Isoflavones Extracted from Sophorae fructus Upregulate IGF-1 and TGF-$\beta$ and Inhibit Osteoclastogenesis in Rat Born Marrow Cells

  • Isoflavones have been a central subject in research on the natural phytoestrogens found in Leguminosae. Their effects on bone formation and remodeling are important in that they can act like estrogen by binding on estrogen receptors on the target cell surface. We, therefore, believed that isoflavones may help in the treatment of patients with estrogen deficiency disease such as estrogen replacement therapy (ERT) for osteoporosis. As commonly known, osteoporosis is one of the hormonal deficiency diseases, especially in menopausal women. When estrogen is no longer produced in the body a remarkable bone remodeling process occurs, and the associated events are regulated by growth factors in the osteoblast lineage. In the present study, we investigated whether isoflavones (Isocal) extracted from Sophorae fructus affect the growth factors IGF-I and TGF-$\beta$ that have been known to be related with bone formation. In the study, we found that the active control (PIII) effectively enhanced the level of nitric oxide (NO) and growth factors, and thereby inhibited osteoclastogenesis. The most efficient concentration was $10^{-8}$% within five days, whereas the comparative control (soybean isoflavone) was not as effective even at a lower concentration. In conclusion, the products which contain enriched glucosidic isoflavone and nutrient supplements such as shark cartilage and calcium can be used for osteoporosis therapy by enhancing the production of IGF-I and TGF-$\beta$. Furthermore, the NO produced through endothelial constitutive NO synthase (ecNOS) may playa role in inhibiting bone reabsorption.

Value of spray-dried egg in pig nursery diets

  • High-quality protein ingredients have been used in nursery diets, in spite of expensive ingredients, to minimize nutritional deficiency and disease problems. Recent dramatic increases in prices of protein products for nursery diets have exacerbated the challenge. Spray-dried egg may be a part of the solutions. Therefore, this review describes the value of spray-dried egg in nursery diets as a high-quality protein source. Spray-dried egg is egg by-product and is produced by only eggs without shell that are below the USDA Grade B standards. Spray-dried egg is an excellent nutrient source: 1) highly digestible, 2) excellent balance of amino acids, 3) rich content of fat, and 4) high metabolizable energy. These can be attributed to growth of nursery pigs. Beyond the provision of bioavailable nutrients, spray-dried egg also may provide specific physiological benefits. Spray-dried egg contains 1) immunoglobulin antibodies (IgY: IgG in egg yolk) that may attach to intestinal pathogens and excrete them and 2) lysozymes antimicrobial protein that can damage bacteria cell wall. Thereby feeding spray-dried egg may reduce concentration of intestinal pathogen and thus improve potential gut health or enteric disease resistance in nursery pigs. This is important for physiologically immature weaned pigs. Based on these benefits, spray-dried egg is believed to have the same benefits as spray-dried plasma protein and milk products in diets for nursery pigs. Therefore, it is suggested that spray-dried egg has a great potential as a valuable protein source in nursery diets.

Consumers' perceptions of dietary supplements before and after the COVID-19 pandemic based on big data

  • Purpose: This study identified words closely associated with the keyword "dietary supplement" (DS) using big data in Korean social media and investigated consumer perceptions and trends related to DSs before (2019) and after the coronavirus disease 2019 (COVID-19) pandemic (2021). Methods: A total of 37,313 keywords were found for the 2019 period, and 35,336 keywords were found for the 2021 period using blogs and cafes on Daum and Naver. Results were derived by text mining, semantic networking, network visualization analysis, and sentiment analysis. Results: The DS-related keywords that frequently appeared before and after COVID-19 were "recommend", "vitamin", "health", "children", "multiple", and "lactobacillus". "Calcium", "lutein", "skin", and "immunity" also had high frequency-inverse document frequency (TF-IDF) values. These keywords imply a keen interest in DSs among Korean consumers. Big data results also reflected social phenomena related to DSs; for example, "baby" and "pregnant woman" had lower TD-IDF values after the pandemic, suggesting lower marriage and birth rates but higher values for "joint", indicating reduced physical activity. A network centered on vitamins and health care was produced by semantic network analysis in 2019. In 2021, values were highest for deficiency and need, indicating that individuals were searching for DSs after the COVID-19 pandemic due to a lack an awareness of the need for adequate nutrient intake. Before the pandemic, DSs and vitamins were associated with healthcare and life cycle-related topics, such as pregnancy, but after the COVID-19 pandemic, consumer interests changed to disease prevention and treatment. Conclusion: This study provides meaningful clues regarding consumer perceptions and trends related to DSs before and after the COVID-19 pandemic and fundamental data on the effect of the pandemic on consumer interest in dietary supplements.

