Influence of Autologous and Homologous Blood Transfusion on Interleukins and Tumor Necrosis Factor-α in Peri-operative Patients with Esophageal Cancer

  • Objective: To explore the influence of different ways of blood transfusion on the expression levels of interleukins (IL) and tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$) inperi-operative patients with esophageal cancer. Materials and Methods: A total of 80 patients with esophageal cancer who underwent radical operations were selected as study patients and randomly divided into an observation group (treated with autologous blood transfusion) and control group (with homologous blood transfusion). Changes of intra-operative indexes and peri-operative blood indexes, from hemoglobin (Hb) and hematocrit value (Hct), to levels of inflammatory factors like interleukins-6 (IL-6), IL-8, IL-10 and tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$) were compared. Results: Operations for patients in both groups were successfully conducted, and no significant differences in mean surgical duration and intra-operative hemorrhage volume, fluid infusion volume and blood transfusion volume were detected (p>0.05). Compared with values before surgery, Hb and Hct levels decreased significantly while white blood cell count (WBC) increased 1, 5 and 7 d after operation (p<0.05, p<0.01). In addition, WBC was apparently higher in observation group than in control group 5 and 7 d after operation (p<0.01). Compared with before surgery, in the observation group, levels of IL-6, IL-8 and IL-10 had no significant differences after operation (P>0.05), but TNF-${\alpha}$ level increased y (p<0.01), whereas in control group, IL-6 level had no significant difference (p>0.05), IL-8 level decreased obviously (p<0.05), IL-10 level increased markedly first and then decreased gradually as time passed but its level remained elevated (p<0.01), and TNF-${\alpha}$ level increased first and then decreased, and there was no significant difference 7 d after operation (p>0.05). Conclusions: Decreased IL-8 and increased IL-10 levels are two important reasons for immunosuppression after homologous blood transfusion, whereas autologous blood transfusion can alleviate this while increasing the TNF-${\alpha}$ level, which also has potential to improve anti-tumor immunity in the human body.

The Role of Transhiatal Operation at Esophageal Carcinoma Operation (식도암의 수술에서 transhiatal 술식의 역할)

  • Background : Controversy exists whether patients with esophageal carcinoma are best managed with classical Ivor Lewis esophagectomy(ILO) as combined thoracic and abdominal approach or transhiatal esophagectomy(THO). The THO approach is known to be superior with respect to operative time, morbidity and mortality, and length of stay, especially at poor pulmonary function patient, but may represent an inferior cancer operation due to inadequate mediastinal clearance compared with ILO. Accordingly, we estimated the THO role at esophageal cancer to compare each operative approach. Material and Method : From January 2002 to December 2007, we performed a retrospective review of all esophagectomies performed at Keimyung University Dongsan Medical Center; 36 underwent THO, and 11 underwent ILO. Result : There were all men and squamous cell carcinoma but 1 woman at ILO group, 2 women at THO group. There were no significant differences between THO and ILO with age, sex, location of tumor, mean tumor length. There were significant differences at preoperative pulmonary function test(In ILO group, average FEV1 is $2.65{\pm}0.6\;L/min$ and iIn THO group, average FEV1 is $2.07{\pm}0.7\;L/min$). The amount of blood transfusion, hospital stay, leak rates and respiratory complication, hospital mortality rate were not significantly different. Conclusion : There was no significant difference in the post-operative complication, hospital mortality rate, long-term survival of patients of both operative method. THO method had lower mobidity and mortality at poor pulmonary function patient than ILO method. Hence, THO is a valid alternative to ILO for patients with poor general condition or expected post-operative respiratory complication.

