Comparison of Pre and Post-operational Phonatory Aerodynamic Parameters in Vocal Polyp and Vocal Cord Palsy Patients (성대마비 및 성대용종 환자의 수술 전과 후의 공기역학적 변수 비교)

  • Background and Objectives : Aerodynamic analysis is an examination which provides information regarding various vocalization measures indicating laryngeal efficiency. Voice evaluation using such examination must be capable of distinguishing between normal to abnormal voice. It also observes variables on aerodynamic characteristics by gender in regards to patients of vocal disorders, especially of vocal cord paralysis and vocal polyp, and compares the conditions before and after surgery. This paper therefore, seeks to build a framework for establishing standard levels of aerodynamical characteristic on vocal disorders. Subjects and Methods : The study was intended for a total number of 20 patients with vocal polyp or unilateral vocal cord paralysis. Those with the vocal polyp underwent laryngomycroscopy surgery and the vocal cord paralysis, vocal fold injection using Restylane. Aerodynamic analysis fulfilled the Maximum sustained Phonation (MXPH) and Voicing Efficiency (VOEF) by using PAS Model 6600 (KayPENTAX, USA). Results : In MXPH, increase in PHOT were evident with vocal polyp after surgery. As for patients with vocal cord paralysis, MAXDB, MEADB, DHODB, PHOT all have increased and MEAP, PEF, MEAF decreased after surgery. In VOEF, patients with vocal cord paralysis who underwent surgery showed increase in MAXDB, MEADB, DHODB, FET100, ARES, but decreases in PEF, TARF. Conclusion : Overall, it can be concluded that patients with the vocal polyp and vocal cord paralysis seemed to get closer to the normal values after than before surgery in majority of measures. This confirms that the function of their vocal cord has improved nearly to normality through operations.

Auditory-Perceptual and Acoustic Assessment in Measuring Dysphonia Severity of Vocal Fold Nodules (성대결절 환자의 음성장애에 대한 청지각적 및 음향학적 평가)

  • The purpose of this study was to investigate the relationship between the differences in the acoustic measurements (AVQI) and the auditory-perceptual assessments (GRBAS, CAPE-V) of the normal and vocal fold nodules. For this purpose, Total 335 voice samples were analyzed acoustically and three raters performed auditory-perceptual assessments. in the results, AVQI, G, and OS scores of the normal group were lower than those of the vocal fold nodules group. The correlations between the G scale and the OS scale were highly correlated, and the correlation between the AVQI, and auditory-perceptual results (G and OS) was also high value. The threshold values for discriminating AVQI, G, and OS between the two groups were ${\leq}4.06$, ${\leq}1$, and ${\leq}26$, respectively, and the predictive diagnostic power was 0.840, 0.860, and 0.848. In conclusion, AVQI and auditory-perceptual evaluation can improve potentiality the screening of vocal fold nodules and help to determine the diagnosis and treatment plan of voice disorders.

Usefullness of the Vibration Pick-Up in Detection of Pitch for Synchronization of Laryngeal Stroboscopy (후두 스트로보스코프 검사의 신호 동기화를 위한 진동 검출기의 유용성)

  • Objective and Background: Laryngeal stroboscope is an useful equipment in evaluation of vocal cord vibration and in early detection of mucosal lesion including invasive cancer of the vocal cord. Recently Lee et al. (2006) developed portable stroboscope using voice as synchronization signal. It has been frequently impaired ability to synchronize the flashes even in normal female. Authors tried to investigate various methods including vibration pick-up, microphone, laryngeal microphone, and contact microphone for development of simple and accurate method like electroglottograph signal. The purpose of this study was to estimate wheher the vibration pick-up is available and is consistent with the signal of EGG. Subjects and Methods: Authors compared the signals between EGG and noncontact method such as voice, contact methods including vibration pick-up, laryngeal microphone, and contact microphone in normal twenty adults (male 10 and female 10). The number of peak in one cycle was compared with the number of the peak in EGG, and the percent of phase difference in the peak was compared with EGG Also, authors tried to investigate which site of vibration pick-up was most effective for synchronization of stobo flashes. Three site including anterior neck below the cricoid cartilage, thyroid ala, and suprahyoid region were analysed. Results: Among various methods for synchronization of strobo flashes, vibration pick-up was most effective method in peak detection. And anterior neck below cricoid cartilage was the most available site of the vibration pick-up. Conclusion: Authors suggest that vibration pick-up is most available and effective method for synchronization of strobo flashes.

