  • It is generally acknowledged that congenital coronary artery fistula is an abnormal communication of the coronary artery with the right ventricle, right atrium, left atrium and left ventricle. In young people the symptoms are unusual , but significant symptoms and complications appear among the older age group such as congestive heart failure, subacute bacterial endocarditis, coronary steal syndrome, aneurysm formation, rupture, and pulmonary hypertension. Therefore, early surgical treatment is recommended. We experienced a case of coronary arteriovenous fistula that was involving the circumflex branch of the left coronary artery with the right ventricle. It was 10mm in diameter with multiple vegetation. We repaired the fistula under extracoporeal circulation. The patient was discharged in a healthy condition twelve days after operation .

  • Clinical studies were performed on 119 cases who had HIVD and chronic back pain cured by chuna treatment from August 1984 to June 1997. The results were as follows 1. In 119 cases of HIVD and chronic back pain, 39 cases(32.8%) were male and 80 cases (67.2%) were female. 2. In 119 cases of HIVD and chronic back pain, 19 cases were under the age of 30 and 30 cases were under the age of 40 and 42 cases were under the age of 50 and 28 cases were older than 60 years old. 3. On Duration of Treatment in a Step, 29 cases were taken I -step treatment and 53 cases were taken II -step treatment and 31 cases were taken III -step treatment. 4. In 119 cases of HIVD and chronic back pain, recurrent rate was 12.6%.

  • Purpose: The purpose of this study was to identify influencing factors of using numbers and period of complementary and alternative therapy (CAT) among chronic patients. Methods: The study employed a descriptive design. The participants were 221 chronic patients. Data were collected in a pharmacy near a general hospital from April to July, 2008. Results: Influencing factors on using number of CAT were age, numbers of disease, period of used CAT and monthly expenses on CAT (the ability to explain, 79.9%). Influencing factors on using period of CAT were the period of illness and using number of CAT (the ability to explain, 44.9%). Conclusion: The results of this study suggest that chronic patients need counseling and guides about using CAT especially for those are older and have many kinds and longer period of diseases.

  • Cardiac resynchronization therapy (CRT) is a new treatment for refractory heart failure. However, most patients with heart failure treated with CRT are adults, middle-aged or older with idiopathic or ischemic dilated cardiomyopathy. We treated a 12-year-old boy, who was transferred after cardiac arrest, with dilated cardiomyopathy, left bundle-branch block, and ventricular tachycardia. We performed cardiac resynchronization therapy with a defibrillator (CRT-D). After CRT-D, left ventricular ejection fraction improved from 22% to 4 4% assessed by echocardiogram 1 year postoperatively. On electrocardiogram, QRS duration was shortened from 206 to 144 ms. The patient's clinical symptoms also improved. For pediatric patients with refractory heart failure and ventricular arrhythmia, CRT-D could be indicated as an effective therapeutic option.

  • Nurses play an important role in patient education, including providing patients with useful and appropriate information. The purpose of this study was to explore what particular types of information were important to women diagnosed with breast cancer. Sixty seven women with breast cancer an outpatient clinic in W Christian Hospital responded to the structured questionnaires developed by the investigator. Interviews and mail surveys were used in this study. The findings in this study showed that six kinds of information needs arose, ranked as the signs and symptoms of recurrence, the possibility of recurrence, the possibility of metastasis, treatment, diet and physical activities in daily living. Marital status, level of education, and level of monthly income were not related to level of informational need. Younger women rather than older women had a greater need to seek information(r=-.471, p<.01). There was a significantly negative relationship between duration of disease and the score on information needs(r=-.32, p<.05). The findings have implications that the opportunity to ask questions and have accurate information on the recurrence should be provided to women with breast cancer.

  • Superior hypogastric plexus block is extensively recognized as a unique nerve block method for the treatment of low abdominal pain originating from organs of the pelvic area. This block is difficult to perform on older patients, especially those with osteophyte, as they will experience a high degree of pain with this technique. Therefore we reported trans-discal superior hypogastric plexus by method of approach needle through disc. This method is less painful to the patient as compared to bilateral approach; and easier to place the needle tip at precise and proper location.

  • Postherpetic neuralgia, the most feared complication of acute herpes zoster, may agonize the pain clinician because the appropriate management of intractable pain can fail inspite of various and prolonged therapeutic techniques. Of all patients with herpes zoster, approximately 5~10% will develop postherpetic neuralgia. Postherpetic neuralgia is very rare in young patient but very common in patients over 60. In other words, the older, the higher incidence. In our pain clinics, 13 postherpetic neuralgic patients were treated with sympathetic blocks, local infiltration with local anesthetics and steroids, TENS, Laser and various drugs including antiderpressant. The results of management of pain in 13 patents were as follows. 1. Excellent pain control :4(30.8%) 2. Good pain control :3(23.0%) 3. Fair pain control :2(15.4%) 4. No effect :4(30.8%).

  • Eccrine porocarcinoma is a rare malignant tumor arising from the intraepidermal ductal portion of the eccrine sweat gland. It develops either spontaneously or from a long standing benign eccrine poroma. This entity usually affects older people and is commonly located on the lower extremities, the trunk, and the head. We report a case of eccrine porocarcinoma on the left cheek in an 85-year-old male. In our case, the tumor was treated with wide excision and postoperative adjuvant radiation therapy. The patient recovered well without local recurrence and distant metastasis during the 14-month follow-up period. Wide excision and postoperative adjuvant radiation therapy can be considered as a safe and effective treatment option in treating patients with eccrine porocarcinoma.

  • Familial Creutzfeldt-Jakob Disease (fCJD) is characteristic with older age onset, relatively low occurrence rate, slower progression and lower possibility of developing myoclonus, cerebellar, pyramidal signs and visual disturbance compared with classical sporadic CJD. We report a case of 75-year-old male patient presented with sudden onset of right side weakness with Broca's aphasia who has been diagnosed with fCJD with V180I mutation. This case indicates that fCJD with V180I mutation can have stroke-like initial presentation.

  • Rotator cuff tear is the most common cause of shoulder pain in middle-age and older people. Arthroscopic rotator cuff repair (ARCR) is the most common treatment method for rotator cuff tear. Early postoperative pain after ARCR is the primary concern for surgeons and patients and can affect postoperative rehabilitation, satisfaction, recovery, and hospital day. There are numerous methods for controlling postoperative pain including patient-controlled analgesia, opioid, interscalene block, and local anesthesia. Regional blocks including interscalene nerve block, suprascapular nerve block, and axillary nerve block have been successfully and commonly used. There is no difference between interscalene brachial plexus block (ISB) and suprascapular nerve block (SSNB) in pain control and opioid consumption. However, SSNB has fewer complications and can be more easily applied than ISB. Combination of axillary nerve block with SSNB has a stronger analgesic effect than SSNB alone. These regional blocks can be helpful for postoperative pain control within 48 hours after ARCR surgery.
