산업장의 소음폭로수준과 근로자의 스트레스 증상간의 관련성 (The Relationship between Noise Exposure Level in Worksite and Workers' Stress Symptoms)

  • Health services on industrial noisy environment have been provided only for noise-induced hearing loss management until now. But gradually, modem diseases and death have come to be related to stress and mental health deeply, therefore noise-induced mental disorder, like a stress became very important. In this point, the purpose of this study was to analyze the relationship between noise exposure level in worksite and workers' stress symptoms. This study included a survey of 786 manual workers selected from 89 worksites in 21 factories in Puchon. The results were as follows: 1. For demographic characteristics, most of the workers were males(80.8%), the $20\sim29years$ old were 34.5% and those who graduated from high school were 65.3%. The workers whose monthly income ranged from 100,000 to 800,000 won were 37.5% and who have a religion were 47.9%. 2. For occupational characteristics, workers who had worked $1\sim5$ years in the factories were 33.0%. Those who were much for them workload were 43.9% and who worked more than 8 hours a day were 73.9%. Those who were disatisfied with their job and pay were 31.9% and 50.6%. The workers who responsed ventilation condition of their worksites were bad were 51.9% and the dissatisfed with working environment of their worksites were 45.9%. 3. Workers who were suffering from tinnitus were 53.3% and those who perceived hearing loss were 50.1%. Persons who reported they always wore earplugs at work were 35.4%. Those who felt earplugs bigger than their ears were 30.6% and those who experienced eardiseases caused by earplugs were 25.6%. 4. For the noise exposure level in worksite, workers who were exposed to $80\sim90dB$ were 30.3%, $90\sim100dB$ were 26.4% and $50\sim70dB$ were 19.2%. 5. Workers' stress symptoms were significantly related to marital status and their monthly income(p<0.05). Workers who were single and had lower monthly income showed higher PSI (Psychiatric Symptom Index) scores than those who were married and had higher monthly income. Higher PSI scores were also significantly related to $1\sim2$ days hight-work per week, much for them workload, dissatisfaction with their job, and bad relationship with their bosses and coworkers. 6. Higher PSI scores were significantly related to severe tinnitus and perceived hearing losso(p<0.001). Workers who felt the earplugs they use did not fit their ears showed significantly higher PSI scores(p<0.01). Workers who reported that they did not feel they need earplugs showed. significantly higher PSI scores (p<0.05). Increased experience of eardisease caused by earplugs that did not fit were also significantly related to higher PSI scores(p<0.01). 7. The higher noise exposure level in worksite from 80dB was, the more severe stress symptoms including PSI subparts were reported; Anxiety, Anger, Depression, and Cognitive disorder(p<0.001). 5. According to the results of stepwise multiple regression analysis, factors affecting workers' PSI scores were perceived hearing loss$(R^2=0.160)$, noise exposure level in worksite$(R^2=0.110)$, realtionship with coworkers, amount of workload, monthly income and relationship with bosses orderly and the total $R^2$ of this 6 factors was 0.371. 9. The most significant factors that have impact on manual workers' stress symptoms were perceived hearing loss and noise exposure level in worksite, especially noise exposure level in the worksite was the most affective factor on the depression symptom.

학교 급식종사자를 대상으로 한 위험성평가의 실효성에 관한 연구 (A Study on the Effectiveness of Risk Assessment for School Food Service Workers)

  • 2017년부터 학교 급식업무에 대한 산업안전보건법이 전면 적용되고 그에 따라 2019년부터 위험성평가가 시행되었으나 타 업종에 비해 도입이 늦고 비전문가인 영양(교)사에 의해 평가되고 있어 제도 정착의 한계가 있었다. 서울시교육청 산하 모든 학교에서는 고용노동부의 학교 급식업무의 산업안전보건법 및 고용노동부 고시에 따라 매년 안전보건공단에서 제공하는 위험성평가 지원 시스템의 KRAS 기법을 적용하고 있다. 이렇듯 법적 규제에 따라 위험성평가를 적용하고 있으나 위험의 빈도와 강도 기준이 정성적이고 주관적이어서 평가자에 따라 다른 결과를 도출하여 위험성평가의 실효성이 의문시되었으나 지금까지 학교 급식업무에 대한 위험성평가와 관련된 연구는 없었다. 본 연구에서는 급식종사자의 재해통계를 바탕으로 위험의 빈도와 강도 기준을 정량적이고 객관적으로 제시하고 급식업무 중 일부 작업에 대해 새로운 산정기준을 적용하여 위험성평가를 실시하고 기존 위험성평가 결과와 비교하였다. 그 결과 위험도가 높아지고 그에 따른 개선방안이 구체적으로 제시되어 실효성 제고가 확인되었으므로 급식업무 전체에 대해 시범평가를 확대하고 타당성을 확인하여 모든 학교에 확대 적용함으로써 위험성평가의 실효성과 작동성 제고에 기여하기를 기대한다.

