최근 고령화로 인해 복지기술(Welfare Technology)의 개발욕구 및 사용 필요성은 사회적으로 증대하고 있으나 새롭게 개발된 복지기술의 개인적 사회적 경제적 영향에 대한 평가논의는 부족한 실정이다. 따라서 본 연구는 복지기술에 대한 개념과 기술영향평가의 이론적 내용들을 분석하여 고령층을 위한 복지기술영향 평가에 적용할 수 있는 지표개발에 대한 논의를 하는 것을 목적으로 하고 있다. 이를 위해 일반기술과 구별되는 복지기술 대상자의 특징을 살펴보고 복지기술의 개념정립과 이론적 고찰을 시도하였다. 또한 기존 기술영향평가지표와 복지기술관련 연구결과를 분석하여 고령층을 위한 복지기술영향평가 지표들을 도출하였다. 평가지표들은 개인적, 경제적, 사회적, 과학기술적, 환경적, 윤리적 측면 등 총 6개 분야별로 제시되었으며, 사례연구를 위해 2개의 고령층대상 복지기술제품의 영향평가에 적용 활용하였다. 본 연구에서 도출된 고령층대상 복지기술영향평가지표들은 복지기술의 수혜자인 고령층 또는 돌봄제공자들이 중요한 이해관계자로서 참여하여 복지기술영향평가의 실효성을 증대시키는데 중점을 두었다.

Purpose: Several screening tools have been developed to identify sarcopenia in rural community-dwelling older adults. We aimed to compare the diagnostic accuracy of two such tools, namely the SARC-F and SARC-CalF assessments. Methods: This cross-sectional study on 388 community-dwelling older adults comprised 254 women and 134 men with a mean age of 77.8 ± 6.26 year in Korea. We assessed muscle mass, muscle strength, and physical performance using a bioimpedance analysis device, hydraulic hand dynamometer, and 4 m gait speed test, respectively. Three widely-used diagnostic criteria [the Asian Working Group for Sarcopenia (AWGS), European Working Group on Sarcopenia in Older People, and the International Working Group on Sarcopenia] were applied. Sensitivity and specificity analyses were performed on the SARC-CalF and SARC-F tests. We used receiver-operating characteristic curves and the area under the curves (AUCs) to compare the diagnostic accuracy of the assessments with regard to sarcopenia. Results: An analysis using four sets of diagnostic criteria showed that the prevalence of sarcopenia was 27.6% to 41.0%. Using the AWGS 2019 criteria as a reference standard, the SARC-CalF had a sensitivity of 83.02% and a specificity of 53.71% in the entire study population, whereas the SARC-F had a sensitivity of 79.87% and a specificity of 41.92%. The AUCs for the SARC-CalF and SARC-F tests were 0.725 (95% confidence interval 0.678-0.769) and 0.645 (95% confidence interval 0.595-0.693), respectively (p<001). In the analyses using the other three diagnostic criteria, similarity was also confirmed. Conclusion: SARC-CalF showed better sensitivity than did SARC-F when diagnosing sarcopenia in rural community-dwelling older adults. Further studies are needed to verify this finding in different populations.
Purpose: This study aims to identify the factors that influence the experience of falls among older adults living in the community. Methods: The study participants were 70,887 65-year-olds who participated in the 2021 Community Health Survey. The study employed the Rao-scott x 2 test to examine the variation in fall experiences based on the characteristics of the older adults. Multiple logistic regression analysis was conducted to investigate these characteristics' impact on older adults' fall experiences. Results: The proportion of subjects in fall experience was 16.6%. The factors influencing the subject's fall experience were sex (odds ratio [OR]=1.47, 95% confidence interval [CI]=1.37~1.57), age (OR=1.48, 95% CI=1.34~1.65), family structure (OR=1.23, 95% CI=1.15~1.31), body mass index (OR=1.13, 95% CI=1.06~1.20), diabetes (OR=1.12, 95% CI=1.06~1.20), depression experiences (OR=1.56, 95% CI=1.42~1.70), stress (OR=1.12, 95% CI=1.05~1.19), subjective health status (OR=1.77, 95% CI=1.63~1.92), life satisfaction (OR=1.57, 95% CI=1.41~1.76), and chewing discomfort (OR=1.29, 95% CI=1.21~1.38). Conclusion: Efforts should be made to effectively educate and develop various programs aimed at reducing falls among older adults. It is essential to emphasize the importance of continuous and active attention to falls in the older adult population.
