수종의 엔진구동형 Nickel-Titanium file을 이용한 근관형성 방법이 근관만곡도 유지능력에 미치는 영향 (EFFECT OF VARIOUS CANAL PREPARATION TECHNIQUES USING ROTARY NICKEL-TITANIUM FILES ON THE MAINTENANCE OF CANAL CURVATURE)

  • There are increasing usage of Nickel-Titanium rotary files in modern clinical endodontic treatment because it is effective and faster than hand filing due to reduced step. This study was conducted to evaluate the effect of canal preparations using 3 different rotary Nickel-Titanium files that has different cross sectional shape and taper on the maintenance of canal curvature. Simulated resin block were instrumented with Profile(Dentsply, USA), GT rotary files(Dentsply, USA), Hero 642(Micro-Mega France), and Pro-Taper(Dentsply, USA). The image of Pre-instrumentation and Post-instrumentation were acquired using digital camera and overspreaded in the computer. Then the total differences of canal diameter, deviation at the outer portion of curvature, deviation at the inner portion of curvature, movement of center of the canal and the centering ratio at the pre-determined level from the apex were measured. Results were statistically analyzed by means of ANOVA, followed by Scheffe test at a significance level of 0.05. The results were as follows; 1. Deviation at the outer portion of curvature, deviation at the inner portion of curvature were showed largest in Pro-Taper so also did in the total differences of canal diameter(p<0.05). 2. All the groups showed movements of center Profile combined with GT rotary files and Hero 642 has no difference but Pro-Taper showed the most deviation(p<0.05). 3. At the 1, 2, 3mm level from the apex movements of center directed toward the outer portion of curvature, but in 4, 5 mm level directed toward the inner portion of curvature(p<0.05). As a results of this study, it could be concluded that combined use of other Nickel-Titanium rotary files is strongly recommended when use Pro-Taper file because it could be remove too much canal structure and also made more deviation of canal curvature than others.

  • The purpose of this study was to quantify the amount of remaining gutta-percha/sealer on the walls of root canals when three types of nickel-titanium rotary instruments (Profile. ProTaper and $K^3$) and a hand instrument(Hedstrom file) used to remove these materials. The results of this study were as follows: 1. In the total time for gutta-percha removal. Profile group was the fastest and followed by $K^3$, Protaper, Hedstrom file group. 2. In case of the evaluation of the volume of remained gutta-percha from radiograph. $K^3$ group got the highest score and followed by Protaper. Hedstrom file. Profile group in the apical 1/3. 3. In case of the evaluation of the volume of gutta-percha remained from stereomicroscope, $K^3$ group got the highest score and followed by Protaper. Hedstrom file. Profile group in the apical 1/3. These results showed that instrumentation using nickel-titanium rotary instrument groups was faster than that using hand instrument group. The effect of gutta-percha removal using Profile group was better than that using Protaper and $K^3$ group in the nickel-titanium rotary instrument groups.

모형 레진 근관에서 수종의 전동 니켈-티타늄 파일에 대한 screw-in effect 비교 (COMPARISON OF SCREW-IN EFFECT FOR SEVERAL NICKEL-TITANIUM ROTARY INSTRUMENTS IN SIMULATED RESIN ROOT CANAL)

