Innate immunity and carbohydrate metabolism alterations precede occurrence of subclinical mastitis in transition dairy cows

  • Background: This study examined whether activation of innate immunity and alterations of carbohydrate and lipid metabolism precede development of subclinical mastitis (SCM). Methods: Blood samples were collected from the coccygeal vein from 100 Holstein dairy cows at -8, -4, disease diagnosis week, and +4 weeks postpartum. Six healthy cows (controls - CON) and six cows that showed clinical signs of SCM were selected for serum analyses. All serum samples were analyzed for acute phase proteins (APP) haptoglobin (Hp) and serum amyloid A (SAA); proinflammatory cytokines including interleukin 1 (IL-1), IL-6, and tumor necrosis factor (TNF) and serum lactate, BHBA, and NEFA concentration. Data of DMI, milk production, and milk composition were recorded and analyzed. Results: The results showed that cows with SCM had greater concentrations of SAA, TNF (P < 0.01), and lactate before expected day of parturition (P < 0.05) compared to CON cows. Cows with SCM showed greater concentrations of lactate starting at -8 weeks (P < 0.05) and TNF starting at -4 weeks prior to the expected day of parturition (P < 0.01). Interestingly, at -4 weeks, concentrations of IL-1 and Hp were lower in cows with SCM compared to healthy cows (P < 0.01) followed by an increase during the week of disease diagnosis (P < 0.05). Subclinical mastitis was associated with lower DMI, at -4 weeks before calving, milk production (P < 0.05) and increased somatic cell counts (SCC) (P < 0.01). Conclusions: Results of this study suggest that SCM is preceded by activated innate immunity and altered carbohydrate metabolism in transition dairy cows. Moreover the results support the idea that Hp, lactate, and SAA, at -8 weeks, and TNF and IL-1 at -4 weeks can be used as early indicators to screen cows during dry off for disease state.

  • During the first half of twentieth century, even though the importance of non-calorie essential micronutrients of 13 vitamins and 17 minerals has been known to alleviate nutritional disorder; the primary objective of agriculture and plant breeding programs has been to increase the productivity and seed yields, and macronutrients of proteins, fats, and carbohydrates made up the bulk of foodstuff which were used primarily as an energy source. In the last decade it has been found that non-essential micronutrients encompass a vast group of phytochemicals including antioxidants that are not strictly required in the diet but when present at sufficient levels work as health-promoting chemicals. Nowadays agricultural crops are grown for health rather than for food or fiber, and modifying the nutritional compositions of plant foods has become an urgent health issue. To ensure an adequate intake of essential vitamins and minerals, and to increase the consumption of health-promoting phytochemicals, the researches on plant secondary metabolism have been made. The attempt to improve nutritional quality of crops has been blocked by a lack of basic knowledge of plant metabolism. The advent of genomics era enabled new approaches to make crossing regardless of species, family, or phylum barriers, and the accumulation in our basic knowledge on plant secondary metabolism during the coming decade would be tremendous. As the major staple crops contain insufficient amount of many micronutrients, fortification strategy will be a necessary practice. Elevated intake of specific vitamins, C, E, and $\beta$-carotene, mineral selenium, antioxidants, and phytochemicals significantly reduces the risk of chronic disease such as cancer, cardiovascular disorder, diabetis, and other degenerative disease associated with aging. As the attempt to improve the nutritional quality of crops requires the basic knowledges on plant metabolism, plant biochemistry, human physiology, and food chemistry, strong interdisplinary collaboration among plant biotechnologists, human nutritionists, and food scientists will be needed. Inhibition of cancer, cardiovascular disease, and other degenerative disorder may be the biggest goal facing nutritional plant breeders. But the assumption that simply increasing dietary level of any compound will necessarily improve human health is a dangerous idea because many plant secondary products and dietary contaminants have paradoxical (hermetic) effects. Before biotechnical manipulation is undertaken to elevate or reduce any individual constituent of crops, the contribution of the micronutrient to human health must first be investigated.

