산업재해 중 근골격계질환 요인 특성 분석 (An Analysis of Characteristics of Musculoskeletal Disorders Risk Factors)

  • This study was aimed to analyze of characteristics of Musculoskeletal disorders (MSDs) based on the industrial accident statistics which were extracted from the Ministry of Labor. In this study all MSDs cases in 2007, 7,723 cases, were investigated. First of all, for the analysis of a wide view point concerning the characteristics of MSDs, We have inspected characteristics of the user population (i.e. age, gender, employed periods, etc.). Secondly, work-related risk factors such as repetitive motions, awkward postures, manual material handling were analyzed in terms of disease code, injured part of body, types of business, etc. Next, characteristics of handling of heavy materials were analyzed according to weight, types of handling, agency of cause, etc. Finally, we have compared and analyzed the features between the period of hospitalization and other variables. In addition to that we also conducted statistics analysis. The study results showed that there were significant characteristics of MSDs that could be used as basis data for the MSDs prevention policy.

종합병원 간호사들의 노동보상과 직업몰입에 관한 연구 (Work Rewards and Occupational Commitment of Hospital Nurses)

  • The purpose of this study is to empirically investigate the determinants of occupational commitment of hospital nurses. For this study, a causal model of occupational commitment of hospital nurses was constructed based on the exchange theory. The sample of this study consisted of 329 nurses from S general hospitals located in Seoul and south-eastern area of Korea. Data were collected with self-administered questionnaires and analyzed using hierarchical multiple regression. It was found that four task reward variables(variety, significance, workload and resource inadequacy), one social reward variable(supervisory support) and two organizational reward variables(promotional chances and pay) had significant net effect on hospital nurses' occupational commitment. The implications of these findings were discussed and the suggestions for future research wert advanced.

경제환경 변화에 따른 산업보건인력, 조직 및 직무변화 (A Study on the Occupational Health Personnel, Department and Job Change Before and After Economic Crisis)

  • The purpose of this study is to analysis change of industrial health and safety before and after economic crisis(1997~1998). The data were obtained through mail interview with health supervisor(occupational health nurses) during February, 1999. The final sample was consisted of 127 respondents, 97 are selected in this study. The major findings were as follows: First, the percent of industries whose empl safe supervisors, physicians and industry hygi decreased in number is 79.4%, 3.8%, 50.0 21.5% in each. Second, man receiving over 1.5 million won as an average wage of health supervisor is 27.5% in 1997, but it decreased to 25.3% in 1998. Man receiving under 100~150 million won is 48.4% in 1997, but this percent decreased to 36.3% in 1998 and 13.6% of health supervisor regularly were transformed to contingent worker in 1998. Third, especially, budgets for health and saf work place decreased to 13.8% in 1998 than 19. Fourth, industrial accident case, unre accident case, occupational disease decreased than 1997, but mortality from industrial a increased in 250%. Fifth, according to the health diagnosis report, number of case with evidence of disease increased in liver disease, tuberculosis and so on, and number of case with evidence of occupational disease increased in noise induced hearing loss, pneumoconiosis. organic solvent poisoning and so on. Sixth, this survey shows that health council, health education in duty of health supervisor was conducted passive in 1998 thin 1997, and percent of hold two or more positions is 47.6% in medical insurance, 10.7% in accountant's business 8.3 in secretory. 7.1% in telephone operator awl 22.6% in etc. Seventh, distress of health supervisor after economic crisis is 30.9% in employment instability, 19.8% in hold two or more positions and decrease of budgets. For subjugation of the trouble, the respond of active policy of government occupied most.

Relationship between Comorbid Health Problems and Musculoskeletal Disorders Resulting in Musculoskeletal Complaints and Musculoskeletal Sickness Absence among Employees in Korea

  • Background: To investigate the relationship between musculoskeletal disorders and comorbid health problems, including depression/anxiety disorder, insomnia/sleep disorder, fatigue, and injury by accident, and to determine whether certain physical and psychological factors reduce comorbid health problems. Methods: In total, 29,711 employees were selected from respondents of the Third Korean Working Conditions Survey and categorized into two groups: Musculoskeletal Complaints or Musculoskeletal Sickness Absence. Four self-reported health indicators (overall fatigue, depression/anxiety, insomnia/sleep disorder, and injury by accident) were selected as outcomes, based on their high prevalence in Korea. We used multiple logistic regression analysis to determine the relationship between comorbid health problems, musculoskeletal complaints, and sickness absence. Results: The prevalence of musculoskeletal complaints and musculoskeletal sickness absence due to muscular pain was 32.26% and 0.59%, respectively. Compared to the reference group, depression/anxiety disorder and overall fatigue were 5.2-6.1 times more prevalent in the Musculoskeletal Complaints Group and insomnia/sleep disorder and injury by accident were 7.6-11.0 times more prevalent in the Sickness Absence Group. When adjusted for individual and work-related physical factors, prevalence of all four comorbid health problems were slightly decreased in both groups. Conclusion: Increases in overall fatigue and depression/anxiety disorder were observed in the Musculoskeletal Complaints Group, while increases in insomnia/sleep disorder and injury by accident were observed in the Sickness Absence Group. For management of musculoskeletal complaints and sickness absence in the workplace, differences in health problems between employees with musculoskeletal complaints and those with sickness absence as well as the physical and psychological risk factors should be considered.

