The present condition analysis of patients who transferred to the emergency room by 119 Rescue service at night - Focused on the Emergency Center of Chonnam National University Hospital - (119 구급대를 이용하여 야간에 응급실로 내원한 환자 현황 분석 - 전남대학교병원 응급의료센터를 중심으로 -)

  • In general, the patients who transferred to the hospital by 119 rescue service at night go to the emergency room(ER) of general hospital rather than that of their nearby the 1st(clinic) and 2nd(local hospital) hospital. And the hospital is mainly selected not by 119 EMST but by patients or his/her conservators. Therefore we had studied retrospectively with emergency situation diary and medical chart for 697 patients of being transferred to one emergency medical center for 6 months since January 2004, and results are as follows. 1. The 280 patients(42.5%) of being transferred hospital at night by 119 rescue service were not emergency case and their average staying time in hospital was about $7.15{\pm}10.06$ minutes. 2. Transfer time was distributed in each time intervals of 1819, 2021, 2223, and 2401 and patients ratio in each intervals were 15.1%, 17.8%, 16.4%, and 15.2%. 3. In response of ambulance, the average time from the spot to the hospital was $14.53{\pm}9.27min$. and average distance of that was $7.95{\pm}9.21km$. 4. Diseases rather than traffic accidents or traumatic injury were main causes of ambulance calling and its value was 533(76.5%), and accidents were mainly occurred in patients' house and its value was 479(68.7%). 5. In time of transfer by 119 rescue service, hospital was mainly selected by patient/conservator and its value was 648(93.0%). In result. the hospital was selected not by EMST but by patient/conservator. 6. The case that the 1st grade EMT was rode in ambulance was 161(23.1%), and the case that 2nd EMT and emergency team member who educated for emergency were rode in ambulance were 504(72.3%). So the number of the 1st grade EMT was short in fire station of Kwangju metropolitan city than other city. 7. The first aids for patients before reaching hospital were limited to oxygen inhalation, airway control, and BLS for maintaining limbs and spine. So it seems to be a simple patients transfer. Consequently, to establish an efficient emergency medical system, it has been thought that it should be advanced a moderate education and public information about the appropriate use of emergency medical system toward citizen, and also need the hospital selection by the patients categorizing standards for 119 rescue service member, securing the 1st grade EMT, appropriate first-aids education, and securing professional human power in emergency room of the Ist(clinic) and 2nd(local hospital) hospital at night.

Clinical Characteristics of Night Eating Syndrome in Outpatients with Bipolar Disorder (양극성 장애 외래환자에서 야간식이증후군의 임상적 특성)

  • Objectives : The aim of this study is to evaluate the clinical characteristics of night eating syndrome(NES) in bipolar disorder outpatients. Methods : The 14 items of self-reported night eating questionnaire(NEQ) was administered to 84 bipolar patients in psychiatric outpatient clinic. We examined demographic and clinical characteristics, body mass index(BMI), subjective measures of mood, sleep, binge eating & weight-related quality of life using Beck's Depression Inventory (BDI), Pittsburgh Sleep Quality Index(PSQI), Binge Eating Scale(BES) and Korean version of Obesity-Related Quality of Life Scale(KOQoL), respectively. Results : The prevalence of night eating syndrome in bipolar outpatients was 14.3%(12 of 84). Comparisons between NES group and non-NES group revealed no significant differences in demographic characteristics, BMI and clinical status except economic status and comorbid medical illnesses. However, compared to non-NES, patients with NES was more likely to have binge eating pattern and poorer weight-related quality of life. Conclusions : This study is to be the first to describe the clinical correlates of night eaters in bipolar outpatients. Although there were few significant correlates of NES in bipolar outpatients, relatively high prevalence of NES suggest that clinicians should be aware to assess the patients with bipolar disorder on NES, regardless of obesity status of patients.