Can Robotic Gastrectomy Surpass Laparoscopic Gastrectomy by Acquiring Long-Term Experience? A Propensity Score Analysis of a 7-Year Experience at a Single Institution

  • Purpose: It is hypothesized that robotic gastrectomy may surpass laparoscopic gastrectomy after the operators acquire long-term experience and skills in the manipulation of robotic arms. This study aimed to evaluate the long-term learning curve of robotic distal gastrectomy (RDG) for gastric cancer compared with laparoscopic distal gastrectomy (LDG). Materials and Methods: From October 2008 to December 2015, patients who underwent LDG (n=809) were matched to patients who underwent RDG (n=232) at a 1:1 ratio, by using a propensity score matching method after stratification for the operative year. The surgical outcomes, such as trends of operative time, blood loss, and complication rate, were compared between the two groups. Results: The RDG group showed a longer operative time (171.3 minutes vs. 147.6 minutes, P<0.001) but less estimated blood loss (77.6 ml vs. 116.6 ml, P<0.001). The complication rate and postoperative recovery did not differ between the two groups. The RDG group showed a longer operative time and similar estimated blood loss compared with the LDG group after 5 years of experience (operative time: 159.2 minutes vs. 136.0 minutes in 2015, P=0.003; estimated blood loss: 72.9 ml vs. 78.1 ml in 2015, P=0.793). Conclusions: In terms of short-term surgical outcomes, RDG may not surpass LDG after a long-term experience with the technique.

Case report of Kimura's disease Treated by Radical resection and Post Operative Steroid therapy (기무라 병 환자에서 수술적 치료 및 술 후 스테로이드 병합요법의 치험2례)

  • Purpose: Kimura's disease is a relatively rare head and neck tumor that frequently occurs in young orientals accompanied with eosinophila. We shared our experience of two cases of Kimura's disease, treated by radical resection and post operative steroid therapy, so we report the correlation of eosinophil counts and disease progression. Methods: A 25 years old male came to the clinic with a mass localized to the right cheek inferior to the right auricle. We could not resect the mass totally. During the follow up period, we checked the eosinophil counts, and steroid therapy was started 7 months after the surgery. A 34 year old female came to the clinic with a mass localized inferior to the left auricle reaching from the posterior portion of the auricle to the left temporal portion. We tried to remove as much tumor as possible, save the temporal region, in regard to the impairment of blood supply to the auricle. After operation, steroid therapy was started. Results: In the first case, the tumor was easily approachable, and radical resection of the tumor with post operative steroid therapy was an effective treatment. In tumors located at difficult regions to remove, as in the second case, optimal debulking and post operative steroid therapy was also effective in treating Kimura's. There were no recurrences in both cases. Eosinophil counts were always reduced after surgery and steroid therapy, and during the period with low eosinophil counts, there was no recurrence of Kimura's disease. Conclusion: Surgery and post operative steroid therapy were effective in treatment of Kimura's disease, and we could assume eosinophil counts as a good indicator for evaluation of the prognosis of Kimura's disease during the follow up period.

Shear bond strength of a new self-adhering flowable composite resin for lithium disilicate-reinforced CAD/CAM ceramic material

  • PURPOSE. The purpose of this study was to evaluate and compare the effects of different surface pretreatment techniques on the surface roughness and shear bond strength of a new self-adhering flowable composite resin for use with lithium disilicate-reinforced CAD/CAM ceramic material. MATERIALS AND METHODS. A total of one hundred thirty lithium disilicate CAD/CAM ceramic plates with dimensions of $6mm{\times}4mm$ and 3 mm thick were prepared. Specimens were then assigned into five groups (n=26) as follows: untreated control, coating with $30{\mu}m$ silica oxide particles ($Cojet^{TM}$ Sand), 9.6% hydrofluoric acid etching, Er:YAG laser irradiation, and grinding with a high-speed fine diamond bur. A self-adhering flowable composite resin (Vertise Flow) was applied onto the pre-treated ceramic plates using the Ultradent shear bond Teflon mold system. Surface roughness was measured by atomic force microscopy. Shear bond strength test were performed using a universal testing machine at a crosshead speed of 1 mm/min. Surface roughness data were analyzed by one-way ANOVA and the Tukey HSD tests. Shear bond strength test values were analyzed by Kruskal-Wallis and Mann-Whitney U tests at ${\alpha}=.05$. RESULTS. Hydrofluoric acid etching and grinding with high-speed fine diamond bur produced significantly higher surface roughness than the other pretreatment groups (P<.05). Hydrofluoric acid etching and silica coating yielded the highest shear bond strength values (P<.001). CONCLUSION. Self-adhering flowable composite resin used as repair composite resin exhibited very low bond strength irrespective of the surface pretreatments used.

