Scavenging Effects of Free Radicals and Inhibitory Effects of Lipid Peroxidation of Bupleury Radix Aqua-Acupuncture Solution in Vitro (시호 약침제제의 자유기 소거능 및 지질과산화 억제효능에 관한 연구)

  • Bupleury radix has been used for the treatment of fever, liver disease, inflammation in traditional medicine. The present study was carried out to evaluate the antioxidant effects of Bupleury radix aqua-acupuncture solution (BRAS) in vitro. Oxygen derived free radicals produced in the course of normal aerobic life, such as superoxide anion radical($O_2^-$ ), hydroxyl radicaI( OH), hydrogen peroxide($H_2O_2$) and singlet oxygen($^1O_2$) can attack polyunsaturated fatty acid in cell membranes, enzymes, other cell compounds, and give rise to lipid peroxidation, DNA damage, lipofuscin accumulation, structure alteration of cell membrane and cell death. In this study, antioxidant effects of BRAS on lipid peroxidation were determined according to the method of TBA. BRAS inhibited markedly peroxidation of linoleic acid during the autoxidation, and also inhibited lipid peroxidation induced by hydroxyl radical derived from $H_2O_2-Fe^{2+}$ in rat liver homogenate. And BRAS showed 30% scavenging effect on DPPH radical, also exhibited a 30% inhibitory effect on superoxide generation from xanthine-xanthine oxidase system. In addition, BRAS protected the cell death induced by tert-butyl hydroperoxide(t-BHP) and significantly increased cell viability in the normal rat liver cell(Ac2F).

Bilateral Adrenal Neuroblastoma (양측성 부신 신경아세포종)

  • Neuroblastoma is the most common solid malignancy in childhood. However, a neuroblastoma presenting as bilateral and multifocal is very rare. A male newborn weighing 2.7 kg was born by normal vaginal delivery at 39 weeks' gestation to a 27-year-old mother who had a normal pregnancy. He was in good condition at birth but presented palpable masses in the both upper abdomen and both side of the neck. Ultrasound examination showed the lesions to be $3{\times}3cm$ sized calcified mass in right suprarenal area and $5{\times}3cm$ sized homogenous mass in left suprarenal area. The abdominal mass was also examined by computed tomography with similar findings. With the impression of bilateral neuroblastoma or metastatic spread, the laparotomy was performed on the 13th day of life. Frozen section of biopsy of the left neck mass was obtained, which showed neuroblastoma. Intraoperative findings revealed bilateral adrenal masses which were distinct and anatomically separate. Bilateral adrenalectomy was accomplished and the initial postoperative course was uneventful. The patient was discharged for hopeless and expired at age 45 days. We present bilateral adrenal neuroblastoma considered to be simultaneous occurrence rather than metastases from one site to another.

Analysis of the State of Science Teaching Anxiety of Elementary Teachers for the Scientifically Gifted according to Their Personal Variables (교사변인에 따른 초등과학 영재교사의 일반학급과 영재학급에서의 과학교수불안 분석)

  • The purpose of this study is to analyze the state of science teaching anxiety of elementary teachers for the scientifically gifted according to their personal variables such as gender, career in education and gifted education, training course for gifted education, participation in meeting related to gifted education. The subjects in this study were 64 elementary school teachers who were in charge of science gifted education. The instrument to assess their science teaching anxiety was Westerback's S-STAI, modified in this study to be suitable for science gifted education. The collected data were analyzed by the statistical package SPSS. The findings of the study were as follows: The teachers for gifted felt more teaching anxiety in gifted classes than in normal classes on the whole. There were statistically significant differences in their personal variables such as gender, career in education experience, experience of taking gifted education training courses, and participation in meeting related to gifted education. Implications related to reduce the gifted education teachers' teaching anxiety were discussed in depth.

