한국노인의 낙상 요인 연구 (Risk Factors of Falls among Korean Elderly)

  • 본 연구는 전국단위의 패널데이터를 사용하여 한국노인의 인구사회학적요인, 보조기구사용, 신체건강상태, 정신건강상태, 건강행태 중 낙상에 영향을 미치는 요인을 도출하는 것을 목적으로 한다. 이러한 연구목적을 달성하기 위하여 고령화연구패널조사(KLoSA)의 제 1차와 제 2차 조사 자료를 분석에 활용하였다. 연구대상자는 2006년 제 1차 조사시점에서 만 65세 이상의 참여자를 기준으로 제 1차 조사와 제 2차 조사에 모두 참여한 응답자만(N=3,311)을 선택했으며 2008년 제 2차 조사 자료에서 낙상경험을 종속변수로 추출했다. 인구사회학적 요인들, 신체기능 및 질병유무, 건강행태 등에 대한 변수들은 시간의 선행 조건이 충족되어야 하므로 1차 조사로 부터 변수들을 추출했다. 분석방법으로는 변수에 따른 낙상경험 차이가 존재하는지 밝혀내기 위해 x2 혹은 t-test를 실시하였으며 독립변수들이 낙상경험에 유의미한 영향을 미치는지 고찰하기 위하여 로지스틱 회귀분석을 실시하였다. 분석결과로 성, 동거인여부, 거주지역, 보조기구사용, 질병의 수, 우울, 운동 및 흡연 여부에 따라 낙상경험에 통계적으로 유의미한 차이를 보였다. 또한 로지스틱 회귀분석에서 모든 변수를 통제하였을 때 여성, 청각보조기구사용자, 질병수가 2가지, 3가지 이상일 때, 우울이 있는 사람과 운동을 주 5회 이상 하는 응답자의 낙상경험 오즈가 그렇지 않은 사람에 비해 더 증가하였다. 반면 비만, 치매의심, 현재흡연자의 낙상경험 오즈는 준거카테고리에 비해 더 낮아졌다. 결론적으로 질병수 3가지 이상과 우울이 있는 사람의 낙상경험 오즈는 그렇지 않은 사람에 비해 2배 이상 높았기 때문에 낙상예방에 더욱 주의가 요구된다고 할 수 있다. 신체건강상태와 정신건강상태 모두 낙상과 관련이 있으므로 재가노인들의 낙상을 방지하기 위해서는 이러한 요인들을 갖고 있는 노인들을 위한 체계적인 지원이 이루어져야 하겠다.

Cardiometabolic Index, Triglyceride-glucose Index를 이용한 대사증후군 진단 예측지수에 대한 고찰: 부산지역 건강검진대상자 중심으로 (Consideration of Predictive Indices for Metabolic Syndrome Diagnosis Using Cardiometabolic Index and Triglyceride-glucose Index: Focusing on Those Subject to Health Checkups in the Busan Area)

  • This study investigates the utility of the Triglyceride-glucose(TyG) index and Cardiometabolic Index(CMI) as predictors for diagnosing metabolic syndrome. The study involved 1970 males, 1459 females, totaling 3429 participants who underwent health checkups at P Hospital in Busan between January 2023 and June 2023. Metabolic syndrome diagnosis was based on the presence of 3 or more risk factors out of the 5 criteria outlined by the American Heart Association/National Heart, Lung, and Blood Institute(AHA/NHLBI), and participants with 2 or fewer risk factors were categorized as normal. Statistical analyses included independent sample t-tests, chi-square tests, Pearson's correlation analysis, Receiver Operating Characteristic(ROC) curve analysis, and logistic regression analysis, using the Statistical Package for the Social Sciences(SPSS) program. Significance was established at p<0.05. The comparison revealed that the metabolic syndrome group exhibited attributes such as advanced age, male gender, elevated systolic and diastolic blood pressures, high blood sugar, elevated triglycerides, reduced LDL-C, elevated HDL-C, higher Cardiometabolic Index, Triglyceride-glucose index, and components linked to abdominal obesity. Pearson correlation analysis showed strong positive correlations between waist circumference/height ratio, waist circumference, Cardiometabolic Index, and triglycerides. Weak positive correlations were observed between LDL-C, body mass index, and Cardiometabolic index, while a strong negative correlation was found between Cardiometabolic Index and HDL-C. ROC analysis indicated that the Cardiometabolic Index(CMI), Triglyceride-glucose(TyG) index, and waist circumference demonstrated the highest Area Under the Curve(AUC) values, indicating their efficacy in diagnosing metabolic syndrome. Optimal cut-off values were determined as >1.34, >8.86, and >84.5 for the Cardiometabolic Index, Triglyceride-glucose index, and waist circumference, respectively. Logistic regression analysis revealed significant differences for age(p=0.037), waist circumference(p<0.001), systolic blood pressure(p<0.001), triglycerides(p<0.001), LDL-C(p=0.028), fasting blood sugar(p<0.001), Cardiometabolic Index(p<0.001), and Triglyceride-glucose index (p<0.001). The odds ratios for these variables were 1.015, 1.179, 1.090, 3.03, and 69.16, respectively. In conclusion, the Cardiometabolic Index and Triglyceride-glucose index are robust predictive indicators closely associated with metabolic syndrome diagnosis, and waist circumference is identified as an excellent predictor. Integrating these variables into clinical practice holds the potential for enhancing early diagnosis and prevention of metabolic syndrome.

