Objectives: The aim of this study was to evaluate the differences in the polysomnography data between positional and non-positional obstructive sleep apnea (OSA) patients. Methods: Forty-seven patients diagnosed with OSA were evaluated using full night polysomnography. According to the criteria of Cartwright et al., the patients were classified into two groups with 37 positional (supine apnea-hypopnea index [AHI] $\geq$ 2x's the lateral AHI) and 10 non-positional (supine AHI < 2x's the lateral AHI) OSA patients, and the differences of polysomnography data between the two groups were evaluated. Results: There were no significant differences in demographic variables (age, gender, and BMI), daytime sleepiness, overall AHI, total arousal index, and percent time of snoring between two groups. However, AHI, arousal index, and mean oxygen saturation ($SpO_2$) of the REM sleep stage were significantly more severe in the positional OSA group than the non-positional OSA group. Mean $SpO_2$ and the lowest $SpO_2$ during overall sleep stage were also significantly lower in the positional OSA group than the non-positional OSA group. Conclusions: Our results of differences in the polysomnography data of REM sleep stage suggest that non-positional OSA patients may have higher collapsibility of the oropharyngeal airway during sleep than positional OSA patients.

This study aims at the analysis using the Resistivity Estimation Model (REM) to examine the effect of specimen size on the measurement of electrical resistivity. In the experiment, specimens of concrete were fabricated and the apparent resistivity was measured for each electrode interval. The apparent resistivity measured was found to be distorted in the apparent resistivity as the specimen size became smaller and closer to the outside (edge). As a result of comparing the experimental and analysis values, it is expected that REM can be used to examine the effect of the size of the specimen.

