한국 중년의 고밀도지단백(HDL) 콜레스테롤과 관련요인 (The High-density lipoprotein (HDL) cholesterol and related factors in Korean middle-aged)

  • 이 연구는 40-59세의 중년들이 갖고 있는 여러 가지 건강행태 중에서 고밀도지단백(HDL) 콜레스테롤에 영향을 명확히 미치는 건강행태를 파악함으로써 한국 중년의 건강증진에 도움이 되는 기초자료를 제공되고자 한다. 국민건강영양조사의 제4기 3차년도(2009), 제5기의 1차년도(2010), 2차년도(2011), 3차년도(2012), 제6기의 1차년도(2013) 등 총 5개년의 원시데이터를 합하여 사용하여 7,406명을 최종분석 대상자로 선정하였다. 고밀도지단백 콜레스테롤을 40mg/dl 미만과 40mg/dl 이상으로 두 집단으로 구분했다. 다중 로지스틱 회귀분석을 시행한 결과, 여자보다 남자일 경우 OR=3.916배, 비만 유병이 저체중에서 정상일 경우 OR=3.439배, 비만일 경우 OR=7.336배, 음주자일 때 OR=1.629배, 흡연자일 때 OR=1.498배, 중등도 신체활동 미실천일 시 OR=1.426배, 걷기 미실천일 시 OR=1.264배, 탄수화물을 과소 섭취할 때보다 적정 섭취할 때 OR=1.510배, 과다 섭취할 때 OR=1.787배 고밀도지단백(HDL) 콜레스테롤이 40mg/dl 이상에서 40mg/dl 미만으로 될 확률이 증가하였다. 한국 중년의 고밀도지단백(HDL) 콜레스테롤 수치를 높여 적절한 건강을 유지하기 위해서는 유산소 운동, 금연, 절절한 음주, 비만 해소, 건강한 식습관과 건강한 식품 선택으로 인한 외식이 이루어져야 한다. 이를 위해서는 지역사회 내에서 지속적인 홍보와 교육이 필요하며, 건강행태를 생활 속에서 실천할 수 있도록 하는 사회적 환경이 구축되어야 할 것이다.

추나체형진단기와 단순 방사선 검사로 측정된 신체 지표들간의 상관 분석 (Correlation Analysis of Body Parameters between Chuna Posture Analysis System and X-ray)

  • Objectives This study analyzed the correlation between body parameters measured using X-ray and Chuna posture analysis system to determine their clinical value in diagnosing and evaluating skeletomuscular diseases. Methods X-ray and Chuna posture analysis system were performed for 105 patients to measure physical characteristics such as Interacromial angle, Pelvic obliquity angle, Structural leg length discrepancy (SLLD), Midpatella-midtalus angle (MMA) and Q-angle, Anterior head translation (AHT), Anterior superior iliac spine to posterior superior iliac spine angle (ASIS-PSIS angle), Interscapular angle, Scoliotic angle and Cobb's angle. Statistical analysis using statistical analysis techniques and Pearson correlation coefficients was performed to assess the body parameters obtained by X-ray and Chuna posture analysis system. Results Significant correlations were observed between the values for Interacromial angle, Pelvic obliquity angle, SLLD, MMA and Q-angle, AHT, ASIS-PSIS angle, Interscapular angle, Scoliotic angle and Cobb's angle obtained by X-ray and Chuna posture analysis system. Significant correlations were also observed between right MMA and left Q-angle as well as between left MMA and right Q-angle. Conclusions Chuna posture analysis system can be used instead of X-ray measure body parameters and perform posture analysis in clinical practice. This study's findings are expected to serve as a basis for further research on the clinical application of Chuna posture analysis system.

