한약치료의 체중 감량 효과와 간기능 개선: 증례보고 (Effect of Weight Loss and Improvement of Liver Function through Korean Medicinal Treatment: Case Report)

  • Obesity is known as the most common risk factor for non-alcoholic fatty liver disease. Weight loss is needed to prevent liver function damage from progressing to non-alcoholic hepatosteatosis (NASH) and NASH-related liver cirrhosis. The purpose of this study was to observe the recovery of liver function in obese patients with liver dysfunction through traditional Korean obesity treatment. Body weight, liver function levels and renal function levels were examined by prescribing traditional Korean medicine in obese patients with mild elevation of liver function test. Blood tests were conducted at intervals of one month, and it was observed that liver function recovered to the normal range in three patients.

비만 중년 남성의 비만 유형별 체질량지수와 인슐린저항성 및 베타세포 기능간의 관련성 (Relationship between Body Mass Index and Insulin Resistance and Beta Cell Function by Obesity Type in Obese Middle-Aged Men)

  • 이 연구는 비만 중년 남성을 대상으로 복부비만 유무에 따른 체질량지수와 인슐린저항성 및 베타세포 기능간의 관련성을 알아보고자 하였다. 2018년 1월부터 2020년 6월까지 경기도 소재 종합병원에서 건강검진을 받은 비만 중년 남성 797명을 대상으로 하였다. 복부비만 동반군은 327명, 복부비만을 동반하지 않은군은 470명이며, 두 군간에 공복혈당(p<0.001), HbA1c(p=0.003), 인슐린(p<0.001), HOMA-IR(p<0.001)은 차이가 있었다. 체질량지수는 복부비만 동반 유무와 관계없이 인슐린저항성 및 베타세포 기능에 영향을 미치는 요인이었다. 또한 체질량지수는 복부비만 동반 유무와 관계없이 인슐린저항성 및 베타세포 기능저하의 질환 발병과 관련이 있었다. 따라서 중년 비만 남성의 건강관리를 위한 운동 프로그램 수행 및 정기적인 검진을 통해 체질량지수의 지표 관리가 필요할 것으로 사료된다.

Body Mass Index at Presentation of Inflammatory Bowel Disease in Children

  • Purpose: The evidence for an association between inflammatory bowel disease (IBD) and obesity is conflicting. Therefore, we set out to review the body mass index (BMI) at presentation of IBD to understand if the rise of the obesity rate in the general population, lead to an increase of obesity in patients with IBD at the time of diagnosis. Methods: Retrospective review of all patients with IBD seen at Children's Hospital and Medical Center from January 1st 2010 to December 31st 2014. From the initial visit and endoscopy, we obtained: age; sex; BMI; disease phenotype; disease severity. Results: We had a total of 95 patients, 35 patients were excluded due to incomplete data or referral being made after diagnosis was made. 28 were males and 32 were females, Age range was 2-17 years. A 37 had Crohn's disease, 19 ulcerative colitis, and 4 indeterminate colitis. Disease severity in 19 cases was mild, 29 moderate and 12 severe. BMI distribution was as follows-obese (5.0%), overweight (6.7%), normal weight (65.0%), mild malnutrition (8.3%), moderate malnutrition (15.0%), severe malnutrition (1.7%). Conclusion: Our data is consistent with other series. Showing most children had a normal BMI, regardless of disease severity or phenotypes. One confounding factor is the possibility of delay in referral to GI. This could mean some obese children may fall in the normal BMI range at the time of diagnosis due to ongoing weight loss. Future studies should include prospective cohort studies, comparing incidence of IBD in obese and non-obese patients, severity at presentation, duration of symptoms, and clinical outcomes.

Expression of eotaxin in 3T3-L1 adipocytes and the effects of weight loss in high-fat diet induced obese mice

  • Eotaxin is an important inflammatory chemokine in eosinophil chemotaxis and activation and, thus, is implicated in asthma. Recently, obesity was associated with an increased prevalence of asthma, but the relationship between obesity and eotaxin expression has only been partially understood in obese mice and human studies. Therefore, we studied the expression patterns of eotaxin in 3T3-L1 preadipocytes/adipocytes to determine whether eotaxin levels are influenced by body weight gain and/or reduction in diet-induced obese mice. First, we investigated eotaxin expression during differentiation in 3T3-L1 adipocytes. Then, we treated 3T3-L1 preadipoeytes/adipoeytes with tumor necrosis factor-alpha (TNF-${\alpha}$), eotaxin, interleukin (IL)-4, IL-5, or leptin. To examine the effects of weight loss in high-fat diet induced obese mice, we fed C57BL/6 mice a high-fat diet or a normal diet for 26 weeks. Then, half of the high-fat diet group were fed a normal diet until 30 weeks to reduce weight. Epididymal adipose tissue, visceral adipose tissue, serum, and bronchoalveolar fluid of mice were examined for eotaxin expression. The results showed that eotaxin expression levels increased with adipocyte differentiation and that more eotaxin was expressed when the cells were stimulated with TNF-${\alpha}$, eotaxin, IL-4, IL-5, or leptin. An in vivo study showed that eotaxin levels were reduced in visceral adipose tissues when high-fat diet fed mice underwent weight loss. Taken together, these results indicate a close relationship between eotaxin expression and obesity as well as weight loss, thus, they indirectly show a relation to asthma.

