Improving Combination Cancer Therapy by Acetaminophen and Romidepsin in Non-small Cell Lung Cancer Cells

  • Combination chemotherapy is more effective than mono-chemotherapy and is widely used in clinical practice for enhanced cancer treatment. In this study, we investigated the potential synergistic effects of acetaminophen, a common component in many cold medicines, and romidepsin, a histone deacetylase (HDAC) inhibitor, in the A549 non-small cell lung cancer (NSCLC) cell line. The combination of acetaminophen and romidepsin also exerted significant cytotoxicity and apoptosis induced by activation of caspase-3 on tumor cells in vitro. Moreover, combination therapy significantly induced increased production of chemokines that stimulate migration of activated T-cells into tumor cells. This mechanism can lead to active T-cell mediated anti-tumor immunity in addition to the direct cytotoxic chemotherapeutic effect. Activated T-cells led to enhanced cytotoxicity in drug-treated A549 cells through interaction with tumor cells. These results suggested that the interaction between the two drugs is synergistic and significant. In conclusion, our data showed that the use of romidepsin and low concentrations acetaminophen could induce effective anti-tumor effects via enhanced tumor immune and direct cytotoxic chemotherapeutic responses. The combination of acetaminophen with romidepsin should be considered as a promising strategy for the treatment of lung cancer.

비소세포성폐암 환자의 $Monototal^{TM}$ 유용성 평가 (Evaluation of Usefulness for $Monototal^{TM}$ in Non Small Cell Lung Cancer)

  • 목적 : 2008년도 국내에서의 암 사망원인 1위가 폐암이며, 폐암으로 인한 사망자수는 매년 증가 하고 있다. 이 폐암은 대부분이 진행된 상태로 발견되면 이들 중 80%는 비소세포 성폐암(NSCLC)이다. 종양표지자 검사는 질병의 진행과 관련된 신뢰성 있는 결과를 신속히 의료진에게 제공해야 하는데 $Monototal^{TM}$ 검사는 반응시간이 짧으며, 비소세포성폐암 특히, Squamous cell(편평상피세포)에 대한 예민도가 높은 검사로 알려져 있다. 이에 비소세포성폐암의 진단 및 치료경과 관리를 위한 새로운 종양표지자인 $Monototal^{TM}$의 임상적 유용성에 대해 알아보고자 한다. 실험재료 및 방법 : 2009년 3월에서 4월까지 본원에 의뢰된 비소세포성폐암 환자 검체중 CEA가 양성인 검체 48건과 SCC 양성인 검체 15건, CYFRA 21-1 양성 검체 40건과 음성검체 20건을 대상으로 $Monototal^{TM}$을 측정하여 결과를 비교 하였다. 결과 : 비소세포성폐암 진단받은 환자 검체의 87.5%가 $Monototal^{TM}$ 검사에서 양성을 보였다. SCC 양성인 검체에서는 93.3%양성을 보였다. CYFRA 21-1 양성 검체에서는 100%의 양성을 보였고 음성인 검체에서도 50%의 양성을 보였다. 결론 : 종양표지자는 임상적으로 치료 후 환자의 경과 관찰에 유용하게 사용 되므로 중요하다. 실험 결과 비소세포성폐암에서 $Monototal^{TM}$의 양성률이 높게 보고 되었다. $Monototal^{TM}$의 임상적 가치를 평가 하기 위해서는 좀더 많은 검사가 필요하다고 생각한다. 그러나 CYFRA 21-1 보다 짧은 시간에 결과를 보고 할 수 있고 높은 양성률을 보인 것으로 보아 임상적으로 의의가 있다고 판단된다. 따라서 다른 종양표지자와 병행해서 검사하면 폐암의 진단과 예후 추정에 있어서 임상적으로 유용하다고 생각된다.

비소세포폐암 환자에시 Ki-67 단백질 발현증가의 임상적 의의 (Clinical Significance of Increased Ki-67 Protein Expression in Non-small Cell Lung Cancers)