비타민 B 결핍에 의한 노인성 근감소증 (Elderly Sarcopenia and Vitamin B Deficiency: A Relationship?)

  • 노인들에서 근감소증은 의료-간호비용 증가의 주요 원인 중 하나가 되고 있다. 한국에서는 근감소증 예방 대책이 일반적으로 특정 질병이 없는 노인들을 대상으로 하지만, 요양원-요양병원에서 집단 거주하는 노인들의 근감소증 대책도 필요하다. 근감소증은 운동량 감소, 단백질 및 영양분(미네랄, 비타민 포함) 섭취 감소, 테스토스테론 및 성장호르몬 변화, 염증 등의 원인으로 발생한다. 분자 생물학적인 정확한 병태생리 기전의 이해가 요구된다. 근감소증은 골다공증, 낙상으로 인한 골절, 치매, 당뇨병, 심혈관 질환 등의 증상을 연결될 수 있다. 비타민 B 패밀리(B1-3, B5-7, B9 및 B12) 결핍을 근감소증 유발의 연구 대상으로 선택한 이유는 다음과 같다. 이는 비타민 B가 에너지 및 단백질 대사에 직접 관여하여 정상적인 신경 기능 유지에 필수적이다. 비타민 B 결핍은 신경-근육 질환, 신경성 질환으로 나타날 수 있으며, 노인성 근감소증과 병행하는 경우가 많다. 노인들은 적정치 이하의 비타민 B 패밀리 섭취, 흡수 장애 및 무식증 문제 등을 겪을 가능성이 높다. 초고령화 사회에서 elderly가 자립적으로 일상생활을 할 수 있는 'health lifetime'을 유지하는 것은 개인의 행복추구와 사회경제적 부담을 줄일수 있는 최고의 목표이다. 본 연구는 근감소증과 관련하여 노인들의 근육량 감소 및 근육 기능 저하를 조절하는 수용성 비타민 B 패밀리의 최신 정보를 제공한다. 또한, 비타민 B 패밀리를 통한 마이오카인에 의한 근감소증의 조절 가능성도 소개한다.

철결핍성 빈혈을 가진 영유아에서 영양학적 평가 및 영양상담 효과 (Nutritional Assessment and the Effectiveness of Dietary Counseling in Infants and Young Children with Iron Deficiency Anemia)

  • 목 적 : 영양 상담을 통하여 IDA 환아의 식이력을 평가하고 상담에 대한 보호자의 만족도와 순응도를 평가하기 위해 이 연구를 시행하였다. 방 법 : 1997년부터 2001년 8월까지 IDA로 진단받은 120명의 6-36개월 영유아를 대상으로 하였으며, 부모를 대상으로 수유방법 및 이유방법에 대하여 설문조사와 영양상담을 시행하여 다음과 같은 결과를 얻었다. 결 과 : 1) IDA로 진단받은 120명은 호흡기 감염으로 병원에 내원하거나 입원해서 발견된 경우가 47례(39.1%)로 가장 많았으며, 창백이나 빈혈 증상으로 평가를 위해 내원한 경우는 27례(22.5%)였다. 2) 총 120명 중 82명의 보호자들이 영양상담을 받았으며, 그중 56명(68.3%)이 이유식을 하고 있었다. 주요 이유식으로는 쌀죽이나 미음이 30례(53.6%)로 가장 많았으며, 철분강화 이유식이 9례(16.1%), 달걀이나 고기, 생선 등 철분이 풍부한 음식의 경우는 3례(5.4%)였다. 3) 영양상담을 받은 환아들의 문제점으로는 단계적인 이유를 시행하지 않고 우유병을 12개월 이상 사용하는 경우가 40례(48.8%)였고, 철분강화 이유식을 시행하지 않고 모유 수유만을 하는 경우는 26례(31.7%)였다. 4) 영양상담을 시행한 후 만족도는 87.8%로 높았으며, 치료에 대한 순응도는 영양상담을 시행한 군에서 철분제제만으로 치료한 군보다 유의하게 높았다(85.4% vs 72%). 결 론 : IDA를 갖는 환아들의 공통적인 식이력은 주 영양 공급원이 탄수화물인 경우로 다섯 식품군이 골고루 섭취되지 않을때, 12개월 이상이 되어도 고형식의 섭취는 거의 없으면서 액상식품을 우유병으로 먹이고 있을 때, 과자 등의 스낵이나 쥬스등의 섭취, 6개월 이후 철분강화 이유식을 시행하지 않고 모유 수유만을 하는 경우, 알레르기 등의 질환으로 임의적인 음식의 제한을 하는 경우, 12개월 전에 생우유를 먹이거나 하루에 700cc 이상 먹인 경우 6가지로 정리해 볼 수 있었다. 그리고 전문적인 영양평가 및 IDA 치료의 순응도를 높이기 위해서 영양상담의 필요하다고 사료된다.

