First Results of the Single Heartstring Aortotomy for Multiple Off-Pump Vein Grafts: A Case Series

  • To validate the technique of the single Heartstring aortotomy for multiple off-pump venous bypass grafts (described in 2015), the results of a 38-month follow-up study of 18 patients, including high-risk patients, are presented. No early deaths or cardiac or cerebral complications occurred. During the follow-up period, 2 patients died of non-cardiac causes, and 3 developed coronary ischemia. Ischemia occurred due to late graft occlusion in 2 patients, both of whom had normal postoperative courses and correct graft flow. The presence of acute symptoms 24 months after surgery in these patients indicated that technical graft failure was unlikely. This safe technique combines the advantages of simple and reproducible revascularization, the off-pump approach, and minimal aortic manipulation.

  • Ascending aortic aneurysms are usually treated with graft replacement under cardiopulmonary bypass. However, if a candidate for off-pump coronary artery bypass grafting has an enlarged ascending aorta, surgeons may consider wrapping it without cardiopulmonary bypass. Here, we report a 78-year-old female who underwent successful wrapping of the ascending aorta concomitant with off-pump coronary artery bypass grafting, using a new wrapping technique that involves multiple bootstraps.

  • A numerical study was conducted to predict the performance curve of a canned motor pump for SMART(System Integrated Modular Advanced ReacTor). The study used a computational domain which included not only the pump but also a suction pipe and a volute casing with a discharging pipe in order to simulate an experimental setup. The ANSYS CFX program was utilized to obtain flow characteristics inside the pump as well as the overall pressure rise across the pump operating on- and off-design points. Computed results showed that the performance of the pump at off-design points was much lower than expected. Special attention was made to find the cause of the low performance of the pump operating at low flow rate.

  • The objective of present work is to use numerical simulation to investigate the complex three-dimensional and secondary flow structures developed at the inlet and impeller in a centrifugal pump at design and off-design points. The pump impeller is shrouded with 6 backward swept blades and with a specific speed of 0.8574. The characteristic of the pump is measured experimentally with straight and curved intake sections. Numerical computation is carried out to investigate the pump inlet flow structures and subsequently the flow field within the centrifugal pump. The numerical results showed that strong interaction between the impeller eye and intake section. Secondary flow structure occurs upstream at the pump inlet has great influence on the pump performance and flow structure within the impeller.

  • The flow characteristics of double suction pump are investigated by numerically Calculations are performed by using SIMPLE algorithm at the design and off-design points. Symmetric nature of flow fields in blade channels is discovered at design point, but asymmetirc effects are discovered at the off-design point. Numerical results show that the formation of secondary flow in volute of double suction pump shows different trends when compared with the case of single suction pump. Also results show that double vortices are formed in the volute cross section.

The In-hospital Analysis of Outcome of Off-pump CABG and On-pump CABG (비체외순환 및 체외순환하에서의 관상동맥우회로술의 임상 고찰)

  • With the developement of non-invasive surgical techeniques, coronary artery bypass graft without cardiopulmonary bypass has become popular. We compared the preoperative risk factors and in-hospital outcomes of patients having off-pump CABG with these having on-pump CABG. Material and Method: From January 2001 to June 2003, 87 patients underwent CABG. Thirty-sin patients underwent on-pump CABG, fifty-one patients under-went off-pump CABG. Preoperative risk factors, extent of coronary disease, operative time, postoperative endotracheal intubation time, duration of ICU stay & hospital stay, the amount of bleeding and postoperative levels of cardiac enzymes were compared in both groups, Result: The were no differences in their sex ratios, ages, preoperative risk factors, preoperative Ml, Canadian classes, extent of coronary artery diseases and, echocardio-graphic ejection fraction between Off-pump CABG and On-pump CABG groups. Off-pump CABG group had significantly lower mean operative time (270$\pm$79.3 min vs 372$\pm$142.2 min, p<0.001), mean ventilation time (17.1$\pm$13.4 hr vs 24.3$\pm$17.8 hr) and CK-MB level (8,9$\pm$18.7 IU/L vs 25.7$\pm$8.4 IU/L) than on-pump CABG groups, On-pump CABG group had more distal grafts (2.2$\pm$0.5 vs 1.7$\pm$0.71 than Off-pump CABG groups did. There were no differences in their postoperative complications and outcomes including amount of postoperative bleeding for 24 hrs, reoperation for bleeding control, mean in-hospital days, postoperative infection, renal failure and neurologic complications between Off-pump CABG and On-pump CABG groups. Conclusion: This study showed that patients who underwent Off-pump CABG had less operation time & intubation time and lower CK-MB level; however, they also have less distal graft. Even though CABG without CPB provided satisfactory results, more clinical experience & longer follow-up is required.

