본 연구의 목적은 대한민국 20세 이상(50.32±16.14세)의 당뇨질환이 없는 성인에서 만성신장질환과 인슐린저항성(homeostasis model assessment of insulin resistance, HOMA-IR) 및 베타세포기능(homeostasis model assessment of beta cell function, HOMA-B)의 관련성에 대하여 조사하였다. 본 연구는 2015년도 국민건강영양조사 자료를 이용하여 당뇨질환이 없는 20세 이상의 대한민국 성인 4,380명을 대상으로 하였다. 본 연구결과에서 중요한 결과는 다음과 같다. 첫째, 만성신장질환과 HOMA-IR와 관련하여, 관련변수를 보정한 후의 결과에서(Model 4), 그룹 1 (G1; estimated glomerular filtrationrate [eGFR], ≥90 mL/min/1.73 ㎡), 그룹 2 (G2; eGFR, 60~89 mL/min/1.73 ㎡), 그룹 3a (G3a; eGFR, 30~59 mL/min/1.73 ㎡), ≥그룹 3b (≥G3b; eGFR, <30 mL/min/1.73 ㎡)의 HOMA-IR 평균값(M±SE, 95% confidence interval [CI])은 각각 1.78±0.03 (1.73~1.83), 1.87±0.03 (1.81~1.93), 2.16±0.13 (1.91~2.42) 및 2.59±0.24 (2.12~3.06)으로 만성신장질환이 진행됨에 따라 HOMA-IR은 증가하였다(P<0.001). 둘째, 만성신장질환과 HOMA-B와 관련하여, 관련변수를 보정한 후의 결과에서(Model 4), G1, G2, G3a 및 ≥G3b의 HOMA-B 평균값(M±SE, 95% CI)은 각각 87.46±1.21 (85.08~89.84), 89.11±1.38(86.40~91.81), 104.82±5.91 (93.23~116.42) 및 123.97±10.87 (102.66~145.29)으로 만성신장질환이 진행됨에 따라 HOMA-B도 증가하였다(P<0.001). 대한민국 당뇨질환이 없는 성인에서 만성신장질환이 진행됨에 따라 인슐린 저항성이 증가하였고, 베타세포기능 또한 증가하였다.

Objective : This study was carried out to develop a semi-quantitative food frequency Questionnaire (SQFFQ) for estimating average dietary intake to determine the risk factor for lifestyle-related diseases in a conjoint cohort study. Design : We developed an SQFFQ for genomic epidemiological studies based on the data in the'98 Korea Health and Nutrition Examination Survey. A subset of data on informative food items was collected using the 24-hr recall method with 2,714 adults aged 40 or older living in middle-sized cities or in rural areas in Korea. The cumulative percent contribution and cumulative multiple regression coefficients of 17 nutrients (energy, fat, carbohydrate, protein, fiber, iron, potassium, sodium, calcium, phosphorus, vitamin A, retinol, $\beta$-carotene, vitamin $B_1$, vitamin $B_2$, niacin and vitamin C) of each food were computed. Results : Two hundred and forty-nine foods, which were selected based on their 0.9 cumulative percent contribution, and 254 foods, which were selected based on their 0.9 cumulative multiple regression coefficients, respectively, were grouped into 97 food groups according to their nutrient contents. Several popular Korean foods, which were missing from the list due to the seasonality of the survey, were included. The portion sizes were derived from the same data set. The SQFFQ covered 84.8 percent of the intake of 17 nutrients in the one day diet record data of our 326 cohort study subjects. Conclusions . The final list included 103 food items. The foods list in the SQFFQ described herein accounted for 84.8 percent of the average intake of 17 nutrients. Therefore, the list could be used for the assessment of the baseline dietary intakes of the conjoint cohort studies.
Due to the rapid population's aging, the life span's expansion and social & cultural characteristics, the standards of age classes in senescence are changed. Thus, this study aimed to identify the influential factors on health care utilization in the elderly by age class, and targeted 2,937 adults in their ages over 55 years old from the Korea Health Panel's 2009 Yearly Integrated Data. This study investigated the target health care utilization in the elderly in relation with the gender, the education level, the health-care insurance type, the income, the current job state, the chronic disease. In order to analyze the influential factors on health care utilization in the elderly, the multiple linear regression analysis was conducted to the data. As the results from this study, Concerning the influential factors on the health care utilization, the income, the subjective health state, the chronic disease and the regularly meals for the young-old influenced. Concerning the influential factors on the health care utilization, the subjective health state for the old. Concerning the influential factors on the health care utilization, the subjective health state, the income for the older-old influenced. Concerning the influential factors on the health care utilization, the education level, the spouse, the economic activity and the drinking oldest-old influenced. Therefore, it will need to provide systemic health-care & medical services, to develop health-care & medical programs and the health-care & medical policies and to execute them according to the age classes in senescence.
