재가노인의 수면장애 유무에 따른 우울, 신체형장애 및 삶의 질 비교 (Depression, Somatoform Disorders, and Quality of Life between Poor Sleepers and Good Sleepers in Community-Dwelling Older Adults)

  • Purpose: The purpose of this study was to compare depression, somatoform disorders, and quality of life among older adults. Methods: Samples of 280 community-dwelling Korean older adults were included. The age range of the participants was 60 to 90 years (average $72.6{\pm}6.4$). The participants were assigned to one of two groups based on reported scores on the Pittsburgh Sleep Quality Index from September 2006 to March 2007. The two groups were designated as "poor sleepers"and "good sleepers". A T-test was used to compare depression, somatoform disorders and quality of life between the two groups. Results: Forty-six percent of participants reported scores that indicated they were poor sleepers. Poor sleepers reported significantly higher depression scores (p<.001), higher somatoform disorders scores (p<.001), lower for each SF-36 quality of life dimension, and lower mental and physical health summary scores (p<.001) than the reported scores of those participants who were classified as good sleepers. Conclusion: Older adults with poor sleep patterns are more likely to report higher depression, more somatoform disorders and a lower quality of life. Additional research is needed to identify the appropriate nursing interventions aimed at improving sleep quality, depression symptoms, somatoform disorders and the quality of life.

Predictors of Participation in Prostate Cancer Screening among Older Men in Jordan

  • Background: Participation is one of the major factors affecting the long-term success of population-based prostate cancer screening programs. The aim of this study was to explore strong factors linked to participation in prostate cancer screening among older Jordanian adults using the Health Belief Model (HBM). Materials and Methods: Data were obtained from Jordanian older adults, aged 40 years and over, who visited a comprehensive health care center within the Ministry of Health. A pilot test was conducted to investigate the internal consistency of the the Champion Health Belief Model Scale for prostate cancer screening and the clarity of survey questions. Sample characteristics and rates of participation in prostate cancer screening were examined using means and frequencies. Important factors associated with participation in prostate cancer screening were examined using bivariate correlation and multivariate logistic regression analysis. Results: About 13% of the respondents had adhered to prostate cancer screening guidelines over the previous decade. Four out of the seven HBM-driven factors (perceived susceptibility, benefits and barriers to PSA test, and health motivation) were statistically significant. Those with greater levels of susceptibility, benefits of PSA test and health motivation and lower levels of barriers to PSA testing were more likely to participate in prostate cancer screening. Family history, presence of urinary symptoms, age, and knowledge about prostate cancer significantly predicted the participation in prostate cancer screening. Conclusions: Health professionals should focus more on the four modifiable HBMrelated factors to encourage older adults to participate in prostate cancer screening. Intervention programs, which lower perceived barriers to PSA testing and increase susceptibility, benefits of PSA testing and health motivation, should be developed and implemented.

노인의 식후 저혈압과 심박동변이 (Postprandial Hypotension and Heart Rate Variability in Older Adults)

  • Purpose: A study was designed to compare heart rate variability (HVR) of older adults with postprandial hypotension (PPH) with older adults with no PPH. Methods: The samples were community-dwelling older adults (${\geq}$ 60 years old) with PPH (n=35) and without PPH (n=35). HRV was measured by 10 minutes of electrocardiography using Physiolab 400 and analyzed in time domain (SDNN) and with power spectral analysis in frequency domain (TP, LF, HF, LF/HF). Blood pressure was measured every 15 minutes before and until 120 minutes after the meal. Results: All HRV parameters were lower in the PPH group compared to those with no PPH. Specifically the measures of SDNN, TP, LF, and LF/HF of the PPH group were significantly lower (U=385.0, $p$=.008; U=361.0, $p$=.003; U=375.0, $p$=.005; U=368.0, $p$=.004, respectively). Further, the LF and LF/HF of SBP (${\geq}140$ mmHg) group and the LF/HF of DBP (${\geq}$ 90 mmHg) group were significantly lower ($x^2$=6.33, $p$=.042; $x^2$=33.10, $p$ <.001; $x^2$=11.03, $p$=.004, respectively). Conclusion: These findings indicate that the autonomic nervous functions in persons with PPH are less activated than those persons with no PPH. The findings contribute to an understanding of HRV and the association of autonomic nervous function with the mechanism of PPH.

