Protective Effect of Physostigmine and Neostigmine against Acute Toxicity of Parathion in Rats

  • The effects of physostigmine and neostigmine on the parathin induced toxicity were examined in adult female rats. Physostigmine $(100\;{\mu}g/kg,\;ip)$ or neostigmine $(200\;{\mu}g/kg,\;ip)$ inhibited acetylcholinesterase (AChE) and cholinesterase (ChE) activities in blood, brain and lung when the enzyme activity was measured 30 min after the treatment. At the doses of two carbamates equipotent on brain AChE, neostigmine showed greater inhibition on peripheral AChE/ChE. The enzyme activity returned to normal in 120 min following the carbamates except in the lung of rats treated with neostigmine. Carbamates administered 30 min prior to parathion (2 mg/kg) antagonized the inhibition of AChE/ChE by parathion when the enzyme activity was measured 2 hr following parathion. Neostigmine showed greater protective effect on peripheral AChE/ChE. The effect of either carbamate on AChE/ChE was not significant 2 hr beyond the parathion treatment. Carbamates decreased the mortality of rats challenged with a lethal dose of parathion (4 mg/kg, ip) either when treated alone or in combination with atropine (10 mg/kg, ip). Lethal action of paraoxon (1.5 mg/ks ip), the active metabolite of parathion was also decreased by the carbamate treatment indicating that the protection was not mediated by competitive inhibition of metabolic conversion of parathion to paraoxon. The results suggest that carbamylation of the active sites may not be the sole underlying mechanism of protection provided by the carbamates.

Laparoscopic Cholecystectomy 수술 후 Sugammadex와 Neostigmine 간에 첫 자발호흡과 부작용 발현 연구 (Comparison of Sugammadex and Neostigmine on First Spontaneous Breathing and Adverse Effects for Laparoscopic Cholecystectomy)

  • Objective: The purpose of the study was to investigate the time from the injection of muscle relaxants to the first spontaneous respiration between sugammadex and conventional reversal for patients undergoing laparoscopic cholecystectomy. Methods: This study was retrospectively conducted on patients who were diagnosed with gallbladder stone (N802) between January 2014 and April 2017. The data were collected from the electronic medical records of a total of 186 patients (84 patients in the neostigmine group and 102 patients in the sugammadex group). Results: The time required for the first spontaneous respiration in the sugammadex group was shorter than that in the neostigmine group (3.6 min vs 4.9 min; p<0.05). After the injection of intermediate muscle relaxants, the comparison of heart rate and mean arterial pressure in the sugammadex and neostigmine groups revealed that the heart rate in the neostigmine group was higher than in the sugammadex group after 5 min (p<0.05). The mean arterial pressure in the neostigmine group was higher than in the sugammadex group after 10 min (p<0.05). A significant adverse effect of tachycardia was observed in the neostigmine group (p<0.05), but the frequency of rescue antiemetic in the sugammadex group was significantly higher than in the neostigmine group (p<0.05). Conclusion: In this study, the unwanted effect of neostigmine group was tachycardia; therefore, in the case of patients with hemodynamic instability, sugammadex is recommended. At 12 hours after the injection of sugammadex to patients, more antiemetics were required than in the neostigmine group; therefore, more research should be conducted on postoperative nausea and vomiting.

  • 1. 척수가토(脊髓家兎)에서 nicotine, DMPP, McN-A-343, AHR-602, tyramine, angiotensin및 neostigmine에 의한 심박증가(心搏增加)의 작용점(作用點)을 조사(調査)하였다. 2. 상기약물(上記藥物)에 의한 심박증가(心搏增加)는 reserpine 처리가토(處理家兎)에서는 대단히 약(弱)하였고, Propranolol 투여후(投與後)에는 거의 나타나지 않았다. 3. Tetrodotoxin, guanethidine은 isoproterenol, nicotine, DMPP, tyramine에 의한 심박증가(心搏增加)에는 영향(影響)을 미치지 않았으나 McN-A-343, AHR-602, angiotensin, neostigmine에 의한 심박증가(心搏增加)는 이를 현저(顯著)히 약화(弱化)시켰다. 4. Nicotine, DMPP의 심박증가(心搏增加)는 chlorisondamine으로 McN-A-343, AHR-602의 심박증가(心搏增加)는 atropine으로 각각(各各) 억제(抑制)되었다. 5. Isoproterenol, nicotine, DMPP, McN-A-343, tyramine, angiotensin, neostigmine은 적당량(適當量)을 우심방내(右心房內)에 주입(注入)하였을때 이들을 이정맥내(耳靜脈內)에 주사(注射)하였을 때와 비슷한 반응(反應)을 일으킬 수 있었다. 그러나 AHR-602 우심방내(右心房內) 주입(注入)은 유의(有意)한 심박증가(心搏增加)를 일으키지 않았다. 6. Nicotine, DMPP, neostigmine은 우심방내(右心房內) 주입시(注入時)는 현저(顯著)한 심박증가(心搏增加)를 일으켰으나, 동량(同量)을 우심실내(左心室內)에 주입(注入)하였을때는 증가(增加)를 일으키기 않았다. Isoprotereool 및 tyramine에 대한 반응(反應)은 우심실내(右心室內) 주입(注入)보다도 우심방내(右心房內) 주입시(注入時) 훨씬 컸다. 7. McN-A-343, angiotensin의 우심실내(右心室內) 주입시(注入時)는 우심방내(右心房內) 주입시(注入時)보다도 훨씬 현저(顯著)한 심박증가(心搏增加)를 일으켰고, AHR-602의 우심실내(左心室內) 주입(注入)도 현저(顯著)한 반응(反應)을 일으켰다. 8. Nicotine, DMPP, tyramine은 심장(心臟)의 aenergic nerve의 말단(末端) 또는 extraneuronal norepinephrine-store에 작용(作用)하여, Mcn-A-343, AHR-602, angiotensin은 심장(心臟)의 adrenergic nerve에 작용(作用)하여 심박증가(心搏增加)를 일으키며, nicotine, DMPP, tyramine, neostigmine의 작용점(作用點)은 주(主)로 우심방(右心房)에 McN-A-343, AHR-602, angiotenisin의 작용점(作用點)은 주(主)로 우심실(右心室)에 존재(存在)하는것으로 추론(推論)하였다.

