Clinical application of chromosomal microarray for pathogenic genomic imbalance in fetuses with increased nuchal translucency but normal karyotype

  • Purpose: To evaluate the additive value of prenatal chromosomal microarray analysis (CMA) in assessing increased nuchal translucency (NT) (≥3.5 mm) with normal karyotype and the possibility of detecting clinically significant genomic imbalance, based on specific indications. Materials and Methods: Invasive samples from 494 pregnancies with NT ≥3.5 mm, obtained from the Research Center of Fertility & Genetics of Hamchoon Women's Clinic between January 2019 and February 2020, were included in this study and CMA was performed in addition to a standard karyotype. Results: In total, 494 cases were subjected to both karyotype and CMA analyses. Among these, 199 cases of aneuploidy were excluded. CMA was performed on the remaining 295 cases (59.7%), which showed normal (231/295, 78.3%) or non-significant copy number variation (CNV), such as benign CNV or variants of uncertain clinical significance likely benign (53/295, 18.0%). Clinically significant CNVs were detected in 11 cases (11/295, 3.7%). Conclusion: Prenatal CMA resulted in a 3% to 4% higher CNV diagnosis rate in fetuses exhibiting increased NT (≥3.5 mm) without other ultrasound detected anomalies and normal karyotype. Therefore, we suggest using high resolution, non- targeting CMA to provide valuable additional information for prenatal diagnosis. Further, we recommend that a genetics specialist should be consulted to interpret the information appropriately and provide counseling and follow-up services after prenatal CMA.

A Study on Body Satisfaction and Fitness Apparel Based on Body Type by Body Mass Index: In Women 20-50's Years of Age (BMI지수에 의한 신체유형별 신체만족도와 의복적합성에 관한 연구: 20~50대 여성을 중심으로)

  • The purpose of this study is to classify body type by BMI and to inquire about body satisfaction and fitness apparel depending on body type among women 20-50years of age. As a result, body types are classified into three groups: lean, normal, and obese figures. On front silhouette, the normal type occupies most in women belonged to lean figure group, the obese lower part of the bodytype in normal figure group, and the obese upper part of the body type in obese figure group. On the other, in side silhouette, the slender type is prevalent in lean figure group, hip obesity in normal figure group, and trunk obesity in obese figure group. In particular, women in the obese figure group were distributed among the various body types. The obese figure group had a lower fitness apparel in the measurement of circumference(e.g., chest, waist, and hip) related to obesity in comparison with measurement of length. Therefore, the development of an optimal sizing system in response to the various body types in the obese figure group is needed to provide more diversity in aesthetic design and continuity among various sizing systems.

Risk Factors of Osteopenia among Korean Middle-aged Women (중년여성의 골감소증 위험요인)

  • Purpose: This study examined the demographic and lifestyle differences between women with osteopenia and those with normal bone mineral density (BMD) to identify risk factors for osteopenia. Method: Participants comprised 381 women age 40-64 years. Data were collected using surveys, BMD measures, and anthropometric parameters. Results: Prevalence of osteopenia was 29.1%; significant differences in age group, job, age at menarche, age at menopause, and body mass index were found between women with osteopenia and those with normal BMD. Logistic regression analysis indicated that advanced age, unemployment, lower body mass index, and lack of exercise in women were significantly associated with osteopenia. Conclusion: This study suggests the need for strategies to improve bone health and continuous cohort studies to identify risk factors.

A Case of Short-chain Acyl-CoA Dehydrogenase Deficiency Detected by Newborn Screening