알레르기성 질환자의 우울증 유무에 따른 영양 상태 연구: 국민건강영양조사 데이터를 이용하여 (A Study of the Nutritional Status According to the State of Depression of Allergic Disease Patients: Based on the Korea National Health and Nutrition Examination Survey)

  • This study was conducted on the nutritional status of 1,805 patients with allergic diseases (atopic dermatitis, allergic rhinitis, and asthma) aged 19 to 64 years according to their state of depression, based on the data from the Korea National Health and Nutrition Examination Survey (KNHANES). The Patient Health Questionnaire-9 (PHQ-9) was used to diagnose depression. Subjects with a score of 10 or more were categorized into the depression group (n=152) and the rest into the non-depression group (n=1,653). The results of this study were as follows: The proportion of women (75.7%) was higher than that of men (24.3%) in the depressed group (P<0.01). In terms of energy intake per 1,000 kcal, both men and women in the depressed group showed a lower energy intake than the non-depressed group and this intake was less than the estimated energy requirement (EER). The nutrient intakes of protein, calcium, phosphorus, iron, vitamin A, thiamine, riboflavin, niacin, folic acid, and vitamin C were below the estimated average requirement (EAR). Also, the intakes of fiber and potassium were less than the adequate intake (AI) (P<0.001). In the lifestyle parameters, the ratio of eating alone at lunch was 54.1%:33.1%, indicating that more than half of the depression group ate alone. In conclusion, it was observed that the nutritional status of allergic disease patients was imbalanced. The nutritional imbalance was due to insufficient energy intake and inadequate intake of nutrients, which was below the average requirements of vitamins and minerals and this was more evident in the depression group than in the non-depression group.

사무직과 생산직 근로자의 영양실태 비교 및 건강 관련 요인 분석 (Comparative Analysis and Evaluation of Health and Nutritional Status of Male Industrial Workers in Korea)

  • This study was carried out to collect information to establish a framework for meal management and nutritional service for prevention of chronic degenerative disease in the industrial workers. We investigated the health concerned life-style, nutrient intakes, anthropometry and biochemical parameters in the male workers in Korea by work condition. Anthropometric parameters of height, weight, waist, hip and triceps skinfold thickness were measured and biochemical parameters including Urine pH, hemoglobin, blood glucose, total cholesterol, GOT, GPT, $\gamma$-GPT and blood pressure were determined for 101 subjects(50 office workers : 33.9yr, 51 laborers : 34.4yr). To assess the nutrient intakes and diet quality of workers, dietary intake was measured by one day 24-hour recall method. Result of anthropometric parameters of height(p<0.01), weight(p<0.05), hip(p<0.05), TSF(p<0.01) were significantly higher in office workers than in laborers. Biochemical parameters were not significantly difference in both of them. Average daily nutrient intakes of both groups was higher than the Korean RDA and report on 2001 National Health & Nutrition Survey. The calcium(p<0.01), iron(p<0.05), vitamin A(p<0.01), vitamin B1(p<0.01) and niacin(p<0.05) intakes in laborers were significantly higher than in office workers. Nutrition adequacy ratio(NAR), mean adequacy ratio(MAR) and index of nutritional quality(INQ) of laborers were higher than those of office workers. This results indicated that the onset possibility of chronic degenerative disease appeared higher in office workers. Therefore, meal menagement and nutritional service for prevention of chronic degenerative disease of industrial works needs to be more variant guidelines.

만성 폐쇄성 폐질환 환자의 영양상태 평가 (Assessment of nutritional status of patients with chronic obstructive pulmonary disease)