세탁업 종사자의 유기용제 노출과 두통 및 눈의 피로 증상 간의 연관성: 제6차 근로환경조사 자료 분석 (Association between Exposure to Organic Solvents and Symptom of Headache and Eyestrain among Laundry Workers: Analysis of the 6th Korean Working Conditions Survey Data)

  • Background: Laundry workers are known to be exposed to organic solvents in various works including dry cleaning. Long-term exposure to organic solvents may cause adverse health effects among laundry workers. Objectives: To investigate the association between exposure to organic solvents and symptom of headache or eyestrain among laundry workers. Methods: The subjects were total 267 laundry workers who participated in the 6th Korean Working Conditions Survey (KWCS). Responses to exposure to 'inhalation of vapor from organic solvent' was categorized into two (non-exposed: rarely or absolutely not exposed during working hours; exposed: ≥1/4 working hours) or three (non-exposed; low exposure: 1/4 of the working hours; high exposure: ≥1/2 working hours) groups. Logistic regression analysis was performed to evaluate the association between exposure to organic solvents and symptom of headache or eyestrain in the last 12 months among laundry workers. Odds ratios (ORs) and the 95% confidence intervals (CIs) were calculated for experience of the symptom of headache or eyestrain with adjustment for age, sex, education, workplace size (by the number of workers), working hours per week, and current working period (year). Results: The proportions of exposure groups, i.e., non-exposed, low exposure, and high exposure, were 80.4%, 11.1%, and 8.5%, respectively. The proportion of those who had symptom of headache or eyestrain in the last 12 months was 18.7%. There was a significant association between exposure to organic solvents and experience of headache or eyestrain (OR=3.2, 95% CI=1.4~7.4). And, the proportion of those who experienced symptom of headache or eyestrain tended to increase significantly as the level of exposure to organic solvents increased (Ptrend=0.002). Conclusions: Our results suggest that exposure to organic solvents are associated with symptom of headache or eystrain among laundry workers in Korea. However, further studies with more relevant design is needed to replicate the association.

한국 당뇨병 성인의 혈당조절 장애요인 (Barrier Factors Influencing Glycemic Control in Korean Adults with Diabetes)

  • 본 연구는 한국 30세 이상 60미만 성인을 대상으로 당화혈색소 6.5% 이상 그룹과 미만 그룹간의 일반적 특성, 신체적 요인, 정신적 요인, 건강문해력 특성 등의 차이를 비교하고, 혈당조절에 영향을 미치는 요인을 성별에 따라 비교하기 위해 시행하였다. 2019년도부터 2021년도까지 국민건강영양조사 원시자료를 이용히여 층화집락추출 및 가중치를 반영하여 복합표본분석 방법을 적용하였다. 한국 30세 이상 60세미만 성인을 대상으로 당화혈색소 6.5% 이상 그룹과 미만 그룹의 인구사회학적 특성, 신체적 요인, 정신적 요인, 건강문해력 등에 따른 차이는 Rao-Scott χ2 test로 분석하였고, 성별에 따른 영향요인을 확인하기 위해 로지스틱 회귀분석 방법으로 분석하였다. 본 연구의 대상자는 438명 이었으며 당화혈색소 6.5% 이상 그룹은 348명으로 나타났다. 당화혈색소 6.5% 미만 그룹과 이상 그룹 간 유의한 차이가 있는 요인은 성별(RCχ2: 7.9, p=.012), 비만(RCχ2: 13.1, p=.006), 당뇨병 유병기간(RCχ2: 9.55, p=.029)이었다. 건강 전문가는 성별, 비만, 당뇨병 유병기간 등이 고혈당 위험성을 높이는 요인임을 파악하고, 혈당 조절 개선을 위한 중재 적용 시 대상자의 신체적 요인, 건강문해력 등을 고려한 건강 증진 전략을 제시할 필요가 있다.

