Congenital heart disease in the newborn requiring early intervention

  • Although antenatal diagnostic technique has considerably improved, precise detection and proper management of the neonate with congenital heart disease (CHD) is always a great concern to pediatricians. Congenital cardiac malformations vary from benign to serious conditions such as complete transposition of the great arteries (TGA), critical pulmonary and aortic valvular stenosis/atresia, hypoplastic left heart syndrome (HLHS), obstructed total anomalous pulmonary venous return (TAPVR), which the baby needs immediate diagnosis and management for survival. Unfortunately, these life threatening heart diseases may not have obvious evidence early after birth, most of the clinical and physical findings are nonspecific and vague, which makes the diagnosis difficult. High index of suspicion and astute acumen are essential to decision making. When patent ductus arteriosus (PDA) is opened Widely, many serious malformations may not be noticed easily in the early life, but would progress as severe acidosis/shock/cyanosis or even death as PDA constricts after few hours to days. Ductus dependent congenital cardiac lesions can be divided into the ductus dependent systemic or pulmonary disease, but physiologically quite different from each other and treatment strategy has to be tailored to the clinical status and cardiac malformations. Inevitably early presentation is often regarded as a medical emergency. Differential diagnosis with inborn error metabolic disorders, neonatal sepsis, persistent pulmonary hypertension of the newborn (PPHN) and other pulmonary conditions are necessary. Urgent identification of the newborn at such high risk requires timely referral to a pediatric cardiologist, and timely intervention is the key in reducing mortality and morbidity. This following review deals with the clinical presentations, investigative modalities and approach to management of congenital cardiac malformations presenting in the early life.

A Study on Usage and Satisfaction of Newborn Clothing - Focused on Baenaet Jeogori - (신생아복의 사용 실태 및 만족도 연구 - 배냇저고리를 중심으로 -)

  • This study investigates customers' possession, usage, and satisfaction of newborn clothing, and analyzes their satisfaction with textiles of baenaet jeogori (the most basic Korean item of newborn clothing). The survey was conducted on 102 mothers with infants under 24 months old. The baenaet jeogori was the most commonly possessed item, followed by swaddling blanket, shirt and pants set, bodysuit, one-piece, and baenaet gown. About 52.6% of respondents used the swaddling blanket all day, 52% of respondents possessed 4-6 baenaet jeogories, and most respondents used the baenaet jeogori 4 weeks or more. Two most important selection criteria of the baenaet jeogori were textiles and season suitability which are closely related to functionality. Respondents were satisfied with the baenaet jeogori; however, there are a few problems with difficulty in putting and taking off as well as textiles. Most baenaet jeogories chosen by respondents were made of cotton and 68.8% of them were made of eco-friendly textiles. Respondents were dissatisfied with textiles when they are unsuitable for the season. Eco-friendly textiles were chosen for baby health and eco-friendliness, and respondents were satisfied with a baenaet jeogori made of eco-friendly textiles in general. The results show customers' high interest in textiles and the popularization of newborn clothing made of eco-friendly textiles. There was little interest in eco-friendly or safety certification marks despite the significant interest in the functionality and safety of textiles. Therefore, correct information on those certification marks is required.

  • A newborn male baby was transferred to our hospital with a left inguinal mass. The mass was huge measuring $10{\times}10cm$, engorged, and dark-blue colored as a result of internal hemorrhage. Unstable vital signs were combined with DIC and acute renal failure. Emergency operation was performed because of the suspicion of bowel perforation. The peritoneal cavity was full of ascitis and the distal jejunum had a 0.5 cm perforation. Segmental resection of the jejunum and incisional biopsy of the inguinal mass were performed. On pelvic and thigh MRI, the mass protruded into pelvic cavity and encircled large vessels and nerves of the thigh. Pathologic diagnosis was congenital infantile fibrosarcoma. Fifteen days after operation, primary tumor excision was undertaken. The second look operation, performed after 6 times VAC chemotherapy, revealed no remained malignant cell on microscopic section. The baby has been followed closely for the last eight months.

Asymptomatic maternal 3-methylcrotonylglycinuria detected by her unaffected baby's neonatal screening test

  • 3-methylcrotonyl-coenzyme A carboxylase (3MCC) deficiency is an autosomal recessive disorder in which leucine catabolism is hampered, leading to increased urinary excretion of 3-methylcrotonylglycine. In addition, 3-hydroxyisovalerylcarnitine levels increase in the blood, and the elevated levels form the basis of neonatal screening. 3MCC deficiency symptoms are variable, ranging from neonatal onset with severe neurological abnormality to a normal, asymptomatic phenotype. Although 3MCC deficiency was previously considered to be rare, it has been found to be one of the most common metabolic disorders in newborns after the neonatal screening test using tandem mass spectrometry was introduced. Additionally, asymptomatic 3MCC deficient mothers have been identified due to abnormal results of unaffected baby's neonatal screening test. Some of the 3MCC-deficient mothers show symptoms such as fatigue, myopathy, or metabolic crisis with febrile illnesses. In the current study, we identified an asymptomatic 3MCC deficient mother when she showed abnormal results during a neonatal screening test of a healthy infant.

