비소세포성폐암 환자의 $Monototal^{TM}$ 유용성 평가 (Evaluation of Usefulness for $Monototal^{TM}$ in Non Small Cell Lung Cancer)

  • 목적 : 2008년도 국내에서의 암 사망원인 1위가 폐암이며, 폐암으로 인한 사망자수는 매년 증가 하고 있다. 이 폐암은 대부분이 진행된 상태로 발견되면 이들 중 80%는 비소세포 성폐암(NSCLC)이다. 종양표지자 검사는 질병의 진행과 관련된 신뢰성 있는 결과를 신속히 의료진에게 제공해야 하는데 $Monototal^{TM}$ 검사는 반응시간이 짧으며, 비소세포성폐암 특히, Squamous cell(편평상피세포)에 대한 예민도가 높은 검사로 알려져 있다. 이에 비소세포성폐암의 진단 및 치료경과 관리를 위한 새로운 종양표지자인 $Monototal^{TM}$의 임상적 유용성에 대해 알아보고자 한다. 실험재료 및 방법 : 2009년 3월에서 4월까지 본원에 의뢰된 비소세포성폐암 환자 검체중 CEA가 양성인 검체 48건과 SCC 양성인 검체 15건, CYFRA 21-1 양성 검체 40건과 음성검체 20건을 대상으로 $Monototal^{TM}$을 측정하여 결과를 비교 하였다. 결과 : 비소세포성폐암 진단받은 환자 검체의 87.5%가 $Monototal^{TM}$ 검사에서 양성을 보였다. SCC 양성인 검체에서는 93.3%양성을 보였다. CYFRA 21-1 양성 검체에서는 100%의 양성을 보였고 음성인 검체에서도 50%의 양성을 보였다. 결론 : 종양표지자는 임상적으로 치료 후 환자의 경과 관찰에 유용하게 사용 되므로 중요하다. 실험 결과 비소세포성폐암에서 $Monototal^{TM}$의 양성률이 높게 보고 되었다. $Monototal^{TM}$의 임상적 가치를 평가 하기 위해서는 좀더 많은 검사가 필요하다고 생각한다. 그러나 CYFRA 21-1 보다 짧은 시간에 결과를 보고 할 수 있고 높은 양성률을 보인 것으로 보아 임상적으로 의의가 있다고 판단된다. 따라서 다른 종양표지자와 병행해서 검사하면 폐암의 진단과 예후 추정에 있어서 임상적으로 유용하다고 생각된다.

A spindle cell squamous cell carcinoma on the cheek presenting with in-transit metastases and a satellite lesion

  • Spindle cell squamous cell carcinoma (SpSCC) is a biphasic tumor composed of squamous cell epithelial and spindle cell mesenchymal components, both of which are malignant. Cutaneous SpSCC can cause diagnostic and therapeutic difficulties because of its rarity, heterogeneity, morphological similarity to other cutaneous spindle cell neoplasms, and uncertain pathogenesis and prognosis, particularly when the squamous cell carcinoma component is minimal or missing. Intransit metastasis and satellite lesion (satellitosis) constitute a spectrum of non-nodal regional metastases. Here the author reports the first known case of cutaneous SpSCC presenting with intransit metastases and a satellite lesion, which were exceptionally aggressive. A 77-year-old female patient presented with a 3×3×0.5 cm mass on her right cheek. Despite wide excision and postoperative radiation, the patient resulted in local recurrence and multiple distant metastases within 3 months. If many high-risk factors-particularly satellitosis and in-transit metastases are observed in a tumor with epithelial to mesenchymal transition, then further wide excision and adjuvant chemoradiation should be considered early in the treatment process. A multidisciplinary approach could be the key to cure the most aggressive malignancies of the skin, as in other organs.

냉각판으로 제조된 Al-Zn-Mg-Cu계 반응고 알루미늄 합금의 RRA 처리 (RRA Treatment of Semi-Solid Al-Zn-Mg-Cu Al Alloy Fabricated by Cooling Plate)

  • The optimum RRA heat treating conditions and SCC (stress corrosion cracking) resistance of semi-solid Al-Zn-Mg-Cu alloy fabricated by inclined cooling plate were compared with those of conventional mould cast alloys. The non-stirring method characterized by using a cooling plate can effectively eliminate dendritic structure and form a fine globular semisolid microstructure in as-cast Al-Zn-Mg-Cu alloy and the SCC resistance of semi-solid Al-Zn-Mg-Cu alloy was higher than that of conventional mold cast alloy. Also, after retrogressed treatment at RRA heat treatment of semi-solid Al-Zn-Mg-Cu alloy, retrogressed treatment time has increased more than 10 minutes at $180^{\circ}C$ to recovery the T6 heat treatment strength.