의식이 명료한 글루포시네이트 중독환자의 신경학적 예후인자로서 APACHE II의 유용성 (Utility of the APACHE II score as a neurological prognostic factor for glufosinate-intoxicated patients with alert mental status)

  • Purpose: In patients with glufosinate poisoning, severe neurological symptoms may be closely related to a poor prognosis, but their appearance may be delayed. Therefore, this study aimed to determine whether the Acute Physiology and Chronic Health Evaluation II (APACHE II) score could predict the neurological prognosis in patients with glufosinate poisoning who present to the emergency room with alert mental status. Methods: This study was conducted retrospectively through a chart review for patients over 18 years who presented to a single emergency medical center from January 2018 to December 2022 due to glufosinate poisoning. Patients were divided into groups with a good neurological prognosis (Cerebral Performance Category [CPC] Scale 1 or 2) and a poor prognosis (CPC Scale 3, 4, or 5) to identify whether any variables showed significant differences between the two groups. Results: There were 66 patients (67.3%) with good neurological prognoses and 32 (32.8%) with poor prognoses. In the multivariate logistic analysis, the APACHE II score, serum amylase, and co-ingestion of alcohol showed significant results, with odds ratios of 1.387 (95% confidence interval [CI], 1.027-1.844), 1.017 (95% CI, 1.002-1.032), and 0.196 (95% CI, 0.040-0.948), respectively. With an APACHE II score cutoff of 6.5, the AUC was 0.826 (95% CI, 0.746-0.912). The cutoff of serum amylase was 75.5 U/L, with an AUC was 0.761 (95% CI, 0.652-0.844), and the AUC of no co-ingestion with alcohol was 0.629 (95% CI, 0.527-0.722). Conclusion: The APACHE II score could be a useful indicator for predicting the neurological prognosis of patients with glufosinate poisoning who have alert mental status.

Occupational exposure to polycyclic aromatic hydrocarbons in Korean adults: evaluation of urinary 1-hydroxypyrene, 2-naphthol, 1-hydroxyphenanthrene, and 2-hydroxyfluorene using Second Korean National Environmental Health Survey data

  • Background: Polycyclic aromatic hydrocarbons (PAHs) are occupational and environmental pollutants generated by the incomplete combustion of organic matter. Exposure to PAHs can occur in various occupations. In this study, we compared PAH exposure levels among occupations based on 4 urinary PAH metabolites in a Korean adult population. Methods: The evaluation of occupational exposure to PAHs was conducted using Second Korean National Environmental Health Survey data. The occupational groups were classified based on skill types. Four urinary PAH metabolites were used to evaluate PAH exposure: 1-hydroxypyrene (1-OHP), 2-naphthol (2-NAP), 1-hydroxyphenanthrene (1-OHPHE), and 2-hydroxyfluorene (2-OHFLU). The fraction exceeding the third quartile of urinary concentration for each PAH metabolite was assessed for each occupational group. Adjusted odds ratios (ORs) for exceeding the third quartile of urinary PAH metabolite concentration were calculated for each occupational group compared to the "business, administrative, clerical, financial, and insurance" group using multiple logistic regression analyses. Results: The "guard and security" (OR: 2.949; 95% confidence interval [CI]: 1.300-6.691), "driving and transportation" (OR: 2.487; 95% CI: 1.418-4.364), "construction and mining" (OR: 2.683; 95% CI: 1.547-4.655), and "agriculture, forestry, and fisheries" (OR: 1.973; 95% CI: 1.220-3.191) groups had significantly higher ORs for 1-OHP compared to the reference group. No group showed significantly higher ORs than the reference group for 2-NAP. The groups with significantly higher ORs for 1-OHPHE than the reference group were "cooking and food service" (OR: 2.073; 95% CI: 1.208-3.556), "driving and transportation" (OR: 1.724; 95% CI: 1.059-2.808), and "printing, wood, and craft manufacturing" (OR: 2.255; 95% CI: 1.022-4.974). The OR for 2-OHFLU was significantly higher in the "printing, wood, and craft manufacturing" group (OR: 3.109; 95% CI: 1.335-7.241) than in the reference group. Conclusions: The types and levels of PAH exposure differed among occupational groups in a Korean adult population.