  • A multi-type heat pump controls the mass flow rate of the working fluid to cope with variable heat loads when it is under dynamic load condition. This paper describes the exergy analysis associated with the dynamic response of heat pump. First, a basic heat pump cycle is examined at steady state to show the general trends of exergy changes in each process of the cycle. Entropy generation issue in the exchangers is discussed to optimize the heat pump cycle. Second, the performance of the inverter-driven heat pump is compared to that of the conventional one when the heat load is variable. Third, the exergy destruction rate associated with the ON/OFF operations of the heat pump is calculated by simulating the thermodynamic states of the condenser and the evaporator. The inefficiency of the ON/OFF operation during the transient period is quantitatively revealed by the exergy analysis.

Intra-aortic Balloon Pump Therapy for Hemodynamic Instability during Off-pump Coronary Artery Bypass Surgery (심폐바이패스를 사용하지 않는 관상동맥우회술 중 발생한 혈역학적 불안정에 대한 대동맥내 풍선펌프 치료)

  • Background: We assessed the efficacy of intraoperative intra-aortic balloon pump therapy for achieving hemodynamic instability during off-pump coronary artery bypass surgery. Material and Method: We studied seven hundred ninety-six patients who underwent off-pump coronary artery bypass between January 2000 and December 2006. The patient were divided into group I (n=39), which received intraoperative intra-aortic balloon pump therapy, and group II (n=757), which did not receive intraoperative intra-aortic balloon pump therapy. Result: There were no differences in the operative mortalities (2.6%, 1/39 vs 0.8%, 6/757; p=0.195) and morbidities such as atrial fibrillation (p=0.691), stroke (p=0.908) and mediastinitis (p=0.781) between the 2 groups, although the ventilator support time, the length of the intensive care unit stay and the length of the hospital stay were longer in group I than in group II (p<0.05). Multivariate analysis failed to prove that group I was a high risk group for operative mortality (p=0.549). There were 3 intraoperative intra-aortic balloon pump-related complications in group I (7.9%). However, no longer complications occurred after 2003, when the surgeons began using a smaller sized 8 F catheter that was inserted using a sheathless technique. Conclusion: Intraoperative intra-aortic balloon pump therapy for achieving hemodynamic instability during off-pump coronary artery bypass surgery can be performed safely and it showed comparable clinical results to that of not using intraoperative intra-aortic balloon pump therapy.

Risk Factors of On-Pump Conversion during Off-Pump Coronary Artery Bypass Graft

  • Background: Off-pump coronary artery bypass grafting (OPCABG) procedures can avoid the complications of an on-pump bypass. However, some cases unexpectedly require conversion to cardiopulmonary bypass during OPCABG. The risk factors associated with a sudden need for cardiopulmonary bypass were analyzed. Methods: This retrospective study included 283 subjects scheduled for OPCABG from 2001 to 2010. These were divided into an OPCABG group and an on-pump conversion group. Preoperative, operative, and postoperative variables were compared between the 2 groups. Results: Of the 283 patients scheduled for OPCABG, 47 (16%) were switched to on-pump coronary artery bypass grafting (CABG). The mortality of the both the OPCABG and on-pump conversion groups was not significantly different. The major risk factors for conversion to on-pump CABG were congestive heart failure (CHF) (odds ratio [OR], 3.5; p=0.029), ejection fraction (EF) <35% (OR, 4.4; p=0.012), and preoperative beta-blocker (BB) administration (OR, 0.3; p=0.007). The use of intraoperative (p=0.007) and postoperative (p=0.021) inotropics was significantly higher in the conversion group. The amount of postoperative drainage (p<0.001) and transfusion (p<0.001) also was significantly higher in the conversion group. There were no significant differences in stroke or cardiovascular complications between the groups over the course of short-term and long-term follow-up. Conclusion: Patients who undergo OPCABG and have CHF or a lower EF (<35%) are more likely to undergo on-pump conversion, while preoperative BB administration could help prevent conversions from OPCABG to on-pump CABG.

Numerical Investigation on Hydrodynamic Characteristics of a Centrifugal Pump with a Double Volute at Off-Design Conditions

  • Severe radial thrust under off-design operating conditions can be a harmful factor for centrifugal pumps. In the present work, effects of geometry of a double volute casing on the hydrodynamic performance of a centrifugal pump have been investigated focusing on off-design conditions. Three-dimensional steady Reynolds-averaged Navier-Stokes analysis was carried out by using shear stress transport turbulence model. Numerical results for the hydrodynamic performance of the centrifugal pump were validated compared with experimental data. The hydraulic efficiency and radial thrust coefficient were used as performance parameters to evaluate the hydrodynamic characteristics of the centrifugal pump. The cross-sectional area ratio of the volute casing, the expansion coefficient of the rib structure, the distance between the rib starting point and volute entrance, and radius and width of the volute entrance, and length of the rib structure, were selected as geometric parameters. Results of the parametric study show that the performance parameters are significantly affected by the geometric variables and operating conditions. Optimal configurations of the double volute casing based on the design of experiments technique show outstanding performance in terms of the efficiency and radial thrust coefficient.