Effect of Dissolved Oxygen on the Stress Cor rosion Cracking Behavior of 3.5NiCrMoV Steels in High Temperature Water

  • Slow Strain Rate Tests (SSRT) were carried out to investigate the effect of environmental factors on the Stress Corrosion Cracking (SCC) susceptibility of 3.5NiCrMoV steels used in discs for Low-Pressure (LP) steam turbines in electric power generating plants. The influences of dissolved oxygen on the stress corrosion cracking of turbine steel were studied, For this purpose, specimens were strained at variously oxygenated conditions at $150^{\circ}C$ in pure water. When the specimen was strained with $1{\times}10^{-7}s^{-1}$ at $150^{\circ}C$ in pure water, increasing concentration of dissolved oxygen decreased the elongation and the UTS. The corrosion potential and the corrosion rare increased as the amounts of dissolved oxygen increased. The increase of the SCC susceptibility of the turbine steel in a highly dissolved oxygen environment is due to the non protectiveness of the oxide layer on the turbine steel surface and the increase of the corrosion current. These results clearly indicate that oxygen concentration increases Stress Corrosion Cracking susceptibility in turbine steel at $150^{\circ}C$.

Effects of Milk Production, Season, Parity and Lactation Period on Variations of Milk Urea Nitrogen Concentration and Milk Components of Holstein Dairy Cows

  • The study was conducted to assess the effect of milk production, parity, stage of lactation, season and individual milk components themselves on milk urea nitrogen (MUN) concentration and other milk components of 3,219 Holstein dairy cows in Korean dairy farms. The MUN concentrations in Korean dairy cows were estimated to 16.68$\pm$5.87 mg/dl. Milk yield was negatively correlated with fat and protein contents and somatic cell counts (SCC) in milk (p<0.01). The increasing MUN concentration has positive correlation with yield and fat content. By increasing somatic cell, milk yield was reduced and MUN level was increased. Cows in spring and winter produced more milk over 1.43 and 0.93 kg/day, respectively, than cows in summer (p<0.01). Milk urea nitrogen concentrations of milk produced in summer and fall were significantly lower (p<0.01) than those in spring and winter. Both MUN concentration and somatic cell counts were highest in winter. Milk yield was lower (p<0.01) in the first calving than other calving time and was tended to increase until the fifth parity and then decrease. Milk urea nitrogen and SCC were not related to parity of cows in this study. Milk yield and SCC were positively related to lactation period while MUN concentrations and milk fat and protein contents were negatively influenced by stage of lactation. In the present study, the relationship between MUN and reproduction of dairy cows was also investigated. Cow produced milk in high MUN concentrations (greater than 18 mg/dl) had more open days than cows in MUN concentrations less than 18 mg/dl. However, no significant difference between MUN concentration levels and frequency of artificial insemination was found in this study. It is suggested that although MUN values for nutritional management and measures of production or reproduction are used, non-nutritional factors should be considered.

Systematical Analysis of Cutaneous Squamous Cell Carcinoma Network of microRNAs, Transcription Factors, and Target and Host Genes

  • Background: MicroRNAs (miRNAs) are small non-coding RNA molecules found in multicellular eukaryotes which are implicated in development of cancer, including cutaneous squamous cell carcinoma (cSCC). Expression is controlled by transcription factors (TFs) that bind to specific DNA sequences, thereby controlling the flow (or transcription) of genetic information from DNA to messenger RNA. Interactions result in biological signal control networks. Materials and Methods: Molecular components involved in cSCC were here assembled at abnormally expressed, related and global levels. Networks at these three levels were constructed with corresponding biological factors in term of interactions between miRNAs and target genes, TFs and miRNAs, and host genes and miRNAs. Up/down regulation or mutation of the factors were considered in the context of the regulation and significant patterns were extracted. Results: Participants of the networks were evaluated based on their expression and regulation of other factors. Sub-networks with two core TFs, TP53 and EIF2C2, as the centers are identified. These share self-adapt feedback regulation in which a mutual restraint exists. Up or down regulation of certain genes and miRNAs are discussed. Some, for example the expression of MMP13, were in line with expectation while others, including FGFR3, need further investigation of their unexpected behavior. Conclusions: The present research suggests that dozens of components, miRNAs, TFs, target genes and host genes included, unite as networks through their regulation to function systematically in human cSCC. Networks built under the currently available sources provide critical signal controlling pathways and frequent patterns. Inappropriate controlling signal flow from abnormal expression of key TFs may push the system into an incontrollable situation and therefore contributes to cSCC development.

Seasonal and Regional Effects on Milk Composition of Dairy Cows in South Korea

  • For a period of over 6 years, more than 160,000 milk samples were collected and analyzed to determine the influence of different seasonal temperatures and geographic regional location on milk composition in South Korea. Fat, protein, lactose, non fat milk solids (NFMS) and total solids (TS) contents were significantly higher among dairy cows milked in winter season than other seasons (p<0.05). In contrast, freezing point (FP), milk urea nitrogen (MUN) and somatic cell count (SCC) were significantly higher in summer season than other seasons (p<0.05). The average SCC in the autumn season was $358{\times}10^3$/ml, which was lower than any other seasons (p<0.05). These results may be due to the changes in temperature during different seasons. Meanwhile, milk produced by dairy cows in central region had higher fat, protein, lactose, NFMS, TS and MUN and had lower SCC compared to other regions (p<0.05). Fat, TS, FP, MUN and citric acid in northeast region were lower than other regions (p<0.05). The SCC was significantly higher in southeast region than those of other regions (p<0.05). As a result, it might be possible that the differences in feeding management in each different region may affect the milk composition. In conclusion, present results indicated that milk composition is clearly influenced by both season and regional location. Therefore, based on these results, development of different feeding systems, according to season and region is needed to produce high quality and satiable milk production.