  • Deaf are it is difficult to communicate to represent the voice heard, so theay use mostly using the speech, sign language, writing, etc. to communicate. It is the best way to use sign language, in order to communicate deaf and normal people each other. But they must understand to use sign language. In this paper, we designed and implementated finger language translation system to support communicate between deaf and normal people. We used leap motion as input device that can track finger and hand gesture. We used raspberry pi that is low power sing board computer to process input data and translate finger language. We implemented application used Node.js and MongoDB. The client application complied with HTML5 so that can be support any smart device with web browser.

Effects of a singing program using self-voice monitoring on the intonation and pitch production change for children with cochlear implants (자가음성 모니터링을 응용한 가창 프로그램이 인공와우이식 아동의 억양과 음고 변화에 미치는 영향)

  • The purpose of this study was to examine how a singing program using self-voice monitoring for children with cochlear implants (CI) influences on the intonation and the accuracy of pitch production. To verify and estimate the effectiveness, a program was conducted with participants of 7 prelingual CI users, whose aged between 4 years and 7 years. The program adopted three stages from the self-voice monitoring: Listen, Explore, and Reproduce (LER stage). All participants received 8 singing sessions over 8 weeks, including pre-test, intervention, and post-test. For the pre and post-test, participants' singing of an excerpt of a song "happy birthday" and speaking three assertive sentences and three interrogative sentences were recorded and analyzed in terms of the intonation slopes at the end of the sentences and the melodic contour. From the sentence speeches, we found that the intonation slopes of the interrogative sentences significantly improved as they showed similar patterns with that of the average normal hearing group. Also, in regard to singing, we observed that the melody contour had progressed, as well as the range of pitch production had extended. The positive result from the intervention indicates that the singing program was effective for children with CI to develop the intonation skill and accuracy of pitch production.

A Study of Phonation Pharyngogram in Functional Voice Disorders (기능성 음성 질환에서 발성 인두조영술의 사용에 관한 연구)

  • Background and Objectives : There are so many methods to investigate the causes of voice disorders. However, they were almost invasive or non-physiologic methods. And none of them showed the laryngeal movements. Phonation pharyngogram is non-invasive method to see the laryngeal movement directly. Authors studied to evaluate the availability of phonation pharyngogram in laryngeal nodule. Materials and Methods : 30 laryngeal nodules and 10 control groups were evaluated. Acoustic analysis and maximum phonation time were measured and pharyngogram was taken during sustaining /a/ phonation immediately after swallowing the barium. We measured the width of hypopharyngeal wall, shape of hypopharyngeal wall and pyriform sinus apex, level difference between both true vocal folds, angle of subglottis, location of true vocal folds, elevated distance of vocal folds and shape of c-spine. Results : Jitter, Shimmer of laryngeal nodule were higher than control group and maximum phonation time was shorter in laryngeal nodule. There was a significance in width of hypopharyngeal wall, shape of pyriform sinus apex, level difference between both true vocal folds, elevated distance of vocal folds and shape of c-spine. Conclusions : Authors knew that there were differences when we used the phonation pharyngogram between normal group and laryngeal nodule group.

Speech Intelligibility and Vowel Space Characteristics of Alaryngeal Speech (무후두음성의 말 명료도와 모음 공간 특성)

  • This study is aimed at finding out different types of speech characteristics categorized based on voice rehabilitation techniques used on twenty-six patients (all-male) with total or partial laryngectomees. The speech intelligibility of standard esophageal (SE), tracheoesophageal speech (TE), and electriclarynx (EL) was measured by using the CSL and eleven listeners were instructed to rate the speech on a 5-point scale. The vowel space parameters such as vowel space, VAI, FCR, and F2 ratio were measured by averaging 5 repeats of each vowel (/a/, /e/, /i/, /u/) and the results were put into the parameter formula. The results showed significant statistical differences in speech intelligibility and vowel space between SE and TE. The speech intelligibility and vowel space of TE were higher than those of SE or EL and there was a high correlation between speech intelligibility and some parameters (vowel space, VAI, F2 ratio). The results also showed that TE's speech characteristics were most similar to normal groups comparing with SE and EL, but still very deviant in laryngeal speech. This was due to insufficient airflow intake into the esophagus when producing sounds, and because articulation movement was carried out differently among groups. Therefore, these findings will contribute to establishing a baseline related to speech characteristics in voice rehabilitation for patients with alaryngeal speech.