Objectives : The objective of this study was to analyze cognition and status of oral health education of dental hygienists who manage the patients visiting dental clinics and to utilize the findings as continuous study data required for development of systematic oral health education program for management of peridontal disease of adults. Methods : Survey was carried out for 261 dental hygienists working in dental clinics who are performing oral health education of patients with peridontal disease living. Results : 1. In the socio-dynamic diagnosis, it was analyzed that, the younger the age is and the lower the career is, the more the work is centered around assisting medical treatment job and, the older the age is and the higher the career is, the work is centered around oral health education job. 2. In the behavioral diagnosis, it was analyzed that, as to source of knowledge acquisition, guide of dentists and senior dental hygienists was the highest and, as to education media, visual education and demonstration education were mainly used. 3. In educational diagnosis, as to the degree of knowledge about oral health education and the degree of practice, it showed that the degree of knowledge was lower than the degree of practice in peridontal disease process and oral prophylaxis as well as scaling part. 4. In both the cases of independent practice and group practice, it showed that the most frequent obstacle was insufficient educational materials and tools and it was analyzed that acquisition of new knowledge and technology is required as a matter to be improved. Conclusions : It seems to be required for the dental hygienists to develop detailed knowledge and professional technology in order to induce actions and motive of patients during oral health education of peridontal patients and to develop and produce data for efficient education of peridontal patients.
Objective : The purpose of this study was to investigate the effects of yoga program on depression, quality of life, and difficulty of daily activities in community elders. Methods : A Nonequivalent control group pretest-posttest design was used for this study. Elderly people who agreed to participated in the study were assigned to a experimental group(n=17) or a control group(n=17). The yoga program conducted 90minutes, 2times a week for 1 year. Study outcomes were measured by structured questionnaires from March, 2012 to Feb., 2013. For data analysis, Chi-square test, t-test, and Mann-Whitney U test were performed using SPSS version 17.0. Results : Depression was not significant difference between the two groups in pretest and posttest. But there was significant differences between the two groups in quality of life (t=3.984, p<.001), and difficulty of daily activities (t=-3.083, p=.004). Therefore, the experimental group showed significant improvement in quality of life and reduce difficulty of daily activities. Conclusion : The yoga program was effective in increasing quality of life and decreasing difficulty of daily activities in community elders. When developing exercise program to improve health in older adults, it is important to consider physical exercise plus general health management.
BACKGROUND/OBJECTIVES: This study aimed to improve portion size estimation aids (PSEAs) used in the nutrition survey of the Korea National Health and Nutrition Examination Survey (KNHANES) and validate the accuracy and precision of the newly developed aids. SUBJECTS/METHODS: We conducted intensive interviews with survey experts in KNHANES and consulted with experts to collect opinions about improvement of PSEAs. Based on the results of the interviews, 5 types of PSEAs (rice bowl, earthen pots, mounds, measuring spoons, and thickness sticks) were newly developed using 3-dimensional (3D) modeling or modification of color or shape. Validation tests were conducted with 96 adults 20 years old or older. For the rice bowl and earthen pots, the participants were asked to select the more similar PSEA in size after being shown the real dishes. For the mounds, measuring spoons, and thickness sticks, the participants were presented with actual plates of food and asked to estimate the given portion sizes using the given PSEAs. RESULTS: The improved 2-dimensional (2D) picture aid for the rice bowl reflecting the size distortion by angle of view using 3D modeling was perceived more closely to the actual size than the current 2D picture (P < 0.001). The change of the color of 2D pictures and 3D models, the change of shape of the measuring spoons, and the 3-dimensionalization of the 2D mounds had no significant improvement in the subjects' perception. CONCLUSIONS: The currently used 2D PSEAs need to be fully redesigned using 3D modeling to improve subjects' perception. However, change of color or shape will not be necessary. For amorphous foods, it is suggested that more evaluation be performed before reaching a final conclusion in the use of PSEAs, or alternative ways to improve accuracy of estimation need to be explored.
