Objectives: Acceptability of patients towards obesity treatment program is associated with better weight loss outcomes. The purpose of this study was to review previously published study results of the predictive factors associated with patients' acceptability in obesity treatment. Methods: Authors searched for the articles related to acceptability reported as continuation, attendance and adherence, published from 2011 to 2018 found on Pubmed, Scopus, Research Information Sharing Service, and Koreanstudies Information Service System. A total of 23 articles were finally selected. From the study results, unchangeable and changeable predictors were extracted, and these predictors were examined according to detail categories. Results: Regarding the continuation of the treatment, unchangeable factors such as younger age, lower educational level, male sexuality and lower accessibility to physical activity predicted lower continuation. Furthermore, changeable factors such as early and half weight loss, better accessibility to the treatment and financial incentives for retention predicted higher continuation rate. Greater degree of attendance was predicted by unchangeable factors such as economical affordability, and changeable predictors such as half weight loss and proximity to the clinic. Main factors of adherence to the recommendation were unchangeable predictors such as weight loss experience, and changeable predictors such as more physical activity and appropriate dietary habits. Negative psychological state predicted lower continuation, attendance and adherence rate. Conclusions: Our review results suggest that unchangeable and changeable predictors of acceptability of patients should be carefully examined during treatments of obesity.
BACKGROUD/OBJECTIVES: Obesity is globally a major public health issue. Evidence suggests that elevated ferritin levels are associated with obesity, dyslipidemia, insulin resistance, and metabolic syndrome. This study was undertaken to examine the relationship between the serum ferritin level and depression in Korean male adults with respect to classification of the prevailing obesity. SUBJECTS/METHODS: This was a case-control study; subjects were classified into obese group (${\geq}25.0kg/m^2$, 28 subjects) and normal group ($18.5-22.9kg/m^2$, 27 subjects). A survey was conducted to assess the depression levels as per the guidelines suggested by the Center program for Epidemiological Studies-Depression (CES-D). Blood was collected from each group for assessing biomarkers, and isolated plasma was evaluated for fasting glucose, insulin, quantitative insulin sensitivity check index, and ferritin levels. Data were analyzed, and groups were compared with respect to Body Mass Index (BMI), depression scale and biomarkers. RESULTS: The average depression score of the obesity group was 16.86, which was higher than the normal group (12.56). Subjects scoring more than 16 points comprised 53.6% of the population in the obese group, which was more than double that in the normal group, as assessed by the CES-D program. Furthermore, the serum ferritin level of the obesity group was 207.12 ng/mL, which was higher than that of the normal group (132.66 ng/mL). Lastly, the BMI appeared to be significantly correlated with both depression (r = 0.320, P = 0.017) and elevated ferritin levels (r = 0.352, P = 0.008). CONCLUSION: This study provides evidence of existing correlation between ferritin and depression with obesity.
This study was conducted to examine the effects of an 8-week weight loss program, on body composition, body shape satisfaction, body shape esteem in 15 obese children. This program included nutritional education and regular physical fitness. After completion of this program, height and body weight increased slightly but body compositions changed. BMI, degree of obesity, and body fat levels dropped slightly, however these differences were not significant. After the 8-week program body weight increased from $63.61{\pm}8.80\;kg$ to $64.07{\pm}9.11\;kg$, degree of obesity and BMI decreased from $151.73{\pm}13.62%$, and $29.08{\pm}2.35$ to $146.60{\pm}12.92%$, and $28.57{\pm}2.26$, respectively, and the percentage of body fat decreased slightly from $42.70{\pm}4.87%$ to $41.46{\pm}4.88%$. There were slight changes in weight of body muscle and lean body mass from $19.62{\pm}3.48\;kg$ and $36.17{\pm}5.78\;kg$ to $20.30{\pm}3.64\;kg$ and $37.16{\pm}6.18\;kg$, respectively. LBM and amount of muscle were higher after this weight loss program. The body shape esteem score increased from $7.60{\pm}4.01$ to $9.00{\pm}6.20$. There was no significant difference in body shape satisfaction before and after the program, and the participants also wanted to be leaner. Finally, body weight, BMI, and body fat showed negative correlations to body shape esteem.
