  • This study was conducted to assess the nutritional status of the elderly from low income areas and its relationship with the indices of immunity. Ninety nine(male : 28, female : 71) healthy, subjects aged over 65 years staying in homes in Seoul were assessed with regard to their dietary intake and biochemical measurements. Their mean daily energy intakes were 1256kcal and 109kcal for males and females respectively. Mean serum IgG were 2356mg/dl in men and 2161mg/dl for women, higher than the normal range. Mean serum $C_3$ were 120.9mg/dl and 130.6mg/dl for males and females respectively and mean $C_4$ were 30.6mg/dl and 31.9mg/dl. The mean $C_3$ and $C_4$ were within the normal range and the proportion of subjects whose $C_3$ and $C_4$ were at a higher level than the upper limit were 15.4$\%$ and 16.3$\%$ respectively. Complement $C_3$ showed significant correlation with serum albumin(p〈0.01), serum cholesterol(p〈0.00l, serum TG(p〈0.05), LDL-C(p$\ll$$\ll$$\ll$$\ll$$\ll$$\ll$$\ll$$\ll$$\ll$$\ll$$\ll$$\ll$$\ll$q0.001), VLDL-C(p$\ll$0.05), serum copper(p$\ll$0.05) and serum Ca(p$\ll$0.01). • BACKGROUND/OBJECTIVES: Hydrolyzed formula is often fed to infants with gastrointestinal or immune issues, such as malabsorption or cow's milk allergy, because enzymatic treatment has rendered it more digestible and less allergenic than standard cow's milk formula (SF). Partially hydrolyzed formula (PHF) should be considered for those infants who are intolerant to extensively hydrolyzed formula. However, there are concerns about the nutritional insufficiencies of PHF. We aimed to evaluate the effects of PHF on the growth and health indicators in infants who were at high-risk of allergic disease and potential candidates for consuming PHF. SUBJECTS/METHODS: A total of 83 infants aged 0-2 mon with a family history of allergies were assigned to consume either PHF or SF until 24 weeks of age. Anthropometric measures were obtained at baseline, 12 weeks, and 24 weeks; blood samples were drawn and evaluated at the end of the study. RESULTS: No significant differences were observed in weight, height, and weight-for-height at any time point in each sex between the PHF and SF groups. At 24 weeks of age, the weight-for-age and height-for-age z-scores of the SF group were higher than those of the PHF group, but there was no significant difference in the weight-for-height z-score. There were no significant differences in levels of white blood cells, hemoglobin, ferritin, protein, albumin, aspartate aminotransferase, alanine aminotransferase, eosinophil cationic protein, and immunoglobulin E. CONCLUSIONS: In this study, there were no differences in growth and blood panels between the infants consuming PHF or SF. Therefore, infants who are unable to tolerate SF can be fed PHF without nutritional concerns about growth.

  • Food intake patterns and iron nutritional status of male and female college students were studied based on dietary iron density. Dietary data were collected using the method of 24-hour recalls for 3 consecutive days from 106 students, and fasting blood were drawn to measure iron nutritional status indicators such as total iron binding capacity, serum iron, hematocrit, hemoglobin, and red blood cell count. Mean daily iron intakes of male and female students were 13.3㎎ and 10.0㎎, which were 107% and 63% of the RDA, respectively. However, dietary iron density were similar between male and female students as 5.9㎎/1,000㎉ and 5.7㎎/1,000㎉, respectively. The diets were divided into two groups according to iron density; high iron density group (6㎎/1,000㎉ or more) and low iron density group (less than 6㎎/1,000㎉). The students in high density group had lower intakes of energy, especially fat, than those in low density group. Female students in high density group showed significantly higher intakes of iron, and non-heme iron and folate than those of low density group. The students in high density group consumed more rice, hamburger, and eggs, while those in low density group consumed more Ra-myon and alcoholic beverages. The students in high density group consumed greater proportions of iron from plant-origin foods. Vegetables, legumes and seasonings were the food groups that female students in high density group consumed significantly more than those in low density group. Also the percentages of female students with iron deficiency were higher in low density group. These results suggest that diet with high iron density is important to improve iron nutritional status of women, and further research about the effective way to increase iron density in our diet is needed.

