Intensity-Modulated Radiotherapy for Nasopharyngeal Carcinoma: Penang General Hospital Experience

  • Purpose: To study the overall treatment time (OTT) and acute toxicity of intensity-modulated radiotherapy (IMRT) treatment for nasopharyngeal carcinoma (NPC). Methods: This retrospective study covered all NPC patients who underwent radical IMRT treatment at the Penang General Hospital from June 2011 to February 2012. Patients of any age and stage of disease with histologically proven diagnosis were included. Information was collected on patient demographics, clinical stage, treatment received, including any neoadjuvant and/or concurrent chemotherapy, acute toxity and completion of IMRT within the OTT. Results: A total of 26 NPC patients were treated with IMRT during the study period; 88.5% had stage III/IV disease. 45.2% received neo-adjuvant chemotherapy while 50.0% were given concurrent chemo-irradiation. All patients completed the treatment and 92.3% within the 7 weeks OTT. Xerostomia was present in all patients with 92.3% having grade 2. Severe grade III/IV acute toxicity occurred in 73.1% of patients, the commonest of which was oral mucositis (57.6%). This was followed by dysphagia which occurred in 53.8%, skin reactions in 42.3% and weight loss in 19.2%. However, haematological toxicity was mild with only one patient having leucopaenia. Conclusion: IMRT treatment for NPC is feasible in our center. More importantly, it can be delivered within the 7 weeks OTT in the majority of patients. Severe grade 3/4 toxicity is very common (73.1%) and thus maximal nutritional and analgesic support is required throughout the treatment.

Processing Procedures and Feeding Systems for Sorghum-based Diets Given to Lactating Sows

  • One hundred and twenty nine primiparous sows were used to determine the effects of alternative processing procedures and feeding systems on the nutritional value of sorghum grain-based diets for lactating sows. Treatments were a ground sorghum control, steam-flaked sorghum and extruded sorghum fed in meal form and the ground sorghum control given as pellets or gruel (1:1 ratio of water and feed on a volume:volume basis). Average daily feed intake was greater for sows fed pelleted and gruel diet forms than sow fed the diets with steam-flaked and extruded sorghum (p<0.04). However, no differences occurred in sow weight or backfat losses among the treatments (p>0.14). Number of pigs weaned and livability rates were similar among treatments except that steam-flaked sorghum supported greater litter weight gains than extruded sorghum (p<0.02). Apparent digestibilities of DM, N and GE in sows fed steam-flaked and extruded sorghum were greater (p<0.001) than in sows fed pelleted or gruel diets. Of the various treatments, sows fed extruded sorghum tended to have the highest digestibilities of DM, N and GE, and lowest excretions of DM and N in the faeces. Severity of ulceration was not significantly affected by treatments (p>0.35), but keratinization was greatest for sows fed extruded sorghum (p<0.01). In conclusion, the alternative processing methods (steam-flaking and extrusion) and feeding system (pellets and gruel) had little effect on sow and litter performance. However, nutrient digestibilities were improved for all treatments that involved heating (steam flaking, extrusion and pelleting) and, thus, these treatments resulted in less fecal excretion of DM and N.

흡연 및 비타민 D 수준과 한국 성인 남녀 대사증후군의 연관성 (Association between Smoking Status, Vitamin D Levels and the Korean Adult Male and Female Metabolic Syndrome)

