0-8hr}$) was calculated by the linear trapezoidal rule. the $C_{max}$ (maximum plasma drug concentration) and $T_{max}$ (time to reach $C_{max}$) were compiled from the plasma concentration-time data. Analysis of variance was carried out using logarithmically transformed $AUC_{0-8hr}\;and\;C_{max}$. No significant sequence effect was found for all of the bioavailability parameters indicating that the cross-over design was properly performed. The $90{\%}$ confidence intervals of the $AUC_{0-8hr}$ ratio and the $C_{max}$ ratio for CKD $Cefaclor^{(R)}$ and $Ceclor^{(R)}$ were $0.9400{\leq}{\delta}{\leq}1.0345$ and $0.8858{\leq}{\delta}{\leq}1.1021$, respectively. These values were within the acceptable bioequivalence intervals of 0.80-1.25. Thus, our study demonstrated the of CKD $cefaclor^{(R)}$ capsule was bioequivalent to $Cefaclor^{(R)}$ capsule with respect to its bioavailability.

A Study on the Side Effect of the Splint Therapy for the Patient with Craniomandlbular Disorders (교합상을 이용한 치료가 두개하악장애 환자에 미치는 부작용에 관한 연구)

  • 저자는 두개하악 장애로 교합상치료를 받고있는 42명의 환자를 대상으로 치료전, 치료후 1개월, 3개월, 6개월째에 주관적 평가, 임상적 검사, 근전도 검사, 컴퓨터 교합 분석을 시행하여 다음과 같은 결론을 얻었다. 1. 주관적 평가인 visual analogue scale과 Helkimo's anamnestic dysfunction index는 유의하게 감소하였다. 임상적 검사시 MM(mandibular movement), TM(TMJ capsule palpation), EM(extraoral muscle palpation), Di(Helkimo's clinical dysfuction index)는 유의하게 감소하였고 무통성 개구량은 유의하게 감소하였다. 2. 42명의 환자중 10명에서 교합변화가 관찰되었으며, 5명에서 치주질환, 2명에서 치아 과민감, 1명에서 구토경향이 관찰되었다. 3. 치주칠환을 보이는 10명의 환자중 3명은 교합상치료 이전부터 치주질환에 이환된 상태였으며 3명에서는 교합변화가 함께 관찰되었다. 4. 교합변화를 보인 10명의 환자중 교합상의 장착시간을 줄였을 때, 3명이 원래의 교합상태로 회복되었고 4명은 부분적으로 회복되었으며 3명에서는 변화를 관찰할 수 없었다. 5. 악관절 내장 제 5기 환자에서 교합변화가 더 빈번히 나타났으며 통계적으로 유의한 차이를 보였다. 6. Habitual clenching 시와 Maximal clenching 시 사이의 치아접촉점 수, maximal clenching 시의 치아 접촉점의 수와 접촉 치아수 사이에 매우 유의한 상관관계를 보였고, habitual clenching 시의 치아접촉점의 수와 접촉 치아수, Habitual clenching 시와 maximal clenching시 사이의 치아 접촉시간에서도 유의한 상관관계를 보였다.

Enhanced Bioavailability of Itraconazole in Liquid Preparation (난용성 항진균제 이트라코나졸의 액상제제화에 의한 생체이용율 개선)

  • This study was to develop an effective itraconazole liquid preparation which exhibits an enhanced bioavailability. The solubility of itraconazole was increased (72-fold) in itraconazole liquid preparation as compared with itraconazole powder. The dissolution rate of itraconazole was higher for itraconazole liquid preparation filled into a hard gelatin capsule with 90% release within 20 min as compared to 55% for $Sporanox^{\circledR}$capsules. The oral absorption of itraconazole liquid preparation and $Sporanox^{\circledR}$tablets were studied in the rat. The area under the concentration-time curve $(AUC_{0-24hr})$ of itraconazole liquid preparation ($90.25\;{\pm}\;8.36\;{\mu}g{\cdot}hr/ml$) increased by 6.2 times compared to that of Sporanox tablets ($14.58\;{\pm}\;1.26\;{\mu}g{\cdot}hr/ml$) after oral administration of itraconazole 15 mg/rat each. $C_{max}$ also increased to $6.87\;{\pm}\;1.15\;{\mu}g/ml$ after administration of liquid preparation $1.58\;{\pm}\;0.16\;{\mu}g/ml$ of $Sporanox^{\circledR}$tablets. These results indicate that in vivo bioavailability of itraconazole liquid preparation was significantly enhanced as compared with $Sporanox^{\circledR}$tablets.

