Intrapelvic Urethral Anastomosis in a Dog with Complete Obstruction of Proximal Membranous Urethra (막성요도 근위부 완전폐쇄를 지닌 개에서 골반내 요도문합술)

  • A 4.65 kg 13-month-old male Pekingese dog was referred to veterinary teaching hospital of Konkuk University for evaluation of dysuria. On physical examination, severe distention of urinary bladder was found in abdominal palpation. Urinary catheter could not be guided into urinary bladder. On serum biochemistry, blood urea nitrogen (35.6 mg/dl) and creatinine (1.9 mg/dl) were increased. Obstruction part of proximal membranous urethra was founded on urethrogram. The length (13 mm) of obstruction part was callipered by cystourethrogram and urethrogram on operation. Surgical resection of obstruction part of urethra was performed without pubic osteotomy, and anastomosis was performed with 5-0 polyglycolic acid. Omentum was placed around the urethral anastomosis. On first day after surgery, appetite was good. On day 5, complete blood count and serum biochemistry showed normal range but mild urinary incontinence was showed after removing catheter. On day 7, urinalysis showed normal condition. On day 14, no leakage of surgical site was observed in excretory urogram. On day 21, no more urinary incontinence and good micturition were found. On 1 year later, the patient showed healthy condition without recurrence.

  • Thyroid hematoma secondary to a blunt trauma is a rare problem. Despite the rarity, it can be a life-threatening condition with tracheal compression. Both surgical exploration and conservative management have been suggested for thyroid hematoma. However, there is still controversy on the optimal treatment. A 67-year old man who progressed severe dyspnea and neck swelling was transferred to the emergency department with a blunt anterior neck trauma after traffic accident. Contrast enhanced neck computed tomography scan showed huge hematoma within the right thyroid gland and slight tracheal deviation without prominent airway obstruction. One day later, anterior neck swelling was aggravated and the patient was intubated to prevent airway obstruction. After 3 days, hematoma resolution was revealed and extubation was done. We report this case with a review of literature.

  • Annular pancreas is a rare congenital anomaly with the descending duodenum encircled by a ring of pancreatic tissue, which may cause partial or complete obstruction of the duodenum. In newborn, the symptoms can be those of duodenal stasis resulting from partial intestinal obstruction secondary to some degree of duodenal stenosis. A male newborn weighing 2.0 Kg was born by C-section delivery at 37 weeks' gestation to a 27-year-old mother who had a hydramnios. He was in relatively good condition at birth except regurgitation of saliva and intermittent apnea. A plain film of the abdomen showed the double-bubble of gas filled stomach and proximal duodenum, and upper gastrointestinal series showed a dilated proximal duodenum, with a complete obstruction of the descending duodenum. Intraoperative finding revealed encirclement of the duodenal second portion by pancreatic tissue. Duodenojejunostomy was performed. After the operation, he had developed two serious complications, neonatal septicemia by Enterobacter cloacae on postoperative day 12 and systemic candidiasis on postoperative day 19, and been managed with ventilatory support, antibiotics, and antifungal agents with recovery.

  • Pulsus paradoxus has been defined as a decrease in systolic blood pressure (SBP) of 10 mmHg or more during inspiration. This report describes pulsus paradoxus detected by pulse oximetry during dental procedure. Case: A 10 years old boy who had impacted mandibular premolar with malformation scheduled for extraction under intravenous sedation with Fentanyl and Propofol. The patient showed upper airway obstruction with stridor and pulsus paradoxus. Though pulsus paradoxus is generally critical condition, in this case, respiration and other vital sign was maintained comparatively well with care in administering oxygen and considerate monitoring of pulse oximetry and capnography. Discussion: Noninvasive continuous monitoring of pulse oximetry allows recognition of pulsus paradoxus which can lead to serious problems. Clinicians should know very well about it and be able to manage of this kind of situation.

  • Control of ground water is one of the most important factors for long-term operation of tunnel because most tunnels are located under the ground water level. In case of a drainage tunnel, there is no pore water pressure on the lining when the drainage system is properly working. After long-term operation, however, residual pore water pressure can be developed on the lining due to the deterioration of the drainage system. In this study, the water pressure distribution under obstruction condition of drainage material and conduit on the tunnel is numerically investigated using the ICFEP program and compared with the current value being applied to the residual water pressure for rational application plan of residual water pressure on the tunnel linings.

Surgical Treatment of Hemolytic Anemia Induced by Residual VSD and PS (잔여 심실중격결손과 우심실 유출로 협착으로 유발된 용혈성 빈혈의 외과적 치험)

  • A patient with intravascular hemolysis due to residual shunt and right ventricular outflow track obstruction after total correction of TOF was presented. The patient was 29 years old female. She underwented VSD closure with dacron patch, infundibulectomy, pulmonic valvotomy and direct closure of PFO. 8 months after the operation, severe intravascular hemolysis and hemolytic anemia appeared. Conservative therapies were not effective, her general condition and laboratory finding got worse gradually. She underwent reoperation, the shunt was closed and right ventricular outflow tract obstruction was corrected by pulmonary valvotomy, infudibulectomy and transannular patch. After operation, hemolysis disappeared dramatically. Severe hemolysis may induce renal failure and necessitate transfusion frequently. If hemolytic anemia is not corrected by conservative treatment, early reoperation is required.

Evaluation of Thermal Utilization of Dousing System in PHWR Nuclear Power Plant

  • An effectiveness of thermal utilization of a dousing system in the 600 MW PHWR Nuclear Power Plant has been evaluated. The behavior and conditions of water droplet sprayed in a postulated accident conditions in containment configuration has been calculated. In this calculation, two pressure conditions with the consideration of obstruction area and containment wall effect has been established : one being the minimum containment pressure of 7 kPa(g) encountered for dousing shut off and the other being the containment design pressure 124 kPa(g). The results revealed that the effectiveness of the thermal utilization ranges from 93% to 97%. In the analysis on two cases without/with side wall effect in the containment building, the thermal utilization decreases with obstruction area from 89% to 85%, which satisfies the design criteria set for the containment pressure against the accident condition.

Postobstructive Pulmonary Edema in a Yorkshire Terrier Dog

  • A 6 month-old castrated male Yorkshire terrier (weighing 1.0 kg) was presented with acute respiratory distress. Diagnostic imaging studies found post-obstructive pulmonary edema sequel to upper airway obstruction by a rubber plug lodged at thoracic esophagus. The rubber plug was removed endoscopically. After removal of this rubber plug with conventional therapy for pulmonary edema, the clinical condition of dog was stabilized. To the best knowledge of authors, this is the first case report describing postobstructive pulmonary edema in a dog in Korea.

  • Nasal septoplasty is often required to correct a cosmetic deformity, which is a common reason for patients to present to a plastic surgeon. If nasal septoplasty is insufficient, a residual deformity or nasal obstruction may remain after surgery. Even if the nasal septum is corrected to an appropriate position, nasal congestion could be exacerbated if the turbinate on the other side is not also corrected. Therefore, appropriate treatment is required based on the condition of the turbinates. Herein, we survey recent trends in treatment and review previous research papers on turbinoplasty procedures that can be performed alongside nasal septoplasty.

  • Chronic cough is a common problem that can be refractory to medical treatment. Nonpharmaceutical management of chronic cough has an important role in well selected patients. This review article outlines the history of chronic cough management, current approaches to speech pathology management of the condition and new modalities of nonpharmaceutical treatment. There is a need for further research into nonpharmaceutical options with well described randomised control trials.
