The likelihood of achieving pregnancy through timed coitus in young infertile women with decreased ovarian reserve

  • Objective: To evaluate the pregnancy rate and time to pregnancy after timed coitus with or without superovulation in infertile young women younger than 35 years old with low serum $anti-M{\ddot{u}}llerian$ hormone (AMH) levels ( < 25th percentile). Methods: A total of 202 patients younger than 35 years old were recruited retrospectively between 2010 and 2012. Ninety-eight women had normal serum AMH levels (25-75th percentile), 75 women had low serum AMH levels (5th ${\leq}$ & < 25th percentile) and 29 women had very low serum AMH levels ( < 5th percentile), according to reference values for their age group. Results: The clinical pregnancy rate was positively associated with AMH levels, but this trend did not reach statistical significance (43.9% vs. 41.3% vs. 27.6% in the normal, low, and very low AMH groups, respectively). The time to pregnancy was longer in the very low AMH group than in the normal AMH group ($13.1{\pm}10.9months$ vs. $6.9{\pm}6.1months$, p= 0.030). The cumulative live birth rate over 18 months was lower in the very low AMH group than in the normal AMH group, with marginal significance (20.0% vs. 55.9%, p= 0.051). The duration of infertility was negatively correlated with achieving pregnancy (odds ratio, 0.953; 95% confidence interval, 0.914-0.994; p= 0.026). Conclusion: Conservative management, such as timed coitus with or without superovulation, should be considered in young patients who have low ovarian reserve without any infertility factors. However, for women with a long duration of infertility or very low serum AMH levels, active infertility treatment should be considered.

Comparison of Calorie Intake and Satiety Rate by Different Energy Density Level of Kimbab (에너지 밀도 차이에 따른 김밥의 섭취량 및 포만도 비교)

  • We attempted to determine whether energy density would influence calorie intake via cognitive cues, as reflected by satiety. This experiment was designed using two different energy density levels of Kimbab: normal Kimbab (1.6 kcal/g) vs low-density Kimbab (1.0 kcal/g). 26 female college students participated in this study. The subjects ate Kimbab in the lab once a week for 2 weeks. Each week at noon, they were served 24 units of either normal or low-density Kimbab, and we determined the units, grams, and calories of the real & cognitive intake of Kimbab, and also analyzed the satiety rate after eating Kimbab. Our results demonstrated that the real calorie intake from the low-density Kimbab was significantly lower than that of the normal Kimbab (290.3 kcal vs 474.4 kcal, p<0.001), but we noted no significant differences in the units and grams of real and cognitive intake between the normal and low-density Kimbab. However, despite consuming 39% lower caloric intake, the subjects reported similar levels of satiety rates with the two different density levels of Kimbab, as they did not perceive themselves to have eaten more normal Kimbab than low-density Kimbab. Thus, this study provides evidence that the energy density of food is a crucial determinant of caloric intake, and supports the notion that the consumption of low energy-dense foods may result in a reduction of caloric intake without altering satiety.

Characterization of Plasma Carnitine Level in Obese Adolescent Korean Women

  • Carnitine is known to be involved in lipid metabolism and affects body composition as well as energy metabolism of the whole body. Improvement of obesity by L-carnitine supplement suggests that obesity can be related with the abnormality of carnitine metabolism and therefore, plasma carnitine level in normal and obesity groups was investigated. For the characterization of plasma carnitine level in obese people, 60 plasma samples collected from Korean women subjects were analyzed using LC/MS and plasma fatty acid level was also determined using GC/MS. Additionally, several clinical chemical parameters including fasting glucose, cholesterol, AST, and ALT level were measured. All the data obtained were combined and pattern recognition analysis was carried out with the dataset. Obese group showed a different metabolic pattern compared with normal group. Plasma acylcarnitine level of the obese group was found to be $11.7{\mu}g/ml$, which was higher than that of normal group ($8.0{\mu}g/ml$). Statistically significant differences in plasma fatty acid level were not observed between the two groups. Other clinical parameters for the obese group were within normal ranges but AST and ALT levels were slightly elevated compared to normal group. The obese group showed elevated plasma acylcarnitine level.