노인의 건강지각과 건강증진행위 (A Study on Health Perception and Health Promoting Behavior in the Elderly)

  • This study was designed to identify the relationship between health perception arid health promoting behavior in the elderly. The subjects for this study were 92 persons over 65, who could be easily met in households, pavilions for the aged, and parks. The data were collected during the period from March 24th to April 12th, 1997 by interviews with a structured questionnaire. The instruments used for this study are as follows: The Health Perception Questionnaire developed by Ware (1979) and translated by Yu Ji-Su(1985). The Health Promotion Lifestyle Profile(HPLP) developed by Walker, Sechrist & Pender (1987) and partially amended by the author of this study. The Health Promotion Lifestyle Profile(HPLP) was parameterized by nutrition, exercise, stress management, self actualization, health responsibility, and interpersonal support. The data was analyzed by frequency, percentage, mean, standard deviation, t - test, ANOVA, and Pearson's correlation using an SPSS program. The results of this study were as follows: 1. Hypothesis 1 : 'The higher the level of health perception in the elderly, the higher the practice of health promoting behavior in the elderly' was supported(r=0.449, p<0.001). 2. The health perception mean score on a four point scale was 2.564. 3. For health promoting behavior, the mean score on a four point scale was 2.560. The factor of the highest mean score was interpersonal support(2.850) and factor of the lowest mean score was exercise(1.964). 4. Among the general characteristic variables, sex(t=4.12, p<0.001), religion(t=2.47, p<0.05), education(F=3.27, p<0.05), marital status (t=2.22, p<0.05), living situation(F=3.50, p<0.05), and hobby(t= -2.57, p<0.05) showed strong connections to health perception. 5. Among the general characteristic variables, religion(t = 1.83, p<0.05), and marital status(t= 2.12, p<0.05) showed strong connections to health promoting behavior. In conclusion, this study revealed that health perception is an important factor related to health promoting behavior in the elderly. The general characteristic variables showed strong connections to health perception and health promoting behavior. Therefore, nurses should consider health perception and general characteristic variables when they plan nursing intervention for the elderly. Also, educational programs encouraging constructive health perception should be developed.

A Study on the Strategic Vitalization Plan of Korean Integrated Medical Tourism

  • Objectives: As a newcomer to the medical tourism industry, Korea needs to differentiate itself from the leading competing countries to vitalize its early-stage medical tourism industry. This study aims to introduce a strategic plan to vitalize the Korean integrated medical tourism so that Korea can differentiate itself from competing countries and create high added value. Methods: The concept and actual conditions of medical tourism and Oriental medical tourism were examined. A plan to differentiate Korea from the competing countries in the medical tourism industry was studied to create high added-value through strategic vitalization of its medical tourism industry. Results: Korean integrated medical tourism must be developed differently from those of other South-East Asian countries in order to strategically promote the cash-cow medical tourism industry. In order to develop such medical tourism, Korean medical practice, which integrates Western and Oriental medicine, is to be developed through mutual understanding and fusion of other disciplines among medical doctors and Oriental medical doctors who are working in local healthcare for health promotion of local residents and disease prevention and control. This will play a key role in developing a unique medical tourism product of Korea by means of strategic alliances as an integrated medicine. Manpower specialized for integrated medicine is to be specially supplied for Oriental medicine-related business lines at city, county and borough levels, among local governments, that are enthusiastically carrying forward Oriental medical tourism with an interest to promoting more active and strategic business development and raise the effectiveness and efficiency of public health centers handling related medical tourism. Manpower specialized for Korean integrated medical tourism is to be specially supplied for the Ministry of Culture, Health and Tourism, a policy control tower to develop and vitalize high value-added fusion (theme) tourism products such as the Korean integrated medical tourism, in order to discover, promote and support Korean integrated medical tourism's differences from existing medical tourism. Conclusions: The differentiated integrated medical tourism that only Korea can offer in a variety of forms, in order to create a key area of high value-added medical tourism, should be strategically vitalized through a liaison between integrated medicine and tourism and the realization of patient-centered health care services with medical technology developed based on mutual understanding of Western and Oriental medicine.