옥시 토신과 부교감신경흥분제(副交感神經興奮劑)가 돼지의 사산(死産)에 미치는 영향(影響) (Effects of Oxytocin and Parasympathomimetic Drugs on Porcine Stillbirth)

  • To study the effects of oxytocin and three parasympathomimetic drugs such as neostigmine, bethanechol, and pilocarpine in reduction of interval between birth and incidence of intrapartum stillborn pigs, 213, sows and gilts which had been kept under the same condition, were randomly allotted to 4 treatment and one control groups. The side reactions of three parasympathomimetic drug were also examined. The results obtained were summarized as followings. 1. Parasympathomimetic-treated groups revealed tendence to shorter interval between birth than oxytocin-treated or control groups. 2. Stillbirth rate per litter was significantly less (p<0.01) in the parasympathomimetic-treated groups than in the oxytocin-treated or control groups. 3. Moderate to severe salivation and vomiting were found in many clams of the pilocarpine-treated and bethanecol-treated groups. The neostigmine-treated group showed nearly no side reaction and neostigmine found to the safest among three parasympathomimetic drugs.

항코린에스테라제 활성에 대한 파파베린 및 디펜히드라민의 효과 (Effect of Papaverine and Diphenhydramine on the Action of Cholinesterase Inhibitors)

  • This study was designed to examine the effects of diphenhydramine and papaverine on the toxic manifestations of cholinesterase inhibitors. It was found that papaverine increase acetylcholinesterase activity in cerebral cortex of mice. Papaverine pretreatment tended to increase acetylcholinesterase activity against the actions of neostigmine and physostigmine. When diphenhydramine (20~30 mg/kg, s.c.) was treated 20 min before the administration of cholinesterase inhibitor, it significantly extended the onset latency in the signs of toxicosis which were characteristically produced by physostigmine (0.25~1.5 mg/kg, s.c.) or neostigmine (0.125~0.5 mg/kg, s.c.), and it also prevented lethality in all of the animals.

Laryngeal Paralysis in a Cat with Malignant Thymoma

  • A 10-year-old castrated male Domestic Shorthair cat visited a veterinary medical teaching hospital for emergency dyspnea. The cat was hypoxic and hypotensive, and stridorous respiration was remarkable. Visual inspection confirmed laryngeal paralysis and a lack of mobility of the left larynx. Megaesophagus, aspiration pneumonia, cranial mediastinal mass, and positive Tensilon test results using neostigmine were observed, indicating acquired myasthenia gravis secondary to thymic neoplasia. After 10 minutes of neostigmine 0.02 mg/kg IV administration, laryngeal paralysis and dyspnea resolved. Histopathlogical examination for the cranial mediastinal mass after surgical resection confirmed malignant thymoma. Here, we report a case of acquired myasthenia gravis in a cat with a malignant thymoma that presented with life-threatening dyspnea due to laryngeal paralysis. Feline laryngeal paralysis is uncommon, and myasthenia gravis, a cause of laryngeal paralysis in cats, has not yet been reported. Myasthenia gravis should be considered in cats with laryngeal paralysis.