  • 연구배경 : 만성 폐쇄성 폐질환 환자에서 영양결핍이 흔히 나타나며, 그 원인기전은 섭취의 부족보다는 호흡근의 열량요구량의 증가에 따른 과신진대사로 인하여 상대적인 결핍이 초래되기 때문으로 알려져 있다. 또한 만성 패쇄성 폐질환에서 영양결핍은 폐기능, 근력 및 운동수행능 등의 생리적 척도와도 유의한 상관관계를 보이며, 나아가서는 예후에도 영향을 미치는 독립적인 인자로 보고된 바 있다. 방법 : 만성 폐쇄성 폐질환 환자와 유사연령의 건강한 대조군에서 영양섭취량을 조사하였고, 신체계측과 혈액검사 등의 영양상태의 지표를 평가하였으며, 영양상태의 평가지표들과 만성 폐쇄성 폐질환의 중요한 폐기능지표인 1초간 노력성 호기량과의 상관성을 조사하였다. 결과 : 1) 대상 환자는 남자 25예였으며, 평균연령은 66.1세였고, 1초간 노력성 호기량의 예측치에 대한 비율은 $42{\pm}14%$였다. 대조군은 폐기능이 정상인 건강한 남자 26예였고, 평균연령은 65.0세였다. 2) 1일 열량섭취량/열량요구량 비율은 환자군에서 $107{\pm}28%$, 대조군에서 $94{\pm}14%$로 환자군에서 높은 경향을 보였다(p=0.06). 3) 환자군과 대조군의 비만도가 92.8% 및 101.6%로(p=0.024), 신체비만지수가 $20.0kg/m^2$$21.9kg/m^2$ 로(p=0.015), 악력이 29.0kg 및 34.3kg으로(p=0.003), 환자군에서 유의하게 낮았으나, 상완삼두근부위 피부두겹두께, 상완위 근육둘레, 알부민 및 총림프구수는 두 군간에 유의한 차이가 없었다. 환자군에서 저체중군의 비율은 40%(10/25)였고, 대조군에서는 15%(4/26)였다. 4) 비만도, 신체비만지수, 상완삼두근부위 피부두겹두께, 상완위 근육둘레 등은 1초간 노력성 호기량과 유의한 상관성이 있었다. 결론 : 이상의 결과에서 만성 패쇄성 폐질환 환자는 저체중과 말초근력의 저하 등 영양결핍의 소견을 보이나, 그 원인이 섭취량은 부족에 기인하지 않음을 알 수 있었으며, 영양상태 평가의 지표들이 1초간 노력성 호기량과 유의하게 상관됨을 알 수 있었다.

한국 성인 남녀에서 지방섭취제한 행동 변화단계에 따른 영양상태와 건강관련 행위에 대한 신념 비교 (Comparison of Nutritional Status and Beliefs on Health Behavior Regarding Stages of change in Dietary Fat Reduction among Korean Men and Women)

  • This study describes the application of the stages of change construct to fat intake by examining the associations of the stages of change with nutritional status and beliefs on health behavior. Data were obtained from apparently healthy 596 adults(326 females and 270 males) residing in large cities. Stages of change assessed by an algorithms based on 6 items were designed each subject into one of the 5 stages: precontemplation(PC), contemplation(CO), preparation(PR), action(AC) and maintenance(MA). Beliefs on health behavior were assessed by self efficacy as well as 4 belief scales from the Locus of Illness Control(LIC) developed using factor analysis such as internal disease cure and prevention and external disease cure and prevention. Energy and fat intakes were measured by a 39 item short form food frequency questionnaire. Regarding the 5 stages of change, MA stage comprised the largest group(37.9%), followed by Ac(30.7%), PC(11.4%), CO(10.4%) and PR(9.6%). Subjects who were females, older or healthier were more likely to belong to either AC or MA. Stage assignment of individuals was corroborated by their nutritional variables. Those in PC had the most energy and fat and those in MA ate the least for females. BMI was higher in PR than any other stages for both males and females. Those in PC were distinctive in that they were more externally oriented in terms of health control showing higher scores on external disease prevention(for males) and external disease cure(for females), and lower score on internal disease cure. On the other hand, those in MA received the highest scores on internal disease prevention and self efficacy, which suggested that they were more internally oriented. Canonical discriminant function analysis indicated that the 5 stages were importantly discriminated by BMI, self efficacy, internal disease prevention and external disease prevention for males and by fat intake, self efficacy and external disease cure variables for females. The results of our study confirm differences in stages of change in fat intake in terms of nutritional status and beliefs on heath behavior and indicate the need for taking these phases of change into account in nutrition advice. Evaluation and Treatment of Malnutrition and Associated Gastrointestinal Complications in Children with Cerebral Palsy

  • The majority of children with cerebral palsy (CP) have feeding difficulties and are especially prone to malnutrition. The early involvement of a multidisciplinary team should aim to prevent malnutrition and provide adequate nutritional support. Thorough nutritional assessment, including body composition, should be a prerequisite for the nutritional intervention. As in typically-developed children nutritional support should start with dietary advice and the modification of oral feeding, if safe and acceptable. However, for prolonged feeding, in the presence of unsafe swallowing and inadequate oral intake, enteral nutrition should be promptly initiated and early gastrostomy placement should be evaluated and discussed with parents/caregivers. Gastrointestinal problems (oropharyngeal dysfunction, gastroesophageal disease, and constipation) in children with CP are frequent and should be actively detected and adequately treated as they can further worsen the feeding process and nutritional status.