Operative Lung Biopsy in Patients with Findings of Diffuse Infiltrative Lung on Chest X-ray (흉부 단순 촬영상 전반적 폐 침윤을 보이는 환자에서 시행한 수술적 폐 생검)

  • The patient with findings of diffuse infiltrative lung on chest X-ray has various causes; therefore, the prognosis is different and the treatment should be changed according to the cause. We are trying to identify the meaning of operative lung biopsy and to 11nd a more accurate and effective procedure. We reviewed 46 medical records of patients with the findings of diffuse infiltrative lung on chest X-ray who had undergone operative lung biopsy or biopsies for 8 years. The open lung biopsy were done in 22 cases(47.8%) and thoracoscopic lung biopsy in 24 cases(52.2%). There is no significant difference in the rate of diagnosis(p=0.452) and the incidence of complications(p=0.155) between these groups. The number of cases with more than two biopsies are 9(19.6%) and that of one biopsy are 37(80.4%). There are no statistical difference in the rate of diagnosis(p=0.928) and the incidence of complications(p=0.125). The postoperative complications occurred in 8 cases,7 cases of air leak more than 7 days and 1 case of respiratory insufficiency. In the diagnosis and treatment of the patients with findings of infiltrative lung on chest X-ray, the operative lung biopsy is the very necessary course, and shows satisfactory rate of diagnosis with negligible complications.

AN EXPERIMENTAL STUDY ON THE TEMPERATURE CHANGE OF THE PULP CHAMBER INDECED BY THE CO2 LASER IRRADIATION (CO2레이저 조사에 따른 치수강내 온도변화에 관한 실험적 연구)

  • The purpose of this study was to suggest the use of laser energy in the the field of operative dentistry without considerable pulpal damage and significant effects on the dental hard tissue, additionally to find out the methods which could control the temperature rise. The laser beam (CW $CO_2$ laser, output: 6W, beam diameter: 1.5mm) was focused on the center of the occlusal surface of extracted lower molars. A Ge lens (focal length 200mm) was used to focus the primary laser beam. In order to vary the total amount of the same irradiated energy, experimental subjects were devided into three groups: continuously irradiated group, intermittently irradiated group, and water-cooled group after continuous laser irradiation. Temperature changes in the pulp chamber after laser irradiation were measured and recorded by the digital thermometer and recorder. The following results were obtained: 1. Temperatures in the pulp chamber were raised up in the order of the continuously irradiated group, intermittently irradiated group, water-cooled group after continuous laser irradiation. 2. In the continuously irradiated group, the temperature was raised up $1.7^{\circ}C$, $3.8^{\circ}C$, $7.3^{\circ}C$, $17.2^{\circ}C$ after 2, 4, 8, 16 seconds of the irradiation of laser. In the intermittently irradiated group, the changes were $1.2^{\circ}C$, $3.4^{\circ}C$, $6.3^{\circ}C$, $11.1^{\circ}C$, respectively. In the water-cooled group after continuous laser irradiation, the changes were $0.0^{\circ}C$, $0.8^{\circ}C$, $1.6^{\circ}C$, $6.9^{\circ}C$, respectively. 3. The starting time of temperature rise in the pulp chamber had no connection with laser irradiation time.

A STATISTICAL STUDY ON WORKING LENGTH OF UPPER AND LOWER ANTERIOR TEETH IN ENDODONTIC TREATMENT (상·하악 전치부의 근관치료에 사용되는 시술길이(Working Length)에 관한 통계학적 연구)

  • This study was undertaken to obtain the average canal length of upper & lower anterior teeth which was important in canal length measuring procedure of endodontic treatment. It was based upon 827 out-patients who had endodontic treatment on their upper & lower anterior teeth at the Department of Operative Dentistry, Dental Infirmary, Yonsei Medical Center from February, 1978 to June, 1984. The 1249 teeth of these patients were devided into sex and age groups. The root canal length of these teeth were measured. The following results were obtained; 1. The mean root canal length of upper & lower anterior teeth were as follows; Upper central incisors : 21.8mm Upper lateral incisors : 21.0mm Upper canines : 24.1mm Lower central incisors : 18.6mm Lower lateral incisors : 19.9mm Lower canines : 22.6mm 2. There was no significant difference in root canal length between sex. (P > 0.05) 3. There was no significant difference in root canal length between age groups. (p > 0.05) 4. The distribution of upper central incisors showed the highest distribution followed by upper lateral incisors and lower central incisors between 10 to 40 year old age groups, and there was no signigicant difference in the rest of the age groups. There was no significant difference in sex distribution, which was 49.5% for males and 50.4% for females. The number of the upper anterior teeth was 74.3% of all the specimens and the lower anterior 25.7%, and 40.6% of all the specimens were upper central incisors.