The obesity incidence rate of children in Korea has increased from 2 to 3% in 1970 to 15% in 2004. The purpose of this study was to evaluate the effect of diet on obesity, and serum cholesterol and triglyceride concentrations by obese index in Korean elementary school students. The subjects were three hundred and thirteen students(male 157, female 156). Height, weight and body fat were measured. Dietary records by the 24-hr recall method were taken for 3 days, and serum cholesterol and triglyceride concentrations were determined. The obesity index was calculated by the Korean of Pediatrics Association standards. Nutrient intakes were calculated by CAN PRO. Almost twenty percent(19.8%) of the subjects were underweight, 60.1 % were normal, 10.2% were overweight and 9.9% were obesity. The obesity incidence rate of the males was not different from the females. Energy and nutrients intakes containing fatty acids were not significantly different by the obesity index. All the subjects had 59.7%~61.1% of carbohydrate and 24.2%~25.6% of fat for energy. Serum cholesterol concentrations of the obesity group(177.3 mg/dl) was higher than those of the underweight group(161.8 mg/dl) and normal weight group(163.5 mg/dl) and overweight group(163.8 mg/dl). Twenty nine percent of the obesity group had serum cholesterol concentrations over 200mg/dl, as compared with 4.8% of the normal and underweight group. Serum triglyceride in the obesity group(103.0mg/dl) was higher than those of the underweight group(79.7 mg/dL), normal weight group(81.0 mg/dl) and overweight group(81.1 mg/dl). Serum total cholesterol concentrations were correlated with the obese index, PIBW, R hrer index at p<0.001, with BMI at p<0.01 and with waist/hip ratio at p<0.05. Serum triglyceride concentrations were correlated with the obese index, PIBW, R hrer index at p<0.01. It is concluded that obesity increases serum total cholesterol concentrations and triglyceride concentrations, and that the obesity preventive program for normal weight children, and weight reduction program for obese children are necessary for elementary school children's education.
Objectives: This study was performed to identify the current barriers of obesity management for children using Community Child Care Centers and their caregivers (parents and teachers working in the Centers). Further, this study explored the possibility of utilizing a mobile phone application for tailored obesity prevention and management programs to overcome the current difficulties associated with children's obesity management. Methods: The qualitative data were collected through in-depth interviews with 20 obese and overweight children or children who wanted to participate in this study using Community Child Care Centers, 12 teachers working at the Centers, and a focus group interview with five parents of children using the Centers. Data were analyzed with a thematic approach categorizing themes and sub-themes based on the transcripts. Results: The current barriers of obesity management of obese and overweight children using Community Child Care Centers were lack of self-directed motivation regarding obesity management (chronic obesity-induced lifestyles and reduced self-confidence due to stigma) and lack of support from households and Community Child Care Centers (latchkey child, inconsistency in dietary guidance between the Center and household, repetitive pressure to eat, and absence of regular nutrition education). Mobile phone applications may have potential to overcome the current barriers by providing handy and interesting obesity management based on visual media (real-time tracking of lifestyles using behavior records and social support using gamification), environmental support (supplementation of parental care and network-based education between the Community Child Care Center and household), and individualized intervention (encouragement of tailored and gradual changes in eating habits and tailored goal setting). It is predicted that the real-time mobile phone program will provide information for improving nutritional knowledge and behavioral skills as well as lead to sustainable children's coping strategies regarding obesity management. In addition, it is expected that environmental factors may be improved by network-based education between the Community Child Care Centers and households using the characteristics of mobile phones, which are free from space and time constraints. Conclusions: The tailored education program for children using Community Child Care Centers based on mobile phones may prevent and reduce childhood obesity by overcoming the current barriers of obesity management for children, providing environmental and individualized support to promote healthy lifestyles and quality of life in the future.
Purpose: Health professionals and policy makers confront the failure of provider-administered, conventional behavioral interventions in the fight against obesity epidemic. The aim of this study was to develop a tailored, cost-effective delivery system for a child obesity prevention and management program through technology convergence using Web-enabled smart cellular phones. Methods: Assessment of service needs and development of a delivery system for the program were based on a comprehensive literature review and expert reviews, and results from in-depth interviews and a need-assessment survey. Results: The user- and site-centered service delivery system using Web-enabled cellular telephones as the hardware platform for obesity prevention and management has been developed. A tailored informational service and intervention will be provided interactively between stakeholders through the platform. The potential legal issues associated with the service design have also been considered. Conclusion: The user-centered convergence design and platform based on principles of Transtheoretical Model and Stages of Change using the Health Promoting School framework could enable effective intervention and promote acceptance in the long-run.
