A Study on the Knowledge, Attitude and Practice about Industrial Health Care Services of workers -Kwang-ju City and Chonnam Province- (근로자의 산업보건관리에 대한 지식, 태도, 실천 조사연구 -광주, 전남지역-)

  • The purpose of this study was to identify the degree and the relationship of the Knowledge, Attitude and Practice of industrial health care services of workers. This information will provide useful data for an effective industrial health care service. The was collected from 600 data employees in Kwang-ju city & Chonnam province. The data was gathered by questionnaire from Dec 20, 1992 to Jan 20, 1993. The questionnaire was developed by the Academic Affairs of Community Health Nursing Academy. Data was analyzed by using the statistical computer package, SPSS to manipulate the data along with percentages, means, standard deviations, modes, t-test and ANOVA. The results in this study were summarized as follows: 1. General characteristics of workers: Distributions of workers was laborer(76.2%). in the 25-34 age group(50.2%), married(63.3%), males comprised(77.5%), the educational level with the highest percentage was high school graduates (71.1%), with a monthly income of 300-700 thousand won(43.8%), and has been working in the present career over seven years (31.5%). 2. The Knowledge, Attitude and Practice levels of employees about industrial health care services The levels were measured according to a five point scale. The total mean score of knowledge was 2.92 points out of 4. The following are the scores of Knowledge of special medical examination and location of industrial clinic(3.48), periodic medical examination (3.18), occupational disease(3.08), personal protective equipment (2.92), and health education(1.37). The total mean score of Attitude was 2.77 point out of 4. The following are the scores of Attitude in order of working environment (3.35), necessity of periodic medical examination and health education(3.15), the worker's perception influence on the working environment to health was high(3.11). But, the level of satisfaction in the content of periodic medical examination was low(2.19). The total mean score of Practice was 2.70 points out of 4. The scores of Practice in order were, practice of periodic medical examination(3.70), utilization of industrial clinic(2.92), and to participate in health education(1.47). 3. The relationship of general characteristics to Knowledge, Attitude, and Practice of workers: Knowledge had a significant difference by sex (P<.01), marital status(P<.01), education level (P<.05) and monthly income(P<.01). Attitude was significantly different with sex(P<.05) and Practice was significantly related to monthly income (P<.01).

The Relationship between ACE I/D Polymorphism and HDL Cholesterol (ACE 삽입 및 결손 유전자 다형성과 HDL 콜레스테롤과의 관련성)

  • Objectives : The purpose of this study is to evaluate the association of the angiotensin converting enzyme (ACE) insertion/deletion (I/D) polymorphism with cardiovascular disease risk factors. Methods : Out of a total of 608 middle-aged adults who visited local health centers, 424 subjects (104 male, 320 female) who had not been diagnosed with hypertension, diabetes mellitus, or hyperlipidemia were included in this study. ACE genotypes were determined in all subjects by polymerase chain reaction methods. Results : Statistical differences in high-density lipoprotein (HDL) cholesterol levels according to ACE genotype were observed using ANOVA (p<0.05), but no differences were found in other cardiovascular risk factors. Specifically, men with the DD and DI genotypes had significantly lower HDL cholesterol levels than those with the 11 genotype based on the LSD multi-comparison test (p<0.05). Conclusions : In men, the D-allele of the ACE I/D polymorphism was significantly associated with reduced HDL cholesterol levels. In the future, larger studies are needed to confirm this relationship between ACE I/D polymorphism and HDL cholesterol.

Human Epididymis Protein 4 Reference Intervals in a Multiethnic Asian Women Population

