보건소 방문간호 대상자의 건강문제와 방문간호 요구 (Health Care Needs and Health Problems of the Subjects in a Health Center)

  • In order to investigate the basic data for the visiting nursing care plan of Health Center in Korea, the questionnaire survey in regard to health care needs and health problems of the subjects with visiting nursing care was carried out on 131 subjects with visiting nursing care. The results were as follows : 1. The subjects consisted of 38.2% in male and 61.8% in female. 77.1% of the subjects had no job. 2. In the health problem, the subjects symptoms were 31.2% of hypertension, 20.6% of arthritis, and 19.1% of diabetes and other chronical illness. Utilization of medical care facilities were 61.8% of private clinics or general hospitals and 29.0% of Health Center. 3. 10 areas of health care needs that subjects wanted were disease management(19.5%), demand for welfare concerns(15.7%), health promotion and disease prevention(14.2%), information for medical institution(12.3%), health management for the aged(10.8%), hospice care(8.0%), prevention of dementia(8.0%), care for mental hygiene(6.7%), management for drinking, smoking and drug abuse(3.3%), home care nursing and rehabilitation nursing(1.2%).

여수지역 비정규직 플랜트 건설 근로자의 안전보건 실태와 개선방안 (Health Status and Improvement Measures for Irregular Plant Construction Workers at Yeosu National Industrial Complex)

  • This study was conducted to evaluate health status and to propose health protection measures of irregular plant construction workers in Yoesu National Industrial Complex (YNIC). The status of safety and health management was examined in five areas including safety and health education, work environment monitoring, health examination, health management record, and personal protective equipment (PPE) for plant construction workers. The safety training rate for plant construction workers was reached high at 91%, The training was mostly consisted of safety accident related things, but training on hazardous materials was found to be insufficient. Workplace monitoring results showed that the compliance rate for work environment for irregular construction workers was 54% and workplace monitoring during turnaround (TA) period with high risk of exposure to hazardous agents has not been implemented. While 61.4% of irregular workers received the general health examination but only 36.8% received the special health examination. The special health examination was found to be conducted only upon welders from 2-3 years ago. The issue of health management record upon irregular construction workers was not being implemented. In case of PPE, basic safety protective equipments such as safety shoes, safety belt, safety helmet were being supplied well while the supply rate of respirator for organic vapor was relatively low at 40%. Based on this study, two suggestions to maximize the utilization of the current safety and health program were made while boosting its effectiveness in protecting workers' health. First, the role of owners (petrochemical plant) related to safety and health should be strengthened. Second, in consideration of the characteristics of construction workers who usually engage in short term employment and frequent movement, community based health management organization is suggested that can overcome such structural problem and carry out the implementation of health examination and sustained health management.

Global Estimates on Biological Risks at Work

  • Introduction: Biological risks are a major global problem in the workplace. The recent COVID-19 pandemic has highlighted the need for a more comprehensive understanding of the biological risks at work. This study presents data on both communicable infectious biological agents and noncommunicable factors leading to death and disability for the year 2021. Methods: We followed the methodology established by the International Labour Organization (ILO) in their past global estimates on occupational accidents and work-related diseases. We used relevant ILO estimates for hazardous substances and related population attributable fractions derived from literature, which were then applied to World Health Organization mortality data. The communicable diseases included in the estimates were tuberculosis, pneumococcal diseases, malaria, diarrheal diseases, other infectious diseases, neglected tropical diseases, influenza associated respiratory diseases and COVID-19. Noncommunicable diseases and injuries considered were Chronic Obstructive Diseases (COPD) due to organic dusts, asthma, allergic reactions and risks related to animal contact. We estimated death attributable to biological risk at work and disability in terms of disability adjusted life years (DALYs). Results: We estimated that in 2022, 550,819 deaths were caused by biological risk factors, with 476,000 deaths attributed to communicable infectious diseases and 74,000 deaths caused by noncommunicable factors. Among these, there were 223,650 deaths attributed to COVID-19 at work. We calculated the rate of 584 DALYs per 100,000 workers, representing an 11% increase from the previous estimate of the global burden of work-related disabilities measured by DALYs. Conclusion: This is a first update since previous 2007 ILO estimates, which has now increased by 74% and covers most biological risks factors. However, it is important to note that there may be other diseases and deaths are missing from the data, which need to be included when new information becomes available. It is also worth mentioning that while deaths caused by major communicable diseases including COVID-19 are relatively rare within the working population, absences from work due to these diseases are likely to be very common within the active workforce.

