Acoustic Comparisons of Vowel and Plosive Productions between the Normal and the Hearing-Impaired Children (청각장애아동과 건청아동의 모음 및 파열음 산출의 음향음성학적 특성 비교)

  • Twenty normal and 20 severe-to-profound hearing-impaired subjects participated in the present study. The two groups are matched by their chronological age. Each subject made a recording of three vowels of /i/, /a/, and /u/, and nine $VC_{plosive}V$ (hereafter, VCV) disyllables of /epe/, /ep'e/, /$ep^{h}e$/, /ete/, /et'e/, /$et^{h}e$/, /eke/, /ek'e/, and /$ek^{h}e$/, each five times. Formant frequencies of $F_1,\;F_2,\;and\;F_3$ were measured for the three vowels and six measures were made for the nine disyllables. The six measures were (1) the total duration of the disyllable, (2) the duration of the first vowel, (3) the duration of the closed period, (4) the ratio of the first vowel over the first vowel plus the closure period of the consonant, (5) the duration of the aspiration, and (6) the duration of the second vowel. Results shows that the three formants and each of the measures were significantly different between the two groups of subjects.

Tubular Colonic Duplication Presenting as Rectovestibular Fistula

  • Complete colonic duplication is a very rare congenital anomaly that may have different presentations according to its location and size. Complete colonic duplication can occur in about 15% of all gastrointestinal duplications. Double termination of tubular colonic duplication in the perineum is even more uncommon. We present a case of a Y-shaped tubular colonic duplication which presented with a rectovestibular fistula and a normal anus. Radiological evaluation and initial exploration for sigmoidostomy revealed duplicated colons with a common vascular supply. Endorectal mucosal resection of theduplicated distal segment till the colostomy site with division of the septum of the proximal segment and colostomy closure proved curative without compromise of the continence mechanism. Tubular colonic duplication should always be ruled out when a diagnosis of perineal canal is considered in cases of vestibular fistula alongwith a normal anus.

Clinical Experience of Atrial Myxoma in 15 Patients (심방 점액종의 임상적 치험 15례)

  • Fifteen patients[10 female and 5 male from 13 to 73 years of age[mean age 46 years underwent excision of atrial myxomas between 1981 and 1993 at the Chonnam University Hospital. Fourteen patients presented with exertional or resting dyspnea, six with palpitation, five with systemic embolization, and three with syncope. Symptoms were present from 1 week to 14 months before operation. Fourteen tumors originated from the atrial septum of left atrium and one of bilateral atrium. All tumors were pedunculated; tumor sizes were variable from 2x1x1cm to 7x6x5cm. No ventricular tumors identified. The myxomas were successfully removed in all patients, either by shaving them from the atrial septum[n=7 or excising a portion of normal atrial septum with the tumor and ASD patch closure[n=8 . There was no perioperative or late death in our experience. Follow-up is current and complete in all cases[range 6 months to 13 years . Thirteen patients are in New York Heart Association Class I, and the remaining two patients are in Class II. One recurrent left atrial myxoma was identified at 20 months after operation. In this series, excellent results were obtained by simple excision of the tumor, with or without a margin of normal atrial septum. Long-term clinical & echocardiographic follow-up is recommended since late recurrence, although rare, has been reported.

Dysphagia Handicap Index and Swallowing Characteristics based on Laryngeal Functions in Korean Elderly (한국 정상 노인층의 삼킴장애지수와 후두 기능에 따른 삼킴 특성)

  • Larynx plays an important role in phonation and protection of the respiratory tract during swallowing. The reduced anatomical and physiological function in elevation of larynx and glottis closure can cause problems in voice and swallowing. The present study investigated the Korean version of handicap index of dysphagia in elderly Koreans. Therefore, 60 normal elderly Koreans ranged from 65 to 95 and 20 normal Korean young adults aged from 20 to 25 were participated in this study to compare total (T), physical (P), functional (F), and emotional (E) index scores between two groups as well as among sub groups (60s, 70s, 80s) in elderly. For swallowing, total and sub dysphagia handicap index (DHI) scores, voice quality during /a/phonation following swallowing (saliva and water), intensity of coughing, and L-DDK were measured. The results showed that functional (P), physical (P), emotional (E) scores as well as total (T) score were significantly different between young adults and old adults in DHI(p<.05). Additionally, there was a negative correlation between total DHI score and intensity of coughing (r=-.51) as well as L-DDK (r=-.70). These findings suggest that a slow rate in vocal fold adduction and reduced intensity of coughing in the elderly affect swallowing function. Thus, recently translated Korean version of DHI may be useful as supplement in evaluating the swallowing problems in elderly people.

Mercedes Benz Pattern Craniosynostosis: A Case Report (벤츠 패턴을 가진 두개골 조기 유합증: 증례보고)

  • Purpose: Craniosynostosis of three or more cranial sutures was not common. "Mercedes Benz pattern," named by Moore1 was a rare form of craniosynostosis and had an atypical pattern of premature closure of cranial suture. It was not reported in Republic of Korea. We report this case with literature review. Methods: A 13-months-old male patient visited our clinic due to exophthalmos. He showed normal developmental course. Other neurological tests were normal but he was Crouzon syndrome patient. CT scans showed bilateral lambdoid and posterior sagittal sutures were fused and the length of the skull was extended. Cranioplasty with pi craniotomy & Barrel-Stave osteotomy and recombination of the bone flap was performed. Results: The patient was discharged after post operative 10 days without any complications. In follow up visit after 2.7 years, he was in good state without recurrence and functional abnormality of skull. Conclusion: This was the first case of Mercedes Benz pattern craniosynostosis with Crouzon syndrome in Korea. This type of craniosynostosis has to be considered differently from single type of craniosynostosis or typical syndromic craniosynostosis clinically and surgically.