Stem cell niche as a prognostic factor in leukemia

  • Despite high interests on microenvironmental regulation of leukemic cells, little is known for bone marrow (BM) niche in leukemia patients. Our recent study on BMs of acute myeloid leukemia (AML) patients showed that the mesenchymal stromal cells (MSCs) are altered during leukemic conditions in a clinical course-dependent manner. Leukemic blasts caused reprogramming of transcriptomes in MSCs and remodeling of niche cross-talk, selectively suppressing normal primitive hematopoietic cells while supporting leukemogenesis and chemo-resistance. Notably, differences in BM stromal remodeling were correlated to heterogeneity in subsequent clinical courses of AML, i.e., low numbers of mesenchymal progenitors at initial diagnosis were correlated to complete remission for 5-8 years, and high contents of mesenchymal progenitor or MSCs correlated to early or late relapse, respectively. Thus, stromal remodeling by leukemic cell is an intrinsic part of leukemogenesis that can contribute to the clonal dominance of leukemic cells over normal hematopoietic cells, and can serve as a biomarker for prediction of prognosis. Cytotoxicity of SD-994 from Artemisia argyi against L1210 Cells with Concomitant Induction of Antioxidant Enzymes (황해쑥 추출정제물 SD-994의 L1210암세포에 대한 세포독성과 항산화효소의 유발)

  • SD-994 was prepared from methanol extract of Artemisia argyi by stepwise purification of solvent partioning and silica gel chromatography. In the course of this purification, fractions obtained at each step were investigated for their cytotoxicities against L1210 cells. Fractions A~G prepared from chloroform fraction showed considerable cytotoxicities raging 40~90% against L1210 cells. Subfractions I~IX obtained from fraction A exhibited various cytotoxicities and subfraction I (SD-994) was found to be the most effective compound. $IC_{50}$ values of SD-994 were measured to be $0.5{\;}{\mu\textrm{g}}/ml and less than $0.05{\;}{\mu\textrm{g}}/ml against L1210 cells and normal lymphocytes, respectively: When SD-994 was added to L1210 cell as cytotoxic agent, significantly increased amount of superoxide ($O_2^-$) and dramatically augmented activities of superoxide dismutase (SOD), specially MnSOD and glutathione peroxidase (GPx) were observed according to the concentration and incubation time. Whereas, in case of normal lymphocytes under the same condition, cytotoxicities were not apparent and the generation of superoxide ($O_2^-$) or the activity changes of SOD and GPx were insignificant. These results together indicate that the cytotoxic action of SD-994 against L1210 cell may be achieved via necrosis and/or apoptosis induced by reaction oxygen species which could not probably be completely abolished even by drastically increased antioxidant enzymes, SOD and GPx activities.

Tricuspid Atresia: Two Cases of Successful Palliative Surgery (선천성 삼첨판 폐쇄증 (수술 2례 보고))

  • Two patients operated upon for tricuspid atresia by Glenn operation are presented. They were five years old and four years old females who were cyanotic shortly after birth, and remained cyanotic. On physical examination, cyanosis on digits and lips, clubbing of fingers, thrill and grade III systolic murmur on 4th I.C.S. along left sternal border were noted. In the first case, chest roentgenograms showed normal pulmonary markings, and the electrocardiogram was interpreted as showing left ventricular hypertrophy with left axis deviation and peaked P-wave in lead IL Right heart catheterization showed high pressure in right atrium and the catheter tip was easily inserted into the left atrium through septal defect. In the second case, chest roentgenograms showed cardiomegaly with sparse pulmonary vascular markings and narrowed vascular pedicle, and the electrocardiogram showed left ventricular hypertrophy with some element of right atrial hypertrophy. Angiocardiogram showed changes characteristic of tricuspid atresia, including "right ventricular window" The findings of right heart catheterization were similar to those of first case. On the basis of these observations, they were diagnosed as tricuspid atresia, and Glenn operation was performed. Normal position of great vessels combined with ventricular septal defect and pulmonary stenosis were noted on the first case, and on second case, transposition of great vessels was additional finding. Postoperative course was uneventful and favorable outcome was obtained.

A Case of Secondary Hypoadrenocorticism in a Dog (이차성 부신겉질저하증 1례)

  • A 9-year old female Shih Tzu dog was presented to the Veterinary Medical Teaching Hospital of Seoul National University with a history of chronic intermittent anorexia and vomiting of 4-year duration. She visited 3 years ago with the same clinical signs but has not been treated regularly due to waxing-waning course. On the physical examination, no specific findings were found. CBC, serum chemistry, radiography, ACTH stimulation test, evaluation of serum T4 and TSH concentration were performed. Hypoadrenocorticism was diagnosed with the ACTH stimulation test. And then, secondary hypoadrenocorticism was diagnosed with ACTH stimulation test, pre-ACTH aldosterone concentration, endogenous ACTH concentration. As electrolyte concentrations were normal, glucocorticoid (0.15 mg/kg bid PO) alone was administered. She has recovered from the clinical signs and has been doing well. It is suggested that the differentiation of secondary hypoadrenocorticism from primary hypoadrenocorticism is important as the secondary hypoadrenocorticism occurs rarely and the symptoms are non-specific with normal electrolyte concentrations.