Lipoma with Extraoral Swelling in the Labial Vestibule: Report of a Case

  • Lipoma is the most common benign neoplasm of the body with rare occurrence in the oral cavity. It represents 1~4% of benign neoplasms of the mouth, which affect the buccal mucosa, floor of the mouth, tongue and lips. We report a case of lipoma in the labial vestibule with extraoral swelling, which could easily be misdiagnosed as an odontogenic abscess. Excisional biopsy in this case revealed well-circumscribed masses, surrounded by a thin fibrous capsule and composed of sheets of mature adipocytes, arranged in a "chicken wire" configuration. After a computed tomography scan, excisoinal biopsy was done, and there were no recurrence after 5-month follow-up period.

  • The auther embedded the clinically differently used direct resins to the oral submucosa of the rabbits, and compared the tissue reactions to the direct resins with that to the indirect resins. The results were as follows: 1. Direct resins showed more lympocytes infiltration than indirect resins at the early stage of the embedding, but the same degree of the appearance of lympocytes infiltration as indirect resin at the late stage of the embedding. And the formation of fibrosis around the embedding showed the irregular finidng. 2. Fibrosis of direct resin showed the tendency to be finer than indirect resin at the early stage of the embedding, but direct resin formed the similar fibers capsule to that of indirect resin at the late stage of the embedding.

  • Silybin is the main component of Cardus marianus extracts (Silymarin) originated from Silybum marianum, called as milk thistle. It has a hepato-protective effect and is used clinically for the treatment of liver disease. But it is water-insoluble and is poorly absorbed from the gastrointestinal tract, resulting in very low oral bioavailability. Polymeric mixed-micelle precursor formulation containing surfactants, co-solvents, and block-co polymers with Cardus marianus extracts was made to enhance the dissolution rate of silybin and encapsulated with soft gelatin capsule. The Effects of Kami-bangpungtongseong-san Extracts on the Mouse Liver and Kidney Toxicated by Mercuric Chloride Subcutaneous Injection (가미방풍통성산(加味防風通聖散)이 수은의 피하 주입으로 중독된 마우스의 간 및 신기능에 미치는 영향)

  • Objective : For this study, mice on mercurial toxication were given mercuric subcutaneous injection to their abdomen factitiously. After delivering Kami- bangpungtongseong-san(KBT) extracts to the mice by oral administration, we observed changes from liver and kidney of mice. Method : The BALB/c mice were distributed into three groups: No treated group(Normal group), Mercuric chloride subcutaneous injection group(Control group), Kami-bangpungtongseong-san-treated group (Sample group). KBT Extracts were delivered orally in 7 days. We observed involution of liver, necrosis of liver and cell plate loss of liver, lipid peroxidation CYP1A1 expression. We observed involution of proximal convoluted tubules, hypertrophy of Bowman's capsule, periodic acid-Schiff(PAS)'s positive reaction of proximal convoluted tubules, heat shock protein(HSP)700's positive reaction in glomerulus. For the charting the results, image analysis was taken. The result of image analysis was verified significance by Sigmaplot 2000(P<0.05). Result : The mice' liver on mercurial toxication were relieved involution of liver, necrosis of liver, and cell plate loss of liver and also declined lipid peroxidation and CYP1A1 expression. The mice' kidney on mercurial toxication were relieved involution of proximal convoluted tubules, hypertrophy of Bowman's capsule and increasing PAS's positive reaction of proximal convoluted tubules. On the other hand it was declined HSP700's positive reaction in glomerulus. Conclusion : According to the result of study, we think that we can expect to the effect of KBT extracts' therapeutic action to tissue injuries of the mice' liver and kidney on acute mercurial toxication.