  • This study was performed to develop a children's dietary life safety index required by the Special Act on Safety Management of Children's Dietary Life enacted in 2009. An analytical hierarchy process was used to obtain initial weights of dietary life safety evaluation indicators. The Delphi method was applied to develop the weights along with 98 food and nutrition professionals. Three representative policy indicators, nine strategy indicators, 11 main evaluation indicators, and 20 detailed evaluation indicators were selected for the children's dietary life safety assessment. Three policy indicators and nine strategy indicators were the following: children's food safety indicator (support level of children' safety, safety management level of children's favorite foods, and safety management level of institutional food service), children's nutrition safety indicator (management level of missing meals and obesity, nutrition management level of children's favorite foods, and nutrition management level of institutional food service), and children's perception and practice level indicator ("Dietary Life Law" perception level, perception, and practice level for dietary life safety management, perception, and practice level for nutrition management). Weights of 40%, 40%, and 20% were given for the three representative policy indicators. The relative importance of nine strategic indicators, which were determined by the Delphi method is as follows: For children’s food safety, support level of children's safety, safety management level of children's favorite foods, and safety management level of institutional food service were given weights of 12%, 9%, and 19%, respectively. For children's nutrition safety, the missing meals and obesity management level, nutrition management level of children's favorite foods, and the nutrition management level of institutional food service were given weights of 13%, 11%, and 16%, respectively. The "Dietary Life Law" perception level, perception and practice level of dietary life safety management, and perception and practice level of nutrition management were given weights of 4%, 7%, and 9%, respectively.

  • The purpose of this study was to analyze the association between self-reported voice problems and voice disorders in the Korean adult population. Data were collected from the 4th Korea National Health and Nutritional Examination Survey (2008) from 3,135 subjects (1,310 men and 1,825 women) aged 19 years and older. Multi-nominal logistic regression analyses were used to examine the association between self-reported voice problems and voice disorders in the Korean adult population. Adjusting for covariates (age, sex, education level, job, smoking, alcohol drinking, thyroid disorders, pain and discomfort during the last 2 weeks), self-reported voice problems included independently associated functional voice disorders (OR=4.70, 95% CI: 3.14-7.03) and organic voice disorders (OR=3.89, 95% CI: 1.57-9.65). The results of the present study verified that self-reported voice problems are valuable indicators for voice disorders. Further research is needed to ascertain the effect of self-reported voice problems on voice disorder in adults.

  • Background : There has been a concern that the quality of care provided to end-stage renal disease (ESRD) patients in the United States may not be as good as recommended. This paper illustrates a composite measure to assess, the quality of care received by ESRD patients undergoing in-center hemodialysis by incorporating outcomes for 4 major treatment areas. The 4 treatment areas are: dialysis treatments, anemia control, nutritional management, and blood pressure control. Methods : The major data source for the study was the United States Renal Data System (USRDS) Dialysis Morbidity and Mortality Study Wave 1 (DMMS-1) d Sixteen categories of a composite quality indicator were constructed by combining 4 dichotomous variables (16=2*2*2*2). representing the optimal vs. less than optimal level of outcome for each of the 4 treatment outcome measure respectively. Optimal outcome level for each treatment area was defined based on the recommendation from the National Kidney Foundation: (a) delivered dialysis doses (Kt/V) ${\geq}$ 1.2; (b) hematocrit level ${\geq}$ 30%; (c) serum albumin concentration ${\geq}$ 3.8g/dl ; and (d) blood pressure of <140 / <90mmHg. The 16 quality indicator were ranked according to their relative quality weights, which were estimated from its association with the relative risk of survival, adjusting for patient's baseline severity and dialysis facility characteristics. Results : Out of the entire sample of 2,179 patients, only 229 (10%) meet th recommended outcome levels for all 4 treatment areas. Overall, the study patients were distributed evenly over the 16 quality indicators, indicating a great variation in the quality of ESRD care. It appears that the rank of the 16 quality-indicators is driven by serum albumin concentration, suggesting that serum albumin concentration may be the most powerful predictor of ESRD patient survival among the 4 outcome measures. Conclusion : The developed quality indicator has the advantage of describin a range of care for dialysis patients and thus providing a more complete picture of care as compared to previous studies that have focused on only single or few components of the ESRD care.

  • BACKGROUND/OBJECTIVES: Over the past few decades, Malaysia has been experiencing an increased prevalence of overweight and obesity that threatens the health of Malaysians. Poor dietary intake is one of the major contributors to the development of obesity and many non-communicable diseases. The dietary intakes of adults in Malaysia were assessed to determine the association between the dietary intake variables and the body mass index (BMI) and waist circumference (WC) measurements. This study examined whether the dietary intake achieved the recommended nutritional guidelines and compared the intakes between both sexes. SUBJECTS/METHODS: The height, weight, and WC of four-hundred-and-ninety adults (n = 490) in Malaysia were measured using standard procedures. The three-day 24-hour dietary recalls were conducted on 422 out of the 490 adults and their dietary intakes were evaluated in detail. The selected dietary intake variables were used to determine the associations with the obesity indicators. RESULTS: Among the participants, 52.8% were overweight or obese. After data analysis, the mean energy intake was 1,550 kcal/day, in which male participants had a significantly higher energy and macronutrients intake than females. Protein consumption and its percentage of energy contribution exceeded the recommended range. The consumption of fruits, vegetables, and milk and milk products were lower than the recommended number of servings for a healthy diet. The male participants consumed significantly more servings of carbohydrate-based foods, meat, and fats, oils, and sweets than females. Among the selected dietary intake variables, only the carbohydrate intake was negatively associated with the BMI (Estimate b = -0.008) and WC measurements (Estimate b = -0.019) after adjusting for covariates. CONCLUSIONS: This study evaluated the dietary intakes of a sample of Malaysian adults and its association with the obesity indicators. The results highlight the need for improvements and modifications of the dietary intake of Malaysians to reduce the overweight and obesity rates.