Congenital Epulis with Feeding Difficulty: A Case Report (수유장애를 동반한 선천성 치은종: 증례보고)

  • Purpose: The congenital epulis is a rare, benign tumor. It can protrude out of the newborn's mouth to prevent normal closure of the mouth and it can interfere with respiration or feeding. Methods: An 11-day old female neonate presented with a $1.5\;{\times}\;1.5\;{\times}\;2.3cm$ sized mass in the gingival and anterior alveolar ridge of the mandible. We performed a simple excision. Results: In our case, histologically, there was no pseudoepitheliomatous hyperplasia. The staining for S-100 protein, and actin was negative. After 8 months, the patient had normal teeth eruption and no recurrence of the tumor. Conclusion: With early detection and appropriate treatment, we were able to help the baby avoid developing any dyspnea. Nursing was possible after the mass had been removed.

CLINICAL STUDY OF VELOPHARYNGEAL CLOSURE AFTER THE PRIMARY PALATORRHAPHY IN CLEFT PALATE PATIENTS (구개열(口蓋裂) 환자(患者)에 있어서 구개(口蓋) 성형술후(成形術後) 비인강(鼻咽腔) 폐쇄(閉鎖)에 관(關)한 임상적(臨床的) 연구(硏究))

  • In order to find the causes of velopharyngeal incompetency after primary palatorrhaphy in cleft patients, we analyzed the form and function of the velopharyngeal space of fifteen operated cleft palate patients and five normal subjects. The velopharyngeal function was evaluated by lateral cephalometric radiography, velopharyngography and hypernasality cul-de-sac test. The obtained results were as follows. 1. The rate of velopharyngeal incompetency was twenty percent, three of the fifteen operated patients. Two of them were complete cleft palate and the other was incomplete one. 2. The length of soft palate and levator eminence were longer in normal group than those of good speech group and complete cleft palate group during phonation of /i/ (P<0.05). The lengthening rate of soft palate was smaller in good and poor speech group than that of normal group(P<0.05), and, reduced in order, normal group, complete cleft palate group and incomplete palate group(P<0.05). 3. The nasopharyngeal distance had no significant difference between all groups at rest, but, smaller in normal group than that of both cleft palate group(P<0.05), good speech group and poor speech group(P<0.05) during phonation of /i/ The difference in nasopharyngeal distance between rest and /i/ phonation was greater in normal group than that of both cleft palate group, good speech group and poor speech group. 4. The moving distance of sop palate reduced in order, normal group, incomplete cleft palate group, complete cleft palate group(P<0.05). 5. The distance between lateral pharyngeal wall had no significant difference between all groups in rest, but, smaller than that of complete cleft palate group in normal group(P<0.01) and increased in order normal group, good speech group, poor speech group(P<0.01) during phonation of /a/. The mobility of lateral wall was reduced in order, normal group, good speech group poor speech group(P<0. 01). 6. There was low corelationship between the mobility of lateral pharyngeal wall and soft palate. Therfore, it suggest that the movements of lateral pharyngeal wall and soft palate occurs independently.

Laser Frenotomy (레이저를 이용한 설소대절단술)

  • Tongue tie is the congenital anomaly which is manifested by an abnormally short and thick lingual frenulum so that a condition outside of the range of normal anatomic or functional capability. In general, the surgical correction is the treatment of choice: release of tongue tie and closure. We compared the traditional frenotomy with laser frenotomy using Nd:YAG laser to treat 67 cases of tongue tie during a period of JAN. 1993 through Feb. 1996. The advantages of laser frenotomy were decreased blood loss and operation time. So we had a conclusion as the adequate use of laser frenotomy with suture was a good surgical procedure for correction of tongue tie.

ANALYSIS OF TURBULENT HEAT TRANSFER FROM STAGGERED PIN-FIN ARRAYS WITH DIAMOND SHAPED ELEMENTS AT VARIOUS GEOMETRICAL CONFIGURATIONS (엇갈린 다이아몬드형 핀휜의 형상에 따른 난류열전달 성능해석)

  • A numerical study is carried out to analyze the steady three-dimensional turbulent flow and convective heat transfer in a staggered pin-fin array with diamond shaped elements at various geometrical configurations. Steady Reynolds-averaged Navier-Stokes equations and energy equation are solved using a finite volume based solver. Shear stress transport (SST) model is used as turbulence closure. The computational domain is composed of one pitch of pin-fin displacement with periodic boundary conditions on the surfaces normal to the streamwise direction and the cross-streamwise direction. The numerical results for Nusselt number and friction factor are validated with experimental results. The effects of pin angle, pin height and pitch on Nusselt number, friction factor and efficiency index are investigated.

  • In this paper, we mainly discuss Gorenstein Dedekind do-mains (G-Dedekind domains for short) and their overrings. Let R be a one-dimensional Noetherian domain with quotient field K and integral closure T. Then it is proved that R is a G-Dedekind domain if and only if for any prime ideal P of R which contains ($R\;:_K\;T$), P is Gorenstein projective. We also give not only an example to show that G-Dedekind domains are not necessarily Noetherian Warfield domains, but also a definition for a special kind of domain: a 2-DVR. As an application, we prove that a Noetherian domain R is a Warfield domain if and only if for any maximal ideal M of R, $R_M$ is a 2-DVR.