  • Purpose : Several studies suggest that smoking and vitamin D level is a risk factor of metabolic syndrome. The objective of this study is to evaluate the association between smoking status, vitamin D levels and the Korean adult male and female metabolic syndrome. Methods : We assessed 3796 participants aged 19 years and older from the Korean National Health and Nutritional Examination Survey 2013, 2014. Smoking statuses were collected from self-reported questionnaires. Subjects were divided into three categories: non-smokers, former smokers, and current smokers. Vitamin D was tested by Radioimmunoassay method and the value of serum 25-hydroxyvitamin D, which is an index of vitamin D status in the body, was used. The diagnosis of metabolic syndrome was made using criteria modified NCEP-ATP III. Logistic regression analysis was used to calculate odds ratios between smoking status, vitamin D levels, and metabolic syndrome. Results : The overall prevalence of metabolic syndrome was 23.0 % in men and 15.4 % in women. After adjusting for smoking, the odds ratio for men's metabolic syndrome in current smokers was 1.77 (95 % CI, 1.30~2.41), while for former smokers OR was 1.63 (95 % CI, 1.15~2.31) compared with nonsmokers. After adjustment vitamin D, the odds ratio for women's metabolic syndrome in vitamin D deficiency was 1.44 (95 % CI, 1.11~1.87) compared with normal. Conclusion : Smoking status was associated with an increased risk of metabolic syndrome in Korean adult males and decreased vitamin D level was associated with an increased risk of metabolic syndrome in Korean adult females.

수원지역 노인 영양개선 전략 연구 : 식습관 및 식품기호도 분석 (Strategies to Improve Nutrition for the Elderly in Suwon : Analysis of Dietary Behavior and Food Preferences)

  • To promote health status, strategies and interventions to improve nutrition should be based on the proper diagnosis of the subject's eating patterns. The elderly usually have traditional food habits and preferences, and it is very difficult to change them. This study was designed to identify dietary behavior and food preference of the elderly, in order to provide baseline data for the Elderly Nutrition Intervention Program for the Public Health Center. A survey questionnaire was made for use by trained interviewers to query 151elderly people from 5 community elderly centers located in Suwon, Korea. The majority of them ate regularly and partook of all available side dishes. Their major dietary problems were frequent consumptions of salty foods, and eating too quickly. They consumed grains and vegetables regularly, but seldomly ate dairy products, fruits, meat and food prepared with oil. They also tended to eschew ready made processed food, high cholesterol food, and fast food. Also they did not dine out as much as younger people. Desirable eating habit score were not significantly influenced by socioeconomic variables and nutrition-related characteristics. These included nutrition knowledge, Nutritional Risk Index(NRI) and a score of health concerns. However, meal balance scores were significantly higher in the younger group(p<.05), the higher household income group(p<.05). According to stepwise multiple regression analysis, NRI was the most important determinant of a desirable eating habit score for the male elderly, whereas the score of health concerns was mo9st important for female elderly subjects. The greatest predictor of the meal f balance score was nutrition knowledge. The elderly liked sweet tasting food, grains, rice, stews and Korean style soups. They disliked sour food, dairy products, processed food, and bread. The results indicate that the Elderly Nutrition Education Program should focus on increasing consumption of dairy products, fruits and food with oil, prepared by traditional Korean cooking methods. It also suggests that the program planning should consider the socioeconomic status of the elderly, such as income and education level, as well as concern for health.

Effects of rumen cannulation surgery on physiological parameters and rumen fluid pH in Korean native Hanwoo cattle

  • Rumen cannulation is used for nutritional and microbiological research, clinical diagnosis, and rumen component transfaunation. However, the cannulation procedure can affect parameters such as complete blood count findings, serum chemistry, and rumen fluid pH. The objective of this study was to evaluate the health risks related to the rumen cannulation procedure over a 1-month period. We did not identify significant differences in red blood cell numbers or morphologies between pre- and postoperative timepoints. Moreover, no inflammation or infection was detected. Despite the absence of apparent clinical signs after surgery, serum chemistry results revealed changes in blood urea nitrogen levels and the activities of liver enzymes, including aspartate transaminase, lactate dehydrogenase, and creatinine kinase, from postoperative days 1 to 14. Rumen fluid pH, as measured from samples collected via an orogastric tube, was slightly increased after a preoperative fasting period and on postoperative day 1 but decreased thereafter from postoperative day 4, indicating a minor influence of cannulation surgery on ruminal fluid pH. This is the first study to evaluate hematological parameters and rumen pH before and after rumen cannulation surgery in Hanwoo cattle. Further research is required to better elucidate the potential effects of rumen cannulation surgery on animal health.