A Conceptual study on Shanghanlun Jueyinbing :Based on 2 cases treated by Shanghanlun provisions (『상한론(傷寒論)』 궐음병(厥隂病)에 대한 고찰: 변병진단체계(辨病診斷體系)에 입각해 치료한 증례 2례에 근거하여)

  • Objective : This study is to propose a new hypothesis about the interpretation of Jueyinbing in Shanghanlun by studying 2 Jueyinbing cases. Methods : We analyzed the original form of the chinese characters 'Jue(厥)', 'Jueyin(厥陰)' and 'Huoluan(?亂)' and tried to understand their contexts in Shanghanlun. At the same time, we analyzed 2 clinical cases diagnosed and treated as Jueyinbing according to newly hypothesized definition. Results : Both patients suffered from eye diseases which caused by concentrating on a sophisticated work by straining their hands, feet and chest during the night time. After the herbal intervention, their eye diseases are improved firstly. The National Eye Institute 25-item Visual Function Questionnaire (NEI-VFQ) score increased on both patients. Not only all the patients' chief complaints relieved but their other clinical problems such as limb pain, hand tremble also improved. Conclusions : These results highly insist that, at least in 15 characters provision of Shanghanlun, Jueyinbing had had the different usage from the conventional terminology, and it is assumed that Jueyinbing means the disease made by concentrating on a sophisticated work by straining hands, feet and chest during night time.

A Case Report of Chronic Fatigue Syndrome treated by Jisilchija-tang based on Shanghanlun Provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 지실치자탕(枳實梔子湯) 투여 후 호전된 만성피로증후군 1례)

  • Objective : The purpose of this paper is to report the improvement of patient with Chronic Fatigue Syndrome(CFS) treated by herb medication based on Shanghanlun disease pattern identification diagnostic system. Methods : According to 'Disease Pattern Identification Diagnostic System based on Shanghanlun Provisions', the patient was diagnosed with Eumyangyeokchahunobok-byung, number 393 provision, and took Jisilchija-tang herb medication for 75days. The changing symptom of chronic fatigue syndrome was estimated by QOL-CFS(Chalder Fatigue Scale) and VAS(Visual Analogue Scale). Results : The QOL-CFS changed 21 to 8 and VAS changed 10 to 1. Conclusions : It is existing theory that Eumyangyeokchahunobok-byung is caused by 'having too much sex relation'. But the author diagnosed Eumyangyeokchahunobok-byung as 'Symptoms worsen when awaken at night, and sleep at day caused by poor sleep pattern' according to the palaeography about shanghanlun, and got a good results.

A Radial Nerve Palsy Cases Managed by Yinyang Balance Appliance of FCST, a TMJ Therapy for the Balance of Meridian and Neurological System (FCST 음양균형장치를 활용한 요골신경마비치료의 증례보고)

  • Therapeutic effect of Yinyang Balancing Appliance of functional cerebrospinal therapy (FCST) for meridian and neurologic yinyang balance was observed in a radial nerve palsy case. One Radial nerve palsy case was managed with the Yinyang Balancing Appliance on temporomandibular joint (TMJ), combined with acupuncture. Clinical outcome measurement was based on subjective measures and clinical observations. The patient showed positive changes even after the initial treatment and this effect maintained over the follow-up period. further clinical and biological research on FCST is expected.

Insomnia Treated with Baekho-tang Based on the Shanghanlun Disease Pattern Identification Diagnostic System (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 백호탕(白虎湯) 투여 후 호전된 불면증 증례 1례)

  • Objective: This study aimed to report the improvement of a patient with chronic insomnia who was treated with Baekho-tang based on the Shanghanlun disease pattern identification diagnostic system (DPIDS). Methods: The patient's symptoms were checked on days 19, 37, and 52 after taking Baekho-tang. Chronic insomnia was evaluated by the degree of insomnia, hours spent asleep, and the number of times the patient woke up during the night, which were all assessed by a visual analog scale (VAS) with the Leeds Sleep Evaluation Questionnaire. In addition, new clinical implications for the interpretation of the 350th Shanghanlun provision were reviewed. Results: After administering Baekho-tang for 52 days, based on the 350th Shanghanlun provision according to the DPIDS, the VAS score decreased from 10 to 1. Conclusions: The patient recovered from chronic insomnia. This case report suggests that the words "滑," "厥," and "裏" in the 350th provision of Shanghanlun mean physical and psychological causes of insomnia. In addition, Baekho-tang had a therapeutic effect in improving the chief complaint of the patient. Therefore, the interpretation of the 350th Shanghanlun provision requires a new definition that includes the patient's lifestyle, family conflict, and past history of the patient as a cause of chronic insomnia.