LDL-콜레스테롤의 Friedewald 계산값과 실측값 비교: 국민건강영양조사 2009-2010 (Friedewald-Estimated Versus Directly Measured LDL-Cholesterol: KNHANES 2009-2010)

  • LDL-콜레스테롤(LDL-C)은 심뇌혈관질환의 주된 교정 가능한 위험인자로서, 정확한 측정값을 임상에 적용하는 것이 중요하다. 하지만 LDL-C의 측정은 실제 측정이 아닌 Friedewald 공식에 의한 계산방법이 널리 이용되고 있다. 본 연구의 목적은 LDL-C의 Friedewald-추정값과 실측값을 비교하고, 두 방법의 LDL-C 위험수준 분류 일치도를 평가하는 것이다. 표본은 국민건강영양조사 2개년(2009년과 2010년)의 공개된 자료에서 추출되었고, 혈액 검사에서 총 콜레스테롤, HDL-콜레스테롤, 직접 측정한 LDL-C, 그리고 중성지방 중 어느 한 결측치도 없는 4,319명을 연구대상으로 하였다. 중성지방 400 mg/dL 미만일 때, Friedewald-추정값과 실측값은 높은 상관관계를 보였고 (r = 0.958, p < 0.001), 위험수준 분류 일치 백분율은 82.7%이었다. 중성지방 수준이 높을수록, 일치 백분율은 낮았다. 중성지방 수준 150 mg/dL 미만, 150-200 mg/dL, 그리고 200-399 mg/dL일 때, 일치 백분율은 각각 85.4%, 78.2%, 그리고 71.4%이었다. Friedewald 공식은 중성지방 농도 150 mg/dL 미만에서는 LDL-C를 과대평가하는 반면, 중성지방 농도 150 mg/dL 이상에서는 과소평가하는 경향이 있었다. 이에 따라 LDL-C 위험수준 분류에 있어 그 범주가 과대평가된 사람은 382명 (9.1%)인 반면, 과소평가된 사람은 348명 (8.3%)이었다. 이러한 결과는 Friedewald-추정값의 LDL-C 과소평가뿐만 아니라, 과대평가도 심각한 문제일 수 있음을 제시한다.

Relationship of Smoking with Self-perceived Health and Selected Health Behaviors

  • The purpose of this study was to analyze variables associated with smoking and selected health behaviors. This study targeted adult men and women over 20 of age. The sample population was drawn from the national sample, and the samples were chosen from the telephone book. A total of 1,500 cases were collected through a telephone based interview survey. As a tool for this study, a structured questionnaire was developed. the variables included self-perceived health status, and selected 7 health behaviors; smoking behavior, physical activities, eating habit, weight control, alcohol consumption, stress management, and cancer exam. The healthy life practice actions of Trans-theoretical model have been classified into five stages. Smokers were more likely than non-smokers to positive attitudes toward smoking and the impact of smoking in this study. This means that smokers's awareness toward the impact of smoking is very week. Smoking behavior was significantly related to other health related behaviors based on the correlation analysis. However, gender, engaging in regular physical activities, moderate consumption of alcoholic beverages, and receiving a cancer examination were the variables that can explain and predict a person's stages of adopting a non-smoking behavior. Self perceived health status was closely related to other health behaviors. Gender, age, and monthly household income were important demographic variables that have significant relationship with the self-perceived health status. Among the health behaviors, regular physical activities and weight control were significant predictable variables. Similar results have been found in the National health and Nutrition examination survey in Korea. As a result of this study, it was found that among the selected health behaviors, engaging in physical activity was an important variable to increase positive sense of health and non-smoking behaviors. Educational and policy level input is needed to increase awareness and provide chances to participate in fitness activities. To those who maintain exercise, the environmental support and diffusion of knowledge information and education data should be strengthened. To change attitudes toward smoking, more in-dept awareness campaign and education should be provided according to people's different behavioral status. In addition, not only diffusing health education data and delivering knowledge information through related programs, but also environmental support system that helps an individual maintain his/her action is required. Such a support system means settlement of the health enhancement base of school, workplace and community should be carried out, and the policy level support and regular programs should be provided and spread to the unit of community.