영양부족 환자의 조기발견을 위한 선별검사의 적용 및 효용성 평가 (Application and Efficacy Evaluation of Nutritional Screening Tool)

Purpose: Malnutrition has been frequently reported for patients on their admission to the hospital and it has been associated with an increase in morbidity, mortality and the length of the hospital stay. Although a number of screening tools have been developed to identify those patients at risk for malnutrition, there is no' gold standard' for defining malnutrition and the malnourished patients remain largely unrecognized. The aim of this study is to evaluate the efficacy of a nutritional screening tool for use in Dankook University Hospital. Methods Nutritional evaluation was performed for 53 patients who were admitted to the department of surgery and internal medicine between October and December 2004. The screening tool was completed by the ward nurse and the nutritional support team nurse on the same patients within24 hours of admission. The nutritional support team nurse performed the full assessment. The screening sheet included 4 questions regarding body mass index, recent unintentional weight loss, food intake and disease severity. Each answer was scored and a total of 5 was tested as the criterion fey malnutrition. The full assessment included current body weight, recent weight loss, triceps skinfold thickness, mid-arm muscle circumference, serum albumin)in and total lymphocyte count. Malnutrition was defined by 3 or more values below the reference values. The reliability of the screening tool was assessed using kappa statistic. Sensitivity, specificity and accuracy were calculated to evaluate the validity of the screening tool. The receiver operating characteristic(ROC) curve was drawn to choose a cutoff valve that maximizes sensitivity and specificity. Results' The level of agreement between the ward nurse and the NST nurse was good for BMI and food intake and moderate for weight loss and disease severity. The full assessment identified7 patients(13.2%) as malnourished. The screening sheet had a sensitivity of 86% and a specificity of 80%. According to the ROC curve, a score of 5 points provided the best validity. Conclusion The nutritional screening tool is reliable when completed by different observers and it is valid for nutritional assessment.

경기도 일부 노인의 노인장기요양보험 방문요양서비스의 영양상태 평가 및 영양관리 현황 (Nutritional Assessment and Management in Long-term Care Insurance's Home Visit Care Service)

  • The purpose of the study was to examine nutritional management and nutritional care demands of home-visit care service and the nutritional status of serviced elderly. The survey was carried out from $5^{th}$ to $21^{st}$ of January, 2011 among 37 In-Home Service institutions. The number of elderly surveyed by 143 care helpers was 281. Analysis was performed for 203 subjects (male: 44, female: 159). The age distribution was from 50 to 99 years. The grading by long-term care insuranceshowed 18,2% on level 1 and 2, and 81.8% on level 3. For the disease treatment, the proportion of implementing diet treatment turned out to be very low. The proportion of subjects living with their children was 45.3%, and living alone was 29.1%. The proportion of home-visit care among types of In-Home Service was 70%. Subjects who needed additional necessary diet management service turned out to be 40.9%. Diet management was the most necessary services from all levels. MNA (specifiy MNA) scores were significantly (p < 0.001) associated with BMI, mid-arm circumference (MAC), calf circumference (CC), intake problems, and weight loss during the last 3 months. For studied elderly, 45.3% were malnourished, and 46.8% were at the risk of malnutrition by MNA score. Based on the result of this survey, the nutritional care was not systematic on the In-Home Service. Prevention and management of the disease was much better than the treatment to improve the quality of life. We conclude that nutrition management is necessary for the elderly. To improve nutritional status of elderly in home care services, systematic nutrition management should be implemented.

  • This paper reviews the available published literature on the antioxidant, anti-inflammatory, and COX-2 inhibition properties of phycocyanin from Spirulina. The potential application of for the prevention and mitigation of such radical-induced chronic diseases like cancer and heart disease, and age-related degenerative diseases like Alzheimer's disease, Parkinson's disease, diabetes and other conditions are discussed based on the available evidence.