장애아동 어머니의 성격유형에 따른 대처행동에 관한 연구 (Coping Behavior According to the Personality Type of Mothers of Children with Disabilities)

  • 본 연구는 장애아동 어머니의 성격유형에 따른 대처행동에 대해 파악하고자 하였다. 2020년 6월부터 9월까지 장애아동 어머니 102명을 대상으로 성격유형과 대처행동에 대한 설문을 진행하였다. 성격유형은 에니어그램을 사용하여 평가하였고 대처행동은 한국판 장애아부모 대처행동 척도를 사용하여 평가하였다. 연구결과, 장애아동 어머니의 성격유형은 장형이 가장 많았다. 대처능력은 장애자녀를 위한 적극적 문제해결이 가장 높은 점수를 보여주었다. 장애아동 어머니의 성격유형에 따른 대처행동을 분석한 결과, 장애아동 어머니의 대처능력과 부부협력 강화, 사회적-정서적 지지추구에서 유의한 차이가 나타났다. 사후검증 결과 대처능력과 부부협력강화, 사회적-정서적 지지추구 모두 가슴형과 장형 간 유의한 차이가 있었으며, 가슴형이 장형에 비해 점수가 높았다. 장애아동 가족을 위한 중재 개입 시, 장애아동 어머니의 성격유형에 따른 대처행동을 예측하고 개별화된 프로그램을 제시한다면 더욱 효과적인 서비스를 제공할 수 있을 것이다.

일 도시 지역 근무지 형태에 따른 요양보호사의 직무인식과 교육요구도 분석 (A Study on the Job Recognition and Educational Needs of Care Workers according to the Types of Working Place in the Urban Area)

  • 본 연구는 근무지 형태에 따른 요양보호사의 직무인식과 교육요구도를 파악하여 비교 분석함으로써 노인요양 서비스의 전문성 강화를 위한 직무교육 프로그램 개발의 근거를 제공하고자 한다. 연구방법: B 광역시의 노인의료복지시설, 노인주거복지시설, 재가복지시설에서 근무하고 있는 177명의 요양보호사를 대상으로 2019년 4월부터 7월까지 자료를 수집하였고, SPSS Win 21.0 Statistical Program을 사용하여 분석하였다. 연구결과: 노인의료복지시설, 노인주거복지시설, 재가복지시설 요양보호사는 요양보호사의 직업윤리와 자세에 대한 직무인식정도가 가장 높았고, 교육요구도는 노인의료복지시설 요양보호사는 요양보호사의 직업윤리와 자세, 노인주거복지시설 요양보호사는 요양보호 업무 기록 및 보고, 재가복지시설 요양보호사는 요양보호사의 직업윤리와 태도가 가장 높았다. 결과: 근무지별에 따른 요양보호사의 역량강화를 위한 차별화된 직무교육이 요구되며, 초급 요양보호사가 수행하는 직무와 숙련 요양보호사가 수행하는 직무를 구분하여 교육해야 될 것이다. 또한, 요양보호사 스스로가 전문가로 거듭날 수 있는 전문성 향상과 요양보호서비스의 질 확보를 위한 노력을 기울여야 할 것이다.