Cancer Incidence by Occupation in Korea: Longitudinal Analysis of a Nationwide Cohort

  • Background: We performed this study to investigate the inequalities in site-specific cancer incidences among workers across different occupations in Korea. Methods: Subjects included members of the national employment insurance. Incident cancers among 8,744,603 workers were followed from 1995 to 2007. Occupational groups were classified according to the Korean Standard Occupational Classification. Age-standardized incidence rate ratios were calculated. Results: We found that men in service/sales and blue-collar occupations had elevated rates of esophageal, liver, laryngeal, and lung cancer. Among women, service/sales workers had elevated incidences of cervical cancer. Male prostate cancer, female breast, corpus uteri, and ovarian cancers, as well as male and female colorectal, kidney, and thyroid cancer showed lower incidences among workers in lower socioeconomic occupations. Conclusions: Substantial differences in cancer incidences were found depending on occupation reflecting socioeconomic position, in the Korean working population. Cancer prevention policy should focus on addressing these socioeconomic inequalities.

일반건강검진의 이상지질혈증 검진주기 변경에 대한 문제점 고찰 (Consideration of Dyslipidemia Examination Cycle Change in Korea National Health Checkup Program)

  • Background: Korea National Health Checkup Programs are aimed at the prevention and early detection of cardiovascular disease in adults. To establish a countermeasure for this tendency, The current Korea National Health Checkup Programs have been providing Health Risk Appraisal (HRA) since 2009, thereby focusing on individual lifestyle correction. However, from 2018, the dyslipidemia screening exam cycle has been changed from 2 to 4 years. Methods: In this study, we try to investigate whether policy decisions are valid based on domestic reports that have influenced policy decisions. First, considering the epidemiology of the domestic cardiovascular disease, dyslipidemia, and metabolic syndrome, the change of the 4-year cycle is appropriate or not. Second, whether the research method that applies came to make policy decisions appropriate or not. Third, our study also investigates whether the direction of policy decision was suitable for the second comprehensive national examination plan. Results: The data that are used in the previous study were that of 10 years ago and there also was a problem in selecting the data, especially the use of one of the research methods to calculate the signal to noise ratio that was aimed at improving health had some problems. This is a research method that does not match with the aim itself. Conclusion: Changing the screening cycle for dyslipidemia does not match the recent trend of general screening to effectively prevent cardiovascular disease in improving individual lifestyles in the national health checkup plan. Studying the relationship with metabolic syndrome, which can be an intermediate stage of cardiovascular disease, could be a policy direction that is more suitable for the national health examination comprehensive plan.

중·고령자의 삶의 질 향상을 위한 정책적 제안: 라이프스타일을 중심으로 (A Policy Proposal for Improving the Quality of Life for the Middle-Aged People: Focusing on Lifestyle)

  • 본 연구의 목적은 중·고령자의 삶의 질 향상을 위한 정책적 제안을 위해 중·고령자의 서비스 경험, 우울 수준, 라이프스타일, 삶의 질 간 구조방정식을 통해 경로 효과에 대해 알아보기 위함이다. 제13차 한국복지패널을 활용하여 인구통계학적 분석에 SPSS, 구조방정식 모형 구축을 위해 AMOS를 사용해 분석을 진행하였다. 분석을 통해 서비스 경험, 우울 수준, 라이프스타일 모두 삶의 질에 대해 직접적으로 유의미한 영향을 미쳤으며, 각 경로들 간 경로계수가 모두 유의미함을 알 수 있었다. 이를 통해 중·고령자의 삶의 질 향상을 위해 소득 수준과 상관없이 신체적, 정신적 건강을 위한 라이프스타일 중심 관리 서비스가 필요함을 알 수 있었으며, 이를 위한 정책적 제도와 연구를 통한 프로그램 개발이 필요하다는 기초자료를 제공함에 있어서 본 연구의 함의가 있다.

How Much Does My Work Affect My Health? The Relationships between Working Conditions and Health in an Italian Survey

  • Backround: Working condition surveys are widely recognized as useful tools for monitoring the quality of working life and the improvements introduced by health and safety policy frameworks at the European and national level. The Italian Workers' Compensation Authority carried out a national survey (Insula) to investigate the employer's perceptions related to working conditions and their impact on health. Methods: The present study is based on the data collected from the Italian survey on health and safety at work (INSULA) conducted on a representative sample of the Italian workforce (n = 8,000). This focuses on the relationship between psychosocial risk factors and self-reported health using a set of logistic and linear regression models. Results: Working conditions such as managerial support, job satisfaction, and role act as protective factors on mental and physical health. On the contrary, workers' risk perceptions related to personal exposure to occupational safety and health risks, concern about health conditions, and work-related stress risk exposure determine a poorer state of health. Conclusions: This study highlights the link between working conditions and self-report health, and this aims to provide a contribution in the field of health at work. Findings show that working conditions must be object of specific preventive measures to improve the workers' health and well-being.

Advanced Korean Industrial Safety and Health Policy with Risk Assessment

  • This article describes a systematic roadmap master plan for advanced industrial safety and health policy in Korea, with an emphasis on. Since Korean industries had first emergence of industrial safety and health policy in 1953, enormous efforts have been made on upgrading the relevant laws in order to reflect real situation of industrial work environment in accordance with rapid changes of Korean and global business over three decades. Nevertheless, current policy has major defects; too much techniques-based articles, diverged contents in less organization, combined enforcement and punishments and finally enforcing regulations full of commands and control. These deficiencies have make it difficult to accommodate changes of social, industrial and employment environment in customized fashion. The approach to the solution must be generic at the level of paradigm-shift rather than local modifications and enhancement. The basic idea is to establish a new system integrated with a risk assessment scheme, which encourages employers to apply to their work environment under comprehensive responsibility. The risk assessment scheme is designed to enable to inspect employers' compliances afterwards. A project comprises four yearly phases based on applying zones; initially designating and operating a specified risk zone, gradually expanding the special zones during a period of 3 years (2010-2012) and the final zone expanded to entire nation. In each phase, the intermediate version of the system is updated through a process of precise and unbiased validation in terms of its operability, feasibility and sustainability with building relevant infrastructures as needed.