  • 배경: Ki-67 단백질은 세포의 증식활성도를 나타내는 생물표식자로, 비소세포폐암 환자에서 Ki-67 단백질의 증가는 예후에 나쁜 영향을 미치는 것으로 알려져 있다. 이 연구는 비소세포폐암으로 폐절제술을 실시한 환자에서 Ki-67 단백질의 발현정도를 조사하여, 단백질의 발현증가가 환자의 임상적 병리적 양상과 술 후 재발과 생존기간에 미치는 영향을 알아보기 위해 시행되었다. 대상 및 방법: 근치적 폐절제술을 실시한 38명의 비소세포폐암 조직에서 단클론항체 Ki-67로 면역조직화학염색을 실시하여 Ki-67 Labeling Index (LI)를 구하였다. 환자를 Ki-67 증가군$(LI{\ge}20%)$과 Ki-67 비증가군(LI<20%)으로 분류하여, 두 군의 술 전 임상적 병리적 특성, 술 후 생존기간 및 무병생존기간을 비교하였다. 결과: Ki-67 LI는 불균질한 분포를 보였고 평균 LI는 $20.0{\pm}20.1%$였다. Ki-67 증가군과 비증가군 간에나이, 성별, 흡연, TNM 병기, 혈관침윤은 유의한 차이가 없었다. 그러나 증가군은 비증가군에 비해 편평상피암이 많고, 분화도가 나쁘며, 임파침윤이 많았다$(p{\le}0.05)$. 증가군은 중앙 생존기간(47.2 vs. 96.5개월)과 중앙 무병생존기간(18.2 vs. 72.3개월)이 비증가군보다 짧았으나 통계적 유의성은 없었다(각각 p=0.312, p=0.327). 결론: 이상의 연구를 통해 비소세로폐암 환자에서의 Ki-67 단백질 발현증가는 수술 후 환자의 예후에 나쁜 인자로 작용하여 생존기간과 무병생존기간이 짧아지는 경향을 보였으나 통계적 유의성이 부족하여 향후 지속적인 연구가 필요할 것이다.

Prognostic Significance of 14-3-3γ Overexpression in Advanced Non-Small Cell Lung Cancer

  • The 14-3-3 protein has been shown to be involved in the cancer process. However, there is no understanding of the relationship between 14-3-$3{\gamma}$ (14-3-3 gamma) expression and prognosis in advanced non-small cell lung cancer. In this study, we therefore investigated the association between protein levels by immunohistochemistry and clinicopathological features of advanced NSCLC patients. Survival curves were estimated using the Kaplan-Meier method and tested by log-rank. Multivariate analysis was conducted with the Cox's regression model to determine independence of factors. p values less than 0.05 were considered significant. A total 153 patients were studied, with 54.3% being stage III and 45.8% stage IV. Fifty-one cases (33.3%) were squamous cell carcinomas, and 98 cases (64.1%) were adenocarcinomas. High 14-3-$3{\gamma}$ expression was seen in 59.5% and significantly correlated with lymph node metastasis (p=0.010) and distant metastasis (p=0.017). On Kaplan-Meier analysis, high 14-3-$3{\gamma}$ expression was associated with poorer survival with a marginal trend toward significance (p=0.055). On multivariate analysis, age, treatment, and 14-3-$3{\gamma}$ expression proved to be independent prognostic parameters. In vitro experiments indicated that 14-3-$3{\gamma}$ overexpression also played a potential role in cancer invasion. In conclusion, our data suggest that 14-3-$3{\gamma}$ overexpression is associated with invasion and a poor prognosis. Therefore, 14-3-$3{\gamma}$ may be a potential prognostic marker of advanced non-small cell lung cancer.

Waldeyer's Ring 비호치킨 림프종의 병합요법 (Combined Modality Therapy of Non-Hodgkin's Lymphoma of Waldeyer's Ring)

  • Purpose: We performed this study retrospectively to evaluate local control, survival, prognostic factors, and failure patterns in patients with non-Hodgkin's lymphoma of Waldeyer's ring. Materials and Methods: From April 1984 to November 1996,41 patients with non-Hodgkin's lymphoma of Waldeyer's ring were treated with combined chemotherapy and radiation therapy. Age was ranged from 19 to 73 years old with a median age of 55 years, and there were 26 male and 15 female patients. Primary site was tonsil in 26 and base of the tongue in 7 and nasopharynx in 8, and stage distribution showed stage I in 12 and stage II in 29 patients. Pathologic classification was done according to Working Formulation. There were 1 with follicular mixed small cleaved and large cell, 8 with diffuse small cleaved cell, 7 with diffuse mixed small and large cell, and 25 cases with diffuse large cell. All patients were treated with combination of chemotherapy and radiation therapy. Chemotherapy regimen consisted of either CHOP-Bleo(cyclophosphamide, adriamycin, vincristine, prednisolone, bleomycin) or COP-BLAM III(cyclophosphamide, vincristine, prednisolone, bleomycin, adriamycin, procarbazine). Radiation dose ranged from 3600cGy to 6620cGy with a median dose of 5040cGy. Follow-up time was ranged from 15 months to 159 months(median 55 months). Results: The complete response was achieved in 98%(40/41) and partial response in 2%(1/41). The complete response rate were the followings: 66.7% for stage I and 51.7% for stage II after chemotherapy, 100% for stage I and 96.6% for stage II after overall treatment respectively. The overall survival rate and disease-tree survival rates at 5 years were 82.6% and 79.5%, respectively. Prognostic factors for overall survival were age(p=0.007), stage(p=0.03), nodal status(p=0.006) and radiation dose(p=0.003). The factors associated with disease-tree survival were stage(p=0.04), nodal status(p=0.004) and radiation dose(p=0.009). The failure patterns were analized in evaluable 35 patients with complete response. Locoregional failure was noted in 2 patients and distant metastasis in 5 patients. Conclusion: Our results suggest that combined modality therapy is the appropriate treatment for stage I-II intermediate grade non-hodgkin's lymphoma of the Waldeyer's ring. However, our material is small and the analysis is retrospective. Randomized prospective studies for combined therapy, radiation therapy alone and chemotherapy alone are needed.

