The Effects of Platelet-Rich Plasma on Regeneration around Dental Implant Defects (혈소판 농축혈장이 임플랜트 주위 골결손부 재생에 미치는 영향)

  • The current interest in periodontal tissue regeneration has lead to research in bone graft, root surface treatments, guided-tissue regeneration, and the administration of growth factors as possible means of regenerating lost periodontal tissue. Several studies have shown that a strong correlation between platelet-rich plasma and the stimulation of remodeling and remineralization of grafted bone exists, resulting in a possible increase of 15-30% in the density of bone trabeculae. The purpose of this study was to study the histopathological correlation between the use of platelet-rich plasma and a bone xenograft used in conjunction with a non-resorbable guided-tissue membrane, e-PTFE, compared to a control group with regards to bone regeneration at the implant fixture site. Implant fixtures were inserted and graft materials placed into the left femur of in the experimental group, while the control group received only implant fixtures. In the first experimental group, platelet-rich plasma and BBP xenograft were placed at the implant fixture site, and the second experimental group had platelet-rich plasma, BBP xenograft, and the e-PTFE membrane placed at the fixture site. The degree of bone regeneration adjacent to the implant fixture was observed and compared histopathologically at 2 , 4, and 8 weeks after implant fixture insertion. The results of the experiment are as follows: 1. The rate of osseointegration to the fixture threads was found to be greater in the first experimental group compared to the control group. 2. The histopathological findings of the second experimental group showed rapid resorption of BBP with subsequent new bone formation replacing the resorbed BBP. 3. The second experimental group showed new bone formation in the area adjacent to the fixture threads beginning two weeks after fixture implantation, with continued bone remodeling in the areas mesial and distal to the fixture. 4. Significant new bone formation and bone remodeling was observed in both experimental groups near the implant fixture sites. 5. The rate of osseointegration at the fixture threads was greater in the second experimental group compared to the first group, and the formation of new bone and trabeculae around the fixture site occurred after the fourth week in the second experimental group. The results of the experiment suggest that a greater degree of new bone formation and osseointegration can occur at the implant fixture site by utilizing platelet-rich plasma and bone xenografts, and that these effects can be accelerated and enhanced by concurrent use of a non-resorbable guided tissue membrane.

A Histo-Pathological Study of Effect on Periodontal Regeneration with Bioabsorbable Membrane on The Grade II Furcation Defects in Beagle Dogs (성견 치근이개부 병소에서 흡수성 차폐막의 치주조직재생에 미치는 영향에 대한 조직병리학적 연구)

  • The present study evaluated the effects of guided tissue regeneration using xenograft material(deproteinated bovine bone powder), with and without biodegradable membrane in beagle dogs. Contralateral fenestration defects (6 ${\times}$ 4mm) were created 4 mm apical to the buccal alveolar crest of maxillary premolar teeth in 5 beagle dogs. Deproteinated bovine bone powders were implanted into fenestration defect and one randomly covered biodegradable membrane (experimental group). Biodegradable membrane was used to provide GTR. Tissue blocks including defects with soft tissues which were harvested following four & eight weeks healing interval, prepared for histo-phathologic analysis. The results of this study were as follows. 1. In control group, at 4 weeks after surgery, new bony trabecular contacted with interstitial tissue and osteocytes like cell were arranged in new bony trabecule. Bony lamellation was not observed. 2. In control gruop, at 8 weeks after surgery, scar-like interstitial tissue was filled defect and bony trabecule form lamellation. New bony trabecular was contacted with interstitial tissue but defect was not filled yet. 3. In experimental group, at 4 weeks after surgery, new bony trabecular partially recovered around damaged bone. But new bony trabecular was observed as irregularity and lower density. 4. In experimental group, at 8 weeks after surgery, lamella bone trabecular developed around bone cavity and damaged tissue was replaced with dense interstitial tissue. In conclusion, new bone formation regenerated more in experimental than control groups and there was seen observe more regular bony trabecular in experimental than control groups at 4 weeks after surgery. In control group, at 8 weeks after surgery, the defects was filled with scar-like interstitial tissue but, in experimental group, the defects was connected with new bone. Therefore xenograft material had osteoconduction but could not fill the defects. We thought that the effective regeneration of periodontal tissue, could be achieved using GTR with biodegradable membrane.