HEEAS: On the Implementation and an Animation Algorithm of an Emotional Expression (HEEAS: 감정표현 애니메이션 알고리즘과 구현에 관한 연구)

  • The purpose of this paper is constructed a HEEAAS(Human Emotional Expression Animaion System), which is an animation system to show both the face and the body motion from the inputted voice about just 4 types of emotions such as fear, dislike, surprise and normal. To implement our paper, we chose the korean young man in his twenties who was to show appropriate emotions the most correctly. Also, we have focused on reducing the processing time about making the real animation in making both face and body codes of emotions from the inputted voice signal. That is, we can reduce the search time to use the binary search technique from the face and body motion databases, Throughout the experiment, we have a 99.9% accuracy of the real emotional expression in the cartoon animation.

The Effects of Increased Processing Demands on the Sentence Comprehension of Korean-speaking Adults with Aphasia (지연된 자극 제시가 실어증 환자의 문장 이해에 미치는 영향: 반응정확도와 반응시간을 중심으로)

  • The purpose of this study is to present evidence for a particular processing approach based on the language-specific characteristics of Korean. To compare individuals' sentence-comprehension abilities, this study measured the accuracy and reaction times (RT) of 12 aphasic patients (AP) and 12 normal controls (NC) during a sentence-picture matching task. Four versions of a sentence were constructed with the two types of voice (active/passive) and two types of word order (agent-first/patient-first). To examine the effects of increased processing demand, picture stimuli were manipulated in such a way that they appeared immediately after the sentence was presented. As expected, the AP group showed higher error rates and longer RT for all conditions than the NC group. Furthermore, Korean speakers with aphasia performed above a chance level in sentence comprehension, even with passive sentences. Aphasics understood sentences more quickly and accurately when they were given in the active voice and with agent-first order. The patterns of the NC group were similar. These results confirm that Korean adults with aphasia do not completely lose their knowledge of sentence comprehension. When the processing demand was increased by delaying the picture stimulus onset, the effect of increased processing demands on RT was more pronounced in the AP than in the NC group. These findings fit well with the idea that the computational system for interpreting sentences is intact in aphasics, but its ability is compromised when processing demands increase.

The Changes in the Closed Qutient of Trained Singers and Untrained Controls Under Varying Intensity at a Constant Vocal Pitch (음도 고정 시 강도 변화에 따른 일반인과 성악인 발성의 성대접촉률 변화 특성의 비교)

  • Background and Objectives : The most important two factors of the voice production are the respiratory function which is the power source of voice and the glottic closure that transform the air flow into sound signals. The purpose of this study was to investigate the differences between trained singers and untrained controls under varying intensity at a constant vocal pitch by simulataneous using the airway interruption method and electroglottography(EGG). Materials and Methods : Under two different intensity condition at a constant vocal pitch(/G/), 20(Male 10, Female 10) trained singers were studied. Mean flow rate(MFR), subglottic pressure(Psub) and intensity were measured with aerodynamic test using the Phonatory function analyzer. Closed quotients(CQ), jitter and shimmer were also investigated by electroglottography using Lx speech studio. These data were compared with that of normal controls. Results : MFR and Psub were increased on high intensity condition in all subject groups but there was no statistically significance. Statistically significant increasing of CQ. were observed in male trained singers on high intensity condition (untrained male : 51.31${\pm}$3.70%, trained male :55.52${\pm}$6.07%, p=.039). Shimmer percent, one of the phonatory stability parameters, was also decreased statistically in all subject groups(p<.001). Conclusion : The trained singers' phonation was more efficient than untrained singers. The result means that the trained singers can increase the loudness with little changing of mean flow rate, subglottic pressure but more increasing of glottic closed quotients.