  • Background: Ovarian cancer is ranked as the fifth most common cause of cancer death in women. In Malaysia, it is the fourth most common cancer in females. CA125 has been the tumor marker of choice in ovarian cancer but its diagnostic specificity in early stages is only 50%. Hence, there is a critical need to identify an alternative tumor marker that is capable of detecting detect ovarian cancer at an early stage. HE4 is a new tumor marker proposed for the early diagnosis of ovarian cancer and disease recurrence. Currently, none of the normal ranges of HE4 quoted in the literature are based on data for a multiethnic Asian population. Therefore, the aim of this study was to determine reference intervals for HE4 in an Asian population presenting in University Malaya Medical Centre, a tertiary reference hospital. Materials and Methods: 300 healthy women were recruited comprising 150 premenopausal and 150 postmenopausal women, aged from 20-76 years. All women were subjected to a pelvic ultrasonograph and were confirmed to be free from ovarian pathology on recruitment. Serum HE4 levels were determined by chemiluminescent microparticle immunoassay (CMIA, Abbott Architect). The reference intervals were determined following CLSI guidelines (C28-A2) using a non-parametric method. Results: The upper limits of the $95^{th}$ percentile reference interval (90%CI) for all the women collectively were 64.6 pmol/L, and 58.4 pmol/L for premenopausal) and 69.0 pmol/L for postmenopausal. The concentration of HE4 was noted to increase with age especially in women who were more than 50 years old. We also noted that our proposed reference limit was lower compared to the level given by manufacturer Abbott Architect HE4 kit insert (58.4 vs 70 pmol/L for premenopausal group and 69.0 vs 140 pmol/L in the postmenopausal group). The study also showed a significant difference in HE4 concentrations between ethnic groups (Malays and Indians). The levels of HE4 in Indians appeared higher than in Malays (p<0.05), while no significant differences were noted between the Malays and Chinese ethnic groups. Conclusions: More data are needed to establish a reference interval that will better represent the multiethnic Malaysian population. Probably a larger sampling size of equal representation of the Malay, Chinese, Indians as well as the other native ethnic communities will give us a greater confidence on whether genetics plays a role in reference interval determination.

Evaluation of Postural Load during Liquid Weight Measurement Process Using Ratio of Exposure Time

  • The aim of this paper was to prove that if the risk level in combined tasks was improved through evaluation of postural load of liquid weight measurement process, the workload level and ratio of exposure time would be changed, and the time of process would be seen concurrently. Background: According to results of epidemiological studies conducted by Korea Occupational Safety & Health Agency, 122 musculoskeletal disorders occurred during 1992 to 2008, in which manufacturing industry covers 96(78.7%) of total. However, this is an insufficient level and only occupies 39% based on the South Korea's manufacturing standard industrial classification(246 industries). Method: Firstly, the number of batches weighed on one day(460min) was investigated based on the work performed and Weight measured weekly. VCR recording was taken based on the level of liquid ingredients prescribed for 1batch using the Camcorder. After dividing a 356 sec video into 1 sec using the screen capture function in Gom player, the job classification was performed by analyzing the change of working postures, which revealed 148 working postures. Time measurement was decided by time of the postures was being maintained. Then, the REBA analysis was performed for the working postures. The ratio of Exposure time was calculated based on the measurement time and REBA Score. In addition, the recommendations were designed and implementation was carried out for the working postures with REBA Score higher than 3. Finally, after the intervention, REBA measurement, time measurement, and ratio of exposure time were calculated for the comparison of works before and after improvement. Results: The number of work elements was decreased by 30.4% from 148 to 103 after improvement. The results of time measurement showed that the time was reduced by 46.3% from 356 sec to 191 sec. And the ratio of exposure time was also improved by 52.1% from 0% to 52.1% after improvement. Conclusion: The reduction of time was found to improve the productivity of management. Furthermore, because the reduction of ratio of exposure time and the improvement of workload level are the improvement of discomfort, it would contribute to the improvement of the worker's psychological working posture. Application: These results would contribute to musculoskeletal disease prevention and management performance. Further studies for other industries would be needed based on this case study.

Change of Skin Temperature of Workers Using Vibrating Tools in Anthracite Mines (진동공구 사용근로자의 피부온도 변화)

  • By implementing epoch-making policies for industrial promotion, the national economy has made a remarkable development. As a result of such economic growth, industrial accidents and occupational diseases have become a serious problem in Korean society. In the presidential order for the execution of the Korean Labor Standard Law, neuritis and other diseases stemming from health impairments due to vibrations in industrial processes are designated to be dealt with as vibration diseases. In the case of vibration disease, industrial accident compensation is not effectively paid. In order to investigate the vibration hazards of rock-drill operation, the authors studied the subjective symptoms and performed physical function tests on a total of 79 persons (vibration exposed group) who used rock-drills, and 39 persons (control group) who did not use rock-drills at anthracite mines. The results of the physical function test were as follosws : 1. The right hand was more affected by white finger than the left hand. 2. Independent variables such as duration of rock-drill operation, age, drinking and smoking were indentified as statistically significant factors for the occurrence of white finger. 3. In the pain sense threshold, the group with Raynaud's phenomenon showed a statistically higher level than that of the control group. 4. The skin temperature of the group with Raynaud's phenomenon was lower than that of the control group. The recovery time of skin temperature aftr cooling was delayed compared with the value of the control group.