  • Short-chain acyl-CoA dehydrogenase (SCAD) deficiency is an autosomal recessive mitochondrial disorder of fatty acid oxidation associated with mutations in the ACADS gene. While patients diagnosed clinically have a variable clinical presentation, patients diagnosed by newborn screening are largely asymptomatic. We describe here the case of a 1-year-old male patient who was detected by newborn screening and diagnosed as SCAD deficiency. Spectrometric screening for inborn errors of metabolism at 72hrs after birth showed elevated butyrylcarnitine (C4) level of 1.69 mol/L (normal, <0.83 mol/L), C4/C2 ration of 0.26 (normal, <0.09), C5DC+C60H level of 39 mol/L (normal, <0.28 mol/L), and C5DC/C8 ration of 7.36 (normal, <4.45). The follow-up testing at 18 days of age were performed: liquid chromatography tandem mass spectrometry (LC-MS/MS), urine organic acids, and quantitative acylcarnitine profile. C4 carnitine was elevated as 0.91; urine organic acid analysis showed elevated ethylmalonic acid as 62.87 nmol/molCr (normal, <6.5), methylsuccinate 6.81 nmol/molCr (normal, not detected). Sequence analysis of ACADS revealed a homozygous missense mutation, c.164C>T (p.Pro55Leu). He is growing well and no episodes of seizures or growth retardation had occurred.

Comparison of Relationship between Biochemical Indices and Bone Mineral Density of Pre- and Post- Menopausal Women in Gyeongnam Area (경남 일부지역 폐경 전·후 여성들의 골밀도와 생화학지수들과의 관련성에 관한 비교연구)

  • This study was carried out to compare the relationship between biochemical indices and bone mineral density (BMD) in 50 pre-menopausal and 50 post-menopausal women. The subjects were divided into normal and risk groups according to their bone status, as determined by T-scores of the lumbar spine and femur. The average T-score of the lumbar spine was higher (p<0.05) in pre-menopausal women ($0.42{\pm}0.18$) than post-menopausal women ($-0.08{\pm}0.21$). Serum levels of HDL-cholesterol, P, and Fe were significantly higher in the risk group than the normal group in pre-menopausal women (p<0.05). Serum levels of total protein, globulin, alkaline phosphatase (ALP), and osteocalcin were lower in the risk group than the normal group, whereas the level of estrogen was higher in the normal group than the risk group in post-menopausal women (p<0.05). In pre-menopausal women, P was positively correlated with Ca (p<0.01), and ALP was positively correlated with osteocalcin (p<0.01) and parathyroid hormone (PTH) (p<0.05). Further, insulin-like growth factor-I (IGF-I) was negatively correlated with the vitamin $25(OH)D_3$ and vitamin K (p<0.05). In post- menopausal women, the Ca was positively correlated with vitamin $25(OH)D_3$ (p<0.05) and vitamin K (p<0.01), and P was positively correlated with vitamin K (p<0.01), Ca (p<0.01), and IGF-I (p<0.05) and negatively correlated with PTH (p<0.05). IGF-I was negatively correlated with PTH (p<0.01) and estrogen (p<0.05), and ALP was positively correlated with osteocalcin (p<0.01) and negatively correlated with vitamin K and estrogen (p<0.05). In pre-menopausal women, the lumbar spine BMD was positively correlated with vitamin K level (p<0.01) and negatively correlated with P level (p<0.05). In post-menopausal women, the femur BMD was positively correlated with estrogen level and negatively correlated with PTH leves (p<0.05). These results suggest that vitamin K and P levels are associated with bone health in pre-menopausal women, and estrogen and PTH levels are associated with bone health in post-menopausal women.

The Relationships of Chewing Frequency, Eating Rates, and Food Consumption with the Body Mass Index and Gender of College Students (남녀 대학생의 체질량지수에 따른 저작횟수와 식사속도 및 식품섭취량 비교)

  • In this study, we examined chewing frequency, eating rates, and food consumption volume according to the body mass index of male and female college students. Twenty-five male college students were divided into a normal weight male group (<23 body mass index, n=14) and an obese male group (${\geq}23$ body mass index, n=11). Twenty-five female college students were divided into a normal weight female group (<23 body mass index, n=21) and an obese female group (${\geq}23$ body mass index, n=4). Subjects were provided with abundant fried rice and allowed to consume it at their own pace. Chewing frequency, total mealtime, rate of eating, consumption volume, calorie intake, and the amount of one bite were measured. There were no significant differences in chewing frequency, total mealtime, rate of eating, consumption volume, calorie intake, and the amount of one bite between the normal and obese male groups. There were also no significant differences in chewing frequency, consumption volume, calorie intake, and the amount of one bite between the normal and obese female groups. However, the obese female group had a significantly shorter mealtime and significantly higher eating rate compared with the normal group. Thus, our study suggests that females who eat a meal for a longer time may reduce their food intake, consequently inducing body weight loss.

