Background: Oral frailty is defined as the functional decline of the oral function due to aging, and it is associated with frailty and chronic disease. Most of the frailty intervention is for adults aged 65 years and older. However, early intervention for preventive disorder is most important. The objective of this study was to identify the age at which oral frailty surpass the "normal" range. Methods: This cross-sectional study included 719 adults (aged 30~89 years) residing in Gangwon province in May 2023. Risk of oral frailty was assessed using criteria from The Korean Academy of Geriatric Dentistry including oral function such as swallowing and mastication, and frailty. Frailty was assessed using the Kihon Checklist. To determine when oral frailty surpass the "normal" status, statistical analysis including chi-squared tests and multiple logistic regression analysis were performed using R (ver. 4.3.1). Results: There were 388 (54.0%) individuals who had a "normal" status risk of oral frailty. The risk of oral frailty was higher in the 50~54 age group compared to the 30~34 age group (odds ratio [OR] 0.50, 95% confidence interval [CI] 0.28~0.91), after adjusting for gender, education, income, occupation, and frailty (OR 0.46, 95% CI 0.22~0.94). This means that from 50~54 years old, regardless of gender, education, income, occupation, or frailty condition, there is a distinction from the "normal" status. Conclusion: We found that intervention for oral frailty is needed starting from age 50 years. This is the stage where early indications of oral frailty become apparent. Early intervention for oral frailty can lead to a decrease in the prevalence of diseases and medical expenditure. Therefore, early intervention in middle-aged adults of oral frailty is necessary to improve the quality of life related to oral health.

Frailty models are useful for the analysis of correlated and/or heterogeneous survival data. However, the inferences of fixed parameters, rather than random effects, have been mainly studied. The prediction (or estimation) of random effects is also practically useful to investigate the heterogeneity of the hospital or patient effects. In this paper we propose how to extend the prediction method for random effects in HGLMs (hierarchical generalized linear models) to log-normal semiparametric frailty models with nonparametric baseline hazard. The proposed method is demonstrated by a simulation study.
Purpose : This study was conducted to investigate and analyze the physical health of older Koreans with sarcopenia and locomotive syndrome and identify the related factors. Methods : In this study, the sarcopenia and locomotive syndrome evaluations were applied to 210 elderly people, and the sarcopenia group was 36, the locomotive syndrome group was 164, and the normal group was 10. After group selection, a physical health status survey was conducted. The physical health status was assessed via body composition analysis, physical characteristics survey including measurement of waist and calf circumference, investigation of diseases currently being diagnosed by a doctor, and frailty measurements. The collected data were statistically analyzed using one-way ANOVA, the Kruskal-Wallis test, and the Chi-square test. Results : There were significant differences between groups in all elements of physical characteristics including body mass index, waist circumference, and calf circumference. Among them, a consistent result was found that the normal group had the largest amount of muscle mass and the sarcopenia group had the least amount of muscle mass in the factors related to muscle mass. However, the factors relating to fat mass and obesity also showed significant differences between the groups, but the results were not consistent. Considering the group differences in current diseases, a significant difference was only detected for osteoporosis among 12 diseases. Moreover, those in the sarcopenia group had the highest rate of osteoporosis. And there was no significant difference between the groups in the total score of the frailty measurement, but there was a significant difference between the groups in the frailty measurement levels. Conclusion : This study on physical health status confirmed that muscle mass-related factors, osteoporosis, and frailty levels were significantly related to sarcopenia and locomotive syndrome.
Background: Oral frailty has garnered considerable interest following its identification as a risk factor for physical frailty. The Korean oral frailty diagnosis criteria have emphasized the need for extensive research on oral frailty diagnostic items and interventions. Our study performed an in-depth analysis of the tongue-palate pressure patterns in healthy community-dwelling older adults. Methods: Of the 217 older adults aged ≥60 years who visited a senior center in Wonju, 205 participants who completed tongue pressure measurement were included in the final analysis. Pressure changes over time were recorded by instructing the participants to press their tongue against the hard palate with for 7 seconds per cycle. The participants were divided into the normal and abnormal tongue pressure (NTP and ATP, respectively) groups based on whether they achieved the target tongue pressure at least once; tongue pressure patterns were compared between the groups. Furthermore, the average time taken to achieve the standard tongue pressure value was calculated for the participants in the NTP group and used to evaluate the decrease in tongue pressure in the ATP group. Results: Among the 205 participants, 40.5% had ATP. The tongue pressure graph revealed a gentle and consistent incline that was maintained even after achieving standard tongue pressure in the NTP group. The graph was more extreme in the ATP group, and the changes in the pressure type varied across individuals; the tongue pressure was only 48.4%, 40.7%, 31.9%, and 22.6% of the NTP in the participants in their 60s, 70s, 80s, and ≥90s, respectively (p<0.05). Conclusion: Tongue pressure weakness was observed in 40.5% of the healthy community-dwelling older adults. Furthermore, ATP graphs were observed in the patients with tongue pressure weakness. Thus, activities improving the oral function in community-dwelling older adults and systematic oral rehabilitation programs should be devised to promote normal swallowing.