Predictor of Liver Biochemistry Improvement in Patients with Cytomegalovirus Cholestasis after Ganciclovir Treatment

  • Purpose: Cholestasis resulting from cytomegalovirus (CMV)-induced hepatitis manifests in 40% of patients with a CMV infection. Ganciclovir treatment in children with CMV infections has proven to be highly effective. Until now, there are very few studies have identified predictive factors for liver biochemistry improvement after ganciclovir therapy. This study aimed to identify the predictors of liver biochemistry improvement in patients with CMV cholestasis after ganciclovir treatment. Methods: A retrospective cohort study was conducted using medical records from Dr. Sardjito General Hospital Yogyakarta, Indonesia from 2013 to 2018. CMV cholestasis was confirmed based on serum CMV IgG and IgM positivity and/or blood and urine CMV antigenemia positivity. Incomplete medical records and other etiologies for cholestasis, such as biliary atresia, choledochal cyst, metabolic diseases, and Alagille syndrome, were excluded. Patient age at cholestasis diagnosis and ganciclovir treatment, duration of CMV cholestasis, history of prematurity, central nervous system involvement, and nutritional status were analyzed and presented as an odds ratio (OR) with a 95% confidence interval (95% CI). Results: CMV cholestasis with ganciclovir therapy was found in 41 of 54 patients. Multivariate analysis showed that a shorter duration of CMV cholestasis (OR: 4.6, 95% CI: 1.00-21.07, p=0.04) was statistically significant for liver biochemistry improvement after 1 month of ganciclovir treatment. The remaining factors that were analyzed were not significant predictors of liver biochemistry improvement in patients with CMV cholestasis after ganciclovir treatment. Conclusion: A shorter duration of CMV cholestasis is the predictor of liver biochemistry improvement after 1 month gancyclovir treatment.

Reproductive management of dairy cows: an existing scenario from urban farming system in Bangladesh

  • Background: Reproductive management practices play crucial roles to maximize the reproductive performance of cows, and thus contribute to farm profitability. We aimed to assess the reproductive management of cows currently practiced in the dairy farms in an urban farming system. Methods: A total of 62 dairy farms were randomly selected considering all size of farms such as small (1-5 cattle), medium (6-20 cattle) and large farms (> 20 cattle) from selected areas of Dhaka city in Bangladesh. The reproductive management-related parameters viz. estrus detection, breeding method, pregnancy diagnosis, dry cow and parturition management, vaccination and treatment of reproductive problems etc. were obtained in a pre-defined questionnaire during the farm visit. Results: The visual observation method was only used (100.0%; 62/62) for estrus detection irrespective of size of the farms; while farmers observed cows for estrus 4-5 times a day, but only for 20-60 seconds each time. Regardless of farm size, 89.0% (55/62) farms used artificial insemination (AI) for breeding the cows. Intriguingly, all farms (100.0%) routinely checked the cows for pregnancy at 35-40 days post-breeding using rectal palpation technique by registered veterinarian. However, only 6.5% (4/62) farms practiced dry cow management. Notably, all farms (100.0%) provided nutritional supplements (Vit D, Ca and P) during late gestation. However, proper hygiene and cleanliness during parturition was not practiced in 77.4% (48/62) farms; even though 96.7% (60/62) farms treated cows by registered veterinarian for parturition-related problems. Conclusions: While farmers used AI service for breeding and timely check their cows for pregnancy; however, they need to increase observation time (30 minutes/ observation, twice in a day: early morning and early night) for estrus detection, consider dry cow management and ensure hygienic parturition for maximizing production.