The Effect of Kinesio Taping on the Quadriceps Femoris to the Gait Characteristics (대퇴사두근에 키네시오 테이핑 적용이 보행특성에 미치는 효과)

  • Background : The purpose of present study was to investigate the effect of quadriceps femoris taping in normal gait using 3D motion capture technique. Method : Twenty healthy volunteers, have no musculoskeletal problems, were recruited as subjects for this study. In experimental group, 20 healthy young(males 10 and females 10) were included. The subjects were assessed during two conditions: control tape(no muscle stretched) and quadriceps (muscle stretched)taping application. To obtain the dynamic data, we captured the motion of subject attached markers without taping during repeated gaits five times or more in 7 m Capture volume of gait analysis center. The result was obtained as a mean value in three times. After taping on quadriceps femoris, the same procedure was carried out. Statistical analysis were performed using statistical software packagess SPSS WIN 12.0(SPSS, Chicago, IL, USA). Differences were tested for statistical significance using paired t-test, independent t-test, chi-squared test for comparisons between the muscle stretched and no muscle stretched. Results : The date of 20 subjects who carried out the whole experimental course were statistically analyzed. 1. gait velocity was showed that muscle stretched group had more significantly increased than no muscle stretched group(p<.05). 2. step length was showed that muscle stretched group had more significantly increased than no muscle stretched group(p<.05). 3. cadens was showed that muscle stretched group had more significantly increased than no muscle stretched group(p<.05). Conclusion : kinesio taping on quadriceps femoris promoted cadence, gait velocity, step length in normal subject (muscle stretched) group.

Mercedes Benz Pattern Craniosynostosis: A Case Report (벤츠 패턴을 가진 두개골 조기 유합증: 증례보고)

  • Purpose: Craniosynostosis of three or more cranial sutures was not common. "Mercedes Benz pattern," named by Moore1 was a rare form of craniosynostosis and had an atypical pattern of premature closure of cranial suture. It was not reported in Republic of Korea. We report this case with literature review. Methods: A 13-months-old male patient visited our clinic due to exophthalmos. He showed normal developmental course. Other neurological tests were normal but he was Crouzon syndrome patient. CT scans showed bilateral lambdoid and posterior sagittal sutures were fused and the length of the skull was extended. Cranioplasty with pi craniotomy & Barrel-Stave osteotomy and recombination of the bone flap was performed. Results: The patient was discharged after post operative 10 days without any complications. In follow up visit after 2.7 years, he was in good state without recurrence and functional abnormality of skull. Conclusion: This was the first case of Mercedes Benz pattern craniosynostosis with Crouzon syndrome in Korea. This type of craniosynostosis has to be considered differently from single type of craniosynostosis or typical syndromic craniosynostosis clinically and surgically.

The necessity for education on endotracheal intubation through video laryngoscope - A focused on paramedic students - (비디오 후두경을 통한 기관내 삽관 교육의 필요성 - 응급구조과 학생을 중심으로 -)

  • Purpose: The aim of this study was to verify the necessity of endotracheal intubation through video laryngoscope and to provide basic data to inform the provision of video laryngoscope education. Methods: Eighty paramedic students participated in this study. A survey was conducted from November 5, 2018 to December 7, 2018. Data were analyzed with independent t-tests, and the chi-squared test. Results: The video laryngoscope is a highly usable instrument that can easily be applied during training. The instrument provides better visual evaluation of the normal airway (p=.004), the airway in case of cervical collar and head fixation (p=.000), and the airway in case of tongue edema (p=.000). The time of endotracheal intubation in the normal airway was significantly less with the video laryngoscope compared with the direct laryngoscope. The success rate of tracheal intubation was significantly higher in the video laryngoscope group than in the direct laryngoscope. Conclusion: This study suggests the necessity of education on endotracheal intubation through video laryngoscope in the professional airway maintenance training course of emergency department students. The video laryngoscope is easier to apply than the direct laryngoscope in cases of intubation in various clinical situations.
