Effects of YideungJetong-Tang on Peripheral Neuropathy Induced by Taxol and Compression Injury in the Rat Sciatic Nerve (이등제통탕(二藤除痛湯)이 Taxol 처리 및 좌골신경의 압박 손상 후 유발된 랫드의 말초신경병증에 미치는 영향)

  • Background: Most antitumor agents have the side effect of chemotherapy-induced peripheral neuropathy (CIPN). Cancer patients who take antitumor agents suffer from CIPN, but there is no known treatment for it. Unlike the central nerve system, the peripheral nerve can self-repair, and the Schwann cell takes this mechanism. Objectives: In this study, we researched the effect of YideungJetong-Tang (YJT) extract on taxol-induced sciatic nerve damage, through in vitro and in vivo experiments. Also, we studied the effect of YJT extract on neurite recovery and anti-inflammatory effect after compression injury of sciatic nerve in vivo. Methods: Vehicle, taxol and taxol+YJT were respectively applied on sciatic nerve cells of rat in vitro, then the cells were cultured. The sciatic nerve cells and Schwann cells were then observed using Neurofilament 200, Hoechst, ${\beta}$ -tubulin, S-$100{\beta}$, caspase-3 and phospho-Erk1/2. CIPN was induced by taxol into the sciatic nerve of rat in vivo, then YJT extract was taken orally. The axons, Schwann cells and neurites of the DRG sensory nerve were then observed using Neurofilament 200, ${\beta}$-tubulin, Hoechst, S-$100{\beta}$, phospho-Erk1/2 and caspase-3. YJT was taken orally after sciatic nerve compression injury, and the changes in axon of the sciatic nerve, Schwann cells and TNF-${\alpha}$ concentration were observed. Results: The taxol and YJT treated group showed significant effects on Schwann cell recovery, neurite growth and recovery. In vivo, YJT compared with control group showed Schwann cell structural improvement and axons recovering effect after taxol-induced Schwann cell damage. After sciatic nerve compression injury, recovery of distal axon, changes of Schwann cell distribution, and anti-inflammatory response were observed in the YJT. Conclusions: Through this study, we found that after taxol-induced neurite damage of sciatic nerve in vivo and in vitro, YJT had significant effects on sciatic nerve growth and Schwann cell structural improvement. In vivo, YJT improved recovery of distal axons and Schwann cells and had an anti-inflammatory effect.

Effect of purinoceptor to perivascular nerve stimulation on isolated coronary artery of pig (돼지 적출 심관상동맥에 있어서 혈관주위 신경자극에 의한 purinoceptor의 효과)

  • To elucidate whether or not innervation of purinergic nerve and functional action of $P_{2X}$-, $P_{2Y}$-purinoceptor, the neurogenic effects of perivascular nerve stimulation were investigated using by physiograph recording system in isolated coronary artery of pig. 1. The contractile responses induced by perivascular nerve stimulation (20V, 0.5msec, 20sec) were the frequency (2~64Hz) dependent manner. 2. The neurogenic contractile responses induced by perivascular nerve stimulation were inhibited by the pretreatment with either ATP or adenosine ($10^{-7}{\sim}10^{-4}M$). 3. The neurogenic contractile responses induced by perivascular nerve stimulation (20V, 16Hz, 0.5msec, 20sec) were increased by the pretreatment with reactive blue 2, but were not affected by the pretreatment with 8-phenyltheopylline ($10^{-5}M$). 4. The neurogenic contractile responses induced by perivascular nerve stimulation (20V, 16Hz, 0.5msec, 20sec) were inhibited by the desensitization of the P2X-purinoceptor using by treatment of $10^{-5}M$, -methylene ATP as 3 times over again. The accomplished present study on isolated coronary artery of pig suggest that purinergic nerve is innervated and that the neurogenic contractile response was mediated by activation of $P_{2X}$-purinoceptor and the neurogenic relaxative response was mediated by activation of both $P_1$ and $P_{2Y}$-purinoceptor.

  • Nerve conduction study (NCS) is an electrophysiological tool to assess the overall function of cranial and peripheral nervous system, therefore NCS has been diagnostically helpful in the identification and characterization of disorders involving nerve roots, peripheral nerves, muscle and neuromuscular junction, and are frequently accompanied by a needle Electromyography. Furthermore, NCS could provide valuable quantitative and qualitative results into neuromuscular function. Usually, motor, sensory, or mixed nerve studies can be performed with using NCS, stimulating the nerves with the recording electrodes placed over a distal muscle, a cutaneous sensory nerve, or the entire mixed nerve, respectively. And these findings of motor, sensory, and mixed nerve studies often show different and distinct patterns of specific abnormalities indicating the neuromuscular disorders. The purpose of this special article is to review the neurophysiologic usefulness of NCS, to outline the technical factors associated with the performance of NCS, and to demonstrate characteristic NCS changes in the setting of various neuromuscular conditions.