Background: This study investigated the predictive value of the frailty index calculated using laboratory data and vital signs (FI-L) in patients who underwent coronary artery bypass grafting (CABG). Methods: This study included 508 patients (age 67.3±9.7 years, male 78.0%) who underwent CABG between 2018 and 2021. The FI-L, which estimates patients' frailty based on laboratory data and vital signs, was calculated as the ratio of variables outside the normal range for 32 preoperative parameters. The primary endpoints were operative and medium-term all-cause mortality. The secondary endpoints were early postoperative complications and major adverse cardiac and cerebrovascular events (MACCEs). Results: The mean FI-L was 20.9%±10.9%. The early mortality rate was 1.6% (n=8). Postoperative complications were atrial fibrillation (n=148, 29.1%), respiratory complications (n=38, 7.5%), and acute kidney injury (n=15, 3.0%). The 1- and 3-year survival rates were 96.0% and 88.7%, and the 1- and 3-year cumulative incidence rates of MACCEs were 4.87% and 8.98%. In multivariable analyses, the FI-L showed statistically significant associations with medium-term all-cause mortality (hazard ratio [HR], 1.042; 95% confidence interval [CI], 1.010-1.076), MACCEs (subdistribution HR, 1.054; 95% CI, 1.030-1.078), atrial fibrillation (odds ratio [OR], 1.02; 95% CI, 1.002-1.039), acute kidney injury (OR, 1.06; 95% CI, 1.014-1.108), and re-operation for bleeding (OR, 1.09; 95% CI, 1.032-1.152). The minimal p-value approach showed that 32% was the best cutoff for the FI-L as a predictor of all-cause mortality post-CABG. Conclusion: The FI-L was a significant prognostic factor related to all-cause mortality and postoperative complications in patients who underwent CABG.
가속수명검사는 시간과 경비를 현실적인 여건에 맞추기 위하여 정상조건보다 열악한(강도가 높은) 조건에서 검사를 수행하여 그 결과를 정상조건에서의 수명분포와 관련된 관심사(기대수명, 신뢰도) 등을 추정하는 것을 목적으로 하는 검사 방법이다. 이 연구에서는 고장률이 다른 두 개의 부품이 직렬형으로 구성된 시스템의 가속수명검사 자료를 분석할 때 사용되는 하나의 모형을 제안하고 분석 과정 및 기존의 두 개의 부품이 서로 독립이라고 가정한 모형과 비교하는 것을 주요 내용으로 하였다. 시스템이 작동될 때 받는 관찰이 가능하지 않은 총체적인 환경의 효과를 동일 시스템내의 두 개의 부품이 공유하게 됨으로써 발생되는 두 부품의 수명의 의존성을 frailty 모형을 이용하여 모형화하였고, 분석 과정에서는 Profile 우도함수를 사용하여 최대 우도 추정 과정에서의 초기치 문제를 해결하는 방법을 제시하고 모의실험을 통하여 독립성의 가정을 검토하여 보았다.