예비치과위생사의 취업에 관한 의식 조사 (A Survey on Preliminary Dental Hygienists' Senses of Employment)

  • The purpose of this study was to conduct a survey on the senses that preliminary dental hygienists have on employment, who applied for the National Dental Hygienist Examination, so that it could provide a basic reference required for fostering good human resources specializing in oral health with a sense of mission and responsibility as professional. To meet these goals, a self-administered questionnaire survey was conducted for participants in a refresher education meeting for preliminary dental hygienists across three major regions(Seoul, Daejeon and Busan), which was held by the Korean Dental Hygienists Association(KDHA) on Jan. 15, 2006. Total 289 sheets of questionnaire were collected, and 262 sheets except invalid or incomplete ones were used for examination and analysis. As a result, this study came to the following conclusions: First, it was found that 40.4% respondents wanted to be employed in dental clinic or hospital. Most respondents(90.1%) answered that faithfulness is most valued as prerequisite qualification for employment in hospital, and highest percentage of respondents(39.3%) wanted to earn 16 to 17 million Korean won annually. For internship opportunities, 61.9% respondents replied that they could willingly accept the internship, provided that basic pay and switchover to regular employee are all assured. Second, as the results of survey on employment circumstances around respondents, it was found that most respondents(58.3%) were employed in dental clinic or hospital, and highest percentage of respondents(35.0%) earned 15 to 16 million Korean won. Third, it was found that interpersonal relationships among employees had most significant effects(4.81) on selection of employment, which was followed by in-house welfare benefits 1(monthly/annual vacation, resting room, etc; 4.56). For possible influential factors on the selection of employment depending on whether employed or not, it was found that there were Significant differences in pay, welfare benefit 2(seminar, orientation, refresher training opportunities, etc), post-wedding continued employment and merit system between employees and non-employees(pE0.05). Fourth, the survey on respondents' occupational senses of dental hygienist showed that major tasks of dental hygienists were represented by oral disease prevention(92.6%), case management(71.5%), oral health education(66.3%), hospital/clinic management and dental management support(10.4%) and public oral health activities(6.7%) respectively. For job satisfaction, it was found that almost half respondents(48.9%) showed satisfaction at their job and 32.6% felt satisfied at and proud of their job. That is, majority of respondents(81.5%) felt satisfied at their job as dental hygienist. For follow-up requirements to qualify for dental hygienists, it was found that 73.3% felt it necessary to give more investments to self-development, 62.2% respondents considered protection of membership's rights as one of future assignments that KDHA should be more committed to. For future social awareness about dental hygienists as occupation, 69.2% respondents expected that they would be better developed and treated as professional. And desired training courses as a part of qualification for professional dental hygienists were represented by case management(services, manners, etc; 33.3%), implant(28.9%), esthetics(correction, prosthesis, whitening; 18.9%) and so on. For an item on working years, it was noted that 75.9% respondents would keep working as dental hygienists as long as they could.

응급구조사 직무기술서 작성에 따른 일의 요소별 빈도 및 중요도 조사 연구 (Investigating Research on the Degree of Frequency and Importance of Tasks for Framing the EMTs Occupational Description)