Effect of ZnO Nanoparticle Presence on SCC Mitigation in Alloy 600 in a Simulated Pressurized Water Reactors Environment

  • This study investigates the synthesis, characterization, and application of zinc oxide (ZnO) nanoparticles for corrosion resistance and stress corrosion cracking (SCC) mitigation in high-temperature and high-pressure environments. The ZnO nanoparticles are synthesized using plasma discharge in water, resulting in rod-shaped particles with a hexagonal crystal structure. The ZnO nanoparticles are applied to Alloy 600 tubes in simulated nuclear power plant atmospheres to evaluate their effectiveness. X-ray diffraction and X-ray photoelectron spectroscopy analysis reveals the formation of thermodynamically stable ZnCr2O4and ZnFe2O4 spinel phases with a depth of approximately 35 nm on the surface after 240 hours of treatment. Stress corrosion cracking (SCC) mitigation experiments reveal that ZnO treatment enhances thermal and mechanical stability. The ZnO-treated specimens exhibit increased maximum temperature tolerance up to 310 ℃ and higher-pressure resistance up to 60 bar compared to non-treated ZnO samples. Measurements of crack length indicate reduced crack propagation in ZnO-treated specimens. The formation of thermodynamically stable Zn spinel structures on the surface of Alloy 600 and the subsequent improvements in surface properties contribute to the enhanced durability and performance of the material in challenging high-temperature and high-pressure environments. These findings have significant implications for the development of corrosion-resistant materials and the mitigation of stress corrosion cracking in various industries.

Experimental investigation on flexural behaviour of HSS stud connected steel-concrete composite girders

  • In this paper, experimental investigations on high strength steel (HSS) stud connected steel-concrete composite (SCC) girders to understand the effect of shear connector density on their flexural behaviour is presented. SCC girder specimens were designed for three different shear capacities (100%, 85%, and 70%), by varying the number of stud connectors in the shear span. Three SCC girder specimens were tested under monotonic/quasi-static loading, while three similar girder specimens were subjected to non-reversal cyclic loading under simply supported end conditions. Details of casting the specimens, experimental set-up, and method of testing, instrumentation for the measurement of deflection, interface-slip and strain are discussed. It is found that SCC girder specimen designed for full shear capacity exhibits interface slip for loads beyond 25% of the ultimate load capacity. Specimens with lesser degree of shear connection show lower values of load at initiation of slip. Very good ductility is exhibited by all the HSS stud connected SCC girder specimens. It is observed that the ultimate moment of resistance as well as ductility gets reduced for HSS stud connected SCC girder with reduction in stud shear connector density. Efficiency factor indicating the effectiveness of high strength stud connectors in resisting interface forces is estimated to be 0.8 from the analysis. Failure mode is primarily flexure with fracturing of stud connectors and characterised by flexural cracking and crushing of concrete at top in the pure bending region. Local buckling in the top flange of steel beam was also observed at the loads near to failure, which is influenced by spacing of studs and top flange thickness of rolled steel section. One of the recommendations is that the ultimate load capacity can be limited to 1.5 times the plastic moment capacity of the section such that the post peak load reduction is kept within limits. Load-deflection behaviour for monotonic tests compared well with the envelope of load-deflection curves for cyclic tests. It is concluded from the experimental investigations that use of HSS studs will reduce their numbers for given loading, which is advantageous in case of long spans. Buckling of top flange of rolled section is observed at failure stage. Provision of lips in the top flange is suggested to avoid this buckling. This is possible in case of longer spans, where normally built-up sections are used.

공정평균 이동을 탐지하기 위한 적응 합성 관리도 (An Adaptive Synthetic Control Chart for Detecting Shifts in the Process Mean)

  • The synthetic control chart (SCC) proposed by Wu and Spedding (2000) is to detect shifts in the process mean. The performance was re-evaluated by Davis and Woodall (2002), and the steady-state average run length (ARL) performance was shown to be inferior to cumulative sum (CUSUM) or exponentially weighted moving average (EWMA) chart This paper proposes a simple adaptive scheme to improve the performance of the synthetic control chart. That is, once a non-conforming (NC) sample occurs, we investigate the next L-consecutive samples with larger sample sizes and shorter sampling intervals. We employ a Markov chain model to derive the ARL and the average time to s19na1 (ATS). We also propose a statistical design procedure for determining decision variables. Comprehensive comparative study shows that the proposed control chart is uniformly superior to the original SCC or double sampling (DS) Χ chart and comparable to the EWMA chart in ATS performance.
