The Effects of High-frequency, Non-noxious TENS on RIII Nociceptive Flexion Reflex and Temporal Summation in Human Subjects (정상인에서 고빈도의 무통증성 경피적 신경자극이 RIII Nociceptive Flexion Reflex와 Temporal Summation에 미치는 영향)

  • Background: Transcutaneous electrical nerve stimulation (TENS) has been used widely, but its effects are controversial. This is probably due to the varying intensity and type of pain. We designed a study to assess the effects of the TENS on the RIII nociceptive flexion reflex as the resting pain level and the temporal summation as a repeated, movement related pain in 7 normal volunteer subjects. Methods: High frequency (80 Hz), non-noxious TENS was applied over the left popliteal fossa for 20 minutes. Ipsilateral RIII reflexes induced by single electrical stimulus and temporal summation of pain responses to repeated stimuli (five stimuli at 2 Hz) were recorded before, during (just before stopping), and subsequently at 20 minutes after TENS. Results: R (III) nociceptive flexion reflex activity during and after TENS was more significantly decreased than before treatment. However, the temporal summation threshold was not changed. Conclusions: We conclude that high frequency, non-noxious TENS could be effective on resting pain relief in the same segment but not on the movement related pain.

  • Canavan disease, also known as van Bogaert-Bertrand disease, is a rare autosomal recessive disorder characterized by early an onset and a progressive spongyform degeneration of the brain, associated with an edema of the central nerve system, intramyelinic swelling and neurologic symptoms. This disorder is most prevalent in people of Ashkenazi Jewish descent but has been observed in other ethnic groups. Patients have severe mental retardation, poor head control, macrocephaly and seizures. Canavan disease is caused by the accumulation of N-acetylaspartic acid(NAA) in the brain as the result of a deficiency of aspartoacylase(ASPA) activity. Most children are reported to have the infantile form, becoming symptomatic between three and six month of age, after unremarkable prenatal and perinatal course. We experienced a case of Canavan disease in a six day old female newborn baby, associated with seizure, degeneration of brain white matter and markedly elevated urine N-acetylaspartic acid(NAA) level. So, we report the case with a brief review of the related literature.

The Effects of Abdominal Relaxed Breathing Training on Stress response and Blood Pressure for Elderly Hypertensionve Patient (고혈압 노인에서 복식호흡 이완훈련이 혈압 및 스트레스 반응에 미치는 영향)

  • In order to improve self-management of elderly hypertensives in this study, the researcher examined the effect of blood pressure regulation by an abdominal relaxed breathing training which regulates physical response to stress and lessens the activity of the sympathetic nerve. Method: A quasi-experimental pre-test and post-test design was used. Thirty elderly hypertensive patient ; Sixteen in the abdominal breathing group and fourteen in the control group participated in this study. The abdominal relaxed breathing training consisted of eight sessions twice a week for four weeks. There was no intervention for the control group. Result: 1. There was a significant decrease in systolic and diastolic blood pressure between the experimental group and control group over three different times, and interaction by groups and over time. 2. There was no significant difference in the level of total stress response, physical stress response, behavioral-cognitive stress response between the experimental group and control group over three different times, and interaction by groups and over time. Conclusion: Findings indicate that this study will contribute to develop nursing strategies for the regulation of blood pressure for the elderly, which is easy for the elderly to learn as a nonpharmacologic approach.

Intracellular cAMP-modulated Gate in Hyperpolarization Activated Cation Channels

  • Hyperpolarization-activated nonselective cation channels (HCNs) play a pivotal role in producing rhythmic electrical activity in the heart and the nerve cells. In our previous experiments, voltage-dependent $Cd^{2+}$ access to one of the substituted cysteines in S6, T464C, supports the existence of an intracellular voltage-dependent activation gate. Direct binding of intracellular cAMP to HCN channels also modulates gating. Here we attempted to locate the cAMP-modulated structure that can modify the gating of HCN channels. SpHCN channels, a sea urchin homologue of the HCN family, became inactivated rapidly and intracellular cAMP removed this inactivation, resulting in about eight-fold increase of steady-state current level. T464C was probed with $Cd^{2+}$ applied to the intracellular side of the channel. We found that access of $Cd^{2+}$ to T464C was strongly gated by cAMP as well as voltage. Release of bound $Cd^{2+}$ by DMPS was also gated in a cAMP-dependent manner. Our results suggest the existence of an intracellular cAMP-modulated gate in the lower S6 region of spHCN channels.

  • 본 연구에서는 외부 자극과 내적인 교감신경의 변화에 따라 나타나는 땀 반응으로 얻어진 생체신호를 다루었다. 즉, 땀 반응을 지칭하는 피부전도도를 측정할 수 있는 시스템과, 더불어 차량 운전시 혹은 운동 등의 동적 자극에 의한 영향을 최소화하여 교감신경에 의한 피부전도도의 신뢰도를 높일 수 있는 방안을 강구하였다. 기존의 두 개의 전극 간 리지스턴스(Resistance)를 측정하는 방식을 내부에서 분리된 한 개의 전극을 사용하여 신체적 움직임의 제한점을 해소하고 피부전도도로 나타난 생체신호가 외적인 움직임에 따른 영향인지 내적인 신경의 변화에 따른 영향인지를 추정할 수 있는 모듈을 개발하였다. 20명의 건강한 청년을 대상으로 피실험하여 기존의 생체신호 측정 장비인 MP-150의 EDA모듈과 동시 측정한 결과, 두 데이터가 약 88% 일치하는 것으로 확인하였다.