컴퓨터 인지강화 프로그램이 경도인지장애 노인의 인지기능, 우울, 자아존중감 및 일상생활 수행능력에 미치는 효과 (Effects of a Computerized Cognitive Training on Cognitive Function, Depression, Self-esteem, and Activities of Daily Living among Older Adults with Mild Cognitive Impairment)

  • Purpose: The aim of this study was to evaluate the effects of a computerized cognitive training on older adults affected by mild cognitive impairment (MCI) in terms of cognitive function, depression, self-esteem, and activities of daily living (ADL). Methods: This study used a non-equivalent control group non-synchronized design. A total of 53 older adults who reside in long-term care facilities were recruited, 26 subjects for an experimental group and 27 subjects for a control group. A computerized cognitive training was performed for 20~40 minutes/day, three days/week for ten weeks. Data were analyzed using SPSS/WIN 21.0 with $x^2$ test, Fisher's exact test, and t-test. Results: Scores of MMSE (t=3.30, p=.002), depression (t=-2.15, p=.036), and self-esteem (t=2.76, p=.008) were significantly better in the experimental group than the control group. However, the difference in ADL (t=-1.01, p=.316) was not significant between the two groups. Conclusion: These findings suggest that the computerized cognitive training can be used as an effective nursing intervention to improve cognitive function and self-esteem and lower depression among older adults with MCI.

농촌 노인의 세대간 사회적 지원 교환과 생활만족: 성별 및 연령집단별 비교 (Intergenerational Social Support Exchanges and Life Satisfaction Among the Rural Elderly: Sex and Age Group Differences)

  • This study focused on individual differences in social support among older adults. The purposes of this study were to investigate sex and age group differences in social support and to examine the effects of intergenerational social support on life satisfaction among the rural elderly. Data were from 545 elderly over 60 years of age living separately from adult children in the rural area. With regard to sex differences in support exchanges, no significant differences were found in support-giving and support-receiving. Men reported giving more financial support to children than women, while women reported receiving more financial support from children than men. With regard to age group differences in support exchanges, there was less support-giving in older age group. Older parents in their 60s reported giving more financial, instrumental, and emotional support and receiving less financial support than the group of age 70+ Regression analyses showed that life satisfaction of both men and women was affected by support size and the frequency of contact with children. Giving financial and instrumental support was significantly associated with life satisfaction of men, but giving and receiving each type of social support had no effects on life satisfaction of women. Life satisfaction of parents in their 60s was found to be positively associated with support size, giving financial support and receiving emotional support, and negatively associated with giving instrumental support. In the group of age 70+, the frequency of contact with adult children and giving financial support had positive influences on life satisfaction.

성별과 연령에 따른 골밀도와 체질량지수 변화에 대한 연구 (The Study on the Changes of Bone Mineral Density and Body Mass Index According to Sex and Age)

  • Objectives : Osteoporosis is the most common metabolic disease of the bone, and one of the most important public health problems around the world. Obesity rate gradually increases as women grow older. The purpose of the study was to investigate the changes between bond mineral density(BMD) and body mass index(BMI) according to sex and age. Methods : We measured the bone mineral density(BMD) of lumber spine(L2-L4) and femoral neck in 10000 normal adult, using dual energy X-ray absorptiometry (DEXA; DPX-alpha). and also measured the body mass index(BMI) in 9810 normal adult. then we analysed the changes of BMD and BMI according to sex and age. Results : 1. As getting older, male's T-score of lumbar spine(L2-L4) was slowly decreased. On the other hand, female's T-score of lumbar spine was sharply dropped after 46 years old. 2. As getting older, male's T-score of femoral was slowly decreased. On the other hand, female's T-score of femoral was sharply dropped after 49 years old. 3. Irrespective of age, male's BMI keep up the extent within a definite period of numerical value. As getting older, female's BMI continued its upward trend. Conclusions : Much more attention should be needed for female patient suffering from osteoporosis and obesity than male.