Cholinergic Contraction to the Perivascular Nerve Stimulation on the Isolated Coronary Artery of Pig (돼지 적출 심장 관상동맥에 있어서 Perivascular Nerve Stimulation에 의한 Cholinergic 수축 작용)

  • The purpose oi this study is to identify clearly the physiologic significance of autonomic nervous system. This study is to find the loose of endogenous neurotransmitter while using the neural response of the neural excitatory action which is distributed to the perivascular smooth muscle through the electrical stimulation of the smooth muscle of coronary artery of pig. The effects of perivascular nerve stimulation were investigated on isolated coronary artery of pig.1 . The magnitude of contractile response to perivascular nerve stimulation increased with increasing frequency (2-80 Hz) of stimulation. 2. The contractions to perivascular nerve stimulation(40V, 40Hz. 0.5msec, 1 min) were increased greatly by pretreatment of the cholinestrase inhibitor physostigmine. 3. The contraction to perivascular nerve stimulation(40V,40Hz, 0.5msec, 1min) was antagonised markedly by the muscarinic antagonist atropine. 4. The contraction to perivascular nerve stimulation(40V, 40Hz, 0.5msec, 1 min) was blocked by the neural blocker tetrodotoxin. 5. The contractions to perivascular nerve stimulation(40V. 40Hz, 0.5msec, 1 min) were not affected significantly by the -adrenergic antagonist phentolamine or - adrenergic antagonist propranolol. 6. The contractile response by the acetylcholine was increased by the pretreatment of cholinestrase inhibitor physostigmine. The finding suggest that it is powerful excitatory action linked to muscarinic receptor by cholinergic nerve in coronary artery of pig.

Changes in Nerve Excitability Depending on Intensity of Neural Stretching (신경 신장 적용 강도에 따른 신경흥분성 변화)

  • Purpose: Neurodynamic tests are used to examine neural tissue in patients with neuro-musculoskeletal disorders, although this has not yet been established in the intensity of nerve tension application. This study aimed to investigate the acute effects of neural stretching intensity on nerve excitability using the latency and amplitude of nerve conduction velocity test (NCV) analysis. Methods: Thirty young, healthy male and female subjects (mean age = 21.30 years) voluntarily participated in this study. Nerve excitability was assessed using the median sensory NCV test. The latency and amplitude of the NCV test were measured under four different conditions: reference phase (supra-maximal stimulus, without neural stretching), baseline phase (2/3 of the supra-maximal stimulus, without neural stretching), weak stretch phase (2/3 of the supra-maximal stimulus, with weak neural stretching), and strong stretch phase (2/3 of the supra-maximal stimulus, with strong neural stretching). Results: The NCV latency was significantly delayed after one minute of neural stretching at the baseline, weak phase, and strong phase in comparison with the reference phase. The NCV latency was significantly delayed by increasing the strength of neural stretching. Furthermore, the NCV amplitude was significantly increased at the weak and strong phases, which were under neural stretching, in comparison with the baseline phase. The NCV amplitude was significantly increased by increasing the strength of the neural stretching. Conclusion: Transient neural stretching as a neurodynamic test can increase the sensitivity of the nerve without negatively affecting the nervous system. However, based on the results of this study, strong neural stretching in the neurodynamic test may delay the transmission of nerve impulses and hypersensitivity.

  • Objective: Facial nerve palsy causes both facial muscle malfunction and mental illness. Because a facial nerve palsy lesion shows on the face, it can have serious effects on social relationships and mental health. Many facial nerve palsy patients undergo anxiety, depression, and social phobia. In this study, a facial nerve palsy patient with sleep disorder was admitted to the Korean medicine hospital for treatment. Methods: The patient with facial nerve palsy and sleep disorder was treated with herbal medicines, acupuncture, herbal acupuncture therapy, and physical therapy. We used the House Brackmann grading system and Yanagihara's Unweighted Grading System to assess changes in facial nerve palsy symptoms and the Korean Modified Leeds Sleep Evaluation Questionnaire to assess the sleep disorder. Results: The patient was hospitalized for 18 days and showed a recovery from both facial nerve palsy and sleep disorder symptoms without any adverse events. We conclude that patients with facial nerve palsy should be treated from both the physical and mental health perspectives.