THE EFFECT OF $TGF-{\beta}1$ ON THE REGENERATION OF BONE IN GUIDED BONE REGENERATION (탈회동결건조골에 혼합한 형질 변형 성장인자($TGF-{\beta}1$)가 골조직 재생에 미치는 영향)

  • The purpose of this study was to observe the effect of $TGF-{\beta}1$ on the regeneration of bone in guided bone regeneration. Four adult dogs aged 12 to 24 months were used in this study. Experimental bone defects were created surgically with surgical bur and chisel on the 3th. premolars. In experimental group, bone defect were grafted with DFDB and $TGF-{\beta}1$. In control groups, bone defects were grafted with only DFDB. At 1,2,3 and 4 weeks, dogs were serially sacrificed and specimens were prepared with Hematoxylin-Eosin stain and Goldner's stain for light microsopic evaluation. The results of this study were as follows: 1. The infiltration of inflammatory cells was prominent in control groups at 1, 2 and 3 weeks. 2. The lining of osteoblast was observed at 2 weeks in control group, but at 1 week in experimental group. 3. In both groups, osteoid was formed at 2 weeks. In control groups, osteoid was fromed on only bone surface. but in experimental groups, osteoid were formed on both bone & DFDB surfaces. 4. In only experimental groups, The fusion of new bone & DFDB was only observed at 3 weeks. and the fusion of new bone & DFDG was more prominent at 4 weeks. But in control groups, No fusion of new bone& DFDB was oberved at 3 and 4weeks. From the above result, the $TGF-{\beta}1$ was effective in bone formation and increased inductive effect of DFDB in guided bone regeneration technique. Inductive effect of DFDB was increased with $TGF-{\beta}1$.

Effectiveness of Transplantation by Freeze-Dried Bone of Goat to Dogs (동결건조한 산양뼈의 개이식 효과)

  • Freeze-dried cortical bones of the goat were transplanted to the experimental fibular defect of 10 dogs for valuating the possibility of xenogeneic bone implantation and the specificity of BM(Bone Morphogenetic Protein). The . freeze-dried cortical bone eliminated antigens and defatted with chloroform and methanol were freeze-dried at $-80{\circ}C$ for preservation of BMP and then sterilized with 50 gas and storaged in room temperature. Ten freeze-dried cortical implants of the goat were transplanted in experimentally defected regions of bilateral fibula of 5 dogs in clinically normal. The transplanted region had been radiographed for observing state of bone union and BALPOone Alkaline Phosphatase) in the serum of the host was measured for valuating activity of oteoblast per 2 week-interval after transplant procedures. New bone formation had been observed early in one of ten regions around implants about the same time as autoimplant regions. It was incorporated with its host bone during 4-12 weeks after transplantation. In another 2 cases of 2 dogs, new bone formation and absorption of implant had been observed from 4 weeks but they were not incorporated completely until 20 weeks. The rest of the freeze-dried bone implants, 7 cases of 4 dogs had not been observed new bone formation nor absorption of implants. The freeze-drying method for implants means to not influence bone incorporation. Although less of union percentages the union form of this experiment were similar to alloimplantation and it may mean to block immunity reaction that disturbs the bone induction by BMP. It demonsknted that the possibility of the xenogenous bone implantation is recognized by reason of the low specificity of BMP between goat and dog.

DIRECT CURRENT EFFECT ON THE BONE FORMATION AND OSSEOINTEGRATION AROUND TPS-IMZ IMPLANT (미소 전류 자극이 TPS-IMZ 임프란트 주위의 골유착과 골형성에 미치는 영향)

  • The purpose of this study is to investigate the effect of constant direct current electrical stimulation in healing the bone defects and surrounding tissues of the endo-oseous(TPS-IMZ) implants. Implants were inserted in the femur of adult dogs. Then a constrant direct current of approximately $10{\mu}A$ was applied. Artificial bone defects were prepared on one side of the implant site. Experimental groups were divided into 4 : control group : bone defect without treatment group I : bone defect filled with hydroxyapatite powders group II : bone defect, in which a negative and positive electrodes were inserted 5mm apart from both sides of the implant group III : bone defect, in which negative current was directly connected to the IMZ implant and a positive electrode was placed 10mm apart from the implant The animals were sacrificed in the 1st, 2nd, 4th and 8th week after implantation for the light microscopic examination. The results obtained were as follows : 1. In electrically stimulated experimental groups, new bone formation and osseointegration around implants were accelerated. 2. Group III showed the greatest activity in new bone formation. Osteoconductivity around HA particles was observed in group 1. 3. The defect area of the control group was healed by forming new bone, which grew from the underlying cancellous bone. The defect areas of the electrically stimulated experimental groups were healed by newly formed bone, which grew upward from the cancellous bone and downward from the periosteum. 4. 8 weeks after implantation, all the groups showed good osseointegration between the surrounding bone and implants.