Complementary and Alternative Medicine Among Older Adults in the United States: Current Evidence and Future Directions

  • Complementary and alternative medicine has gained popularity and respectability in recent years in the United States. Since aging is often associated with chronic health conditions that commonly lead to physical and psychosocial disabilities (e.g., depression, functional and/or cognitive disabilities, and decreased quality of life), older adults often seek options to maintain health and treat chronic conditions as an adjunct to conventional medical care. Herbal products, the most commonly used among various complementary and alternative medicines (CAM), should be used with caution due to potential herbal-drug interactions (related to polypharmacy) and herbal-disease interactions (related to comorbidities). Five of the most common chronic conditions in older adults are chronic pain, cardiovascular problems, hypertension, diabetes, and chronic lung problems. A high rate of falls or risk of falling is also a problem unique to this older population. For these conditions, only a few types of CAM (e.g., acupuncture, qi gong, tai chi) were tested, with promising results. However, in spite of evidence supporting the use of certain types of CAM to alleviate some common chronic conditions, findings are limited in terms of other types of CAM tested and both short and long-term effects. More rigorous clinical trials of various CAM types are thus warranted to advance scientific knowledge and establish evidence-based practices to care for the growing number of older adults who deserve to have a better quality of life.

돌봄제공자가 인지한 치매노인의 문제행동 연구 (Problematic Behaviors recognized by Caregiver in Demented Older Adults)

  • Behavioral symptoms are frequent and problematic components of dementia. The aim of this study was to detect behavioral problem of the 25 item of dementia problem behaviour(DPB)assesment scale for demented older adults. Seventy-three patients with dementia staying in the day care center, group home, dementia hospital, nursing home were recruited data on problematic behavior obtained through interviews with their caregiver during 2 months from March to May 2004. Results shows that problematic behaviors common occurring in 95% of subjects. The most frequently exhibited problematic behaviors were repetitive movement(1.05), repetitive questions(1.03), restlessness(1.0). indifference(0.97). shouting/screaming(0.92). Problematic Behaviors of the demented older adults were closely associated to the distress for caregiver. Most distressful ones for caregivers were repetitive questions(1.46), repetitive movement(1.42), shouting/screaming (1.42), indifference(1.41). restlessness(1.41). Problematic Behaviors of the demented older adults were divided into six subdivisions, among those restless behavior was positively correlated with aggressive behavior, nervous symptom and psychotic symptom. and then nervous symptom was interrelated to psychotic symptom(P<0.01). This study has limitation that field study data were derived from various primary caregivers.

치과위생사의 치매노인 구강건강관리 실태 및 인식에 관한 질적 연구 - 포커스 그룹 인터뷰 적용 - (A qualitative study on the present working conditions of dental hygienists and the oral health awareness of older adults with dementia - Focus group interviews -)

  • Objectives: This study investigated the working conditions of dental hygienists and their challenges in providing oral care to older adults with dementia using focus group interviews. Methods: We collected data for approximately a month beginning from August 2020 and divided the study subjects into two groups: the health dental hygienist and the clinical dental hygienist groups. A total of 11 subjects participated in this study. Results: The oral health management of older adult patients with dementia has not been efficiently carried out in local communities or dental medical institutions. In addition, dental hygienists encounter difficulties in managing the oral health of these patients and hope to actively learn more about their special cases. Conclusions: Based on the results of this study, it is necessary to develop a manual or program for the professional implementation of oral health interventions for older adults with dementia.

  • Purpose: This study aimed to investigate the influence of social capital on the depression of older adults living in rural areas. Methods: Data sets were obtained from the 2019 Korea Community Health Survey. The participants were 39,390 older adults over 65 years old living in rural areas. Indicators of social capital included trust, reciprocity, network, and social participation. Depression-the dependent variable-was measured using the Patient Health Questionnaire-9 (PHQ-9). Hierarchical ordinal logistic regression was conducted to identify factors associated with depression after adjusting the data numbers to 102,601 by applying the Synthetic Minority Oversampling Technique (SMOTE). Results: The independent variables-indicators of social capital-exhibited significant association with the depression of older adults. The odds ratios of depression were higher in groups without social capital variables. Conclusion: To reduce depression, we recommend increasing social capital. Factors identified in this study need to be considered in older adult depression intervention programs and policies.