본 연구에서는 서울시 일부 취약계층 노인을 대상으로 맞춤형 영양교육과 보충식품으로 구성된 영양꾸러미를 제공하여 식품안정성 확보와 건강·영양 상태 개선에 대한 효과를 평가하고자 하였다. 연구 대상자는 서울시 일부 자치구 내 방문건강관리사업 대상자 중 65세 이상 노인 372명이었다. 맞춤형 영양중재 프로그램은 맞춤 영양교육과 보충식품 제공인 2가지 요소로 구성되었고, 영양중재는 약 4개월간 이루어졌다. 프로그램 효과평가를 위해서 식품 및 영양소 섭취량, 식품안정성, 빈혈 여부, 간이영양평가도구 (MNA)를 활용한 영양불량 여부, 만성 질환관리 수준, 허약 여부가 조사되었다. 영양중재 프로그램 참여 이후, 대상자의 모든 영양소 섭취량이 프로그램 참여 이후 유의적으로 증가하였고 권장섭취기준 대비 50%에 미치지 못했던 대부분의 영양소 섭취는 프로그램 참여 이후 50%를 넘게 섭취하였다. 프로그램 참여 이전 모든 대상자가 식품불안정 상태였으나 프로그램 참여 이후 37.1%가 식품안정성을 확보하였다. 영양 상태에서는 불량과 위험은 감소하였고, 정상이 0%에서 29.8%로 증가하였으며, 빈혈에서는 정상이 18.7%에서 28.5%로 증가하였다. 또한, 질환관리 수준에서는 정기 및 집중관리 수준의 대상자 비율은 감소하였고, 자기역량 관리 수준이 0%에서 4.6% 증가하였으며, 허약 상태에서는 허약 및 허약 위험 비율은 감소한 반면, 정상 비율이 0%에서 9.7% 증가하였다. 연령별로 분석하였을 때, 80세 이상의 고령인 노인일수록 특히 만성질환수준의 관리와 허약 수준에 대한 개선이 미비한 것으로 나타났다. 본 연구 대상자는 질환을 갖고 있는 방문건강관리사업의 대상자로 경제적으로나 건강상태로나 매우 취약한 대상자로서 이들의 질환이 더 악화되지 않도록 유지하거나 개선시키는 부분은 매우 중요하며, 본 연구는 대상자들의 질환 관리 및 영양상태 개선을 위하여 질환별 보충식품의 제공 및 영양교육 등의 전문적인 관리를 통해서 영양 상태 향상뿐만 아니라 질환 개선까지의 가능성도 보여주었다.
목적 : 본 연구는 국내 노인 인구를 대상으로 라이프스타일의 요소가 허약 및 우울 위험도에 미치는 영향을 파악하고자 한다. 연구방법 : 본 연구는 2017년에 수집된 노인실태조사 자료를 활용하여 지역사회에 거주하는 65세 이상 노인 10,072명을 대상으로 수행하였다. 라이프스타일의 요소는 신체활동, 영양 관리, 여가 및 사회 활동 참여 변수를 활용하여 분석하였다. 허약은 허약 척도를 사용하여 측정되었고, 우울 증상은 노인 우울 선별 척도를 사용하여 측정되었다. 로지스틱 회귀분석을 수행하여 노인의 허약 및 우울 위험도에 대한 라이프스타일의 오즈비(odds ratio, OR)를 분석하였다. 결과 : 분석 결과 모든 라이프스타일의 요인은 허약 및 우울 위험도와 유의미한 연관이 있음을 확인하였다. 허약 및 우울의 OR은 주 3회 이상의 정기적인 운동(OR = 0.59, 95% confidence interval [95% CI] = 0.52~0.91; OR = 0.66, 95% CI = 0.59~0.75), 적극적인 영양 관리(OR = 0.86, 95% CI = 0.80~0.91; OR = 0.81, 95% CI = 0.76~0.86), 여가 참여(OR = 0.79, 95% CI = 0.74~0.84; OR = 0.71, 95% CI = 0.66~0.77), 사회 활동의 참여(OR = 0.92, 95% CI = 0.88~0.96; OR = 0.82, 95% CI = 0.78~0.87)를 통해 낮춰지는 것으로 확인되었다. 결론 : 본 연구 결과는 정기적인 운동, 균형 잡힌 영양 관리 및 다양한 활동에 대한 적극적인 참여를 특징으로 하는 건강한 라이프스타일이 노인의 허약 및 우울 위험도를 효과적으로 줄일 수 있음을 시사한다. 궁극적으로 본 연구는 노인의 신체 건강 및 정신 건강에 긍정적으로 작용하는 라이프스타일의 중요성을 강조한다.

사망과 같은 종말 사건은 중간 사건을 중도절단 시킬 수 있지만 재발과 같은 중간 사건은 종말 사건을 중도절단 시킬 수 없는 자료를 준경쟁위험 자료라고 하는데 의학 및 보건, 역학 분야에서는 이와 같은 자료를 자주 접하게 된다. 본 논문에서는 질병-사망 모형에 포함된 세 가지 전이 시간이 모두 구간중도절단된 준경쟁위험 자료를 분석하기 위해 정규 프레일티를 가진 와이블 회귀모형을 제안하였다. 각 개체는 중간 사건과 종말 사건의 발생 여부에 따라 다섯 가지 유형으로 구분되는데 유형별로 조건부 우도함수를 유도하였다. 조정중요표본추출법을 써서 주변 우도함수를 유도한 후 반복의사뉴톤 알고리즘을 써서 최적 추정량을 얻었다. 제안한 추정 방법의 소표본 성질을 살펴보기 위해 모의실험을 수행하였으며 또한 제안한 추정 방법을 Personnes Agées Quid (PAQUID) 자료에 적용하였다.