Correlation of commute time with the risk of subjective mental health problems: 6th Korean Working Conditions Survey (KWCS)

  • Background: Studies conducted so far on the link between commute time and mental health among Koreans remain insufficient. In this study, we attempted to identify the relationship between commute time and subjective mental health using the 6th Korean Working Conditions Survey (KWCS). Methods: Self-reported commute time was divided into four groups: ≤ 30 (group 1), 30-60 (group 2), 60-120 (group 3), and > 120 minutes (group 4). Subjective depression was defined as a score of 50 points or less on the WHO-5 well-being index. Subjective anxiety and fatigue were defined as answering 'yes' to the questionnaire on whether they had experienced it over the past year. The analysis of variance, t-test, and χ2 test was used to analyze the differences among the characteristics of the study participants according to commute time, depression, anxiety, and fatigue. Odds ratios (ORs) and 95% confidence intervals (CIs) for depression, anxiety, and fatigue according to commute time were calculated using multivariate logistic regression models adjusted for sex, age, monthly income, occupation, company size, weekly working hours, and shift work status. Results: Long commute times showed increased ORs and graded increasing trends for depression, anxiety, and fatigue. The ORs for depression increased significantly in group 2 (1.06 [1.01-1.11]), group 3 (1.23 [1.13-1.33]), and group 4 (1.31 [1.09-1.57]) compared to group 1 (reference). The ORs for anxiety increased significantly in group 2 (1.17 [1.06-1.29]), group 3 (1.43 [1.23-1.65]) and group 4 (1.89 [1.42-2.53]). The ORs for fatigue increased significantly in group 2 (1.09 [1.04-1.15]), group 3 (1.32 [1.21-1.43]), and group 4 (1.51 [1.25-1.82]). Conclusions: This study highlights that the risk of depression, anxiety, and fatigue increases with commute time.

Relationship between shellfish consumption and urinary phthalate metabolites: Korean National Environmental Health Survey (KoNEHS) cycle 3 (2015-2017)

  • Background: Phthalates are endocrine disrupting chemicals that are widely used in the production of items of daily life such as in polyvinylchloride plastics, insecticides, and medical devices. This study aimed to determine the association between phthalate exposure and shellfish consumption using data from the Korean National Environmental Health Survey (KoNEHS) cycle 3 (2015-2017), which is a nationally representative survey. Methods: In this study, we analyzed the KoNEHS cycle 3 data of 3,333 (1,526 men and 1,807 women) adults aged more than 19 years. Data related to the variables of sociodemographic factors, health-related behaviors, dietary factors, seafood consumption frequency, and urinary phthalate metabolites concentrations were collected. The concentrations of urinary phthalate metabolites of all the participants were divided into quartiles to define high and low concentration groups based on the 75th percentile concentration. A χ2 test was conducted to analyze the distribution of independent variables. To analyze the relationship between shellfish consumption and phthalate exposure, the odds ratios (ORs) were calculated using logistic regression analysis. Results: Total adults with shellfish consumption frequency of over once a week showed the following adjusted ORs for high concentrations of the following metabolites compared with the group that consumed shellfish once a week or less: 1.43 (95% confidence interval [CI]: 1.01-2.06) for mono-(2-ethyl-5-oxohexyl) phthalate (MEOHP), 1.43 (95% CI: 1.01-2.03) for mono-(2-ethyl-5-carboxypentyl) phthalate (MECPP), 1.57 (95% CI: 1.10-2.24) for ∑di-2-ethylhexyl phthalate (∑DEHP), 2.01 (95% CI: 1.46-2.77) for mono-carboxyoctyl phthalate (MCOP), 1.56 (95% CI: 1.11-2.18) for mono-carboxy-isononly phthalate (MCNP), and 2.57 (95% CI: 1.85-3.56) for mono (3-carboxypropyl) phthalate (MCPP). Conclusions: The concentrations of urinary phthalate metabolites (MEOHP, MECPP, ∑DEHP, MCOP, MCNP, and MCPP) were higher in adults with a higher frequency of shellfish consumption.