Druggable Targets of Squamous Cell Lung Cancer

  • Knowledge of molecular pathogenesis of non-small cell lung cancer has increased remarkably and changed the principles of treatment, especially during the past decade. These advancements have been limited mainly to adenocarcinoma of the lung. Recently, genetic alterations in squamous cell lung cancer (SQCLC) have been detailed and positive results of clinical trials using agents targeting these changes have indicated the potential for improved treatment outcomes for SQCLC.

비소세포 폐암의 방사선 치료 (Radiotherapy of Non-Small Cell Lung Cancer)

  • 131 Patients of non·small cell lung cancer treated with irradiation at Yonsei Canter Center from Jan. 1971 to Dec. 1980 were retrospectively analysed. Overall 5 year survival rate was $7\%$ in 117 cases, treated with radiotherapy alone and $33\%$ in 14 Cases, treated with surgery & postoperative ratiotherapy. Their median survival was 9.6 months in the former, while 11.1 months in the latter. The patients treated with radical aim achieved $10\%$ in 5 YSR and with palliative aim, $0\%$. Also, survival according to histolocial classification and staging was obtained. Treatment failure was mostly distant failure (40 cases/49 cases), and within 6 months (34cases/49cases).

A Case of Primary Pulmonary Lymphoepithelioma-like Carcinoma Misdiagnosed as Adenocarcinoma

  • Primary pulmonary lymphoepithelioma-like carcinoma (LELC) is rare, with a more favorable prognosis compared with that of other types of non-small cell lung cancers. Herein, we describe an interesting case of primary pulmonary LELC confirmed postoperatively, which had been initially diagnosed as poorly differentiated adenocarcinoma. We suggest that despite the rarity of pulmonary LELC, it should be included as one of the differential diagnoses for lung malignancies. Physicians should consider taking a larger biopsy, especially when histologic examination shows undifferentiated nature.

Video-assisted Thoracoscopic Surgery for Treatment of Earlystage Non-small Cell Lung Cancer

  • Objectives: To evaluate the safety, efficacy, and invasiveness of lobectomy by video-assisted thoracoscopic surgery (VATS) in the treatment of stage I/II non-small cell lung cancer (NSCLC). Methods: A total of 148 patients presenting with Stage I or II NSCLC were enrolled into our study, comprising 71 who underwent VATS and 77 patients undergoing conventional thoracotomic lobectomy, in combination with systematic lymph node resection. Results: It was found that VATS was superior to conventional thoracotomy in terms of the duration of surgery, intraoperative blood loss, frequency of the need to administer postoperative analgesia, thoracic intubation indwelling time, post-operative hospital stay, and survival rate (P<0.05). We saw no obvious difference in the number of resected lymph nodes with either approach. Conclusions: VATS lobectomy is a safe and reliable surgical approach for the treatment of Stage I/II NSCLC, characterized by significantly minimal invasiveness, rapid post-operative recovery, and markedly lower loss of blood.

An Improved Convergent Approach for Synthesis of Erlotinib, a Tyrosine Kinase Inhibitor, via a Ring Closure Reaction of Phenyl Benzamidine Intermediate

  • An improved convergent and economical method has been developed for the synthesis of erlotinib, a 4-anilinoquinazoline and an EGFR-tyrosine kinase inhibitor for treatment of non-small-cell lung cancer. The final two steps for the formation of this 4-anilinoquinazoline from suitable 2-aminobenzonitrile intermediate and 3-ethynylaniline were modified and were performed in a simple one-pot reaction. The ring-closing mechanism for the formation of erlotinib from the suitable formamidine intermediate and 3-ethynylaniline was investigated and determined to proceed via the formation of phenyl benzamidine intermediate rather than involving Dimroth rearrangement reported earlier. The new benzamidine intermediate was isolated for the first time and characterized.