THE EFFECTS OF DECALCIFIED FREEZE-DRIED BONE AND SYNTHETIC BONE GRAFTS ON REGENERATION OF ALVEOLAR BONE DEFECTS IN DOGS (탈회동결건조골과 합성골이식재가 치조골 결손부 재생과정에 미치는 영향)

  • The purpose of this study was to compare effects of various bone grafts on periodontal regeneration of alveolar bone defects in dogs. Seven adult dogs aged 12 to 18 months were used in this study. Experimental alveolar bone defects were created surgically with a #1/2 round bur at the furcation area of the buccal surface of the mandibular 3rd, 4th premolars and 1st molar. Each experimental alveolar bone defects were grafted with dense hydroxyapatite, natural coral, and decalcified freeze-dried bone, and respectively divided into DHA, NC, DFDB group. An area without bone graft was divided into control group. At 1,2,4,6, and 12 weeks, dogs were serially sacrificed and specimens were prepared with Hematoxylin-Eosin stain and Mallory stain for light microscopic evaluation. The results of this study were as follows : 1. In control group, the matrix change of granulation tissue was observed at 1 week. And in experimental groups, the appearance of connective tissue around graft materials was loosely formed at 1 week, but densely formed at 2 weeks. 2. In every group, the slight formation of new trabecular bone was seen from remaining bone at 1 week. 3. The DHA and NC particles were gradually encapsulated by new trabecular bone from remaining bone, and the osteoid tissue was directly induced from DFDB particles. 4. The presence of osteoblasts was first observed at 1 week in control group and at 2 weeks in NC group, but at 6 weeks in DHA group. 5. In DHA group, the resorption of particles was not observed until 12 weeks. But in NC and DFDB group, the particles were resorbed at 6 weeks and replaced by new bone. And the amount and size of particles were reduced, and their border represented irregular form. In summary, in three experimental groups the inflammatory or foreign body reaction were slight, but the regeneration of new osteoid tissue and the matrix change of dense connective tissue fiber were observed. Especially, NC and DFDB materials were considered as the biocompatible graft materials which were effective in the regenertion of new bone.

  • The successful implantation necessitate tissue regeneration m site of future implant placement, there being severe bone defect. Therapeutic approaches to tissue regeneration in the site have used bone grafts, root surface treatments, barrier membranes, and growth factors, the same way being applied to periodontal tissue regeneration. Great interest in periodontal tissue regeneration has lead to research in bone graft, guided-tissue regeneration, and the administration of growth factors as possible means of regenerating lost periodontal tissue. The blood component separated by centrifuging the blood is the platelet-rich plasma. There are growth factors, PDGF, $TGF{beta}1$, $TGF{beta}2$ and IGF in the platelet-rich plasma. The purpose of this study was to study the histopathological correlation between the use of platelet-rich plasma and the healing of bone defect around implant fixture site. Implant fixtures were inserted and graft materials were placed into the left femur of in the experimental group, while the control group received only implant fixtures. In the first experimental group, platelet-rich plasma and BBP xenograft were placed at the implant fixture site, and the second experimental group had platelet-rich plasma, BBP xenograft, and the e-PTFE membrane placed at the fixture site. The degree of bone regeneration adjacent to the implant fixture was observed and compared histopathologically at 2, 4, and 8 weeks after implant fixture insertion. The results of the experiment were as follows: 1. Bone remodeling in acid etched surface near the implant fixture of all experimental groups was found to be greater than new bone formation. 2. Bone remodeling in acid etched surface distant to the implant fixture of all experimental groups was decreased and new bone formation was not changed. 3. Significant new bone formation in machined surface near the implant fixture of bothl experimental groups was observed in 2 weeks. 4. New bone formation in machined surface distant to the implant fixture of both experimental groups was observed. Bone remodeling was significant in near the implant fixture and not in distant to the implant fixture. The results of the experiment suggested that the change of bone formation around implant. Remodeling in machined surface distant to the implant fixture of both experimental groups, and new bone formation and remodeling near the implant fixture were significant.