편평세포폐암에서 CT 영상 소견을 이용한 PD-L1 발현 예측 (Predictions of PD-L1 Expression Based on CT Imaging Features in Lung Squamous Cell Carcinoma)

  • 목적 CT 영상 소견을 이용하여 편평세포폐암에서 programmed death ligand 1 (이하 PD-L1)의 발현을 예측하는 모델을 구축해 보고자 하였다. 대상과 방법 PD-L1 발현검사 결과를 포함하고 있는 97명의 편평세포폐암 환자를 포함하였고 종양 치료 전 시행한 CT 영상 소견을 분석하였다. 전체 환자군과 40명의 진행성(≥ stage IIIB) 병기 환자군에 대하여 PD-L1 발현 예측을 위한 다중 로지스틱 회귀 분석 모델 구축을 시행하였다. 각각의 환자군에 대하여 곡선 아래 면적(areas under the receiver operating characteristic curves; 이하 AUCs)을 분석하여 예측력을 평가하였다. 결과 전체 환자군에서 '전체 유의인자 모델'(종양병기, 종양크기, 흉막결절, 폐전이)의 AUC 값은 0.652이며, '선택 유의인자 모델'(흉막결절)은 0.556이었다. 진행성 병기 환자군에서 '선택 유의인자 모델'(종양크기, 흉막결절, 폐소수전이, 간질성폐렴의 부재)의 AUC 값은 0.897이었다. 이러한 인자들 중 흉막결절과 폐소수전이는 높은 오즈비를 보였다(각각, 8.78과 16.35). 결론 본 연구에서의 모델은 편평세포폐암의 PD-L1 발현예측의 가능성을 보여주었으며 흉막결절과 폐소수전이는 PD-L1 발현을 예측하는데 중요한 CT 예측인자였다.

도시와 농촌지역 노인의 건강행태 및 건강수준 비교 (Comparisons of Health Status and Health Behaviors among the Elderly between Urban and Rural Areas)

  • 본 연구는 국민건강영양조사를 이용하여 도시와 농촌지역에 거주하는 노인의 건강행태와 질병수준을 파악하고, 사회인구학적 특성을 통제한 상태에서 도시와 농촌지역의 노인의 건강수준을 비교하고자 수행하였다. 제4기 국민건강 영양조사(2007년-2009년) 자료를 이용하였으며, 65세 이상 노인으로 건강면접조사, 보건의식행태조사, 검진조사에 참여하여 분석에 필요한 변수가 모두 확보된 3,823명을 최종 연구대상으로 하였다. 지역의 분류는 행정지역과 동, 읍면으로 구분된 거주지역을 이용하여 '대도시', '도시', '농촌'으로 분류하였다. 가중치를 적용한 복합표본분석을 시행하여 지역에 따른 건강행태, 정신 건강 및 만성질환수준 증의 건강상태를 비교하였다. 연구결과, 우리나라 65세 이상 노인의 건강수준은 농촌지역이 사회경제적 여건은 도시지역에 비해 열악하지만 노인의 건강행태나 건강상태는 도시지역에 비해 더 양호함을 알 수 있었다. 농촌지역을 대도시 지역과 비교할 경우 건강행태 및 건강상태의 교차비는 독감접종 1.58(95% 신뢰구간 1.25-2.01)로 유의하게 높았고, 유연성운동 0.47(0.37-0.59), 근력운동 0.56(0.38-0.81), 비만 0.76(0.62-0.92)로 유의하게 낮았다. 질환 이환 중 관절염(0.81, 95% 신뢰구간: 0.66-0.99)과 당뇨병(0.70, 0.55-0.89)의 교차비는 농촌지역에서 대도시지역에 비해 통계적으로 유의하게 낮았다. 결론적으로 65세 이상 노인의 건강행태와 건강 수준은 이들의 거주 지역에 따라 차이가 있음을 알 수 있었다. 따라서 노인의 건강 수준의 향상을 위해서는 지역 특성과 여건에 맞는 프로그램의 개발이 필요하며, 거주 지역에 따른 차이를 설명할 수 있는 특성을 파악하고자 하는 연구가 이루어져야 할 것으로 생각된다.
