A Literature Review on Obesity in Postmenopausal Women (폐경기 비만의 임상적 의의에 대한 문헌적 고찰)

  • Objective This study was designed to collect latest papers and to find more effective approach for Postmenopausal Women Methods We searched 85 papers in English from Pubmed(www.ncbi.nlm.nih.gov) and selected 40 in favor of our study. In addition to that, we searched 24 papers in Korean from Kiss(www.kiss.kstudy.com) and Medric(www.medric.or.kr). Results are as follows. Results and conclusion Many studies showed that postmenopausal obesity causes cardiovascular disease, osteoporosis, breast cancer and decrease in immunity. To prevent and treat obesity, many studies suggested that dietary and exercise are most effective methods. If needed, medicine and hormone therapy may be recommended as next steps. But, Herb medication and acupuncture are not used yet as postmenopausal obesity treatment around the world. Postmenopausal obesity is significant, resulting in many medical problems. We hope more effective treatment based on these studies and combined with Korean traditional medicine could be developed after.

The Comparison of Pattern Identification Diagnosis According to Symptom Scale Based on Obesity Pattern Identification Questionnaire (한방비만병증 설문지를 바탕으로 증상 척도에 따른 변증진단 비교)

  • The study was to investigate the distribution for the diagnosis of pattern identification questionnaire and agreement rate between diagnosis of pattern identification based on obesity pattern identification questionnaire and the clinical diagnosis of pattern' identification by medical specialist. The distribution for the diagnosis of pattern identification based on obesity pattern identification questionnaire was shown in order of stagnation of liver Gi, retention of undigested food, deficiency of Yang at scale of 5, 3, 2 score and the diagnosis rate of single pattern identification at scale of 5, 3, 2 score was 89.96%, 79.33%, 54.64%, respectively the agreement rate between the diagnosis of pattern identification based on obesity pattern identification questionnaire and the clinical diagnosis of pattern identification by medical specialist was 0.1013. Therefore, the complementary management in CRF questionnaires with consultation from experts and the study for score difference of pattern identification will improve the accuracy and agreement rate, which will will be helpful for pattern identification of obesity by clinical experts.

Effects of the Obesity Therapy with Korean Herbal Medicine on Liver Function: Case Series (한방비만치료가 비만환자의 간기능에 미치는 영향 증례보고)

  • Many obesity patients were affected by depressed liver function including hepatosteatosis. The aim of this study was to investigate the clinical effect of the obesity therapy with Korean herbal medicine (KHM) on obese patient with abnormal liver function. This is a retrospective observational study performed on obesity patients with abnormal liver function. All subjects have taken serum liver function tests (LFTs) before treatment. Ten patients showed abnormal LFTs. They were treated with KHM at least 15 days. During this treatment, LFTs were evaluated afterwards. Body weight and body mass index of the patients were decreased during treatments. Five of 10 subjects recovered normal levels of LFTs. While other subjects remained at abnormal level but their levels were decreased. This study shows that the obesity therapy including KHM could be effective to improve liver function on obese patient. Additional large scale studies are necessary for more evidences.

Abdominal Obesity as a Risk Factor of Ischemic Stroke in Case-control study (복부비만과 허혈성 뇌졸중)

  • Objective: The purposes of this study are to know the relations of abdominal obesity and ischemic stroke and to know what index could represent the abdominal obesity appropriately. Methods. We have done case-control study and recruited 97 ischemic stroke patients and 117 controls. Case is matched by control by individual matching. All participants had questionnaire, interview and then were examined waist-hip ratio, waist circumference and body mass index. Results : WC, WHR and Hypertension history had differences in two groups, case and control groups. But BMI and other factors weren't significant. According to Quartile of Waist Circumference, the two groups had the differences in hypertension history, diabetes history, smoking status, WHR, BMI, and weight and in the Quartile of Waist-Hip Ratio Quartile, past history of hypertension and diabetes, WC, BMI and weight had the significances. Sex, Age Adjusted and Multivariate Odds Ratio (95% Confidence Interval) of WC Quartile are 2.083, 1.628, 4.491 and 4.418, 4.964, 12.306, and in WHR, the Ors are 2.252, 5.743, 15.776 and 2.632, 8.918, 23.596. Conclusions: We knew from these results that abdominal obesity is very important risk factor of ischemic stroke and WHR more than WC is a good indicator of abdominal obesity, so we should reduce the WHR to prevent of ischemic stroke.

The Case Study of Alcohol & Food Habits in Obese Adult Men (비만한 성인남성의 음주 및 식이습관의 특성연구)

  • Objective: Obesity is closely related to several factors such as genetic factors, energy intake and energy expenditure. It was said that alcohol & food habits were important to formation of obesity. The purpose of this study are to find out the alcohol & food habitual characteristic of obese adult men and to promote the health of these men. Method: From May, 2000 to November, 2001, We collected 75 eases obese patients, who visited to the Health Examination Center Dept. of Kangnam oriental medicine hospital, Dongguk University who took the health examination, and control group who were matched by age and sex. We investigated the alcohol & food habits by questionnaire. Results and Conclusions: 1. The patients over BMI 25 were in excess of the standard of obesity such as Modified Broca $^{\circ}{{\O}}s$ Index. Percent Body Fat, Abdominal Fat Distribution. But, the patients below BMI 20 were normal range of obesity degree. 2. Abdominal Fat Distribution was increased according to age and physical activity was decreased in obese group. 3. The mean of total calory by alcohol in a month was 4324.6kcal in obese group while control group was 2206.8kcal. 4. In the comparison of food habit, obese group is higher than control group in 7 articles of the 13 articles which influenced obesity.