BACKGROUND/OBJECTIVES: The Dietary Reference Intakes for Koreans (KDRIs) were revised in 2020. Due to the rapidly aging Korean population, special consideration was given to reclassify the KDRI age group categories of older adults. This article examines the evidence for modifying the current KDRI age group ranges of older adults (65-74 and ≥ 75 yrs). SUBJECTS/METHODS: We first reviewed the domestic and international data on the elderly, following which we received expert opinions on age classification from the KDRI Advisory Committee. Finally, the 6th and 7th (2013-2017) Korea National Health and Nutrition Examination Survey (KNHANES) data were used to analyze the nutritional intake statuses by considering the age of older adults. RESULTS: According to the review results of domestic and international data and the inputs received from the expert advisory committee, the minimum age considered for the elderly was maintained at 65 yrs. However, the KNHANES data was analyzed to review whether there was a need to subdivide the later periods. Examining the differences in nutrient intakes by age group through the interaction effect term of the piecewise linear regression model revealed the interaction effect was maximum in the groups divided by 65 yrs (50-64 and 65-80), as compared to the groups divided by 70 yrs (50-69 and 70-80) and 75 yrs (50-74 and 75-80). The mean adequacy ratio was calculated per 1 yr of age, and a 3-yr (age) moving average analysis was performed to examine the change in the trends of overall nutrient intake. However, it was challenging to secure a scientific basis for subdivision into age groups in older adults from the results obtained. CONCLUSIONS: This study could not find any scientific evidence for modifying the KDRI age groups for older adults.
The Body Appreciation Scale-2 (BAS-2) measures the extent to which individuals feel, accept, and respond to their own bodies in a positive manner. Given the research need to explore positive body image and its associations with various sociocultural factors and related consequences among individuals with various cultural backgrounds, several studies have established the psychometric properties and factor structures of the BAS-2 in different languages and samples with different characteristics. The current study investigated the psychometric properties and measurement invariance of a Korean version of the BAS-2 in an older Korean adult population (599 older Korean adults with the average age of 70 years). Data were collected using both online and offline (paper-based) survey questionnaires. The results of exploratory factor analyses and confirmatory factor analysis evidenced the unidimensional factor structure and measurement invariance of the Korean BAS-2 among older Korean men and women, after dropping item 1. Scalar invariance was supported across gender, and men and women did not significantly differ in observed mean scores of the Korean BAS-2. The results also supported good convergent validity and criterion validity. Incremental validity was demonstrated by predicting self-esteem over and above measures of age, BMI, subjective financial and health status, body esteem, and ageism. High internal reliability and test-retest reliability over a 2-week period were confirmed. Overall, the results of this study support the reliable use of a Korean BAS-2 to measure positive body image among older Koreans after excluding item 1.
Objectives: This study was to examine the associations between obesity level and major chronic diseases in older population in Korea, using different obesity indicators. Methods: Data was from the 2008 KloSA Biomarker pilot, a nationwide sample of 514 non-institutionalized subjects (age $63.6{\pm}9.8$ years; women 57.2%). Anthropometric information was collected by home visiting nurses. Portable bioimpedence devices(Omran HBF359) were used for measuring fat mass. Obesity cut-offs used the Asian criteria of $BMI{\geq}25$, $WC{\geq}90/85cm$, and $fat%{\geq}25/35$ for men and women. Chronic disease prevalence was defined by physician-diagnosed history of 8 specified diseases. Results: Prevalence of chronic diseases significantly increased with increment of obesity level by BMI, WC and fat %. Odds ratios of acquiring chronic diseases remained significant in the obese group (BMI OR 2.76, 95%CI 1.82-4.19; WC OR 2.73, 95%CI 1.81-4.11; Fat OR 1.87 95%CI 1.26-2.78), after adjusting for age, sex, marital status, education, work participation, household income, smoking, drinking, and exercise. Conclusions: Obesity measured by all three indicators, BMI, WC, and fat mass cannot be disregarded, accentuating the significant effect on increases in disease risks among older population. BMI and WC measure appeared better to assess the risks of chronic diseases.

BACKGROUND/OBJECTIVES: Osteoporosis is characterized by low bone mass and results in vulnerability to fracture. Calcium and vitamin D are known to play an important role in bone health. Recently, potassium has been identified as another important factor in skeletal health. We examined the link between potassium intake and bone health among the Korean older adult population. SUBJECTS/METHODS: This retrospective, cross-sectional study included 8,732 men and postmenopausal women over 50 years old who completed the Korean National Health and Nutrition Survey (KNHANES) between 2008 and 2011. Potassium consumption was evaluated using a 24-hour recall method. Bone mineral density (BMD) was measured at three sites (total hip, femur neck, and lumbar spine) by dual-energy X-ray absorptiometry (DEXA). Multinomial logistic regression was used to examine the link between potassium intake and prevalence of osteoporosis and osteopenia, after controlling for potential confounding variables. RESULTS: The BMD of the total femur and Ward's triangle were significantly different according to the potassium intake among men (P = 0.031 and P = 0.010, respectively). Women in the top tertile for potassium intake showed higher BMD than those in the bottom tertile at all measurement sites (all P < 0.05). Daily potassium intake was significantly related to a decreased risk of osteoporosis at the lumbar spine in postmenopausal women (odds ratios: 0.68, 95% confidence interval: 0.48-0.96, P trend = 0.031). However, the dietary potassium level was not related to the risk of osteoporosis in men. CONCLUSION: Current findings indicate that higher dietary potassium levels have a favorable effect on bone health and preventing osteoporosis in older Korean women.