Efficacy Test of Polycan, a Beta-Glucan Originated from Aureobasidium pullulans SM-2001, on Anterior Cruciate Ligament Transection and Partial Medial Meniscectomy-Induced-Osteoarthritis Rats

  • The object of this study was to assess the efficacy of Polycan from Aureobasidium pullulans SM-2001, which is composed mostly of beta-1,3-1,6-glucan, on osteoarthritis (OA)-induced by anterior cruciate ligament transection and partial medial meniscectomy (ACLT&PMM). Three different dosages of Polycan (85, 42.5, and 21.25 mg/kg) were orally administered once a day for 84 days to male rats a week after ACLT&PMM surgery. Changes in the circumference and maximum extension angle of each knee, and in cartilage histopathology were assessed using Mankin scores 12 weeks after Polycan administration. In addition, cartilage proliferation was evaluated using bromodeoxyuridine (BrdU). As the result of ACLT&PMM, classic OA was induced with increases in maximum extension angles, edematous knees changes, and capsule thickness, as well as decreases in chondrocyte proliferation, cartilages degenerative changes, and loss of articular cartilage. However, these changes (except for capsule thickness) were markedly inhibited in all Polycan- and diclofenac sodium-treated groups compared with OA control. Although diclofenac sodium did not influence BrdU uptake, BrdU-immunoreactive cells were increased with all dosages of Polycan, which means that Polycan treatment induced proliferation of chondrocytes in the surface articular cartilage of the tibia and femur. The results obtained in this study suggest that 84 days of continuous oral treatment of three different dosages of Polycan led to lesser degrees of articular stiffness and histological cartilage damage compared with OA controls 91 days after OA inducement, suggesting that the optimal Polycan dosage to treat OA is 42.5 mg/kg based on the present study.

A Descriptive Research on Drug Use Pattern of Narcotic Analgesics: a Case of University Hospital in Korea (마약성 진통제의 사용양상에 관한 서술적 조사연구: 한 대학병원의 경우)

  • The objective of this study was to find out drug use pattern of narcotic analgesics in university hospitals in Korea. A university hospital located in Kyungbuk province was chosen for this study. The drug use pattern was analyzed in terms of ingredient, administration route, patient type, and attending department. Amount of drug usage was counted by unit dose defined by the number of ampule or vial for injectable, tablet or capsule for oral, and each for patch preparations. Result showed that 11 narcotic analgesic ingredients were used during 2007-2009, and the drug usage was increased by about 20% annually during the period. Proportion of oral preparations used for pain management was about two third of all narcotic analgesics usage and kept increasing during the period. Proportion of the drug usage for outpatients was also steadily increased. Notably, the usage of oral preparations of oxycodone, morphine, and hydromorphone was rapidly increased for the management of cancer pain while the usage of codeine and codeine-containing composite preparations for cancer pain were minimal (<10%). About 90% of all narcotic analgesics were used by physicians in Internal Department, especially in Oncology Division of the Department. These findings suggest that pain management is becoming more aggressive and in agreement with WHO's guidelines regarding selection of administration route. However, in terms of 3-step ladder for cancer pain management, the drug use pattern was not congruent to WHO's guidelines. Therefore, in conclusion, it appears that physicians need to try to be congruent to the guidelines when using narcotic analgesics for cancer pain.

SOLITARY NEUROFIBROMA OF THE CHIN (이부에 발생한 신경섬유종)

  • Tumors originated from peripheral nerve tissues are neurofibroma, neurilemoma, plexiform neurofibroma, malignant schwannoma, and granular cell tumor. Neurofibromas seem to occur in two forms : The first one is circumscribed solitary neurofibroma and the second group is neurofibromatosis or Von Recklinghausen's disease, which is a congenital and familial disease, presenting abnormalities of the skin, nerve system, bones and soft tissue. A solitary neurofibroma is a benign, relatively circumscribed, noncapsulated tumor which often presents in the skin and subcutaneous tissue as a soft sessile or pedunculated mass. It may occur anywhere in the head and neck, but the common site of the occurrence is the tongue, buccal mucosa, palate in frequency. Since solitary neurofibroma is a relatively radioresistant and its recurrence rate seem to be low, the treatment of choice is surgical excision. The author would like to present a case of unusually large solitary neurofibroma occured in the chin, which was successfully treated with surgical excision and reconstructed using deltopectoral flap and tongue flap.