수면단계는 수면감성을 평가하는데 있어서 중요한 생리지표로서 사용되어왔다. 그러나 수면다원검사를 이용한 전통적 수면단계 분류방법은 뇌전도, 안전도, 심전도, 근전도 등을 종합적으로 측정하므로 수면단계를 비교적 정확히 분류할 수 있지만 피험자에게 심한 구속감을 주는 문제가 있다. 본 연구에서는, 각성상태에서 교감신경계가 지배적인 반면에 수면 중에는 부교감신경계가 더 활동적인 점에 착안하여 수면단계를 간단히 분류할 수 있는 방법을 찾고자 수면단계에 따른 심박동변이도(heart rate variability: HRV)를 분석하였다. 단일채널 심전도를 이용하여 수면단계별로 HRV 의 교감신경계/부교감신경계 활성도의 비율을 분석한 결과, W(wakefulness) 단계가 NREN(non REM) 2 단계, 3 단계, 4 단계에 비하여 높게 나타났으며, NREM 4 단계는 REM(rapid eye movement) 단계와 NREM 1단계에 비하여 낮게 나타났다. 또한 교감신경계/부교감신경계 활성도 비율의 수면단계에 따라 변화하는 양상은 W, REM, NREM 1, 2, 3, 4 단계의 순으로 단조 감소하였다.
The aim of this study was to determine the statistical significance of heart rate variability(HRV) between sleep stages, Apnea-hypopnea index(AHI) and age in patients with obstructive sleep apnea(OSA). This study evaluated the main parameters of HRV over time domain and frequency domain in 40 patients with sleep apnea. The non-REM(sleep stage) was statistically validated by comparing the AHI degree of the three groups(mild, moderate, severe) of sleep apnea patients. The NN50(p=0.043), pNN50(p=0.044), VLF peak(p=0.022), LF/HF(p=0.028) were statistically significant in the R-R interval of patients with sleep apnea from the control group (p<0.05). The LF / HF (p = 0.045) and HF power (p = 0.0395) parameters between the non-RAM sleep (sleep 2 phase) and REM sleep in patients with sleep apnea were statistically significant in the control group(p<0.05). We may be able to provide a basis for understanding the correlation among AHI, sleep stage and age and heart rate variability in patients with obstructive sleep apnea.
Rem Koolhaas has pursued new architectural approaches breaking with conventional ones. Around the 1990s when large-scale projects occurred with the union of Europe ahead, Koolhaas recognized the limits to the existing methods for responding to such changes. Accordingly, he came to use design methods based on 'congestion' and 'void' as strategical alternatives, which became the moment for him to leap forward from the previous working sphere based in Europe to becoming an architect who would be commissioned a number of large-scale global projects. Therefore, this study intends to investigate his design methods which utilized congestion and void, and to derive spatial characteristics from the projects based on such methods. First of all, the study looked into the historical background, definition and process of congestion and the void as design methods, and analyzed his projects to which such methods were applied by classifying them into the following categories: 1) the void that removes a space of singularity; 2) the void that penetrates space while making a flow; and 3) the void that is formed by vertical extrusion. Then, the characteristics of architectural spaces made in this way were identified as 1)the single-body appearance made by congestion and the following types of space made by the void: 2) the non-uniformly shaped space that looks like floating; 3) the flexible space with various flows and directions; and 4) the space with virtual possibilities that embrace contingent events. This understanding of Rem Koolhaas's design methods which were attempted in various ways at his critical turning point will be the foundation to understand the overall world of his works.
Objectives: This study was performed to review the research trends in treatment for REM sleep behavior disorder (RBD) in Traditional Chinese Medicine (TCM) and Kampo in Japan. Methods: We searched articles in CNKI (China National Knowledge Infrastructure) under the key words, "RBD", and Chinese words related with it in Traditional Chinese Medicine, Traditional Chinese Medicinal Herbs and Combination of Traditional Chinese Medicine With Western Medicine' field, and also in CiNii (Citation Information by NII); we also searched articles in Kampo Square in Japan under the key words, "RBD" and Japanese words related with it. We found 10 papers, and then selected 6 of them except the non-clinical and unrelated studies. We then analyzed their way of diagnosis, treatments, study type and etc.. Results: 6 studies were divided into 4 case reports, one control study, and one literature review study. All of the studies reported that Herbal medicine for RBD was effective as much as Western medicine like clonazepam and paroxetine. However, the quality and the quantity of these clinical studies were not enough. Conclusions: It seems that the researches for RBD have gradually been performed in TCM and Kampo. We hope that our study can activate/push forward clinical research for this disorder in Korean traditional medicine.
The semiotics philosopher Roland Barthes declared 'the death of the author'. This means a sense of skepticism about author-centered writing in literature, and that readers and critics must also change their reading attitudes. In architecture, too, author-centered design appears. Architects sometimes try to express their will and concepts strongly through their work. However, this architecture is oriented toward identity, self-revealing, self-centered, and eventually without inspiration. On the other hand, the signifier does not result in a single meaning, and it is architecture as text that allows readers to enjoy the pleasure of deciphering. The purpose of this study is to investigate that unilateral by the will of the architect. Or whether there is a potential for non-intelligence and the possibility of generating meaning. In particular, we will analyze the works of Rem Koolhaas, SANAA and Sou Fujimoto to see if they are free from the will of the architect and can obtain the user's true freedom. As a result, we want to reveal the potential of various interpretations by users in contemporary architecture.

Catathrenia is a rare sleep disease characterized by monotonous groaning sounds that appear to be related with prolonged expiration, commonly experienced during rapid eye movement (REM) sleep. Catathrenia is also known as nocturnal groaning or sleep-related groaning and is currently categorized as a sleep-related breathing disorder. We present a rare case of a 19-year-old male with nocturnal groaning during non-REM sleep. We suggest that if catathrenia is suspected, polysomnography should be utilized to differentiate it from various sleep disorders such as snoring, central sleep apnea, sleep talking, parasomnia, and sleep-related movement disorders.