우리나라 일부 심부전 환자의 영양소 섭취량 평가 연구 (The Assessment for Nutrient Intakes of Korean Patients with Heart Failure)

  • 본 연구에서는 우리나라 심부전 환자 78명을 대상으로 식이 섭취 빈도법을 통한 영양소 섭취량 조사를 시행하여 한국인 영양섭취기준과 비교하였으며 그 결과는 다음과 같았다. 1) 에너지와 단백질, 철분의 평균 섭취량은 한국인 영양섭취 기준에 비해 충분히 섭취하고 있었으나 나트륨 섭취가 과다하고, 칼륨 섭취가 부족한 불균형의 영양문제가 파악되었다. 2) 칼슘의 섭취가 부족한 환자들이 상당수 존재하여 심부전 환자의 골연화증 및 골다공증 위험이 높을 가능성이 있음을 확인하였다. 3) 비타민 $B_{12}$, 엽산의 섭취가 부족하며, 이외 리보플라빈, 나이아신 등의 비타민 섭취 부족의 가능성이 있음을 확인하였다. 4) 70세 이상의 고령 환자에서도 칼륨, 칼슘, 비타민 $B_{12}$, 엽산의 섭취가 부족한 것으로 나타났고 이러한 부족 현상은 고령 환자에서 더욱 심각한 것으로 파악되었다. 위의 결과로 보아 국내 심부전 환자의 경우 무기질, 비타민 등의 미량 영양소의 섭취 부족이라는 영양문제가 존재하고 있었다. 따라서 이러한 영양 결핍 문제가 지속된다면 심부전 환자의 장기적인 예후에 영향을 미칠 수 있을 것이므로 영양섭취 부족을 해결하기 위한 다각적 측면에서의 영양 섭취 평가 및 영양관리 대책과 지침이 마련되어야 할 것이다.

Nutrient Supplementation in the Elderly

  • Populations both in the US and worldwide are aging. It is projected that by the year 2030 the population of Americans over 65 will increase to 70 million, more than twice their number in 1998. About one-third of elderly over the age of 65 have debilitating chronic health conditions that greatly impede their activities. Age-associated chronic diseases are believed to be associated with free radicals and the imbalance in antioxidant and oxidative stress contributes to development of several chronic health conditions. Diet and nutrients can have great impact in the health status of elderly. Several factors may contributor to the inadequate consumption of nutrients in elderly, including an inability to chew food adequately, polypharmacy, living along and limited income. Low intake of energy and several micronutrients are common among the elderly. Although overt deficiency of nutrients among the elderly is not common. a recent study showed that while elderly consume more fat, the total energy intake is low among the elderly. Inadequate intake of antioxidant nutrients such as vitamin E in elderly may contribute to the development of chronic health problems. Intake of higher than normal RDA levels of vitamin E for long periods of time has been shown to reduce the risk of many degenerative diseases, such as cardiovascular diseases(CVD) and cancer, and improve immune response in elderly. High intake of other antioxidants is also associated with reduced risk chronic diseases. Dietary antioxidants are abundant in fruits and vegetables. However, due to variety of factors, the elderly may not be able to receive these and other micronutrients through diet. Therefore, supplemental intake of micronutrients in the form of multi vitamins/minerals and/or specific micronutrients is a more practical approach to the maintenance of health status in the elderly.

