만성 폐쇄성 폐질환 환자에서 병기에 따른 영양상태 평가 (Nutritional Status of Chronic Obstructive Pulmonary Disease Patients according to the Severity of Disease)

  • The purpose of the study was to investigate nutritional status of chronic obstructive pulmonary disease (COPD) patients and to find out the differences according to the stages of disease. From March to October, 2006, 41 stable male patients of mild to severe COPD patients were recruited from Seoul National University hospital. The patients' of body weight and fat free mass were assessed by bioelectrical impedance analysis. The nutritional status of the patients was also assessed by 3-day recall, index of nutritional quality (INQ), dietary diversity score (DDS), dietary variety score (DVS), food group index pattern and dietary quality index (DQI). The total of 41 patients were classified into three groups, stage I, stage II and stage III groups according to the classification of Global Initiative for Chronic Obstructive Lung Disease (GOLD) standard. The mean age of the patients in each stage were 67.2-66.9 years showing no significant difference. The ratio of $FEV_1$/FVC were $57.5{\pm}7.3$, $46.9{\pm}7.6$ and $38.2{\pm}6.8%$, respectively showing significant differences according to the stages of disease. The fat free mass of the stage II ($48.2{\pm}4.7kg$) and III ($47.3{\pm}4.5kg$) was significantly lower than that of stage I ($53.1{\pm}6.9kg$) patients. There were significant correlation of fat free mass with $FEV_{1}$, and BMI (body mass index) with $FEV_{1}$/FVC ratio (p < 0.05). COPD patients showed the diet-related clinical symptoms of anorexia, dyspnea, dyspepsia, and chewing difficulty. Daily intakes of calorie, K, vitamin $B_2$ and folate of the patients were very low ($83.8{\pm}20.7%$, $58.9{\pm}14.4%$, $70.7{\pm}19.6%$ and $74.4{\pm}10.2%$, respectively) however, they did not significantly different according to the stages of disease. Daily intake of calcium was significantly lower in the stage III patients (p < 0.05). The mean scores of dietary variety score was significantly lower in the stage III patients (p < 0.001). Dietary quality index of the patients were not different among the stages of disease and the scores indicated poor quality of diet. As a summary, we found that body fat free mass, regularity of exercise, frequency of having snacks and dietary variety score were significantly associated with the severity of chronic obstructive pulmonary disease.

치주질환 유무에 따른 건강행태와 영양상태 (Health behavior and nutritional status according to the presence or absence of periodontal disease)

  • Objectives: This study investigated health behavior and nutritional status according to the presence or absence of periodontal disease using data from the Sixth National Health Nutrition Survey. Methods: Data from the 2013-2015 National Health Nutrition Survey were analyzed by frequency analysis, chi-square test, and Pearson's correlation analysis using SPSS 21.0. Results: Periodontal disease was more frequent in the group of patients who perceived that their subjective health status was "very bad" or were underweight according to the BMI classification. In men, periodontal disease occurred more frequently in those who were taking calcium (p<0.05), and periodontal disease occurred more frequently in the age group of 20-39, regardless of sex (p<0.05). In women, periodontal disease was less frequent in the group taking vitamin A. In both men and women, the presence of periodontal disease correlated with high fasting blood sugar and glycated hemoglobin levels. Conclusions: A customized program should be developed for each patient to identify factors that affect the relationship among periodontal disease, nutrient intake, and blood parameters. Such information will help improve the oral health of the population.

간이영양상태조사지(Mini Nutritional Assessment)로 조사한 일개 복지관 여성노인의 영양상태 평가와 관련요인 분석 (Nutritional Status among Elderly Korean Women and Related Factors)

  • Purpose: The purpose of this study was to evaluate the nutritional status of elderly Korean women and to investigate related factors. Methods: A total of 100 elderly women attending a local elderly welfare center were recruited for study. Evaluation criteria included demographics, clinical status, nutritional status (using a mini-nutritional assessment), food intake, and nutritional bioparameters, including homocysteine, transferrin, prealbumin and lymphocyte levels, and their antioxidant power status. Results: Among the test group, 83% of the subjects were classified as undernourished, and they often had hyperlipidemia and a lower intake of fiber ($p$ <.05, respectively) and vegetables than normal subjects. In a correlation analysis, their nutritional status was associated with abdominal circumference, the Korean Geriatric Depression Scale score and the Enriched Social Support Inventory score. Nutritional bioparameters and antioxidant power were associated with age among undernourished subjects. Old age, the prevalence of hyperlipidemia, and poor social support significantly increased the risk of undernutrition. Poor social support, the prevalence of diabetes mellitus and depression significantly increased the risk of nutritional deficiency after controlling for age, educational and economic variables. Conclusion: More strategies to increase social support in the Korean female elderly population may improve their nutritional status.