실버 의류산업 활성화를 위한 노년 비만여성의 체형연구 (Study on the body shapes of old aged obese women for the activation of the silver clothing industry)

  • This study analyzed the body-type characteristics of 340 old-aged obese women that had been on the rise as a part of efforts to activate the silver clothing industry. The subjects were in the age range of 60-79 and met some obesity requirements, including a Rohrer Index of 1.6 or higher, a BMI of 25 or higher, and a WHR of 0.85 or higher. Old-aged obese women showed increased thickness of the torso with age, which suggests that they revealed the characteristics of regardless of gender. In other words, they became bigger in the waist and abdomen, shorter in height, slimmer in the lower body, and thicker in the torso. There are three types of obesity: Type 1 is lower-body obesity with a higher degree of obesity in the abdomen than the upper body. Type 2 is abdominal obesity with a higher degree of obesity in the upper body than in the lower body. Type 3 is whole-body obesity with balanced obesity of the whole body. As for changes to the types of obesity according to age, those who are in their sixties usually fall into the categories of upper-body and whole-body obesity, and those who are in their seventies are much more concentrated in the categories of abdominal obesity and upper-body obesity with a decreased percentage of whole-body obesity. It is apparent that the percentage of abdominal and upper-body obesity rises with age due to fat accumulation in the abdomen.

노년 비만남성의 체형연구를 통한 실버 의류산업 활성화 기반 구축 (Building up the foundation for the elderly apparel industry through the study on the body shapes of elderly obese males)

  • The purpose of this study is to promote the elderly apparel industry for the increasing numbers of elderly obese male population. In the study, a total number of 249 males between the ages of 60 to 85 were studied to analyze their body types and differences. The group had a Rohrer Index of 1.6 or higher and BMI of 25 or higher. The noticeable physical differences in the group were shorter waist front length, bigger waist and hip circumferences with increasing age and slimmer limbs that are associated with the natural aging process with or without obesity. The obese body types have been classified in the following 3 different categories. Type 1 is the group that has lower body obesity with broad shoulders and relatively slimmer abdomen than a heavy bottom. A total number of 84 people belonged to the type 1 obesity category which makes up 33.8% of the total. Type 2 is the group that has upper body obesity with especially large abdominal obesity. A total number of 76 people, 30.5% of the total, were classified as type 2. Type 3 is the group that has whole body obesity with balanced obesity in the whole body. A total number of 89 people, 35.7% of the total, made up type 3.

Validation of G-protein beta-3 subunit gene C825T polymorphism as predictor of obesogenic epidemics in overweight/obese Korean children

  • Purpose: We investigated the potential interaction between the G-protein beta-3 subunit gene (GNB3) C825T polymorphism, a risk factor for chronic disease in various ethnicities, and obesogenic environments in overweight/obese Korean children. Methods: The present study was conducted as a cross-sectional study using measures of anthropometry, blood pressure (BP), and fasting blood samples as well as 3-day food records. Subjects were recruited from seven elementary schools in an urban district in Seoul, South Korea, between 2007 and 2008. A total of 1,260 children aged 8-9 years were recruited in the study, including 633 boys (50.3%) and 627 girls (49.7%). Results: The allele frequencies of the GNB3 polymorphism were C allele = 49.7% and T allele = 50.3% in subjects. In general, boys with T allele had higher BMI, systolic BP (SBP), and triglycerides, although their energy intake was not significantly different from boys with C allele. In contrast to boys, girls with T allele had lower BMI but higher SBP and energy intake than those with C allele. The girls with T allele had a significantly lower BMI and waist circumference in both the normal weight group and obese group (OB). T allele carriers in both genders had significantly higher TC than C allele carriers in the OB group. At last, girls with T allele in OB appeared to have significantly lower HOMA-IR than those with C allele. Conclusion: Unlike higher risk for negative health outcomes by the GNB3 polymorphism in various ethnicities, GNB3 polymorphism did not influence obesogenic environments in overweight/obese Korean children.