Heritage of Traditional Childbirth and Childcare on Jeju (제주도 전통 산육속(産育俗)의 전승)

  • The characteristics of traditional childbirth and childcare custom on Jeju were studied by interviews of 134 elder women over 60. This established the nature of traditional custom of prenatal care, delivery, child rearing, and traditional ceremonies for baby as well as popular beliefs. The survey of current childbirth and childcare practices included 168 parents of kindergarteners. Results showed that traditional childbirth and childcare custom of Jeju is based in self-reliance, effectiveness, labor, and popular beliefs and that the heritage consisting of traditional postpartum foods, bathing of the newborn baby, use of a cradle, shamanistic prescriptions, and acupuncture is prevalent today.

Clothes for Newborn Celebration Event from the 1920s to 1950s - Focusing on the Central Region - ($1920{\sim}1950$년대의 출생의례복 - 중부지방을 중심으로 -)

  • This study is on the children's clothing in Seoul Gyeonggi-do, Chungcheong-do and Gangwon-do between the $1920s{\sim}1950s$, by comparing positive data collected from pictures and literatures, remains and interviews. A baenaet jeogori was made of soft white cotton fabrics and was used as a charm when the baby had grown and had an test or a big occasion. A dureong chima and pungcha trousers were clothes for both boys and girls from their birth to the age of $4{\sim}5$ when they could have bowel movements by themselves. Occasions for celebrating a baby's growth were the one-hundredth day and the first birthday. In general, ordinary families had their babies' one-hundredth day in a simple way without special clothes. On the first birthday, however, even ordinary families prepared new clothes for their babies, and read their fortune and prayed for their well being and long life through events such as doljabi. In the age when medicine was poor and the infant mortality was high, the meaning of such a ceremony was to congratulate on the baby's safe growth through dangerous moments.

A Study on the Perceived Stress Level of Mothers in the Neonatal Intensive Care Unit Patients (신생아 중환자실에 입원한 환아 어머니의 스트레스)

  • This descriptive study was conducted to understand the contents and degree of parental stress level in the NICU patients, and to give a baseline data in developing nursing intervention program. Subjects were the 62 mother of hospitalized newborn in NICU of 1 University Hospital in Taejon City from May 1st, 1999 to November 30th, 1999, who agreed to take part in this study. The instrument used in this study were Parental Stressor Scales : NICU(PSS:NICU) developed by Miles et al. and validated by 3 NICU practitioners and 3 child health nursing faculties. The questionnaire has 4 dimensions and 45 items; sight and sounds of NICU(5 items), babies' appearance and behavior(19 items), parental role alteration and relationship with their baby(10 items), communication with health team(11 items). The questionnaire asks parents to rate each item on a five-point Likert type scale that ranges from (1) to (5). Total scores representing overall stress from the NICU environment are calculated by summing response to each item. A high score indicates high stress. A subscale score is calculated by summing the responses to each item in the subscale. Cronbach's α coefficients were .93. The data was analyzed as average, Frequency, Standard deviation, t-test, ANOVA, Pearson correlation coefficient by use of SPSS/PC+. The results of this study is summarized as follows ; 1. The total perceived stress level score of mothers was slightly high(3.6±.7). The highest scored dimension was 'appearance and behavior of the baby'(3.9±1.5), and next were 'relationship with their baby and parental role change'(3.5±1.4), 'communication with health team'(3.4±.9), 'sight and sounds of NICU'(3.2±.8). 2. Two variables were statistically significant with PSS:NICU total scale ; mother's perceived severity of the baby's condition (r=.482, P=.002) and mother's religious attendance(t=2.83, P=.01). The more the mother perceive their baby's condition severe, the higher the total stress score. There were high stress score noted in the mother of no religious attendance. 3. Four variables were statistically significant with NICU environment subscale ; mother's educational background(F=3.45, P=.04), religious attendance(t=2.28, P=.04), sex of the baby(t=2.83, P=.01) and NICU patients' hospital day(r=.359, P=.004). That is mother with high educational background and girl baby were high NICU environment subscale score. 4. Four variables were statistically significant with appearance and behavior of the baby subscale ; when first saw baby(F=3.52, P=.04), incubator care(t=2.83, P=.01), mother's perceived severity of the baby's condition(r=.303, P=.017), number of NICU visit(r=.441, P=.002). That is, seeing the baby first in the NICU and recieved incubator care was very stressful. Also, the more the mother perceive their baby's condition severe and more NICU visit, the higher the appearance and behavior of the baby subscale stress score. 5. Four variables were statistically significant with relationship with their baby and parental role change subscale ; when first saw baby(F=3.37, P=.04), sex of the baby(t=2.36, P=.03), incubator care(t=5.60, P=.00), mother's perceived severity of the baby's condition(r=.401, P=.001). That is, seeing the baby first in the NICU and girl baby was very stressful. Also, the more the mother perceive their baby's condition severe, the higher the relationship with their baby and parental role change subscale stress score. 6. Three variables were statistically significant with communication with health team subscale ; mother's educational background (F=3.63, P=.04), incubator care(t=4.24, P=.00), gestational age(r=-.394, P=.047), and birth weight(r=-.460, P=.004). That is, mother with high educational background and receiving incubator care were high communication with health team subscale score. Also, the shorter the gestational age and smaller the baby's birth weight, the higher the communication with health team subscale score. In conclusion, information about physical environment of NICU, the mother's perceived severity of baby's illness state, maternal role change related variables and the knowledge of characteristics of NICU patients must be included in nursing intervention program of mother's of NICU patients in reducing the maternal stress and anxiety level.