  • This research, aiming at analyzing the Emergency Medical Technicians' duties prior to establishing a baseline for question development in the national exams to qualify for the EMTs, investigated the importance and frequency per unit specified in the description with the EMTs to be subjects working on the task spot by means of questionnaires. The EMT duties were classified into 9 items like 1) the notification and response, 2) the scene size-up, 3) the patient assessment, 4) the emergency care, 5) the patient transport, 6) the duties at hospital clinics, 7) the operational management, 8) the receiving and responding to a call, 9) the self-development, and put the functional tasks into 52 items, describing the task elements into 177 items, and then questioned 112 EMTs working on the spot from July 21 through August 30, 2000, which showed the following results. 1) The distribution of subjects' career showed the highest rate with 33.9% for those who careered "less than a year", only 13.4% for those with more than 4 years and the highest rate with 43.8% for those who aged at 20-25. And 70.5% of all those who were questioned was the junior college graduates, 58.9% for those working at fire station and 29.5% working at hospital clinics. 2) Looking at the distribution of frequency and importance for each task element, questioning 'the patients main symptoms', 'accidental type', 'place of the patients identification' showed the highest rate in both frequency and importance in the field of "notification and response". 3) In the "scene size-up", identifying the patients showed the highest rate of frequency and importance, compared to other field of tasks, among which "identifying the patients' state" showed the highest rate of frequency (2.66) and importance (2.81). 4) In the "patient assessment", "identifying the patients" showed the high rate of frequency and importance in most elements of task, especially the importance showed the highest rate with 2.83 for the task of airway management and the cervical immobilization during "the primary assessment", and the frequency showed the highest rate for questioning the past case of the task of grasping the patients' history. 5) In the "emergency care", "the management of the heart attacked patients" and "the advanced cardiac life support" showed a high rate in the importance, whereas the frequency showed a very low rate. The high rate of frequency during the emergent task was the management of "musculoskeletal system injury patients" and of "the gastro-intestinal and urinary genital system injury patient support." In care of the patients with heart attack, the management of the airway showed the highest rate with 2.95, whereas the management of alcoholic abused patients and of dying patients, showed comparatively lower rate of assessment in the importance. The frequency of tasks showed the highest rate with 2.69 in the control of bleeding and the lowest with 0.47 in the management of abnormal delivery of child. 6) As to the patient transport, "the emergency transport" showed the highest rate with 2.74 and the unemergent transport with 2.55 in the importance, and the task importance at hospital showed the highest rate with 2.89 in managing the cardio-pulmonary resuscitation and with 2.60 in identifying the patient state. 7) Of all the tasks related with "operational task", the high importance was to "educate for the management of the first responder", but the frequency of tasks mostly showed a low rate and "receiving and responding to a call" showed relatively a high rate of importance and frequency. And related with "the self-development", "the health care management" and "the stress control" on working spot showed a high rate, but the frequency mostly showed a low rate.

병원도산 예측에 관한 연구 (Predicting hospital bankruptcy in Korea)

  • 본 연구는 우리 나라 병원도산 예측모형을 도출하기 위한 연구로 1992년에서 1997년 사이 5년간의 전국 병원 경영통계 자료를 이용하여 1995년부터 1997년 사이에 도산한 병원중도산전 3년까지의 연속된 자료가 있는 31개 병원을, 비교군 병원은 도산병원과 유사한 병상규모를 가지고 당기순이익이 발생한 31개 우량병원을 선정하여 단계적 판별분석에 의한 실증연구를 시행하였다. 본 연구의 구체적 연구결과는 다음과 같다. 첫째, 도산전 각 연도별로 도산병원과 우량병원간에 연구변수의 단순 평균치분석 결과, 자본구조 지표인 자기자본비율과 수익성지표인 총자본의료이익을, 의료수익의료이익을, 총자본경상이익을, 의료수익경상이익율, 총자본순이익을 등은 도산 1, 2, 3년전 모두에서 도산병원과 우량병원간에 유의한 차이를 보였다. 자본고정성지표는 도산 1년전에 고정비율이 유의한 차이를 보였고, 유동성지표는 도산 1년전에는 유동비율과 당좌비율이 유의한 차이를 보였고 도산 2년전에는 당좌비율만이 유의한 차이를 보였다. 활동성지표로는 도산 1년전에 총자본회전율과 재고자산회전율이 유의한 차이를 보였고 도산 2년전에는 총자본회전율과 의료미수금회전율이, 도산 3년전에는 의료미수금회전율만이 유의한 차이를 보였다. 생산성지표로는 도산 2년전에 총자본투자효율이, 도산 3년전에는 조정환자1인당 부가가치가 유의한 차이를 보였다. 진료실적지표로는 도산 3년전 일평균재원환자수가 유의한 차이를 보였다. 둘째, 도산 1, 2, 3년전 판별함수는 각각 도산 1년전 Z=($0.0166\times$당좌비율)-($0.1356\times$총자본경상이익을)-($1.545\times$총자본회전을), 도산 2년전 Z=($0.0119\times$당좌비율)-($0.1433\times$총자본의료이익율)-($0.0227\times$총자본투자효율), 도산 3년전 Z=($0.3533\times$총자본순이익율)-($0.1336\times$의료미수금회전율)-($0.04301\times$조정환자1인당부가가치)+($0.000119\times$일평균재원환자수)이었다. 셋째, 도출된 도산 1, 2, 3년전 각 판별함수의 예측력은 77.42%, 79.03%, 82.25% 이었다.