The Study on New Poverty and Change of Poverty Policy in Korea (한국의 신빈곤현상과 탈빈곤정책에 관한 연구: 근로빈곤층(the working poor)의 실태를 중심으로)

  • The object of the study is to examine the change of social-economic structure and poverty-shape to escape poverty. In Korea, the working poor have been increased by flexibility and division of labor market since the economic crisis in 1997, and are faced with hard conditions due to the vulnerable welfare system. Especially the workers who engage in irregular jobs were increased by restructure of labor market. Besides they are in unstable employment terms such as low payment, low-skill and exclusion from welfare-benefit. Many small independent businessmen are also in danger of poverty for enterprises trend to move abroad by globalization. Poverty policy in our country was focused on the absolute poor class that has relation with old age, unemployment, disable, disease etc, so they were the object of welfare policy. The poverties, however, are increasing rapidly after the economic crisis, and they work so hard but are still poor, that is, participation in labor market doesn't become an element to escape poverty. Thus the emergence of new poverties whose core consists of the working poor becomes to need new poverty policy. The study is to survey change of their economic conditions, their welfare conditions, their experiences and responses of social dangers after the economic crisis, then to explore the policy to escape poverty. As the result of the study, it shows that the working poor experienced many kinds of social dangers like unemployment, decrease of income etc. In their welfare conditions as their responses to the social dangers, the benefit of social insurance, enterprise welfare like legal retirement pay and paid leave and private welfare such as private pension and insurance are low. The working poor are faced with social dangers, moreover, they don't have skill or education for adapting themselves to information society. The study says that it needs variable policies for the working poor to escape poverty, and suggests payment & tax policies as stable income policy, occupational discipline and skill-education for promoting the quality of employment, moreover, social insurance as expansion of social welfare policy and housing & education policies whose objects are the working poor.

A Study on The Periodic Medical Examination, and Health Care Management Programs of Industries -Busan City Province- (산업장의 건강검진과 보건관리실태에 관한연구 -부산 지역을 중심으로-)

  • The Purpose of this study was to explore the condition of periodic medical examination and the health care services of industries in order to offer some basic data on developing industrial nursing care. To achieve this goal a self - administered questionnaire (developed by the academic affairs of community health nursing) was provieded to the nurses in 56 industries from Dec. 10, 1992 to Jan. 20, 1993. The statistical computer package, SPSS, was used to manipulate the data along with T-test and ANOVA. The results were as follows : 1. General characteristics: The greater part of the industries were manufacturing company, and below 300 employees of industry were 55.4%. The shift system was mostly one shift(66.1%) and three shift(23.2%), and 50.0% them organized the Industrial Safety and Health Commitee. Average a number of employees was 631 person. 2. Periodic Medical Examination: Most of the workers were receiving periodic medical examination from the designated hospital (95.71%). From the about 12.89% were gone through a colse medical examination. In colse medical examination 58.41% were decided 'C' and 36.73% were decided 'D'. About 6.23% off those who had any clinical findings were work-time shortening(7.84%), work-transition(8.12%), recoverating at home. The majority of the workers receive the result of the periodic medical examination individually (78.5%). 3. Special medical examination: The rate of those who are receiving special medical examination were 76.82% and about 8.24% were decided 'C' and 1.23% were decided 'D' Those who had any health problems were receiving follow-up checking (9.10%) and medical treatment while working (15.04%). The health managers in the company can consult (85.7%) those who had any suspicious sign and symptoms of occupational disease. 4. Health care services: The average score of health care services were 17.57 point out of 28 point, and the score was lower in health assessment and environmental hygiene than medical diagnosis and health education. There were significant differences in environmental hygine (F=3.72, P=0.017), health care services(F=3.94, P=0.013) according to the size of the size of the industries The other's significancy is not shown by any type of industrial nurse. The level of health care services were higher in the wokers who had better health and showed no singificant differences(T=-0.73, P=0.470).