  • Screw-in effect는 니켈-티타늄 전동 파일을 사용한 근관형성시 나타나는 현상으로 근관형성을 어렵게 한다. 이 연구의 목적은 다양한 니켈-티타늄 전동 파일들 사이의 screw-in effect를 비교하고자 하는 것이다. 본 연구에서는 여섯 가지의 다른 니켈-티타늄 전동 파일 기구들, 즉, $K3^{TM}$ (SybronEndo, Glendora, CA USA), $M_{two}$(VDW GmbH, Munchen, Germany), 삭제능이 있는 첨단을 가진 것과 삭제능이 없는 첨단을 가진 NRT (Mani Inc., Shioya-gun, Japan), ProFile$^{(R)}$ (Dentsply-Maillefer, Ballaigues, Switzerland), 그리고 ProTaper$^{(R)}$ (Dentsply-Maillefer, Ballaigues, Switzerland)가 사용되었다. 기구가 동일한 경사도와 크기를 가지게 하기 위해, 각 기구는 경사도 0.06, size 20을 선택하였으며, ProTaper$^{(R)}$의 경우에는 이와 유사한 S2를 사용하였다. 각 기구당 10개씩 총 60개의 투명레진 블록의 모조 단일 만곡근관(REF A0177, Dentsply-Maillefer, Ballaiguez, Switzerland)에서, 전동파일의 회전속도는 분당 300회전으로 하고, 단일 pecking 동작이 되게 하여 기구조작을 하였다. 장치를 고안하여 일정한 힘의 pecking 동작을 재현하고 screw-in effect의 힘을 측정하였다. 고안한 장치의 dynamometer가 근관형성 과정동안 screw-in force를 측정하였고, 기록된 data는 고안된 소프트웨어를 이용하여 컴퓨터에 저장되었다. 데이터는 one-way ANOVA로 통계처리를 하였고, Tukey's multiple range test를 사용하여 95% 수준에서 유의성을 검정하였다. ProTaper$^{(R)}$가 가장 큰 screw-in effect를 나타내었다(p < 0.001). $K3^{TM}$$M_{two}$와 ProFile$^{(R)}$ 보다 큰 screw-in effect를 나타내었다(p < 0.001). 그러나 $M_{two}$, NRT와 ProFile$^{(R)}$ 사이에서는 유의한 차이가 나타나지 않았고(p > 0.05), 삭제능이 있는 첨단을 가진 NRT와 삭제능을 가지지 않는 첨단을 가진 NRT 사이에도 유의한 차이를 나타내지 않았다. 이상의 연구결과를 통해 볼 때, 실험에 사용된 수종의 Ni-Ti 전동파일들 사이에서 screw-in effect의 차이가 나타날 것으로 보이며. 특히, Ni-Ti 전동파일의 radial lands와 rake angle이 screw-in effect의 차이를 나타낼 수 있을 것으로 생각된다.

네 가지 전동 Ni-Ti 파일의 danger zone에서의 근관성형력 (Shaping Ability of Four Rotary Nickel-Titanium Instruments to Prepare Root Canal at Danger Zone)

  • 하악 대구치 근심치근의 danger zone에서 수종의 전동 니켈-티타늄 파일의 근관성형력을 근관성형 전후 치질 두께의 변화를 측정하여 평가하고자 하였다. 기구에 따라 총 40개의 하악 대구치를 10개씩 $Profile^{\circledR}$, GT Rotary file, Quantec 및 ProTaper 4개 군으로 나누고 각 치아당 2개의 근관을 straight up-and-down과 anticurvature 군으로 나누어 근단부 근관을 모두 30번 크기로 일정하게 확대하였다. 수정된 Bramante법을 사용하였으며 술전 및 술후의 근관 상아질 두께를 치수저 하방 1, 3 및 5 mm지점에서 측정, 이원변량분석법으로 통계분석하였다. 모든 군의 danger zone과 safe zone에서의 straight up-and-down 동작과 anticurvature 동작 사이에는 치근상아질 두께변화에 현저한 차이를 나타내지 않았다 (p > 0.05). ProTaper는 danger zone과 safe zone 모두에서 다른 기구에 비해 많은 량의 근관상아질 삭제를 보였으며 특히 분지부 3 mm 수준에서 현저하였다(p < 0.05).