Effect of Alendronate on Bone Regeneration in Defect of Rat Calvaria (Alendronate가 백서에서 두개골 결손의 재생에 미치는 영향)

  • Previous studies have demonstrated an increase in bone mass and density with the use of bisphosphonate in osteoporosis. This agent acts as an inhibitor of osteoclastic activity, and results in increase of net osteoblastic activity. Currently, it has been reported that bisphosphonate has direct effect on osteoblast. This study was designed to evaluate the effect of alendronate on bone regeneration in defect of rat calvaria. The animals used for these experiments were 48 male rats, over 6-8 weeks old. They were divided into three groups according to the dose of alendronate($MK-217^{(R)}$, Merck, USA) administered. After the calvarial defects were surgically created, the rats received a peritoneal alendronate(0.25mg/kg) in group I, a peritoneal alendronate(1.25mg/kg) in group II, and a peritoneal normal saline injection in the control group. Three and six weeks later, blood was sampled and evaluated for alkaline phosphatase activity. The animals were sacrificed for histological observation and histometric analysis of the level of bone formation. The alkaline phosphatase activity was similar in three groups at 3 weeks of experiment. The activity at 6 weeks increased more than twice, compared to 3 weeks, and was slightly higher in group I than the other two groups. In histological observation, all the groups at 3 weeks, osteoblast rimming and new bone formation were observed along the defect margin. At 6 weeks, the defect was almost closed with new and more mature bone, but new bone is thinner than original bone in the central portion of defect. In histometric analysis, group I and II at 3 weeks showed significantly greater new bone formation than the control, and all the groups at 6 weeks showed similar amount of bone formation. These result suggest that alendronate administration in the dose of 0.25mg/kg and 1.25mg/kg promote osseous regeneration.

  • Calcium sulfate(plaster of Paris) has been used in dental and orthopedic surgery for about 100 years. It is well known that the plaster is bioresorbable, biocompatible, defect conformable and moldable. The purpose of this study is to evaluate two effects of calcium sulfate on bone regeneration, that is, the effects of graft materials and barrier for bone regeneration. Cortical bone defects were formed for recipient site on the femurs of 19 Sprague-Dawley rats. The autogenous particulated bone and calcium sulfate were grafted to the defects. Calcium sulfate paste, $Gore-Tex^R$ membrane(W.L. GORE & ASSOCIATES LTD., U.S.A.) and rubber sheet were used for the shielding materials. The results were as follows : 1. Calcium sulfate that had been grafted in the cortical bone defect was almost resorbed before bone remodeling, resultantly had little effect on bone regeneration. 2. Resoption process of calcium sulfate grafted on the bone grafting area tends to be accelerated, as being divided into numerous small particles progressively. Under the situation where the calcium sulfate was protected, with the coverage of fascia, $Gore-Tex^R$ membrane or rubber sheet, new bone formation was confirmed with presence of calcium sulfate particles over 6 weeks after grafting. 3. In the case of calcium sulfate covered with membrane, distinct bone formation was observed on the marrow space of femur adjacent to the plaster mass. 4. Rubber shielded plaster group revealed new bone trabeculae under the rubber sheet, but it showed ischemic degeneration of superficial cortical bone.

Usefulness of Bone Scan for Diagnosis of Osteomyelitis in Diabetic Foot (당뇨 발 환자의 골수염 진단에 있어서 골 주사 검사의 유용성)

  • Objectives: To validate usefulness of the three phase bone scan for the diagnosis of osteomyelitis in the diabetic foot with soft tissue inflammation. Materials and Methods: Fourteen diabetic feet with soft tissue inflammation were included in this study. We took the bone biopsy from the site of hot uptake on the three phase bone scan but no abnormal findings on the plain radiographs. We observed whether the bone has evidence of osteomyelitis on the patholgic findings such as inflammatory cell infiltration within bone, dead bone, new bone formation and fibrosis. Results: Thirteen of fourteen cases (92.8%) were compatible with osteomyelitis on the pathologic criteria. Inflammatory cell infiltration within bone was observed in thirteen cases, dead bone in twelve cases. new bone formation in four cases. fibrosis in eight cases. All of the four findings were observed in three cases. Conclusion: In the diabetic foot with soft tissue inflammation, the osteomyelitis should be included in differential diagnosis if the lesion reveals increased uptake on three phase bone scan, even though the lesion does not show any abnormal findings on the plain radiographs.