Patients with obstructive sleep apnea (OSA) often have more aggravated symptoms in the supine position. We tried to investigate the clinical characteristics and the predictive factors for positional OSA. Polysomnographic data were reviewed for OSA patients (apnea hypopnea index, $AHI{\geq}5$) from April, 2008 to April, 2011 at the Asan Medical Center. Clinical data, comorbid medical condition data and questionnaires (SF-36, MFI-20, ESS, BDI, STAI) were assessed. All patients were classified into two groups: positional patients (PP) group and non-positional patients (NPP) group. PP was defined as a patient who had the AHI in the supine position was at least twice as high as that in the lateral position. The body position of patients was confirmed by sleep position sensor and video monitor. All patients had at least 30 minutes of positional and 30 minutes of non-positional sleep. We compared clinical, medical, polysomnographic data, and questionnaire results between two (PP and NPP) groups and investigated predictive factors for the PP group using binary logistic regression analysis. In total, 371 patients were investigated. 265 (71.4%) was categorized as PP group and 106 (28.5%) as NPP group. The mean age ($mean{\pm}SD$) was higher in the PP group ($52.4{\pm}9.8$) than in the NPP group ($49.5{\pm}11.9$) (p<0.05). Comparison of sleep parameters between the PP and the NPP group showed that the PP group had significantly lower BMI (PP: $26.1{\pm}3.2kg/m^2$; NPP: $27.8{\pm}4.3kg/m^2$, p<0.001), neck circumference (PP: $39.7{\pm}2.8cm$; NPP: $41.5{\pm}3.7cm$, p<0.001) and hypertension rate (PP: n=89/265 (33.5%); NPP: n=48/106 (45.2%), p=0.0240). In the PP group, the percentage of deep sleep (PP: $8.7{\pm}8.1%$; NPP: $5.6{\pm}7.0%$, P=0.001) and rapid eye movement (REM) (PP: $17.5{\pm}6.1%$; NPP: $14.0{\pm}6.9%$, p<0.001) were significantly higher whereas the percentage of light sleep (stage N1) was significantly lower than the NPP group (PP: $30.4{\pm}12.3$; NPP: $44.5{\pm}20.8%$, p<0.001). During the sleep, the AHI in the supine position (PP: $48.6{\pm}19.5$; NPP: $60.5{\pm}22.6$, p<0.001) and in the non-supine position (PP: $9.4{\pm}8.9$; NPP: $48.4{\pm}24.8$, p=<0.001) were significantly lower and the minimal arterial oxygen saturation in non-REM sleep was significantly higher in the PP group (PP: $80.3{\pm}7.6$; NPP: $75.1{\pm}9.9$, p=<0.001). There were no significant differences in all questionnaires including quality of life. The results of the binary logistic regression analysis showed that age, the amount of REM sleep(%) and AHI were significant predictive factors for positional OSA. The significant predictive factors for positional OSA were older age, higher percentage of REM and lower AHI. The questionnaire results were not significantly different between the two groups.

Objectives: The purpose of this study is to investigate the effect of exercise load on sleep structure and stress hormone secretion during sleep. Methods: Five male physical education students were included in this study after giving their written, informed consents in the Research Institute for Sports Science at the University of Hanyang. All subjects have performed for at least 3 years in a regular aerobic exercises such as football, basketball, and running. The subjects were divided into three groups ; NOE(non-exercise), MDE(middle duration exercise), LDE(long duration excercise). MDE group maintained a total of 120 min exercise, and LDE group maintained a total of 300 min exercise by football, basketball or badminton. All subjects were acclimatized to the experimental sleep condition by spending one night under expermental conditions, including the placement of an intravenous catheter. During the subsequent night(24:00-08:00), somnopolygraphic sleep recordings were obtained, and blood for measuring growth hormone, cortisol, testosterone, and $\beta$-endorphin was collected every 120 min throughout the night. Blood samples were obtained from prominent forearm veins of subjects. Then, the samples were immediately placed in ice and centrifuged within 10 min at 3000 rpm at $4^{\circ}C$. Statistical analyses were performed using the SPSS/$PC^+$. Data were analyzed by one-way ANOVA with repeated measures. Results: No significant differences among groups were observed in sleep latency, total sleep time, stage 2 sleep, and slow wave sleep. However, daytime exercise produced significant changes in stage 1 sleep, REM sleep, stage 2 sleep latency, REM sleep latency and sleep efficiency. Stage 1 sleep, stage 2 sleep latency, and REM sleep latency significantly increased in LDE compared to those of NOE and MDE groups. But the amount of REM sleep significantly decreased in LDE. Sleep efficiency of MDE was higher than those of NOE and LDE. The blood concentrations of growth hormone, testosterone, and cortisol during night sleep were significantly lower in LDE than in NOE. $\beta$-endorphin concentrations in blood during night sleep were not different among groups. Conclusion: The daytime exercise load was significantly related to sleep structure and stress hormone secretion during night sleep. Long duration exercise showed a harmful effect on sleep structure and hormone secretion. However, middle duration exercise had a beneficial effect on sleep structure and hormone secretion during sleep.