Glide path 형성용 니켈티타늄 회전 파일의 효율: in-vitro 예비 연구 (Efficiency of the nickel-titanium rotary instruments for glide path preparation: in-vitro preliminary study)

  • Objectives: This preliminary study compared the effects of glide path establishing instruments prior to substantial root canal preparation. Materials and Methods: Glide path was established by enlargement of the 2nd mesiobuccal root canal of Dentalike by using three kinds of glide path preparation nickel-titanium file; PathFile, One G and ProGlider. The pre- and post-instrumented Dentalikes were weighed in the resolution of 1 / 10mg. In addition, after glide path preparation, torque generated during shaping using the WavoOne file was measured. The data were analyzed by one-way ANOVA and Tukey post-hoc test at a significance level of 95%. Results: The ProGlider had the significantly larger amount of reduced weight than other instrument groups (p < 0.05). There was no significant difference between group of glide path preparation with ProGlider and without glide path preparation in maximum torque and total stress generation during the shaping with WaveOne. Conclusions: Glide path preparation instruments may have different efficiency according to their geometries. The Dentalike artificial teeth were revealed to have discrepancies in the size of root canals by microCT examination. It is impossible to make a meaningful judgment of the results due to the reliability or resolution problem of the root canal size of the artificial tooth selected as the standardized tooth.

니켈티타늄 전동 파일의 기계적 특성 비교: Aurum Blue vs. Aurum Pro (Comparison of mechanical properties of nickel-titanium rotary files: Aurum Blue vs. Aurum Pro)

  • AIM: The purpose of this study was to evaluate and compare the torsional fracture resistances, cyclic fatigue resistance, and bending stiffness of two nickel-titanium (NiTi) rotary instruments made of different heat-treated alloy: Aurum Blue (heat-treated) and Aurum Pro (conventional). Methods: Forty-five Aurum Blue and Aurum Pro NiTi files were selected for the three mechanical tests (n=15). For the torsional resistance test, 3 mm file tip was fixed and the shaft was driven clockwise at 2 rpm until fracture occurred by using a customized device. Cyclic fatigue resistance was evaluated by rotating instruments in artificial canal with dynamic mode. Bending stiffness was tested by observation of the bending moment on attaining a 45° bend. The results were analyzed by student-t tests at a significance level of 95%. The fractured surface of each groups were examined under a scanning electron microscope (SEM). Results: Aurum Blue showed significantly higher toughness, ultimate strength, distortion angle, and number of cycles to failure than those of Aurum Pro (p < 0.05). However, Aurum Blue and Aurum Pro did not differ significantly in terms of bending stiffness. SEM showed typical topographic appearances of the cyclic fatigue and torsional fracture. Conclusions: Under the limitations of this study, heat-treated instruments showed higher flexibility and fracture resistances than conventional NiTi instruments.

Screw-in forces during instrumentation by various file systems

  • Objectives: The purpose of this study was to compare the maximum screw-in forces generated during the movement of various Nickel-Titanium (NiTi) file systems. Materials and Methods: Forty simulated canals in resin blocks were randomly divided into 4 groups for the following instruments: Mtwo size 25/0.07 (MTW, VDW GmbH), Reciproc R25 (RPR, VDW GmbH), ProTaper Universal F2 (PTU, Dentsply Maillefer), and ProTaper Next X2 (PTN, Dentsply Maillefer, n = 10). All the artificial canals were prepared to obtain a standardized lumen by using ProTaper Universal F1. Screw-in forces were measured using a custom-made experimental device (AEndoS-k, DMJ system) during instrumentation with each NiTi file system using the designated movement. The rotation speed was set at 350 rpm with an automatic 4 mm pecking motion at a speed of 1 mm/sec. The pecking depth was increased by 1 mm for each pecking motion until the file reach the working length. Forces were recorded during file movement, and the maximum force was extracted from the data. Maximum screw-in forces were analyzed by one-way ANOVA and Tukey's post hoc comparison at a significance level of 95%. Results: Reciproc and ProTaper Universal files generated the highest maximum screw-in forces among all the instruments while M-two and ProTaper Next showed the lowest (p < 0.05). Conclusions: Geometrical differences rather than shaping motion and alloys may affect the screw-in force during canal instrumentation. To reduce screw-in forces, the use of NiTi files with smaller cross-sectional area for higher flexibility is recommended.