Purpose: The purpose of this study was to compare and analyze differences in effects of postoperative daily activity on headaches in patients who underwent surgery under spinal anesthesia. Methods: The study was conducted with 219 adults, 20 years or older. Official approval (Approval number: KMC IRB 1434-01) was received from K university hospital clinical trials review board. The research design was a nonequivalent control group non-synchronized design with a daily activities group and the bed rest group. Data was collected after receiving written consent from the participants. Results: There were no participants in either group who experienced headaches. Changes in a physiological index were also not significantly different between the daily activities group and the bed rest group. Conclusion: The results indicate that allowing daily activities in the ward, rather than maintaining bed rest for 6 hours, the existing method of nursing care for the prevention of postoperative headaches, in spinal anesthesia patients, is not detrimental to these patients post operatively.
Purposes: The purpose of this study, was to identify similar factors between reasons for unscreening and unmet health care needs through prior research, and based on this, we wanted to figure out the relevance between the medical screening and unmet health care needs. Methodology: The analysis was conducted using data from 9,640 adults aged 19 or older who don't have a missing value from 16,277 participants in the 7th, 1st&2nd Year (2016&2017) of the National Health and Nutrition Examination Survey (KNHANES). Unmet health care needs were investigated as a self-reported questionnaire of whether medical service was required but not received. And the analysis was performed through the Chi-Square Test and Multi-logistic Regression analysis. Findings: As a result of the analysis, unmet healthcare needs were higher who received only one type of screening comparative to who screened both. and were highest who screened neither. Practical Implications: Unmet health care needs are the center of a vicious cycle, such as morbidity and mortality, which is detrimental to the quality of life, and continues to increase. Therefore, it is necessary to find ways of realizing health care that guarantees the health rights of all citizens by policy guarantee and support for the subjects to recognize the importance of thorough education of screenings rather than only health screening or cancer screening.
Objectives: The purpose of this study is to identify the association between oral health status and pulmonary ventilatory defects. Methods: The 6th (2013-2015) National Health and Nutrition Examination Survey data was used. The study subjects were those aged 40-79 who had pulmonary function examination. Complex samples general linear model analysis, Complex samples cross-tabulation analysis, and Complex samples logistic regression analysis were conducted. Results: In terms of restrictive ventilatory defects, the study subjects (8.3%) who recognized that their oral health status was bad outnumbered those who recognized that their oral health status was good (6.1%) (p<0.05). Many of the study subjects who experienced difficulty in biting, chewing and/or speech, and who had an unhealthy periodontal had restrictive ventilatory defects and obstructive ventilatory defects (p<0.05). The association between oral health status and pulmonary ventilatory defects was analyzed. The findings showed that those who had unhealthy periodontal had a 1.33 times higher probability of pulmonary ventilatory defects than those who had a healthy periodontal (p<0.05). After taking into account general characteristics (age, sex, incomes, education, and smoking) of the subjects, the association between oral health status and pulmonary ventilatory defects was analyzed. The result found that only in cases where one experienced dental caries was an association with pulmonary ventilatory defects found. In other words, those who had dental caries showed a 0.73 times higher probability of pulmonary ventilatory defects than those who had no dental caries (p<0.05). Conclusions: Based on the findings of this research, oral health status was found to be associated with pulmonary ventilatory defects. To improve oral health, it is necessary to provide life-cycle stages based oral health education. Therefore, it is required to develop an oral health education program and develop a national oral health policy.
People have experienced physical problems including vision problems with aging; some are normal and some are abnormal. When they have abnormal and impaired vision, they have a lower level of life satisfaction than their sighted peers. For the elderly with sight loss, well-designed housing could be an important indicator for their quality of life. Housing adjustment and modification could help the elderly cope with sight loss, do daily activities more independently, and lead to better quality of life. The purpose of this study is to investigate housing conditions for the elderly with vision impaired, state of housing adjustment or modifications, and the effects of housing conditions on the wellness of the elderly in Korea. For this study, 18 housing cases were investigated and a total number of 65 visually impaired people aged over 55 participated. Housing condition of those participated was poor. Most observed home hazards are level differences in living areas, obstacles in the hallway, confined space that is not allowed structural modification, swing doors from wardrobe or cabinets, color contrast, poor lightings, and etc. Majority of households participated in this study did not make modification or refurbishments, rather many of them tend to adapt themselves to their residential environments. Older adults living in better housing condition and under less difficult environments are likely to have higher life satisfaction. The effects of housing condition on wellness could be more influential when individual capabilities (e.g. health condition) are weaker. The findings underline that with intervention of individual characteristics, the effects of housing and physical environment on negative outcomes of vision loss would get ameliorated or reduced.