  • The present study was aimed at evaluating whether white rice consumption has any association with selected measures of dietary intake and nutrients, including various variables of energy intake, major vitamin, mineral intakes, and the risk of health characteristics for over 60 years older. A total of 1,433 subjects (658 male and 775 females) 60 years or older from the fifth Korea National Health and Nutritional Examination Survey(KNHANES) 2010 participated and the Food Commodity Intake Database were used in this study. Levels of percent energy intake from rice were classified into 4 groups (R1, R2, R3, R4 groups: 25% of each) between male and female elderly using data from KNHANES. One of the interesting findings was that the higher rice consumers had a significantly sufficient intakes of a range of nutrients such as total energy intake, protein, carbohydrate, calcium, mineral, sodium, potassium, and niacin in female elderly, but not much in male. Other finding was that as the higher intake of rice group, prevalence of hypertension (53.5%, 63.4%, 50.3%, 46.3%, p=0.035), dyslipidemia (16.6%, 18.0%, 14.5%, 11.1%, p=0.008), sarcopenia (46.1%, 46.1%, 40.2%, 28.8%, p=0.012), sarcopenic obesity (25.4%, 31.5%, 23.7%, 15.5%, p=0.008) were significantly less than other groups in female, but not much in male. In conclusion, we obtained valuable basic information on recommended rice-centered diet could give us good nutritional status and eventually bring the prevention of some chronic diseases in elderly, especially in female.

  • It has been more than three decades since the first assay assessing circulating 25 (OH)D in human subjects was performed That publication as well as several that followed it defined 'normal' nutritional vitamin D status in human populations. Recently, the wisdom by which 'normal' circulating 25 (OH)D levels in human subjects were assigned in the past has come under question. It appears that sampling human subjects, who appear to be free from disease, and assessing 'normal' circulating 25 (OH)D levels by plotting a Gaussian distribution is grossly inaccurate. There are many reasons why this method is inaccurate, including race, lifestyle habits, sunscreen usage, age, latitude, and inappropriately low dietary recommendations for vitamin D. For instance, a 400 IU/day. AI for vitamin D is insignificant when one considers that a 10-15 minute whole body exposure to peak summer sun will generate and release up to 20,000 IU vitamin $D_3$ into the circulation. Recent studies, which orally administered up to 10,000 IU/day vitamin $D_3$ to human subjects for several months, have successfully elevated circulating 25 (OH)D levels to those observed in individuals from sun-rich environments. Further, we are now able to accurately assess sufficient circulating 25 (OH)D levels utilizing specific biomarkers instead of guessing what an adequate level is. These biomarkers include intact parathyroid hormone (PTH), calcium absorption, bone mineral density (BMD), insulin resistance and pancreatic beta cell function. Using the data from these biomarkers, vitamin D deficiency should be defined as circulating levels of 25 (OH)D$\leq$30 ng/mL. In certain cases, such as pregnancy and lactation, significantly higher circulating 25 (OH)D levels would almost certainly be beneficial to both the mother and recipient fetus/infant.

  • The animal production sector in Jordan is characterized by shortages of locally produced feedstuffs derived from rangeland, forage plants or from human food crops as by products. This is exacerbated by insufficient rainfall, overgrazing, early grazing and high stocking rate. Thus, subject to these constraints, other technological improvements are highly desirable to meet the needs of crop growth and animal production. Alternative adapted technologies are also desirable in order to meet the increased demand for red meat in relation to population growth along with the changes in the price subsidy for feedstuff. The technologies are those, which have been introduced to the animal production sector, obtained in agricultural research stations besides on-farm demonstrations. They include technologies suited for increasing birth and twining rates, synchronizing the mating period, introducing the early weaning method, and animal feed and sheep production. Economic assessments conducted in this study demonstrate promising results of hormonal and nutritional practices in improving production efficiency of Awassi sheep in Jordan. Jordanian published data between 1991 and 1998 were used. The examined practices were: 1) use of PMSG in estrus synchronization in ewes, 2) introduction of early lamb weaning program, 3) supplementation with $AD_3E$ for ewes and 4) the use of agro-industrial feed block as a feed supplement for grazing lambs. Production data were then subjected to partial budgeting for economical evaluation. The use of PMSG outperformed the control groups in fertility and net returns per ewe by US$ 8.36/ewe. The early weaning of lambs increased the net returns by US$ 3.90/lamb. The injection with vitamin $AD_3E$ showed an average additional net return of US$ 5.66/ewe. Feeding agriculture by-product blocks improved weight gain in the feed block groups and resulted in additional net returns of US$ 3.5/lamb. The economic viability and reproductive performance indicators demonstrate that efforts should be undertaken to disseminate these new practices in the development program.