Pain Physiology and Principles of Physical Therapy (통증 생리와 물리치료 원리)

  • The pain is common among individuals with physical disabilities. It can interfere with therapy since patients with pain can become uncooperative and reluctant to move. This paper reviews the natural physiological mechanisms that can reduce pain perception, and considers physiological mechanisms which contribute to clinical pain by describing how the pain system changes its sensitivity depending upon the body's needs. The peripheral and central mechanisms contributing to sensitised nociceptive system are described with reference to the symptoms of clinical pain such as hyperalgesia, allodynia sopntaneous 'on-going'-projected and referred pain. It is suggested that in some chronic pain the nociceptive system maintains a state of sensitivity despite the absence of on-going tissue damage and under such circumstances the nociceptive system itself may have become dysfunctional. Such situations are often initiated by damage to nervous tissue which results in changes in the activity and organization of neuronal circuits within the central nervous system. The ability of the nociceptive system to operate in a suppressed state is also discussed with reference to pain modulation. The physical therapist can help facilitate the activation of these mechanisms through a combination of noninvasive modalities, functional activities, and the therapeutic use of self.

  • This study proposed a new algorithm to assess autonomic function activity using Time-Frequency Representation(TFR). TFR is a way of describing the time-valiant energy of a signal. A discrete Wigner representation that is capable of filtering out any cross terms occuring in the Wigner-Ville Distribution(WVD) is used for time-variant energy distribution of heart rate variability(HRV) signals. And the marginal condition are evaluated to estimate power spectrum of HRV signals. The proposed algorithm showed that estimated power spectrum of HRV signals well describe the autonomic nerve system function and also showed the dynamics of autonomic nervous system response.

Requirement of EGF Receptor Kinase for Signaling by Calcium-Induced ERK Activation and Neurite Outgrowth in PC12 Cells

  • Membrane depolarization in PC12 cells induces calcium influx via an L-type voltage-sensitive calcium channel (L-VSCC) and increases intracellular free calcium, which leads to tyrosine phosphorylation of epidermal growth factor (EGF) receptor and the associated adaptor protein, She. This activated EGF receptor complex then can activate mitogen-activated protein (MAP) kinase, as in nerve growth factor (NGF) receptor activation. In the present study, we investigated the role of EGF receptor in the signaling pathway initiated by membrane depolarization of PC12 cells. Prolonged membrane depolarization induced phosphorylation of extracellular signal-regulated kinase (ERK) within 1 min in undifferentiated PC12 cells. Pretreatment of PC12 cells with the calcium chelator EGTA abolished depolarization-stimulated ERK phosphorylation, but NGF-induced phosphorylation of ERK was not affected. The chronic treatment of phorbol ester, which down-regulated the activity of protein kinase C (PKC), did not affect the phosphorylation of ERK upon depolarization. In the presence of an inhibitor of EGF receptor, neither depolarization nor calcium ionophore increased the level of ERK phosphorylation. These data imply that the EGF receptor is functionally necessary to activate ERK and neurite outgrowth in response to the prolonged depolarization in PC12 cells, and also that PKC is apparently not involved in this signaling pathway.

Effects of Interferential Current Stimulation on the Peripheral Blood Velocity in Healthy Subjects (간섭전류자극이 말초 혈류속도에 미치는 영향)

  • The purpose of this study was to determine whether percutaneous interferential current stimulation on thoracic sympathetic ganglia with amplitude modulated frequency (AMF) $90\~100$ bps and subthreshold of muscle contraction for 10 minutes on peripheral blood flow velocity in healthy subjects. Thirty-seven healthy volunteers were assigned randomly into an experimental group (n=25) and a control group (n=12). the experimental group received interferential current stimulation with subthreshold of the muscle contraction of current at AMF $90\~100$ bps on $1st\~5th$ thoracic sympathetic ganglial region for 10 minutes. The control group received same handling and electode placement, but no current was applied. Using a Doppler blood flow meter, the radial arterial blood flow velocities and the pulse raters were determined for two-way analysis of variance for repeated measures on time and group. There were no significant difference between the two groups with respect to the changes in arterial blood flow velocity and pulse rate over the four measurement times. Interferential current stimulation did not change in mean blood flow velocity and pulse rate. We conclude that interferential current stimulation on the thoracic sympathetic ganglia, as used in this study, did not dilate peripheral artery. This results suggests that interferential current stimulation dose not alter the activtiy of sympathetic nerve.

  • Capsaicin. a vanillyl amide(8-methy1-N-vanilly1-6-nonenamide) with a molecular weight of 305.42, was substance, interrupting the pain conducting pathway Until recently the neurotoxic effects of Capsaicin to adult animals were thought to be limited to the peripheral nervous system. But several reports suggest the possibility of central nervous system changes after Capsaicin administration to the adult rat. Capsaicin desensitization is defined as long lasting, reversible suppression of sensory neuron activity. How fast and for how long the desensitization develops is related to the dose and time of exposure to Capsaicin, and the interval between consecutive dosing. In the long term Capsaicin treatment can lead to morphological degeneration and changes in some small sensory neurons, predominantly unmyelinated C fiber afferent nerve fibers. Clinical interest has recently been roused by evidence that Capsaicin's desensitizing action may be of therapeutic value and that an endogenous Capsaicin-1 ike substance may exist. This study summarizes the fundamental knowledge(mechanism, receptors, et al of Capsaicin) of Capsaicin for physical therapists.