Background: The objective of this study was to elucidate the relationships musculoskeletal pains with combined vulnerability in terms of age, gender, and employment status Methods: The fifth European Working Conditions Survey (EWCS) in 2010 (43,816 participants aged 15 years and over) analyzed for European employees and the third Korean Working Conditions Survey (KWCS) in 2011 (50,032 participants aged 15 years and older) analyzed for Korean employees. In this study, three well known vulnerable factors to musculoskeletal pains (older age, female gender, and precarious employment status) were combined and defined as combined vulnerability. Associations of musculoskeletal pains with combined vulnerability were assessed with prevalence ratios (PRs) and 95% confidence intervals (CIs) estimated by Poisson regression models with robust estimates of variance. Results: The prevalences of musculoskeletal pains were lower but the absolute and relative differences between combined vulnerabilities were higher among Korean employees compared with the European employees. Furthermore, the increased risk of having musculoskeletal pains according to combined vulnerability was modestly explained by socioeconomic factors and exposure to ergonomic risk factors, especially in Republic of Korea. Conclusions: The results of this study showed that the labor market may be more unfavorable for female and elderly workers in Republic of Korea. Any prevention strategies to ward off musculoskeletal pains, therefore, should be found and implemented to mitigate or buffer against the most vulnerable work population, older, female, and precarious employment status, in Republic of Korea.
본 연구는 고령화 사회에 대한 사회구성원들의 인식을 살펴보고 그에 영향을 미치는 요인을 인구고령화와 관련된 가치체계와 세대차이의 맥락 속에서 고찰해 보는데 목적이 있다. 전국을 대상으로 할당표집을 통해 추출된 20세 이상 성인남녀 1,498명으로부터 수집한 자료를 분석하였다. 분석결과에 의하면 고령화 사회 인식의 8개 영역 중 응답자가 긍정적으로 생각하는 영역은 세 개 이하였다. 노화에 대한 지식, 부양인식, 노후불안, 세대 갈등으로 측정된 인구고령화와 관련된 변수들 중, 노후에 대한 불안이 모든 연령 집단에서 고령화 사회에 대한 부정적 인식에 영향을 미치는 것으로 드러났다. 65세 이상 연령 집단이 고령화 사회에 대한 인식이 20~44세, 45~64세 집단보다 긍정적인 것으로 나타났으며 이는 세대갈등 변수의 영향을 받는 것으로 분석되었다. 세대 갈등적인 인식은 20~44세와 45~64세 집단들에서 고령화 사회에 대한 보다 긍정적인 인식에 영향을 미쳤으나 65세 이상 집단에서는 두 요인 간에 관계가 없는 것으로 나타났다. 연구결과를 바탕으로 사회 구성원의 고령화 사회 인식의 전환의 방향과 후속연구의 필요성에 대해 논의하였다.
목 적 : Cystatin C는 최근 사구체 여과율의 정확한 지표로 제시되고 있다. 본 연구에서는 소아 및 청소년에서 연령에 따른 혈청 cystatin C 농도의 변화, 혈청 cystatin C 농도의 정상 범위, 혈청 cystatin C 농도와 사구체 여과율의 연관성을 분석하여 소아 및 청소년에서 사구체 여과율의 지표로서 혈청 cystatin C 농도의 유용성을 알아보고자 하였다. 방 법 : 2004년 5월부터 7월까지 중앙대학교 용산병원을 방문하여 혈액 검사를 시행받은 환아들 중에서 신질환의 증거가 없는 276명의 환아들을 대상으로 혈청 cystatin C와 creatinine을 측정하였고, 사구체 여과율은 Schwartz's formula로 추정하였다. 결 과 : 1) 혈청 cystatin C 농도는 1세 이상보다 1세 미만에서 유의하게 높았다. 2) 혈청 cystatin C 농도는 1세 미만에서 연령과 음의 상관관계를 보였으나, 1세 이상에서는 유의한 상관관계를 보이지 않았다. 3) 1세 이상에서 혈청 cystatin C 농도의 정상 범위는 0.46-1.05 mg/L였다. 4) 1/cystatin C은 Schwartz's formula로 추정된 사구체 여과율과 양의 상관관계를 보였다. 결 론 : Cystatin C는 소아 및 청소년에서 사구체 여과율의 지표로 유용하게 사용될 수 있을 것으로 생각된다.