심뇌혈관질환자의 소득과 교육수준에 따른 건강행태 비교 - 2010-2011년도 국민건강영양조사 자료를 이용하여 - (Comparison of the Health Behaviors according to Income and Education Level among Cardio-Cerebrovascular Patients; based on KNHANES data of 2010-2011)

  • 본 연구는 심뇌혈관질환자들을 대상으로 소득수준과 교육수준이 건강행태에 미치는 영향을 분석하여 질병관리와 예방에 도움을 주는 방안을 제시하는데 있다. 2010-2011 국민건강영양조사 원시자료를 이용하여 30세 이상의 심뇌혈관질환자 3,687명을 대상으로 건강행태, 일반적(사회인구학적) 특성, 신체계측 및 혈액검사를 소득수준별, 교육수준별로 상관관계를 분석하였다. 본 연구의 결과, 소득수준과 교육수준이 낮을수록 심뇌혈관질환의 유병율이 높고, 현재흡연에서의 OR(95% CI) 값은 고소득자 0.71(0.52-0.96), 고교육자 0.41(0.29-0.58)로 소득과 교육수준이 낮을수록 건강수준의 감소와 관계되는 현재흡연과의 상관성이 높았다. 한편 1개월음주와 고위험음주에서 고소득자의 OR(95% CI) 값은 각각 1.55(1.22-1.95), 1.42(1.11-1.82)로 음주율이 고소득자에서 높았으나 교육수준에서는 유의한 차이가 없었다. 걷기운동의 실천율은 소득과 교육수준 모두 유의한 차이가 없었다. 따라서 소득수준이 높은 환자군에서는 절주홍보를 강조하고, 소득과 교육수준이 낮은 환자군에서는 금연의 중요성을 강조하는 것 이외에도 서비스공급을 위한 제도적인 뒷받침이 필요할 것으로 보인다.

서울시내 직장인의 스트레스와 식생활관리 (A Study on the Stress and Dietary Life of Office Workers in Seoul)

  • The purpose of this study was to investigate the relationship between the dietary attitude and health on the stress status of office - workers in Seoul. This research was conducted on 389 office workers (224 males and 165 female). In relation to stress, it was found that the degree of stress as an office workers was ′slight′ and much for 45.2% and 40.6%, respectively, of the study population. Consequently, most office workers were found to be under stress. The amount of food intake under stress was found to be ′reduced′, unchanged and increased in 38.3, 37.0 and 24.7%, respectively, of those surveyed. The preferred foods when under stress were found to be alcoholic and non-alcoholic beverages, in 57.6% of cases, which was the highest proportion regardless of age, sex, marital status, occupation and educational level. The desired taste when under stress was found to be ′hot′ and sweet in 34.7 and 26.0%, respectively. As to the feeling after food intake, 51.7% responded that food intake was not useful for the relief of stress, which was the highest proportion and many salaried females worried about weight gain. With regard to the relationship between stress, health and nutrition, 69.9% of office workers responded that they thought the relationship of these factors was very high. As for the greatest cause triggering stress, 50.3% responded "due to workplace and job". The best method for relieving stress was to do exercise, with a 30.1 % response, The average score for the degree of stress and tension was 7.2, with 64.0% of office workers having an average level between 6- and 9. The degree of stress was higher for females than males, for unmarried salaried and hot-tempered persons. Those not doing exercise were subject to the highest levels of stress, and those a having sufficient sleep were found to be subject to less stress. Those interested in their health were found to have a good condition and lower levels of stress. In correlation with eating habits, stress, the degree of tension and personality, the F-value was found to be 43.505 (p<0.01), with an explanatory power of 0.294 (29.4%), indicating significant differences. This means that office workers with higher degrees of stress and tension tended to have poorer eating habits. Since bad eating habits have an adverse effect on both the state of physical health and individuals emotional development, for the purpose of correctly managing stress, office workers should make efforts to practice good eating habits.