A survey on patient preferences who had visited Dept. of Ophthalmology, Otolaryngology & Dermatology Hospital of Dong Shin University (동신대 목동한방병원 안이비인후피부과 내원환자의 성향에 관한 설문조사)

  • Objective : The purpose of this study is to understand preferences of patients who had visited the Dept. of Ophthalmology Otolaryngology & Dermatology Method : We conducted questionnaire survey on the patients who had visited the Dept. of Ophthalmology, Otolaryngology & Dermatology Hospital of Dong Shin University from March to September, 2007. Result : The result were as follows. 1. Distribution of sex: male 41 % female 59% 2. The age distribution of patients showed highest in 30-39 years(23%), followed by 20-29 years, 40-49 years in order. 3. The classification of patients according to disease: Skin disease (38%), Nose (23%), Face (10%), Eye (9%), Throat (8%), Ear (7%) 4. Other offices patients visited before coming to this clinic: Local clinic (41%). Hospital (18%), Oriental clinic or hospital (5%), None (22%) 5. The length of time the patient has had the disease: 2years (26%), more than 3years (15%), 2-4weeks (13%), 6months (11%), 1year (10%) in order 6. The expected length of treatment: 1week (21%), 2week (12%), 1month (21%) 7. 26% expected about 100,000 won as medical expenses. 8. 88% of the patients are willing to have a check-up when it needed and 40% expected about 50,000 won as check-up expenses. 9. 52% of the patients are not willing to withstand pain, and would rather heal slowly. 10. 69% of the patients said that treatment would be affected by company(44%), school(19%) and business(16%). 11. 73% of the respondants are positive about night-time treatment and 92% is positeve about making an appointment for the treatment. Conclusion : Through the research, we could understand preferences of patients who had visited Dept. of Ophthalmology, Otolaryngology & Dermatology

A Case of Osteoid Osteoma Diagnosed during Treatment of Herniated Nucleus Pulposus (요추 추간판 탈출증의 통증치료시 발견된 유골골종 -증례 보고-)

  • We experienced a case of osteoid osteoma in thoracic vertebra accompanied with herniated nucleus pulposus during the management of back pain. A 32 year old male patient with herniated nucleus pulposus complained of back pain and radiation to right leg. Lumbar epidural block with 1% mepivacaine 5 ml was performed for pain control and it relieved the radiating pain. However patient continued to experience severe exacerated back pain at night which responded to aspirin. Therefore we performed further examination for existence of disease of the spine and diagnosed osteoid osteoma in the right pedicle of T12 vertebra. In conclusion, we recommend physicians when evaluating patients with back pain to be congnizant of possible existence of neoplastic disease of the spine and incorporate it in differential diagnosis.

Body Discomfort and CTS Survey for Korean Dentists in Accordance with Clinic Activity (치과의사의 진료활동에 따른 신체불편도 및 손목관 증후군 발생현황 조사)

  • The purpose of this study is to investigate the physical disorders of CTDs prevalent among dentists who are exposed to job risk factors. The average total work time per week of subjects(N=117) was 44$\pm$6 hours, work time with one patient was 20$\pm$10 minutes, and their daily tasks were consisted of extraction, filling, preparation, nerve amputation of tooth. The results of this study are as follows: 1) The discomfort test shows that dentists suffer from discomfort/pain in upper extremities. 2) The work environment test related to CTDs shows that female dentists feel more uncomfortable in the height of chairs than male dentists. 3) The symptom test shows that female dentists suffer from more pain in fingers or wrists at night than male dentists.

  • Review is made on the sweat in 'Donguibogam' from the view point of Hyungsang medicine. The conclusions are as follows: Clinically sweet is an important indicator for the condition and constitution of the patients. Sweat should be taken into consideration in diagnosis and treatment. Sweat is one of the symptoms that appears when there is a disharmony among Jung(精), Ki(氣) and Shin(神). The cause and phase of sweat varies with every patient. Dam type persons (膽體) are inclined to have night sweat, while Bangkwang type persons(膀胱體) are apt to have day sweat.