The Relationship Between Thiamine Intake and Long Sleep Duration: Results From the Korea National Health and Nutrition Examination Survey

  • Objectives: Thiamine is thought to modify sleeping patterns, while alcohol use diminishes internal thiamine levels. We investigated the association between thiamine intake and sleep duration and explored possible heterogeneity in the effect according to alcohol use. Methods: In total, 15 384 participants aged 19-64 were obtained from the Korea National Health and Nutrition Examination Survey 2012-2016. Nutrient intake, including thiamine, was measured using a food frequency questionnaire. Sleep duration was measured by a self-reported questionnaire. The highest thiamine intake quartile was set as the reference group. Participants were divided into 3 groups, with 7-8 hours of daily sleep as a reference group and those who slept more or less than that as "oversleeping" and "insufficient sleeping," respectively. Multivariate logistic regression was used, adjusting for socioeconomic, medical, and nutritional factors. Additionally, participants were stratified according to high-risk alcohol use defined by the World Health Organization standards on alcohol use. Results: Low thiamine intake was associated with oversleeping (Q3: odds ratio [OR], 1.06; 95% confidence interval [CI], 0.86 to 1.32; Q2: OR, 1.24; 95% CI, 0.99 to 1.55; Q1: OR, 1.49; 95% CI, 1.16 to 1.91) and showed a significant trend for higher ORs at lower intake levels (p-trend<0.001). The effect was stronger in the high-risk alcohol use group (Q1: OR, 1.78; 95% CI, 1.28 to 2.49). Conclusions: Low thiamine intake was associated with oversleeping, and alcohol use intensified that association. These results were found in a context where overt clinical symptoms due to thiamine deficiency are considered rare. More awareness of the potential relationship of thiamine intake with oversleeping and its related risks should be considered.

노인의 식이섭취실태와 건강상태에 관한 연구 I -서울지역을 중심으로- (Astudy on the dietary intake and health of aged person I -Based on elderly person in Seoul-)

  • The purpose of this study was to investigate the dietary-intake and that of health by mean of questionarie and interview for male and female old persons living in Seoul area. The contents of study included general aspects, physical status and health, nutrient intake, and food intake frequency. The results from the above survey are summarized as follows ; 1. The age group in the range of 60 to 79 years old was 71%, and the average number of family was 4.9. The educational level was 56% of elderly persons were elementary or middle school graduates . 42.8% of elderly persons had an average monthly income of \490,000 to 300,000. 2. The aged average height, weight, and physical index were 164.9cm, 55.8kg and 20.4 in male, 152.7cm, 46.0kg and 20.3 in female which were lower than the Korean average standard. (male; 167.0cm, 61.0kg, female; 156.0cm, 55.0kg) In the degree of health self-consciousness, percentage distribution of poor and very poor was 29% in male, 59% in female. Among the condition of disease, neuralgia was 23.8%, hypertension was 17.2%, diabetes was 5.4%. 3. Average daily calorie intake was $63.9{\sim}70.4%$ for male and $76.4{\sim}83.9%$ for female which were lower than the Recommended Dietary Allowances for Koreans. Protein intake was $42.9{\sim}57.3g$ (which was $72.8{\sim}82.6%$ RDA) for elderly person, the proportion of animal protein to total protein intake were $24.3{\sim}28.2%$($12.9{\sim}16.2g$). Iron, Vitamin $B_1$, $B_2$ Niacin intake exceeded the RDA, but the intake of Calcium, Vitamin C were far less than that of RDA. 4. In the correlations between nutritional intakes and environmental factors and health, economic living situations and educational level as the factors which might influence the condition of nutritional intake was significant(P<0.01). 5. Food intake frequency of meats, fishes, eggs, for average of $1{\sim}2$ days per week were $44.8{\sim}50.5%$, that of milk and milk products for scarecely week were 42.9.% Correlation of food intake frequency was divided three levels-good, fair, poor. Food intake frequency as the factors which might influence the condition of nutritional intake was significant(P<0.01). The results of the survey reveal that many of elderly show evidence of general nutrient intake deficiency, it requires first of all importance of nutrition to improve nutritional level through to promotion of elderly health.