영유아의 영양평가 (Nutritional evaluation of infants and young children - About Definition, Method, necessity, Importance and Korean Medical Approach -)

  • Objectives: This study was conducted to review the overall concept of nutritional evaluation of infants and young children. Methods: Literature search was done to study definition, method and importance of nutritional evaluation of infants and young children. Pubmed and Google scholarship were used in the research. Search words were 'Nutritional evaluation', 'infants' and 'young children'. Results: Nutrition evaluation is mainly used by physical measurement, clinical evaluation, dietary intake survey, and biochemical test depending on the age and disease. The treatment method for 治未病, which is a concept of korean medicine, and 健兒法 for treating the concept of weak children are considered to have very high applicability in solving various problems found through nutrition evaluation in korean medical approach. Conclusion: Infants and children are the fastest growing period of their lives, showing rapid growth in physical, and emotional development along with rapid brain growth. Therefore, inadequate nutrition during this period affects mental and physical growth and development not only during that period but also throughout life. Nutrition evaluation is divided into regular evaluation that includes children in all growing seasons and screening evaluation for problem solving, and follow-up nutritional evaluation that evaluates nutritional status in the long run. For each nutritional evaluation, evaluation is conducted by physical measurement, clinical evaluation, dietary intake survey, and biochemical test. By comprehensively interpreting them, nutritional status is determined. This study provides basic data on the proper nutrition evaluation method to identify and correct nutritional problems such as growth level, nutritional deficiency, and nutritional excess early.

  • 대사체학이 질병, 약물, 스트레스, 식이, 생활습관, 유전적 차이, 장내 미생물 등에 의해서 발생하는 비정상적인 대사 메커니즘을 규명하고 관련 바이오 마커 발굴에 중요한 역할이 증명됨에 따라, 식품 영양학과 대사체학이 융합된 영양 대사체학의 역할이 더욱 중요해지고 있다. 특히 잘못된 식생활에 따른 미래의 질병 예측이 가능해지고 있어 향후 적절한 질병 예방이나 치료를 위한 적절한 식생활이나 식이에 대한 정보를 제공함으로써 건강 증진은 물론 개인별 맞춤식이나 맞춤약물 처방을 통한 개인 맞춤형 건강관리(personalized health care) 시대가 멀지 않았다. 또한 복잡한 식생활 패턴, 대사 반응에 대한 개인 간 차이 그리고 방대한 대사체 데이터와의 관계들을 효과적으로 밝혀낼 수 있는 기술에 대한 지속적인 개발과 영양 대사체학(nutritional metabolomics)이 유전체학(genomics or transcriptomics)과 단백체학(proteomics) 기술과 융합적으로 연구가 이루어질 때 질병과 식사 섭취 사이의 관계가 더욱 투명하게 규명될 것이다.

Nutritional Modulation of Resistance and Resilience to Gastrointestinal Nematode Infection - A Review

  • Disease susceptibility is linked to nutritional status for a wide range of human and animal diseases. Nutritional status can influence both resistance (ability to resist the pathogen) and resilience (ability to tolerate or ameliorate the effects of the pathogen). This review focuses on the nutritional modulation of gastro-intestinal nematode infection in domestic ruminants, primarily sheep. It highlights the duality of the adverse consequences of infection on host nutritional status and the adverse consequences of poor host nutritional status on resistance to infection. Central to both phenomena is the complex, gut-based immune response to gastrointestinal nematode infection. The potential for strategic nutritional supplementation to enhance host resistance and resilience is reviewed together with recent findings on responses to increased ME supply, and long term effects on host immunity of short term protein supplementation.

동물(젖소) 건강 Monitoring System 모델 개발 III. 목장에서 빈발하는 질병의 비용 평가 (Development of a Model for a National Animal Health Monitoring System in Gyeongnam III. Cost Estimates of Selected Dairy Cattle Diseases)

  • A study was conducted to estimate cost of major dairy cattle diseases. Forty (n=40) of the 167 dairy herds in Gyeongnam (Chinju) area were stratified and selected randomly for participation in the national animal health monitoring system. Gyeongsnag University veterinarians, Gyeongnam Livestock Promotion Institute veterinarians and clinic veterinarian visited each herd once a month for a total periods of 12 months. At a each visit data on disease, production, management, finance, treatments, preventive activities, animal events, and any other relevant events were collected. Monthly and annual cost estimates of disease treatment were in computed in each herd and stratum(including cost of prevention). Results were expressed as cost per head and given separately for cows, young stock, and calves. In cows, the most expensive seven diseases entities (from the most to the least) were : (1) clinical mastitis; (2) breeding problems; (3)gastrointestinal problems; (4) multiple system problem; (5) birth problems; (6) metabolic/nutritional disease; (7) lameness. In young stock, the most costly disease were the multiple system problems, breeding problems, respiratory disease, gastrointestinal disease, and lameness. In calves, the most costly disease problems were gastrointestinal problems, respiratory disease, integumental, multiple system problems, and metabolic/nutritional problems.