농작업에서 안전보건정보 표시의 농업인 이해도 조사 연구 (A Questionnaire Survey about the Degree of Understanding of the Safety and Health Information by Agricultural Workers)

  • This study surveyed the reading discomforts and the reasons for discomfortable reading of the safety and health information (texts and icons) presented on the agricultural vehicles/machines, pesticides, fertilizers, and feeds. Eighty seven people residing in rural area participated in the survey interview. Questionnaire survey showed that the most discomfortable product in reading the texts was pesticides. Forty three (49%) among participants had very-discomfortable or discomfortable in reading the texts used in the label of pesticides, and the main reason for the discomfort was small text size. The reading discomforts in reading the texts (varied from 4 point to 19 point and presented in 50 cm reading distance) showed different pattern according to the age groups. Three age groups (50s, 60s and older than 70s) showed a similar discomfort pattern, but different from the group of 30s and 40s. Forty four people (51%) had a problem in understanding the meaning of the icons and the main reasons were the small size and the difficulty in inferring the meaning of the icons. Thus, the more detailed and practical guidelines for the presentation format, particularly about the text heights and the size of icons, are required. Also, more comprehensive research is needed to investigate the readability and legibility of texts and icons.

  • Schools have significant and serious indoor environmental health problem, of which indoor air quality (IAQ) in school building may affect the health of the students and indirectly affect learning performance. Schools are of special concern when regarding indoor exposure to air pollutants, because students are particularly sensitive to pollutants and spend a significant amount of time in that environment. Therefore researches for improvement of indoor air quality have been developed such as installation of air cleaning device, ventilation system, titanium dioxide(TiO2) coating and so on. However, it is difficult to evaluate the magnitude of improvement of indoor air quality in field study because indoor air quality can be affected by source generation, outdoor air level, ventilation, decay by reaction, temperature, humidity, mixing condition and so on. In this study, evaluation of reduction of formaldehyde emission rate in school indoor environments by far-Infrared ray coating material was carried out using mass balance model in indoor environment. we proposed the evaluation method of magnitude of improvement in indoor air quality, considering outdoor level and ventilation. Since simple indoor concentration measurements could not properly evaluate the indoor air quality, outdoor level and ventilation should be considered when evaluate the indoor air quality.

외국인 근로자와 내국인 근로자의 건강수준 및 결정요인 비교 (A Comparative Study on Health Status and Health Determinants of Foreign Workers and Native Workers)

  • Purpose: The purpose of this study was to compare the health status between foreign workers and native workers and to examine the influencing factors of health status of foreign and native workers. Methods: This was a secondary analysis of the data collected from the 3rd (2011) Korean Working Conditions Survey (KWCS). The sample included 101 foreign workers and 101 native workers matched by age and gender. Results: The difference in health status between foreign workers and native workers was not statistically significant. The most commonly complained health problem of foreign workers was muscle pain of the upper limbs. Among foreign workers, hazard exposure, coworker support and supervisor support were significant predictors of health status. Among native workers, income level and obesity were significant predictors of health status. Conclusion: For health management and health promotion of foreign workers, employers and health providers need to establish coworker supporting systems in workplace and improvement strategies of work environment. Particularly, they must pay closer attention to management of foreign workers' musculoskeletal disorder.

남자 제조업 근로자의 직무스트레스, 피로도, 안전보건서비스의 경험이 건강문제에 미치는 영향 (The Effects of Job Stress, Fatigue, and Health and Safety Services on Health Issues of Male Manufacturing Workers)

  • Objectives: The purpose of this study was to investigate whether there were differences in manufacturing industry worker's health issues depending on job stress, fatigue, and health and safety services. Methods: The purpose of the questionnaire survey and its contents were explained to manufacturing industry health personnel, and the questionnaires were collected from 666 workers from 17 manufacturing workplaces. Questionnaires with missing data were excluded from the analysis so a total of 632 questionnaires were included. Results: Overall, 74.1% of the respondents had experienced health issues during the preceding month. The factors affecting worker health were number of workers(OR=1.000034, p=.013), job demand(OR=1.82, p=.009), MFS(OR=1.85, p=.003), provision of safety & health information(OR=1.85, p=.048), wearing protector(OR=2.58, p=.021). Conclusion: It is necessary to strengthen the safety and health education to mitigate hazardous and risk factors. Employers should provide health managers in charge of safety and health education to control workplace risk factors. In case of small businesses, it may be necessary to strengthen safety and health education in existing worker health centers and government support projects.