Effects of the Obesity Therapy with Korean Herbal Medicine on Menstrual Cycle Disorder: Case Series (한방비만치료가 월경주기 이상에 미치는 영향: 증례보고)

  • Obesity is associated with menstrual cycle disorder. The aim of this study was to investigate the clinical effect of the obesity therapy with Korean herbal medicine (KHM) on obese female patient with oligomenorrhea or amenorrhea. This is a retrospective observational study performed on obesity patients with oligomenorrhea or amenorrhea. Nine patients were treated with KHM and lifestyle management at least 2 months. During the treatment, body weight, body mass index (BMI), percent body fat (PBF) and menstrual cycle length were checked afterwards. Body weight, BMI, and PBF of the patients were decreased during treatments. Five of 9 subjects recovered to normal menstrual cycle. Two subjects improved menstrual cycle. The other subjects remained at oligomenorrhea or amenorrhea. This study shows that the obesity therapy including KHM could be effective to improve menstrual cycle on obese patient. Further large scale studies are necessary for more evidences.

The Systematic Review on Clinical Studies of Traditional Korean Medicine Treatment for Obesity in Menopausal Women (폐경 여성의 비만에 응용되는 한방치료에 대한 문헌 고찰)

  • Objectives: The purpose of this study is to review clinical studies of herb medicine and acupuncture treatment on obesity in menopausal women. Methods: Key words "Obesity", "Menopause", "Herb medicine", "Acupuncture", "Moxibustion", "Catgut embedding" were searched on 9 database systems (PubMed Central, Cochrane Controlled Register of Trials, Embase, China Academic Journals, Korean Traditional Knowledge Portal, Oriental Medicine Advanced Searching Integrated System, Korean Studies Information Service System, National Digital Science Library, DBpia) on April 30th 2019. Results: 1 case report and 17 clinical trials were collected in accordance with the selection and exclusion criteria. Among the 17 trials, 6 were randomized controlled trials, 1 was controlled clinical trial, and 10 were single-arm trials. The types of intervention were herb medicine, electroacupuncture, acupuncture, auricular acupuncture, warm needle acupuncture, moxibustion, laser acupuncture, and catgut embedding. The study design, study results and method of intervention were analyzed. Conclusions: 1 case report described the effectiveness of pharmacopuncture, 4 trials described the effectiveness of herbal medicine, 2 of electroacupuncture, 1 of laser acupuncture, and 2 of catgut embedding. Among the 17 trials, 2 studies showed that herbal medicine treatment was more effective than no treatment or selective serotonin reuptake inhibitors, and 1 study showed that electroacupuncture was more effective than hormone therapy. All of 18 selected studies reported the effectiveness of weight reduction and abdominal obesity reduction after the traditional Korean medicine treatment for obesity in menopausal women.

Analysis of Lower-body Obesity-type and Development of Suit-pants Size System for the Abdomen-obese Adult Males (복부비만 성인 남성을 위한 하반신 비만 유형분석과 정장 하의류 치수체계 연구)

  • This study helps out-size consumers purchase ready-made clothes and improve fitness by classifying the lower bodies of abdomen-obese adult males in their 20s and older; in addition, it creates a size system based on each size interval of obesity type. The criteria for the 559 male subjects surveyed in this study were over $25kg/m^2$ of BMI, over 90 cm of waist, and over 0.85 waist hip ratio. The results are as follows. First, the higher the age group, the higher the degree of abdominal obesity in each age group. The degree of obesity then decreases somewhat as their age increases; however, the risk of abdominal obesity also increases. Second, 3 clusters were categorized by cluster analysis, into abdominal obesity, larger oval-type, trapezoid-type and small cylinder-type. Third, the size system establishment according to lower-body types resulted in basic body sizes and reference body sizes being different according to types even in commonly-appeared size names in the sections of respective types. The above research findings show it is necessary to understand obesity types according to waist and hip sizes that represent basic sizes and to design patterns in consideration of the characteristics of obese body shapes when lower-half body clothes are designed among obesity groups.

Relationship between Bisphenol A Exposure and Obesity in Korean Adults from the Second Stage of KoNEHS (2012-2014) (한국 성인의 비스페놀 A 노출과 비만과의 관련성 연구: 제2기 국민환경보건기초조사(2012-2014))

  • Objectives: Bisphenol A (BPA) has been extensively used in a variety of consumer products, resulting in widespread non-occupational human exposure. It is often detected in the human body. Studies have reported many health effects associated with endocrine and metabolic disruptions, including obesity, diabetes, hypertension, and cardiovascular diseases. This study was performed to explain the relationship between BPA exposure and obesity in the Korean adult population. Methods: The second stage of the Korean National Environmental Health Survey (KoNHES) was conducted from 2012 to 2014 with 6,478 persons participating. Using the results of the survey, we analyzed the exposure levels for BPA and the influence on obesity of BPA. Results: In model 1, the volume-based measure concentration of BPA, total, female and the 30s to 60s age group were positively related with BMI. In model 2, creatinine adjusted as a covariate and positive associations for BPA with BMI were observed in the female group and was marginally significantly associated in low body weight group. In model 3, creatinine adjusted (g/g-creatinine), BPA exposure, and BMI were positively related with sex, in females, and there was a marginally significant association with the low body weight group in the BMI categories. BMI was significantly associated with BPA in the female group in all three models. Conclusion: This study added further evidence that exposure to EDCs, include bisphenol A, is related with obesity among the general population. Given the environmental health concerns over BPA, it is necessary to develop comprehensive measures to reduce BPA exposure.