Collagen 유발(誘發) 관절염(關節炎) 동물모델에 대(對)한 전침자극(電鍼刺戟)의 진통효과(鎭痛效果) 및 그 기전(機轉)에 관(關)한 연구(硏究) (The Study on the Analgesic Effect and its Cholinergic Mechanism of Electroacupuncture in the Rat Model of Collagen-induced Arthritis)

  • Objectives : To investigate the analgesic effect and its cholinergic mechanism of electroacupuncture(EA) in the rat model of collagen-induced arthritis(CIA). Methods : Immunization of male Sprague-Dawley rats with bovine typeII (CII) collagen emulsified in Freund's incomplete adjuvant, followed by a booster injection 14 days later, leads to development of arthritis in more than 70% of rats by 21 days postinjection. After three weeks of first immunization, EA stimulation(2 Hz, 0.07 mA, 0.3 ms) was delivered into Jogsamni($ST_{36}$) for 30 minutes. Analgesic effect was evaluated by tail flick latency(TFL). We compared the analgesic effect of EA with TFLs between pretreatment of normal saline and pretreatment of Atropine (1 mg/kg, intraperitoneal) and Neostigmine ($100{\mu}g/kg$, intraperitoneal) in CIA. Results : 1. TFLs were gradually decreased in CIA as increasing severity of arthritis. 2. Jogsamni($ST_{36}$) EA stimulation in CIA increased TFLs and the effect lasted for 60 minutes. 3. Increased TFLs with Jogsamni($ST_{36}$) EA stimulation were inhibited with pretreatment of atropine in CIA 4. Increased TFLs with Jogsamni($ST_{36}$) EA stimulation did not show an obvious synergistic effect with pretreatment of neostigmine in CIA. Conclusions: Jogsamni($ST_{36}$) EA showed analgesic effects in CIA. The analgesic effects of Jogsamni($ST_{36}$) EA were inhibited by atropine pretreatment and combined application of Jogsamni(ST36) EA and neostigmine did not show an synergistic effect. These observations suggest that intrinsic muscarinic cholinergic pathways represent an important modulating system in pain perception of inflammatory pain in CIA It is suggested that, the active mechanism of analgesic effect in EA may involve the release of acetylcholine in the spinal cord.

  • 제왕절개술 환자 10인에게 Succinylcholine Chloride 및 atracurium을 준 결과 다음과 같은 결론을 얻었다. 1. Succinyline chloride 1mg/kg와 atracurium 0.4mg/kg를 주어 충분한 근육이완을 유지하였다. 2. 혈압과 맥박은 atracurium주사 후 의의있는 변화는 없었다. 3. 태아의 Apgar score는 1분과 5부때 8~10점으로 모두 양호한 상태였다.

코브라과 뱀에 물린 환자에서 코브라 항독소를 사용하여 치료한 1례 (A Case of Cobra Antivenom Therapy in a Patient Bitten by Elapid Snake in South Korea)

  • Elapid snakes have neurotoxic venom which causes diverse neuroparalytic manifestations, including fatal respiratory failure. In South Korea, since elapid snakebites are very rare, the cobra antivenom, which is effective against neurotoxicity, was only introduced recently. Most physicians in South Korea have little experience in the treatment of patients who have been bitten by elapid snakes. A 19-year-old man was brought to the emergency department with sudden diplopia, 1 hour after a snakebite on the left 2nd finger. The patient presented with drowsiness and complained of mild dizziness and binocular diplopia. After 1 hour, he had sudden onset of dyspnea and dysphagia and appeared to be agitated. He was immediately intubated and received mechanical ventilation as he was unable to breathe on his own. A total of 2.5 mg of neostigmine diluted with normal saline was slowly infused, and 1 vial of cobra antivenom was infused for an hour, 5 times every 2 hours, for a total of 5 vials. He slowly recovered self-breathing; on the 3rd day of hospitalization, he showed tolerable breathing and was extubated. He was discharged without any neurological deficits or other complications.

위암 수술 후 장마비에 대한 한의학적 치료의 효과 : 체계적 문헌 고찰 및 메타분석 (The Effectiveness of Traditional Korean Medicine for Treating Postoperative Ileus in Gastric Cancer Patients: A Systematic Review and Meta-Analysis)

  • 목적: 본 연구는 위암 수술 후 장마비의 한의학적 치료 효과를 평가하기 위해 수행하였다. 방법: 2007년 1월 1일부터 2022년 5월 11일까지 출판된 연구들을 6개의 데이터베이스를 통해 수집하였다. 수술 후 한의학적 치료를 받은 위암 환자를 장마비 관련 지표들을 통해 관찰한 연구들을 선정하였다. 결과: 한약, 침, 뜸, 전기혈위자극, 이혈요법, 족삼리혈의 약물 주입을 수술 후 장마비 치료법으로 사용한 27편의 연구를 선정하였다. 한약, 한약과 침 병용요법, 침, 뜸, 전기혈위자극, 이혈요법, 족삼리혈의 네오스티그민 주입 치료군에서 수술 후 첫 가스 배출까지의 시간이 감소하였고 (p<0.00001), 수술 후 첫 배변까지의 시간이 한약. 한약과 침 병용요법, 침, 뜸, 전기혈위자극, 이혈요법, 족삼리혈의 네오스티그민 주입 치료군에서 유의미하게 감소하였다 (p<0.00001). 심각한 이상반응은 나타나지 않았다. 결론: 한의학적 치료는 위절제술 후 위암 환자에게 수술 후 장마비의 예방과 치료법으로 활용될 수 있으며 수술 후 장마비 치료의 임상적 효과를 명확히 하기 위해 후속 연구가 필요하다.