Risk Factors for Postmenopausal Osteoporosis in Korea (한국 폐경 여성의 골다공증 위험 인자에 관한 연구)

  • Objective : To identify the risk factors for postmenopausal osteoporosis in Korea Materials and methods : Bone mineral density (BMD) at the lumbar spine and femoral neck was measured by dual energy X-ray absorptiometry in 808 apparently normal postmenopausal Korean women. Questions about life style, demographic parameters, medical history and social habits etc. were asked on these women; 2ll women with normal bone mineral density, and 597 women with osteopenia or osteoporosis. Results 'Age of >50 years, low body mass index (BMI; <18.5 kg/m2), long duration of menopause(>10 years), and previous history of fracture were associated with increased prevalence of osteopenia or osteoporosis. Women without the outside activity also showed a higher frequency of low bone mass, Risk for osteopenia or osteoporosis was low in women with high BMI (>23 kg/m2) and women with job. The prevalence of low bone mass appeared to be independent of the following parameters: socioeconomic status, familial history, smoking, drinking, exercise, previous use of oral contraceptive, coffee or milk consumption, and degree of sunlight exposure. Conclusion 'Age, BMI, duration of menopause, previous history of fracture and degree of outside activity are the risk factors for postmenopausal osteoporosis in Korea.

A Comparison of the Results from Somatotype Evaluation with Different Evaluation Tools

  • Supposing that somatotype evaluation results would have significant differences between the public group with less amounts of exercises and the special group with intensive exercises for three to four times a day, this study aimed at comparing the mutual consistency between the results determined by somatotype evaluation tools such as visually calculated index(VCI), R$\ddot{o}$hrer's Index(RI) and Body Mass Index(BMI). The public, taekwondo players and judo players groups were composed of fifty persons, taekwondo players and judo players passed through VCI determination, respectively. Their height and weight were examined and analyzed with somatotype evaluation tools. Comparison of somatotype dispersion of RI and VCI showed that most women were determined by VCI as lean type but were determined by RI as normal type. And that women were determined by VCI as fat type but were determined by RI as normal type. Therefore both men and women showed significant differences in VCI and RI. Comparison of somatotype dispersion of VCI and BMI showed that both men and women were overestimated or underestimated by VCI rather than by BMI. Comparison of somatotype dispersion of RI and BMI showed that men were less determined by BMI as lean type compared with women; both men and women less determined by BMI rather than by RI as normal type; and both men and women, in particular, were more determined by BMI as fat type but men were more determined by BMI rather than by RI as fat type. Total somatotype consistency by tools showed that VCI has the greatest possibility of determining the public group, compared with other groups as lean type and that the consistency of the three tools were relatively higher for the taekwondo players and judo players groups, compared with the public.

The frequency of chromosomal abnormalities and the prenatal cytogenetic analyses for couples with recurrent abortions

  • Between 1988-1998, cytogenetic analyses were performed for 1,476 couples and 162 women with recurrent abortions. We applied GTG-banding, high resolution-banding and FISH (fluorescent in situ hybridization) techniques in this study. The frequency of balanced translocations was 3.6% (112/3114). Of them, 74 cases (2.38%) were reciprocal translocations and 38 (1.22%) were robertsonian translocations. Chromosome aberrations were more frequent in women (80 cases) than in men (32 cases). No phenotypical abnormalities were found in all carriers who had experienced recurrent spontaneous abortions or experienced giving birth to malformed offsprings. Prenatal cytogenetic analyses were carried out on 40 subsequent pregnancies for carrier couples with balanced translocation. The fetal karyotypes showed that 13 cases (32.5%) were normal, 25 (62.5%) were balanced translocations, and two (6%) were unbalanced translocations. It is believed that the frequency of chromosomal abnormalities in patients with recurrent spontaneous abortion is higher than that of the normal population. Most of the fetal samples showed normal karyotypes or balanced translocations matching that of one of their parents. Although the incidence of chromosomal imbalance in the fetuses was relatively low in prenatal cytogenetic analysis, individuals with balanced translocations are predisposed to giving birth to malformed offsprings with partial trisomy or monosomy. Therefore, we recommend the cytogenetic and the prenatal cytogenetic analysis for those who experiences recurrent abortion as well as in case they become pregnant, to prevent the birth of offsprings with chromosomal abnormalities.

A Screening Tool for Identifying High-Risk Pregnant Women of Fe Deficiency Anemia : Process II (임신부의 철분 영양잠재위험집단의 조기선별을 위한 스크리닝 도구의 개발 II)

  • Pregnant women in South Korea are a highly risk group fur iron deficiency anemia. Previous studies indicated that the 24-hour recall method was insensitive in distinguishing iron deficiency anemic women from normal women. This method is also impractical to when used at community health centers where no public health dietitians are employed. The objective of this study was to develop a convenient tool to evaluate the usual iron (Fe) intake of pregnant women. The study participants were 115 pregnant women (age 23 to 37 years) at gestational stage of 13 to 24 weeks. Anemic subjects were classified on the basis of their serum ferritin < 12.0 ${\mu}$g/L and hemoglobin < 12.0 g/dL levels. Food frequency questionnaires with 46, 29, and 15 commonly consumed food items were used to measure the usual intake of iron of the subjects. Hemoglobin and serum ferritin were measured from fasting blood samples. Nutrients intake was assessed on three consecutive days using the 24-hour recall method and the food record method. The iron index score calculated using the food frequency method showed a significantly positive correlation with iron intake for the three days dietary intake. The iron index showed a significantly difference (p < 0.05) between the normal and anemic groups. However, there was no significant difference in the iron intake between the anemic and the normal women as measured by the 24-hour recall and food record method. Our study indicated that the 29-food items questionnaire could be used as a screening tool to identify poor dietary intake of iron. (Korean J Community Nutrition 8(2) : 160170, 2003)

