Clinical Outcomes of Pulsed Radiofrequency Neuromodulation for the Treatment of Occipital Neuralgia

  • Objective : Occipital neuralgia is characterized by paroxysmal jabbing pain in the dermatomes of the greater or lesser occipital nerves caused by irritation of these nerves. Although several therapies have been reported, they have only temporary therapeutic effects. We report the results of pulsed radiofrequency treatment of the occipital nerve, which was used to treat occipital neuralgia. Methods : Patients were diagnosed with occipital neuralgia according to the International Classification of Headache Disorders classification criteria. We performed pulsed radiofrequency neuromodulation when patients presented with clinical findings suggestive occipital neuralgia with positive diagnostic block of the occipital nerves with local anesthetics. Patients were analyzed according to age, duration of symptoms, surgical results, complications and recurrence. Pain was measured every month after the procedure using the visual analog and total pain indexes. Results : From 2010, ten patients were included in the study. The mean age was 52 years (34-70 years). The mean follow-up period was 7.5 months (6-10 months). Mean Visual Analog Scale and mean total pain index scores declined by 6.1 units and 192.1 units, respectively, during the follow-up period. No complications were reported. Conclusion : Pulsed radiofrequency neuromodulation of the occipital nerve is an effective treatment for occipital neuralgia. Further controlled prospective studies are necessary to evaluate the exact effects and long-term outcomes of this treatment method.

Two Cases of Herpes Zoster and Post-herpetic Neuralgia Improved by Oriental Medicine with Bee Venom Acupuncture (蜂藥針療法을 병행한 한방치료로 호전된 대상포진 및 대상포진후 신경통환자 2례)

  • After administration of Oriental medicine with Bee venom acupuncture to two cases of Herpes zoster and Post-herpetic neuralgia. We can observe remarkable efficacy on relief from pain. This results suggest that Oriental medicine with Bee venom acupuncture can be one of a good remedy for Herpes zoster and Post-herpetic neuralgia.

  • Purpose : Trigeminal neuralgia has been known a rebellious disease and there has been no case report of trigeminal neuralgia during pregnancy in Oriental medicine. Experienced good effect on treatment for trigeminal neuralgia during pregnancy, we wanted to report a case report of trigeminal neuralgia during pregnancy. Methods : 5-month-pregnancy, patient with trigeminal neuralgia was hosptalized from the 10th, June, 2003 to the 23th, June, 2003 in OO Oriental Medical Center. In acupuncture therapy, SI19(Tinggong, 聽宮), S7(Hagwan, 下關), LI19(Hwaryo, 禾?), S4(Chichang, 地倉), S2(Sabaek, 四白), Tel7(Yepung, ?風), B2(Chanjuk, ?竹), G14(Yangbaek, 陽白), 太陽(taeyang), 魚腰(yuje) were used. In Moxa Therapy, SI19(Tinggong, 聽宮), S7(Hagwan, 下關), LI19(Hwaryo, 禾?), S4(Chichang, 地倉), S2(Sabaek, 四白), Te17(Yepung, ?風) were used. In Herbal Therapy, Boikyangwytang was used. Pain was measured by VAS(Visual Analogue Scale). Results : At the 10th, June, 2003, Pain was measured VAS(100). At the 23th, June, 2003, Pain was measured VAS(10) and Pain was resolved, Tic(intermediate) was remain. Conclusion : We could get effective result that a case of trigeminal neuralgia during pregnancy, was treated conservative therapy of oriental medicine.

  • Sciatic neuralgia has been considered as the symptom of herniated lumbar disk. but disk disease is not the only causes of sciatic neuralgia. Sciatic neuralgia uncombined with disk disease in thought to be a sign of myofascial syndrome of the piriform is muscle. Local anesthetic injection into piriform is muscle is recommended for the therapeutic treatment of sciatica without lumbago.