Effects of reducing inclusion rate of roughages by changing roughage sources and concentrate types on intake, growth, rumen fermentation characteristics, and blood parameters of Hanwoo growing cattle (Bos Taurus coreanae)

  • Objective: Reducing roughage feeding without negatively affecting rumen health is of interest in ruminant nutrition. We investigated the effects of roughage sources and concentrate types on growth performance, ruminal fermentation, and blood metabolite levels in growing cattle. Methods: In this 24-week trial, 24 Hanwoo cattle ($224{\pm}24.7kg$) were fed similar nitrous and energy levels of total mixed ration formulated using two kinds of roughage (timothy hay and ryegrass straw) and two types of concentrate mixes (high starch [HS] and high fiber [HF]). The treatments were arranged in a $2{\times}2$ factorial, consisting of 32% timothy-68% HS, 24% timothy-76% HF, 24% ryegrass-76% HS, and 17% ryegrass-83% HF. Daily feed intakes were measured. Every four weeks, blood were sampled, and body weight was measured before morning feeding. Every eight weeks, rumen fluid was collected using a stomach tube over five consecutive days. Results: The mean dry matter intake (7.33 kg) and average daily gain (1,033 g) did not differ among treatments. However, significant interactions between roughage source and concentrate type were observed for the rumen and blood parameters (p<0.05). Total volatile fatty acid concentration was highest (p<0.05) in timothy-HF-fed calves. With ryegrass as the roughage source, decreasing the roughage inclusion rate increased the molar proportion of propionate and decreased the acetate-to-propionate ratio; the opposite was observed with timothy as the roughage source. Similarly, the effects of concentrate types on plasma total protein, alanine transaminase, Ca, inorganic P, total cholesterol, triglycerides, and creatinine concentrations differed with roughage source (p<0.05). Conclusion: Decreasing the dietary roughage inclusion rate by replacing forage neutral detergent fiber with that from non-roughage fiber source might be a feasible feeding practice in growing cattle. A combination of low-quality roughage with a high fiber concentrate might be economically beneficial.

들깻잎과 생산환경의 미생물 분포 (Distribution of Microorganisms in Perilla Leaf and Cultivation Area)

  • 본 연구는 국내 일일 평균 섭취량이 많은 들깻잎을 대상으로 미생물 안전성을 평가하고 미생물에 대한 오염도를 조사하였고, 검출빈도가 높은 S. aureus와 B. cereus의 오염원을 조사하였다. 들깻잎의 미생물 오염도 조사 결과 일반세균수 수준은 4.82 log CFU/g이었다. 대장균군의 오염수준은 0.83-5.33 log CFU/g으로 농가 간 차이가 크게 나타났으며 50% 농가의 들깻잎에서 4.00 log CFU/g 이상 검출되었다. 또한 병원성미생물 분석결과, E. coli는 3%(4/114)의 들깻잎에서 1.00 log CFU/g 이하로 검출되었으며 S. aureus의 경우 검출빈도는 7.9%(9/114)였고 1.6%(2/114)의 들깻잎에서 S. aureus가 2.00 log CFU/g 이상으로 다소 높게 검출되었다. B. cereus는 0-3.81 logCFU/g 수준이었고 검출빈도는 46.5%(53/114)였으며 2곳의 농가에서 3.00 log CFU/g 이상으로 높게 검출되었다. 한편 E. coli O157:H7, Salmonella spp, 그리고 L. monocytogenes는 모두 검출되지 않았다. 또한 들깻잎 생산환경(토양, 수질, 작업자 및 포장대 등)에서 위생지표세균과 S. aureus와 B. cereus를 조사한 결과 일반세균수와 대장균군이 대부분의 시료에서 3.00 log $CFU/100cm^2$ 이상 검출되었고 일부 손, 수확용기 및 포장대에서 대장균군이 4.00 log $CFU/100cm^2$, hand 이상으로 높게 검출되었다. S. aureus는 관개용수, 포장대, 포장비닐, 작업자의 손과 복장에서 검출되었고 검출 수준은 0.33-1.31 log $CFU/100cm^2$이었다. B. cereus의 경우는 다양한 시료에서 빈번하게 검출되었고 특히 앞서 위생지표세균결과에서 오염도가 높았던 수확용기와 포장대에서 최고 5.50 log $CFU/100cm^2$까지 검출되었으며 수확 전 들깻잎보다 수확 후 들깻잎에서 B. cereus 수준이 증가되는 경향을 보였다. 이상의 결과를 통하여 들깻잎의 유해미생물 오염은 생산단계부터 발생되고 있으므로 이러한 유해미생물의 오염을 사전에 관리하는 GAP제도를 도입해야 할 것이다.