인슐린 비의존형 당뇨병 남성 환자의 영양소 섭취량, 혈중 지질 및 비만도에 관한 연구 (Study of Nutrient Untake, Blood Lipids, and Obesity in Non Insulin Dependent Diabetes Mellitus Male Individuals)

  • This study was an attempt to investigate various factions, such as the calorie, nutrient intake, physical activity, blood lipids, obesity prevalence and body fat distribution on NIDDM male diabetics. General characteristics, physical activity and exercise levels of subjects were invesigated by interviewing, daily calorie and nutrient intake were measured by convenient method. The following anthropometric measurements were made on all participats : weight, height. Also waist and hip circumference were measured on 174 male diabetics to get waist-to-hip circumference ratio as index of the body fat distribution. For measurement of plasma lipids, 12-hour fasting blood samples were drawn The results of this study were summarized as follows : 1. At the onset of diabetes, the major self-diagnosed symptoms were polydipsia, fatigue, and body weight redution 2. The average of daily energy intake of male diabetics was 2106 Kcal which is 96% of the RDA Percentage of energy is that carbohydrate:protein : pat=70:14:16. 3. Among the NIDDM male subjects, 59% was exercise regularly. 4. Obese subjects above in the ideal body weight of 120% are presently 17%, but 39% of subjects were reported to be obese in the past. The mean BMI of the male NIDDM diabetics is 23.3${\pm}$2.6 and the past mean BMI was 25.2${\pm}$2.7 The mean WHR was 0.93${\pm}$0.10. 5. When diabetics were divided into obese and nonobese group according to RBW, energy intake, blood pressure, blood glucose and total cholesterol were not significantly different between the two groups, but LDL and VLDL-cholesterol were significantly higher in the obese group. 6. RBW did not correlate with cholesterol and triglyceride, but WHR correlated significantly with cholesterol and triglyceride. In conclusion, these results from a present study support previous findings indicating that not only the degree of obesity but also the localization of fat is a risk factor for diabetes.

A Comparison of Obesity Prevalence between Korean-American Children and Korean Children

  • This study is to compare the prevalence of obesity between Korean-American children and Korean children, and to investigate the role of environmental factors in obesity development. Prevalence of child obesity is compared by their residence, parents' anthropometric data, education and occupation, and the length of immigration, and birth place of the children. A total of 593 children between the ages of 9 and 12 were examined. 262 Korean-American children (KcUS group) from New Jersey, USA and 331 Korean children (KcK group) from Seoul, Korea were compared. KcUS group showed a higher obesity rate (male: 12.6% and female: 8.2%), compared to KcK group (male: 8.0% and female: 5.1 %). KcK male children showed lower weight and prevalence of obesity than Korean-American male children who lived in the United States for more than three years. Korean-American female children had higher weight and obesity rate than Korean female children. Waist circumference and hip circumference were also higher in obese children in both KcK and KcUS groups. The children who live in America had an odds ratio of 1.69 to be obese compared to KcK, while those born in America and those who have lived in America for more than 3 years had odds ratios of 1.53 and 1.25 to be obese, respectively. This study found that environmental factors, immigration to America for instance, could playa bigger role in child obese development than the genetic factor.

서울지역 중년에 있어서 정상체중군과 과체중군의 영양상태 비교 (Comparison of Nutritional Status among Obese, Overweight and Normal Subjects in Seoul)

  • The nutritional status of middle-aged overweight and control normal subjects were evaluated for the purpose of providing the background information of the degenerative disease control. A survey was conducted with 293 healthy subjects (121 males and 172 females) between the ages 40-64. The average BMI (body mass index) of male subjects was 24.5 $\pm2.75$, and that of females was 23.5 $\pm2.87$. The average values of WHR (waist-hip ratio) were 0.88 $\pm0.04$, for males and 0.82 $\pm0.07$, for females. The normal BMI group (BMI 18.5-22.9) comprised 28.9% of males and 47.1% of females. The percentage of overweight subjects (BMI 23-24.9) was 26.4% of males and 30.8% of females. The obese group (BMI > 25) was 44.6% of males ana 22.1% of females, showing the greater rate of obese state among male subjects. The average energy intakes were 76.6 $\pm14.9$% for males and 77.8 $\pm12.6$, % for females, protein intakes were 108.0 $\pm24.6$% for males and 111.2 $\pm22.7$, % for females of the RDA levels. The average intakes of other nutrients were above the 75% of RDA levels except calcium. The average nutrient intakes of the three subgroups according to their BMI values were not different for both males and females. There were weak correlations between obesity and blood biochemical indices. There were positive correlations between BMI or WHR and hemoglobin, hematocrit, fasting glucose, total cholesterol, triglyceride, AST or ALT. There were negative correlations between BMI or WHR and HDL-cholesterol. These results suggest that the obesity rate of middle-aged is an influential factor of chronic disease. The middle-aged subjects of this study with higher educational and socioeconomic background tend to have desirable nutrition knowledge and attitude, but the application of their knowledge or attitude was relatively poor.