  • Orofacial pain can be caused by intracranial disorders or can be musculoskeletal, vascular, internal derangemental, and neurologic in origin. The neurologic pain is derived from structural and functional disorders of nerve, and the trigeminal neuralgia is the typical manifestation. Trigeminal neuralgia is known from centuries ago, and is one of the most common pains in human. We present our experience with three patients who have trigeminal neuralgia. The first case is a 50-year-old female who had no specific evidence radiographically. Second is a 50-year-old male with microvascular compression on right trigeminal nerve. The third case is a 60-year-old female who had a neoplasm in cerebellopontine angle with associated mass effect.

  • Objective : This study aims to explain the effect of Buja-tang on Postherpetic Neuralgia with Chronic Fatigue Syndrome which a Soeum-byeong patient has. Methods : This is a case report on a female patient who has been suffering from Postherpetic Neuralgia with Chronic Fatigue Syndrome. According to Diagnostic system based on Shanghanlun provisions, Buja-tang was provided for one month. Results : After a series of Buja-tang treatments, the patient's symptoms and result improved. Conclusions : This case study show an effectiveness of using Buja-tang according to Diagnostic system based on Shanghanlun provisions on Postherpetic Neuralgia with Chronic Fatigue Syndrome.

  • Objectives : The purpose of this study is to report the effect of Traditional Korean Medical treatment on Postherpetic Neuralgia. Methods : Five patients suffering from postherpetic neuralgia were treated with Warm Needling, Bee venom Acupuncture, Cupping Therapy, Sipjeondaebo-tang. To evaluate the results of this treatment, we used Visual Analogue Scale(VAS), Pain Rating Scale(PRS), Quality of Sleep(QSP). Results : After treatment, VAS, PRS, QSP and clinical symptoms of 5 patients were improved remarkably. Conclusions : According to the results, These findings suggest that Traditional Korean Medical treatment can be effective for the treatment of postherpetic neuralgia.

  • During the early stage of postherpetic neuralgia, an epidural block on the affected segment is helpful in controlling pain and preventing progression to a chronic state. The main neurologic complication following an epidural block is cord compression symptom due to an epidural hematoma. When neurologic complications arise from an epidural block for the treatment of postherpetic neuralgia, it is important to determine whether the complications are due to the procedure or due to the herpes zoster itself. We report a case of a patient who was diagnosed with herpes zoster myelitis during treatment for postherpetic neuralgia. The patient complained of motor weakness in the lower extremities after receiving a thoracic epidural block six times. Although initially, we believed that the complications were due to the epidural block, it was ultimately determined to be from the herpes zoster myelitis.

  • Objectives: The purpose of this study is to report the effect of Korean medicine on trigeminal neuralgia in a patient with systemic sclerosis. Methods: We treated a 49-year-old male patient who has systemic sclerosis and trigeminal neuralgia by Korean medicine. He was treated from October 29th, 2022 to November 12th, 2022. The treatment applied was herbal medicine, acupuncture, pharmacopuncture. The evaluation was conducted by Visual Analogue Scale (VAS). Results: After 14 days of treatment, the symptoms were alleviated. Facial pain and dullness of taste improved. VAS of discomfort was decreased from 10 to 2. Conclusions: These results showed that Korean medicine may have an effect on reducing symptoms of systemic sclerosis and trigeminal neuralgia.

  • Background : There are several treatment of trigeminal neuralgia including Gasserian Ganglion Block. This paper retrospectively analyze the efficacy of Gasserian Ganglion Block as main treatment of trigeminal neuralgia. Methods : We studied 55 patients for followings; sex, age, duration from onset to visit, consulted mediator, causes, characteristics of pain, affected area, types of block, kinds and volume of used neurolytic agents, duration from first block to repeat block, types of repeat block, complications, if surgery was necessary after block. Results : Characteristic items of trigeminal neuralgia are similar with other studies. Mean duration of pain relief was 23 months. The duration was shortened in the repeat block. There were no significant complication. Conclusion : Considering Gasserian Ganglion Block as main treatment of trigeminal neuralgia is an effective and safe method. Therefore we would like to recommend the need to increase public awareness of trigeminal neuralgia and its possible treatment in pain clinics through mass commanication.