Preference of undergraduate students after first experience on nickel-titanium endodontic instruments

  • Objectives: This study aimed to compare two nickel-titanium systems (rotary vs. reciprocating) for their acceptance by undergraduate students who experienced nickel-titanium (NiTi) instruments for the first time. Materials and Methods: Eighty-one sophomore dental students were first taught on manual root canal preparation with stainless-steel files. After that, they were instructed on the use of ProTaper Universal system (PTU, Dentsply Maillefer), then the WaveOne (WO, Dentsply Maillefer). They practiced with each system on 2 extracted molars, before using those files to shape the buccal or mesial canals of additional first molars. A questionnaire was completed after using each file system, seeking students' perception about 'Ease of use', 'Flexibility', 'Cutting-efficiency', 'Screwing-effect', 'Feeling-safety', and 'Instrumentation-time' of the NiTi files, relative to stainless-steel instrumentation, on a 5-point Likert-type scale. They were also requested to indicate their preference between the two systems. Data was compared between groups using t-test, and with Chi-square test for correlation of each perception value with the preferred choice (p = 0.05). Results: Among the 81 students, 55 indicated their preferred file system as WO and 22 as PTU. All scores were greater than 4 (better) for both systems, compared with stainless-steel files, except for 'Screwing-effect' for PTU. The scores for WO in the categories of 'Flexibility', 'Screwing-effect', and 'Feeling-safety' were significantly higher scores than those of PTU. A significant association between the 'Screwing-effect' and students' preference for WO was observed. Conclusions: Novice operators preferred nickel-titanium instruments to stainless-steel, and majority of them opted for reciprocating file instead of continuous rotating system.

Effect of adaptive motion on cyclic fatigue resistance of a nickel titanium instrument designed for retreatment

  • Objectives: The aim of this study was to evaluate the cyclic fatigue resistance of the ProTaper Universal D1 file (Dentsply Maillefer) under continuous and adaptive motion. Materials and Methods: Forty ProTaper Universal D1 files were included in this study. The cyclic fatigue tests were performed using a dynamic cyclic fatigue testing device, which had an artificial stainless steel canal with a $60^{\circ}$ angle of curvature and a 5 mm radius of curvature. The files were randomly divided into two groups (Group 1, Rotary motion; Group 2, Adaptive motion). The time to failure of the files were recorded in seconds. The number of cycles to failure (NCF) was calculated for each group. The data were statistically analyzed using Student's t-test. The statistical significant level was set at p < 0.05. Results: The cyclic fatigue resistance of the adaptive motion group was significantly higher than the rotary motion group (p < 0.05). Conclusion: Within the limitations of the present study, the 'Adaptive motion' significantly increased the resistance of the ProTaper Universal D1 file to cyclic facture.

Influence of glide path on the screw-in effect and torque of nickel-titanium rotary files in simulated resin root canals

  • Objectives: The purpose of this study was to investigate the screw-in effect and torque generation depending on the size of glide path during root canal preparation. Materials and Methods: Forty Endo-Training Blocks (REF A 0177, Dentsply Maillefer) were used. They were divided into 4 groups. For groups 1, 2, 3, and 4, the glide path was established with ISO #13 Path File (Dentsply Maillefer), #15 NiTi K-file NITIFLEX (Dentsply Maillefer), modified #16 Path File (equivalent to #18), and #20 NiTi K-file NITIFLEX, respectively. The screw-in force and resultant torque were measured using a custom-made experimental apparatus while canals were instrumented with ProTaper S1 (Dentsply Maillefer) at a constant speed of 300 rpm with an automated pecking motion. A statistical analysis was performed using one-way analysis of variance and the Duncan post hoc comparison test. Results: Group 4 showed lowest screw-in effect ($2.796{\pm}0.134$) among the groups (p < 0.05). Torque was inversely proportional to the glide path of each group. In #20 glide path group, the screw-in effect and torque decreased at the last 1 mm from the apical terminus. However, in the other groups, the decrease of the screw-in effect and torque did not occur in the last 1 mm from the apical terminus. Conclusions: The establishment of a larger glide path before NiTi rotary instrumentation appears to be appropriate for safely shaping the canal. It is recommended to establish #20 glide path with NiTi file when using ProTaper NiTi rotary instruments system safely.