Adverse Birth Outcomes Among Pregnant Women With and Without COVID-19: A Comparative Study From Bangladesh

  • Objectives: Pregnant women are especially vulnerable to respiratory infections such as coronavirus disease 2019 (COVID-19), but insufficient research has investigated pregnancy and its outcomes in women with COVID-19. This cross-sectional study compared birth outcomes related to COVID-19 between Bangladeshi pregnant women with and without COVID-19. Methods: The study was conducted at 3 tertiary referral hospitals in Dhaka, Bangladesh, from March to August 2020. Pregnant women admitted for delivery at these hospitals with laboratory results (reverse-transcription polymerase chain reaction) were analyzed. Using convenience sampling, we included 70 COVID-19-positive and 140 COVID-19-negative pregnant women. Trained and experienced midwives conducted the interviews. Data were analyzed using the t-test, the chi-square test, and univariate and multivariable linear and logistic regression. Results: Pregnant women with COVID-19 were more likely to give birth to a preterm baby (adjusted odds ratio [aOR], 2.15; 95% confidence interval [CI], 1.06 to 4.37) and undergo a cesarean section (aOR, 3.27; 95% CI, 1.51 to 7.07). There were no significant differences in birth weight, premature rupture of membranes, and the Apgar score at 1 minute or 5 minutes post-delivery between women with and without COVID-19. All the newborn babies who were born to COVID-19-positive women were COVID-19-negative. Conclusions: Our study suggests that pregnant women with COVID-19 were more likely to give birth to a preterm baby and undergo a cesarean section. For this reason, physicians should be particularly cautious to minimize adverse birth outcomes among pregnant women with COVID-19 and their newborn babies.

Concept analysis of transition to motherhood: a methodological study

  • Purpose: Although the term "transition to motherhood" is commonly used in research, the concept is not clear. This study, hence, was conducted to clarify the concept of "transition to motherhood." Methods: The concept analysis framework developed by Walker and Avant is used to analyze the concept of transition to motherhood. Results: Transition to motherhood is defined as the physical, psychological, social, and relational (mother-baby relationship/interpersonal relationship) changes that happen to a woman after pregnancy and delivery of a baby. The attributes of the transition to motherhood include: 1) adapting to physical changes after pregnancy and childbirth; 2) experiencing various psychological changes; 3) changing of her social perception from being a woman to someone's mother; and 4) forming and developing a relationship with the newborn, adjusting priorities, and redefining the relationship between family and others. Meeting the newborn is regarded as an antecedent of the transition to motherhood. Redefining identity and physical image, ensuring mother's well-being, maternal attachment, and confidence in the maternal role are regarded as consequences of the transition to motherhood. The concept was clarified by the presentation of model, borderline, and contrary cases. Conclusion: The significance of this study lies in the clarification of the concept of transition to motherhood and defining its attributes. It is recommended that tools be developed to measure transition to motherhood based on the results of this study. Furthermore, nurses and midwives can use study findings to better understand the concept of transition to motherhood in providing care and support to mothers who experience it.

A case of amebiasis in the newborn period (신생아에서 나타난 아메바증 1례)

  • Entamoeba histolytica (E. histolytica) is a worldwide protozoan parasite causing a wide rage of intestinal disease in human, including asymptomatic cyst carriage, acute diarrhea, fulminant dysentery, and chronic nondysenteric colitis. Extraintestinal amebiasis such as hepatic abscess also occurs. Spread of the infection occurs due to consumption of food and water that is contaminated with the cyst. Amebiasis is rarely reported in the newborn period. We report a case of a 6-day-old female newborn with watery diarrhea that progressed to bloody mucoid diarrhea with persistent fever. The fresh stool microscopic examination disclosed E. histolytica trophozoites. She was born of spontaneous vaginal delivery at 41 weeks of gestation after premature rupture of membrane. She was given breast-feeding after birth, then entered postpartum care center. Her mother's stool microscopic examination showed negative result. Infection might have occurred from giving sweetened water following birth or contamination of the baby's bottles at the postpartum care center. She was treated with oral metronidazole for 24 days.