Association between taste perception, nutrient intake, and mental health in young Japanese women

  • BACKGROUND/OBJECTIVES: Taste perception is influenced by both nutritional factors and psychological factors. This study was undertaken to measure the 4 basic taste perceptions, nutrient intake, and mental health, and to examine the factors that affect insensitive taste perception in young Japanese women. SUBJECTS/METHODS: Young women in their late teens and twenties were enrolled as subjects. Taste perception was measured by applying the filter-paper disc method over areas of the chorda tympani nerve. Nutritional status was evaluated using brief, self-administered diet history questionnaires. The index of nutritional status was based on the 2015 Japanese dietary reference intakes. Mental health was assessed using the Japanese translation of the Profile of Mood States short version. This study was approved by the ethical committee at Osaka University. RESULTS: The normal taste perception group (four basic tastes [sweet, salty, sour, and bitter] identified as normal taste perception) comprised 55.4% of the subjects, while the abnormal taste perception group (more than 1 abnormal taste perception was perceived, regardless of flavor) comprised 44.6% of the subjects. There were no significant differences in nutrient intake (except manganese) and mental health between the normal and abnormal taste perception groups. Subjects who took 5 mg to less than 7.1 mg zinc per day were at significantly decreased risk of insensitive taste perception compared to subjects who consumed less than 5 mg zinc per day [Regression coefficient 0.831, 95% confidence interval 0.694-0.996]. CONCLUSION: The present results suggest that insensitive taste perception could be associated with zinc deficiency in young women in their late teens and twenties.

The Pattern of PCBs Level in Adipose Tissue and Serum of Breast Cancer and Normal Women (유방암 환자와 정상여성의 혈액 및 지방조직 중 PCBs 형태에 관한 연구)

  • The purpose of this study was to identify the distribution of non-ortho and mono-ortho PCB congeners and homologues in adipose tissues and sera of women with breast cancer. The collected samples were 25 adipose tissues and 33 sera from women with breast cancer. The samples from the control group were 49 adipose tissues and 52 sera. The levels of three non-ortho and eight mono-ortho PCBs identified in adipose tissue and serum samples were determined by GC/MSD and GC/ECD analyses. Non-ortho and mono-ortho PCB congeners were more dominant in the control group than in the case group for serum samples. The Tetra-PCB and the Hexa-, Hepta-PCB were more dominant in tale and control groups, respectively. The level of PCB homologues in normal women was similar to that of the normal human milk samples. However, the levels of PCB homologues from breast cancer patients were almost same the level of sample from environment. As a result of this study, it is suggested that breast cancer could be related to environmental factors such as PCB level in stack gas and soil sample. More extended research should be to verify this result.

A Study of Energy Intake and Body Fat Percent of College Students in Seoul

  • To investigate the correlation between college women's energy intake and body fat percentage, this study was undertaken with 116 college women aged 19-24 years. The subjects were 20.9 years old on the average, and mean height and weight was 161.3 cm, 51.2 kg. Blood pressures were 112.4 mm Hg(systolic pressure), 70.2 mmHg(diastolic pressure). The number of pulse was 75.5 count/min. The energy intake and percent body fat were examined. The average intake of energy was 7652kcal/day(82.3% of RDA), which was lower than the recommended amount. It was similar to the level of energy intake of Korean women. BMI was 19.7 and body fat percent was 24.4%, so they appeared plump. According to BMI, 43.1% of subjects was grouped into normal, 56.9% was thin ; there was no difference between groups in age and stature. Also, normal group(27.1%) shoed significantly higher body fat percent(27.1%) than thin group(22.3%). But, body fat percent of thin group according to BMI was shown normal. Therefore, it may be thought that BMI is unsuitable indicator in this study. As a result of analyzing the correlation between body-measuring value and food intake, body fat percent against body weight and BMI was significant at the level p<0.001, but thee was no significant correlation with food intake. Therefore, it is desirable to recommend a healthful dietary lifestyle and activities for young women who want to reduce body weight due to excessive concerns regarding their outward figure.