Analysis of the Work-related Accidents and Diseases in Gunsan, Buan, and Gochang Areas from 2002~2011 (군산시, 부안군 및 고창군의 2002~2011년의 산업재해 분석)

  • Objectives: This study was conducted to assess the trend of work-related accident rate in the past 10 years (2002~2011) in Gusan district, Ministry of Employment and Labor, Korea. Methods: To analyze the change of work-related accident rate in the past 10 years in Gunsan District, we analyzed the work-related accident and disease by the type of business, business scale and analyzing the victim's age and job period using workplace information management system(PKMS) in the Ministry of Employment and Labor. Results: The average rate of work-related accidents in Gunsan district was 0.96% in the past 10 years and it was a little higher than that of national average 0.75%. The rates of work-related accidents tended to decrease from 1.11% in 2002, to 1.09% in 2006, and to 0.79% in 2011. Mortality (per 10,000) rates also tended to decrease in the years 2002, 2006, and 2011 as 3.07, 2.46, and 1.53. By the type of business, the average rate of work-related accidents in the past 10 years was the highest in agriculture as 1.77%, followed by forestry farming at 1.76%. By the business scale, the average rate of work-related accidents was the highest in the group of under 5 employees as 2.05% and it was the lowest in the group of more than 300 employees as 0.50%. The ratio of work-related accidents in the 30's age group was decreased from 41.7% in 2002 to 25.2% in 2011 and the ratio for the 50's age group was increased from 28.6% in 2002 to 49.5% in 2011. Conclusions: Although the rate of work-related accident was decreased in the past 10 years, the polarization of work-related accidents rated by the business scale and age groups show a significant difference. So it is needed that the planning and implementing of work-related accidents prevention policies.

Clean Room Structure, Air Conditioning and Contamination Control Systems in the Semiconductor Fabrication Process (반도체 웨이퍼 제조공정 클린룸 구조, 공기조화 및 오염제어시스템)

  • Objectives: The purpose of this study was to examine clean room(C/R) structure, air conditioning and contamination control systems and to provide basic information for identifying a correlation between the semiconductor work environment and workers' disease. Methods: This study was conducted at 200 mm and 300 mm semiconductor wafer fabrication facilities. The C/R structure and air conditioning method were investigated using basic engineering data from documentation for C/R construction. Furthermore, contamination parameters such as airborne particles, temperature, humidity, acids, ammonia, organic compounds, and vibration in the C/R were based on the International Technology Roadmap for Semiconductors(ITRS). The properties of contamination control systems and the current status of monitoring of various contaminants in the C/R were investigated. Results: 200 mm and 300 mm wafer fabrication facilities were divided into fab(C/R) and sub fab(Plenum), and fab, clean sub fab and facility sub fab, respectively. Fresh air(FA) is supplied in the plenum or clean sub fab by the outdoor air handling unit system which purifies outdoor air. FA supply or contaminated indoor air ventilation rates in the 200 mm and 300 mm wafer fabrication facilities are approximately 10-25%. Furthermore, semiconductor clean rooms strictly controlled airborne particles(${\leq}1,000{\sharp}/ft^3$), temperature($23{\pm}0.5^{\circ}C$), humidity($45{\pm}5%$), air velocity(0.4 m/s), air change(60-80 cycles/hr), vibration(${\leq}1cm/s^2$), and differential pressure(atmospheric pressure$+1.0-2.5mmH_2O$) through air handling and contamination control systems. In addition, acids, alkali and ozone are managed at less than internal criteria by chemical filters. Conclusions: Semiconductor clean rooms can be a pleasant environment for workers as well as semiconductor devices. However, based on the precautionary principle, it may be necessary to continuously improve semiconductor processes and the work environment.

Bactericidal Efficacy of Vital-Oxide®, Disinfectant Solution Against Salmonella Typhimurium and Brucella Ovis

  • $Salmonella$ spp. and $Brucella$ spp. have caused a considerable disease of farmed animals and economic loss in animal farming and food industry. In this study, the disinfection efficacy of Vital-$Oxidel^{(R)}$, a commercial disinfectant, composed to chlorine dioxide, betaine hydrochloride, and propylene glycol was evaluated against $S.$ $typhimurium$ and $Brucella$ $ovis$. A bactericidal efficacy test by broth dilution method was used to determine the lowest effective dilution of the disinfectant following exposure to test bacteria for 30 min at $4^{\circ}C$. Vital-$Oixdel^{(R)}$ and test bacteria were diluted with distilled water (DW), hard water (HW) or organic matter suspension (OM) according to treatment condition. On OM condition, the bactericidal activity of Vital-$Oixdel^{(R)}$ against S. typhimurium and Brucella ovis was lowered compared to that on HW condition. As Vital-$Oxidel^{(R)}$ possesses bactericidal efficacy against animal pathogenic bacteria such as $S.$ $typhimurium$ and $Brucella$ $ovis$, this disinfectant solution can be used to control the spread of bacterial diseases.