Effect of Tailored Music Intervention on Intra-operative Anxiety among Those Undergoing Regional Anesthesia (환자의 선호도를 고려한 음악중재가 부분마취 환자의 수술 중 불안에 미치는 효과)

  • Purpose: Music intervention has long been used in research of patients undergoing surgical operation in reducing anxiety level and improve surgical outcome. However, there are few studies that have considered a patient's music preference. We investigated the effect of the tailored music intervention which chose music according to the patient's preference on anxiety level and vital signs. Method: The subjects were 50 patients who received regional anesthesia for surgical operation at D hospital in Pohang city from April, 2006 to November, 2006. All of the subjects were randomly assigned either music group (30 subjects) or non-music group (20 subjects). As the patients arrived in the operating room, vital signs were monitored until the subjects were transferred to the recovery room, while Spielberger's STAI-KYZ questionnaires were applied twice to measure preoperative and intra-operative anxiety. The data were analyzed by 2-test, t-test, one-way ANCOVA and repeated measures ANOVA using SPSS 12.0/PC+. Results: There were no significant differences between the two groups on vital signs. All of the vital signs increased when the subjects arrived in the operating room, but decreased quickly once the operation began, regardless of the groups. However, the music group reported significantly less intra-operative anxiety, compared to the non-music group (F=15.208, p<.000), when preoperative anxiety was treated as a covariance. Conclusion: The findings support that the use of music which was chosen by patients during the surgery significantly reduced patient's intra-operative anxiety during regional anesthesia.

Heart Valve Replacement With Ionescu-Shiley Valves: Report Of 265 Cases (IonescuShiley 판막 사용 심장판막이식술[265례 보고])

  • In this department 504 cases of valve replacement were done since 1968 to the end of October 1981. Since October 31, 1978 to the end of October 1981 ,333 Ionescu-Shiley bovine pericardial xenograft bioprosthetic cardiac valves were replaced in 265 patients. There were 149 males and 116 female. Ages ranged from 2 to 63 years with 25 cases under 15 years of age. Among 265 cases of Ionescu valve replacement there were 157 MVR, 36 VAR, 6 TVR, 45 MVR+ AVR, 16 MVR+TVR and 5 MVR+AVR+TVR cases with mortality of 5.7%, 8.3%, 16.7%, 8.9%, 18.8% and 20% for each group respectively. Over all mortality rate in 265 Ionescu valve replacement cases was 7.9% with 21 total deaths. Main causes of operative deaths were due to LCOS in 7, bleeding in 5, arrhythmia in 3, air embolism in 2,and heart block in 2 cases. There were 12 late complications with 6 deaths. Over all long-term survival rate was 89.8%. MVR showed the highest long-term survival rate with 92.4%, and MVR+AVR+TVR the lower with 80% lower with 80%.Average follow-up period was 14 month duration. Twenty five congenital anomaly cases were operated with Ionescu-valve replacement that consisted of 7 VSD+AI, and 5 Ebstein anomaly cases with over all operative mortality of 16% and late mortality of 14.3% among 21 operative survivors. There were 25 Ionescu valve replacement cases in pediatric patients under the age of 15 years, with 4 operative deaths. Fourteen MVR, 7 AVR, and 3 TVR cases were found. Even though long-term follow-up study was short in postoperative period with total of 33~.0 months among 244 operative survivors ranging one to 36 months, the late survivors showed beneficial long-term results two thromboembolic episodes in 244 patients were found. More cases and longer term follow-up study are warranted for valve replacement in pediatric and TVR cases with Ionescu-valves which have advantageous hemodynamic structures compared with other bio-prosthetic heart valves.