남성 근로자의 직종별 문제음주 관련 요인 (Factors related to Problem Drinking of Male Workers according to Occupational Classes)

  • 본 연구는 직종별 문제음주 관련 요인을 확인하고자 시도되었다. 본 연구는 2012년 1월부터 12월까지 수집된 국민건강영양조사 자료를 이용하여 분석하였다. 전체 대상자 중 본 연구의 목적에 따라 사무직 391명, 생산직 707명, 서비스직 309명을 최종 대상자로 선정하였다. 자료는 Rao-Scott 카이제곱 검정과 로지스틱 회귀분석으로 분석하였다. 알코올사용장애 선별검사 도구를 이용하여 8점 이상을 문제음주군, 8점 미만은 정상음주군으로 분류하여 분석한 결과 문제음주율은 사무직이 45.1%, 생산직 39.1%, 서비스직 42.3%로 나타났으나 직종간 통계적으로 유의한 차이는 없었다. 문제음주 관련 요인은 직종에 따라 차이가 있었는데 사무직은 흡연만이 관련 요인으로 확인되었으며 흡연자의 문제음주 위험이 4.44배 높았다. 생산직은 배우자가 있는 경우 문제음주 위험이 0.46배 낮고 흡연자는 4.07배 높았다. 서비스직은 20대에 비해 60대 이상의 문제음주 위험이 0.02배 낮은 반면 흡연자는 11.17배 높게 나타났다. 본 연구는 직종별 문제음주 관련 요인의 차이를 확인하였으며 향후 본 결과가 음주 중재프로그램 개발에 의미있게 활용될 수 있을 것이라 생각한다.

Workplace Violence and Safety Issues in Long-Term Medical Care Facilities: Nurses' Perspectives

  • Background: Workplace violence (WPV) is becoming an issue that needs immediate attention in the United States, especially during this period as more states are adopting the "stand your ground laws to promote worker protection." This study was conducted to investigate how WPV has contributed to an unsafe environment for nurses and nursing assistants who work in long-term medical care facilities. Methods: A structure questionnaire was used to collect data for the study. Three facilities were sampled and 80 nurses and certified nursing assistants participated in the study. Ninety-two percent (n = 74) were female and 8% (n = 6) were male. Approximately 62% were black or African American, approximately 33% were Caucasians, and only 2% were from other ethnicities. Results: We found that 65% of the participants had experienced WPV while 41% believed that management shows little or no concern for their safety. Approximately 23% of respondents believed that reporting supervisor's WPV act is an unsafe action. In addition, 22% of those who reported that they have experienced WPV believed that the work environment is not safe to perform their duties. This significant difference in perception of workplace safety between those who had experienced WPV and those who had not was significant (t = 3.95, df = 158, p < 0.0001). Conclusion: WPV is an epidemic problem that affects all health-care professionals. The findings of this study could help long-term medical care facilities' management identify the areas to focus on mitigating, controlling, and/or eliminating incidents of WPV.

독거노인과 가족동거 노인의 시간관리능력과 일상생활활동의 비교 (Comparison of Time-Management Ability and ADL between Elderly People Living Alone and Living with Family)

  • 목적 : 본 연구는 대전광역시에 거주하는 독거노인과 가족동거 노인들을 대상으로 시간관리능력과 일상생활 활동을 비교하고, 집단 간의 관계를 파악함으로써 노인의 건강관리 방안을 찾는 기초자료를 제공하고자한다. 연구방법 : 연구대상은 65세 이상의 독거노인과 가족동거 노인을 MMSE-K로 선별한 후 각 40명씩 총 80명을 선정하였다. 시간관리능력은 설문지를 사용하였고, 일상생활활동은 FIM을 사용해 평가하였다. 연구기간은 2013년 5월부터 6월까지 진행되었다. 본 연구의 자료분석은 SPSS 20.0 프로그램을 사용하였다. 결과 : 독거노인과 가족동거 노인의 시간관리능력 및 FIM에 대한 총점의 점수비교는 두 집단 간에 통계적으로 유의한 차이를 보이지 않았다. 집단 간 세부항목 점수의 비교에서는 FIM의 세부항목 중 사회적 인지항목에서 사회적 상호작용, 문제해결능력, 기억력 모두 두 집단 간에 통계학적으로 유의한 차이를 나타냈다. 결론 : 본 연구를 통해 가족동거 노인에 비해 상호작용의 기회가 부족한 독거노인이 사회적 인지기능에 저하가 있음을 알 수 있었다. 이를 바탕으로 향후 지역사회 작업치료 안에서 노인의 인구 사회학적인 특성을 고려한 다양한 프로그램 개발과 연구가 이루어져야 할 것이다.