Comparison of food and nutrient intake according to the presence of glaucoma among Korean older adults

  • BACKGROUND/OBJECTIVES: The purpose of this study was to compare nutrient intake according to the presence of glaucoma among Korean individuals aged 60 yrs or older and to establish evidence for the necessity of dietary habits and nutritional management in the prevention and management of glaucoma. SUBJECTS/METHODS: The study was conducted on 4,195 older adults aged 60 or older who participated in the Korea National Health and Nutrition Examination Survey in 2017 and 2018. The subjects were classified into glaucoma (n = 186) and non-glaucoma (n = 4,009) groups based on the doctor's diagnosis of glaucoma. Nutrient intake was estimated using a 24-h recall method, and the intake of nutrients between the 2 groups was compared. RESULTS: The comparison of food group and nutrient intake according to the subjects' prevalence of glaucoma showed that the average intake of potatoes and starches was higher in the glaucoma group than in the non-glaucoma group (P = 0.049), whereas the average intake of meat, fish, and shellfish was higher in the non-glaucoma group than in the glaucoma group (P = 0.045, P = 0.018). Additionally, the average intake of omega-3 and vitamin C was higher in the non-glaucoma group than in the glaucoma group (P = 0.022, P = 0.045), while the average intake of niacin was higher in the glaucoma group than in the non-glaucoma group (P = 0.046). CONCLUSION: The intake of meats, fish, and shellfish, omega-3, and vitamin C was higher in the non-glaucoma group than in the glaucoma group. These results suggest that a healthy diet might be necessary for the prevention and management of glaucoma.

산전검사 대상 질환에 대한 법적 규제의 문제점에 대한 고찰 (Controversial issues in the legal restriction for prenatal genetic testing in Korea)

  • 현재까지 보고 된 유전질환을 포함한 희귀질환은 6000종이 넘으며, 이 중 2007년 12월 현재, 1,500종(임상검사 목적 1,211종과 연구 목적 289)의 유전자 검사가 가능하다. 외국의 경우, 원인 유전자가 밝혀지고 진단이 가능한 모든 유전질환에 대해 착상 전 및 산전 유전자검사가 가능한데 반해, 국내에서는 2005년 제정된 생명윤리 및 안전에 관한 법률 제25조 2항에 의해 착상전 및 산전 유전자검사가 가능한 유전 질환은 63종으로 제한되어 있다. 이 보고에서는, 63종으로 제한된 검사항목에 포함되어 있지 않아 산전 진단을 할 수 없게 된 최근의 증례를 검토하고 문제점과 대안에 대해 논의하였다. X-성염색체 연관 열성질환인 MNK의 보인자로 확진된 L씨(여 38세)는 2명의 자녀를 출산하였는데, 그중 1명은 MNK에 이환된 남아로 출생 후 사망하였다. L씨는 2003년에 산전 유전자검사를 실시하여 정상의 남아를 출산하였다. 현재 임신 중인 L씨는 MNK에 이환된 남아를 또다시 출산할 가능성이 50%로 산전 유전자 검사가 필요하지만, 2005년에 제정된 생명윤리 및 안전에 관한 법률에 의해 산전 유전자검사가 법적으로 불가능하였다. 이 증례는 유전질환 검사항목을 63종으로 제한한 현행법의 문제점과 질환 형평성의 문제점을 실질적으로 보여주고 있다. 질환 명에 상관없이 유전질환의 가족력이 있는 가족에게는 산전 유전자검사에 대한 자기결정의 기회가 제공되어야 할 것이며, 만일, 현행법의 개정이 현실적으로 불가능하다면, MNK처럼 3년 내에 사망에 이르는 등 질병의 정도가 심하며, 효과적인 치료 방법이 없는 질환에 대해서는 산모나 가족이 원하는 경우 전문의의 전문적인 판단에 근거하여 예외가 인정되어야 한다고 사료된다.