The purpose of the study was to investigate the status of the nutritional education program on educational perception, facilities, contents and systems, which were focused on the school obesity education in the elementary school. A five point Likert scale was used to evaluate the importance and performance of each item in four sections of the survey and Importance-Performance Analysis (IPA; gap between importance and performance) matrix analysis was applied to determine improvement priority. The findings from IPA matrix analysis are in the following. The items of the sections to be improved intensively are recognition of parents and in-charge teacher, availability of the education place, preparation of consulting room, preparation of various teaching media, interest of an education program, meal adjustment of overweight children, combination with education and exercise, education for psychological factors, development of scientific and systematic education program and reduction of meal service duty for education. Accordingly, the IPA matrix analysis suggested that an intensive improvement area should be excessively concentrated on for better performance. The regional gap between importance and performance showed significant difference for textbook and diet of overweight children in Kyunggi-Do. There were significant differences for the government perception, dietary habits and food-life education, fast-food education, education for breakfast importance, and education of school meal indication system and method to read food nutrition indication in Chollabuk-Do. As for the age gap between importance and performance, the twenties showed significantly bigger gaps in perception of school dietitians and linking between school meals and diet education. Therefore, more proactive efforts for the education for obesity prevention are necessary to prevent childhood obesity in elementary school and to help children to possess better health throughout their entire lives.
본 연구는 20대 비만 여대생의 복합운동 시 운동화 속 인솔의 높이의 차에 따른 비만관련 신체생리 및 체력적 기능 변화를 알아보기 위해 실시하였다. 비만 여대생 36명을 인솔의 높이에 따라 0cm군, 5cm군, 9cm군으로 군당 12명씩 분류하여 비만 관련 신체 생리는 기초대사량과 체지방률을 측정하였고, 건강 관련 체력은 넙다리네갈래근 등속성 근력, 근지구력, 유연성, 순발력을 측정하여 운동 전과 후 각 그룹 간을 효과적 차이를 비교 분석하였다. 그 결과 넙다리네갈래근의 등속성 근력과 기초대사량은 세 군 모두 증가하였으며, 5cm군이 0cm군과 9cm군에 비해 더 유의하게 증가된 것으로 나타났다. 결론적으로 적절한 인솔의 높이를 적용한 복합운동은 비만 여대생의 근력과 기초대사량에 긍정적인 영향을 미친다.
Background & Methods: In order to study the effect of electro-lipolysis-acupuncture, 57 outpatients who have had herbal medication in Kirin Oriental Hospital were devided to two groups; acupuncture-treated group(46) vs non-acupuncture-treated group(11) Acupuncture-treated group was given electro-lipolysis-acupuncture at least two times a week and administered with herbal diet medication, whereas non-acupuncture-treated group was administered only with herbal diet medication. Results: Mean age, body weight and BMI of these two group at the start point of the treatment were $27.22({\pm}7.64)\;vs \;29.09({\pm}8.73),\;67.76({\pm}9.34)\;vs\;67.00({\pm}10.69),\;and\;26.20({\pm}3.02)\;vs\;26.14({\pm}4.10)$ in the order of acupuncture-treated and non-acupuncture groups. After one month of treatment, change rate of body weight, BMI, total fat, percentage of fat was significantly higher in acupuncture-treated group. Also the change rate of arm circumference, hip circumference and thigh circumference was significantly higher in acupuncture-treated group. The change rate of abdominal circumference and WHR of acupuncture-treated group was higher than that of non-acupuncture-treated group with no statistical significance. The circumference reduction rate according to herbal dieting program was higher in this order, abdomen, arms, chest, thighs and hips. On the other hand, chest circumference change rate over weight loss rate, which is the portion no acupuncture treatment was given to, and AMC change rate was higher in non-acupuncture-treated group. Conclusion: These results imply that herbal dieting program combined with electro-lipolysis-acupuncture is more effective on reducing body fat, size reduction of localized obesity and preservation of muscle than herbal prescription only dieting program.
A weight control program has been prescribed to investigate its effects on weight reduction of obese adults. The prescribed weight control program has been followed by 4 overweight adults and fad diets has been followed by 6 ovenveight adults for 3 to 7 months. The prescribed weight control program was basically the low calorie diet (1,500-1,800 kcal per day) and nutrition education was applied to enhance its effects. Fad diets were chosen among the fashionable diets and they were "apple diet" , "yogurt diet" and "Lee Hijae diet" Anthropometric measurements, biochemical parameters, and percent of body fat were measured after the respective period of weight control program. Body weight was significantly (p<0.01) reduced and the rate of obesity was also significantly lowered with the prescribed weight control program while the body weight and rate of obesity were not changed with the fad diets. Weight loss from the prescribed weight control program also led to the change of total cholesterol levels while LDL-cholesterol, HDL-cholesterol, triglyceride were not significantly changed. Nutrition education and counseling of the weight control subjects induced more weight loss and better food behavior. It is suggested that a weight loss program should be based on the low calorie diet with the well-planned nutrition education The fashionable diets were attractive for a short time period in weight reduction but the rapid weight gain was noticed right after the diets ceased.