Cryoanalgesia vs Bupivacain Intercostal Nerve Block for the Post -thoracotomy Pain (개흉술 후 흉부동통억제를 위한 늑간신경냉동 요법과 약물주입요법에 관한 비교연구)

  • Post-thoracotomy pain is so severe that lead to postoperative pulmonary complications due to the patient`s inability to cough, deep breathing and chest wall motion. Many authors have been trying to reduce the post-thoracotomy pain, but there is no method of complete satisfaction. In 1990, we reported the result that the cryoanalgesia reduce the immediate postoperative pain significantly. We try to compare the effect of cyroanalgesia with the effect of Bupivacain intercostal nerve block for the post-thoracotomy pain control. Ninety patients, who undergoing thoracotomy in Korea University Medical Center Guro Hospital between the January 1993 and September 1993, were evaluated. The patients were divided into three groups : Group A, control, the patients without pain control procedure [N=30], Group B, the patients with Bupivacain intercostal nerve block [N=30], Group C, the patients with cryoanalgesia [N=30]. Postoperative analgesic effects were evaluated by the scoring system which made arbiturary by author. The results were that the Bupivacain intercostal nerve block markedly reduced the immediate postoperative pain compare with two other groups, the cryoanalgesia reduced the immediate postoperative pain significantly compare with control group, the pain reduction effect of the two groups- cryoanalgesia and Bupivacain intercostal nerve block- were not different at postoperative seventh day, probably due to the action time of Bupivacain and the result that there were no significant complications of the procedures.

Case Report of Korean Medical Treatment on Acute Peroneal Nerve Palsy Patient Caused by Prolonged Immobilization (장시간 부동 자세로 야기된 급성 비골신경 마비 환자의 한방치료 증례보고)

  • Objectives This study was carried out to investigate the clinical effects of Korean medical treatment on acute peroneal nerve palsy caused by prolonged immobilization. Methods One patient with acute peroneal nerve palsy was treated with various korean medical techniques such as acupuncture, moxibustion, cupping and herbal medicine, bee-venom acupuncture for 12 weeks. Manual Muscle Test, Range of motion, Numerical Rating Scale, Ankle-hindfoot scale, Digital infrared thermographing imaging system were used to evaluate treatment effect. Results After treatment, all the scales mentioned above were improved significantly. Conclusions This result showed that Korean medical treatment is effective on acute peroneal nerve palsy caused by prolonged immoblization.

The Effect of Manual Lymph Drainage on the Changes of Autonomic Nervous System and Pain in Stressed Hospital Office Employees

  • This study is to investigate the effects of manual lymphatic drainage of stressed hospital office employees on the autonomic nervous. A total of 30 stressed hospital office women voluntarily participated in the study. The participants were randomized to the manual lymphatic drainage groups and rest groups. The intervention was conducted for 20 minutes in each group. There were significant differences in sympathetic nerve, parasympathetic nerve, and pain within manual lymphatic drainage groups(p<.05). There were significant differences between groups for the sympathetic nerve, parasympathetic nerve, and pain(p<.05). Therefore, manual lymphatic drainage is an effective intervention for reducing the stress and pain of stressed hospital office employees.

Analysis of Pharmacopuncture Used in Facial Nerve Palsy Articles that Published in the Journal of Korean Medicine (한의학 학술지에 게재된 안면신경마비 증례 연구에서 사용된 약침에 관한 고찰)

  • Objectives : The purpose of this study is to analyze the trends of phamacopuncture used in facial nerve palsy related articles that have been published in the Korean medicine journal. Methods : 242 papers related to facial nerve palsy were retrieved from Oriental Medicine Advanced Searching Integrated System(OASIS) and Korean Traditional Knowledge Portal(KTKP). We selected 13 research papers treated with pharmacopuncture for facial nerve palsy. We analyzed for type of pharmacopuncture used, treatment point and clinical type. Results : 1. The number of searched articles is thirteen. 2. The most common clinical type of facial nerve palsy is Bell's palsy. Other types are accompanying tympanitis type, herpes zoster oticus, caused by traumatic temporal bone fracture type, Foville syndrome, Foville-Millard-Gubler syndrome and multiple cerebral neuritis. 3. In the case of pharmacopuncture, the most frequently used type is Hominis Placenta Pharmacopunture. The second is Hwangreonhaedok-tang and Hwangreon Pharmacopuncture. Other than that BV, Sinbaro, Jungsongouhyul Pharmacopuncture, Cornu Cervi Pantotrichum Pharmacopuncture and Ginseng pharmacopuncture are used. 4. The most frequently used part as a treatment point is ST4, ST6, TE17.