간호사의 가정간호를 위한 교육요구 분석 (A study on educational need of nurses for home care)

  • This study was conducted from July to December 1990, in order to diagnose nurses' educational need for home care. The study subjects consisted of 145 nursing educators, and the 3 groups of nurses, namely 250 senior nursing students of diploma and collegiate program, 235 health center nurses, 521 university' hospital nurses in Seoul. Four types of questionaires were formulated by Delphi method. Two questionaires for the nursing educators were designed to measure their expectations of nurses' knowledge and of their skill for home care, and another two questionaires for the nurses to measure their actual home care knowledge and skill. The results of the study were as follows : 1) The mean scores of educators' expectation for home care knowledge were 17.68 for the care of dependence on medical equipment, 17.44 for the care of mobility impairment patient, 16.56 for the care of cardiopulmonary impairment patient, 16.40 for the care of nutrition and elimination impairment patient, '1.20 for the care of psychiatric disorder patient and 9.03 for the care of cancer and terminally ill patient,. 2) The mean scores of nurses' home care knowledge tested by 20 items were 14.36 for the care of mobility impairment patient, 13.28 for the c8;re of dependence on medical equipment, 13.78 for the care of cardiopulmonary impairment patient, 12.92 for the care of nutrition and elimination impairment patient, and those of tested by 10 items were 7.08 for the care of psychologic disorder patient, 7.80 for the care of cancer and terminally ill patient. The sum of means marked 69.23. As for the nurses' home care knowledge categorized by tasks in terms of the group, significant difference were shown in the care of mobility impairment(P=0.00), cancer and terminally ill(P=0.03), nutrition and elimination impairment(P=0.00) and psychologic disorder patient(P=0.00). No significant difference were shown in the care of dependence on medical equipment and cardiopulmonary impairment patient. 3) Regard to educational need of nurses' home care knowledge categorized by task according to the group it was found that all sampled nurses had educational need in the care of mobility impairment, dependence on medical equipment, cardiopulmonary impairment, cancer and terminally ill patient. It was found that health center nurses had educational need in the care of psychologic disorder. No educational need were found in the health center nurses whose career less than 2 years, in the care of mobility impairment, cardiopulmonary impairment and psychologic disorder patient, and in those of career with 2-5 year in the care of psychologic disorder patient. No educational need were found in the hospital nurses whose career more than 15 years, in the care of cardiopulmonary impairment patient and in those of career with 11-15 year, in the care of cancer and terminally ill patient. 4) The mean scores of educators' expectation for home care skill measured by Likert 5 points scale were 4. 21 for assessing, 4.49 for planning, 4.29 for basic care, 4.42 for curative care, 4.40 for rehabilitative care, 4.36 for emergency care, 4.53 for medication, 4.31 for nutritional care, 4.32 for other means for care, and 4.38 for evaluation. 5) Regard to nurses' home care skill measured by Likert 5 points scale of self evaluation, there was a significant difference between the nurses' home care skill and group(P=0.00l). The higher scores reported by students were vital sign checking and basic care while the scores of below medium were curative care and emergency care. The higher scores reported by health center nurses were vital sign checking, other means for care and care of specimen while the scores below medium were curative, emergency and nutritional care. The higher scores reported by hospital nurses were vital sign checking, care of specimen and basic care, while the score below medium was emergency care. 6) Regard to educational need of nurses' home care skill by nursing process activity according to the group it was found that health center nurses had educational need in all nursing skills including vital sign checking, care of specimen, health assessment, socioeconomic assessment, nursing diagnosis, care plan, basic care, curative care, rehabiitative care, psychological care, emergency care, medication, nutritional care, other means for care and evaluation. And students had educational need in all nursing skills except vital sign checking, and hospital nurses had educational need in all nursing skills except vital sign checking, care of specimen and basic care. 7) In short, the result of this study suggests that the curriculum should be organized in accordence with nurses' educational background and their career for the education of nurses for home care. It should be considered to develop the short term educational program focused on curative and rehabilitative care for health center nurse or community health nurse practitioner and which was focused on family care for hospital nurse. Concerning about this field practice for home care nurse, they are required not only community practice but also . clinical practice including emergency, curative and rehabilitative care.