Study on Body Proportion Using Body Indexes of Mongolian Men and Women in 20's (몽골 20대 남녀의 지수치를 이용한 인체 프로포션 연구)

  • The purpose of this research is to compare the body proportion of the Mongolian and Korean men and women in their 20' s by comparing the characteristics of the body type that uses the body indexes, in order to provide the base data for the increased fit of the apparels. The results are as follows. 1) Histogram of the Rohrer' s Index for the Mongolian men and women in their 20s was examined. the Mongolian men, normal body type took up 48.0% while small body type comprised 50.0% and obese body type comprised 2.0%. the Mongolian women, normal body type comprised 61.5%, small body type comprised 22.1% and obese body type comprised 16.4%. 2) The body proportion of Korean men has 1:1.62 with upper body and low body part on a waist basis, and Mongolian men has 1:1.65. Thus Mongolian men has longer pan of lower body than Korean men. 3) Mongolian women has longer part of lower body and has higher rates of hip height, crotch height, knee height than Korean women. Mongolian women has high rate of hip line and calf length. 4) As a result comparison using the body index of Mongolian and Korean women (bust thickness/ bust width, waist thickness / waist width, hip thickness / hip width, when regarding the height as 100 ), it was found that Mongolian women are flatter than korean women.

Pregnancy and Delivery by Round Spermatid Injection into Oocytes in Human (원형정자세포의 주입에 의한 임신과 분만 1례)

  • We demonstrated that the normal pregnancy and delivery by round spermatid injection (ROSI) into oocytes was achieved from nonobstructive azoospermia patient. In this case, the normal fertilization rate was 50%. All of the two pronuclear stage embryos cleaved and were transferred to the patient's uterus. A singleton pregnancy was achieved and resulted in the birth of normal female infant. This resuJt show that intracytoplasraic injection of round spermatid seems to be new treatment of nonobstmctive azoospermia male infertility. Further research is needed to evaluate the required culture conditions to induce progression of the round spermatid into a more elongated stage.

Study on the obesity and nutrition status of housewives in Seoul and Kyunggi area

  • This study was conducted to evaluate the rate of obesity of 212 women (age 45-60 years) in Seoul and the Kyunggi area through analysis of BMI and the dietary life factors related to obesity using a survey on dietary habits, dietary assessment, and nutrient intake. The height of the underweight group was taller than normal. The height of the obese group was equal to that of the normal group, but the weight was 8.5 kg greater than the normal group. Women in the underweight group consumed meals irregularly, and only 33.4% ate breakfast. Additionally, the rate of overeating was low in the underweight group, and milk, dairy products (yogurt, etc.), fruit, and fruit juice were consumed more than once a day. It was found that 62.1% of the women in the obese group never ate out, and the rate of eating one serving of fruit, drinking one cup of fruit juice, and eating various kinds of foods was high. The average point of women's dietary life was $21.9{\pm}2.9$, and 12.7% of all women responded that their dietary life was good. However, in the obese group, only 6.9% of the women reported that their dietary life was good. Evaluation of snacking habits revealed that the underweight group consumed a high level of carbonated drinks and ice cream, whereas for in the obese group, 24.1% of the women consumed milk and its products and 5.6% regularly consumed fast and fried foods. Evaluation of nutrient intake revealed that the consumption of energy, protein, vitamin A, vitamin $B_1$, $B_2$, $B_6$, niacin, vitamin C, and vitamin E was high in all of the groups, but the intake of folic acid in the underweight group was lower than the required level. Overall, 24.1% of the women in the obese group were found to have metabolic diseases, mostly hypertension (43%). In conclusion, a balanced diet to avoid excessive nutrient intake is needed to prevent obesity.
