Relationship between Colonic Polyp Type and the Neutrophil/Lymphocyte Ratio as a Biomarker

  • Aim: We designed this study to investigate the neutrophil lymphocyte ratio as a biomarker in distinguishing colonic polyps which are neoplastic or non-neoplastic. Materials and Methods: One hundred and twenty-five patients with colonic polyps were enrolled into the study. The following data were obtained from a computerized patient registry database: mean platelet volume (MPV), uric acid (UA), platelet count (PC), alkaline phosphatase (ALP), gamma-glutamyl transpeptidase (GGT) and the neutrophil to lymphocyte ratio (NLR). Exclusion criteria were active infectious disease, hematological disorders, and malignancies. Colonic polyps divided into two groups as neoplastic polyps (tubular adenoma, villous adenoma, tubulovillous adenoma) and non-neoplastic polyps (hyperplastic polyps, inflammatory pseudopolyps etc). The relationship between colonic polyp type and NLR was evaluated with statistical analysis. Results: There were 67 patients (53.6%) with neoplastic and 58 (46.4%) patients with non-neoplastic polyps. Mean NLRs of neoplastic and non-neoplastic groups were respectively $3.32{\pm}2.54$ and $2.98{\pm}3.16$ (P<0.05). Conclusion: Although sensitivity and specificity are not high, NLR may be used as a biomarker of neoplastic condition of colonic polyps.

주타액선부위에 발생한 종괴의 비교분석 (The Relative Incidence of Neoplastic Versus Non-neoplastic Mass in Major Salivary Gland Area Diagnosed by Surgery)

  • It is the purpose of this paper to determine the relative frequency of the neoplastic vs. non-neoplastic disease in a series of surgically removed parotid and submandibular masses. The study was conducted a retrospective review of 249 consecutive, operated patients' records seen over a 9-year period from 1985 through 1993 at Department of Otolaryngology-Head and Neck Surgery, Korea Cancer Center Hospital. One hundred and fifteen patients had parotid mass and 134 patients had submandibular mass. All the specimen surgically removed were studied histopathologically. The numbers and percentages of patients are reported along with a pathological classification of the lesions encountered: neoplastic(benign, malignant) and non-neoplastic (inflammatory, non-inflammatory). There were 59 benign, 46 malignant, 6 inflammatory, 4 non-inflammatory in 115 parotid mass, and 33 benign, 48 malgnant, 47 inflammatory, 6 non-inflammatory in 134 submandibular mass. One-hundred-eighty-six of entire 249 patients had tumor involvement, for a 74.7% incidence of neoplasia. There was a neoplastic predominance in parotid mass(91.3%) and a relative inflammatory predominance in submandibular mass(35.1%).

Utility of narrow-band imaging with or without dual focus magnification in neoplastic prediction of small colorectal polyps: a Vietnamese experience

  • Background/Aims: Accurate neoplastic prediction can significantly decrease costs associated with pathology and unnecessary colorectal polypectomies. Narrow-band imaging (NBI) and dual-focus (DF) mode are promising emerging optical technologies for recognizing neoplastic features of colorectal polyps digitally. This study aimed to clarify the clinical usefulness of NBI with and without DF assistance in the neoplastic prediction of small colorectal polyps (<10 mm). Methods: This cross-sectional study included 530 small colorectal polyps from 343 consecutive patients who underwent colonoscopy at the University Medical Center from September 2020 to May 2021. Each polyp was endoscopically diagnosed in three successive steps using white-light endoscopy (WLE), NBI, and NBI-DF and retrieved for histopathological assessment. The diagnostic accuracy of each modality was evaluated with reference to histopathology. Results: There were 295 neoplastic polyps and 235 non-neoplastic polyps. The overall accuracies of WLE, WLE+NBI, and WLE+NBI+NBI-DF in the neoplastic prediction of colorectal polyps were 70.8%, 87.4%, and 90.8%, respectively (p<0.001). The accuracy of WLE+NBI+NBI-DF was significantly higher than that of WLE+NBI in the polyp size ≤5 mm subgroup (87.3% vs. 90.1%, p<0.001). Conclusions: NBI improved the real-time neoplastic prediction of small colorectal polyps. The DF mode was especially useful in polyps ≤5 mm in size.

Uterine Adenocarcinoma in a Lionhead Rabbit

  • An 8-year-old, female lionhead rabbit with clinical sign of hematuria and vaginal discharge with/without blood was submitted to a local animal hospital. On exploratory laparotomy, three round to oval masses were observed in both uterine horns. The lumen of uterus was severely obstructed and distorted because of massive neoplastic proliferation. Histopathologically, the uterine masses revealed papillary projections along with irregular glandular structures into the lumen. The neoplastic foci were composed of numerous irregular sized neoplastic glands originated from uterine glands. These neoplastic cells showed very strong invasive tendency to muscle layer, therefore emboli of neoplastic cells were located in lymphatics. According to immunohistochemistry, the tumor cells in uterine masses demonstrated strong positive signals for cytokeratin, but negative for vimentin. Based on the gross, histopathologic and immunohistochemical features, this case was diagnosed as uterine adenocarcinoma in lionhead rabbit.

CK2 Enzyme Affinity Against c-myc424-434 Substrate in Human Lung Cancer Tissue

  • CK2 is a serine threonine kinase that participates in a variety of cellular processes with more than 300 defined substrates. This critical enzyme is known to be upregulated in cancers, but the role of this upregulation in carcinogenesis is not yet fully understood but c-myc, one of the defined CK2 substrates, is a well-known proto-oncogene that is normally essential in developmental process but is also involved in tumor development. We evaluated the optimal enzyme and substrate concentrations for CK2 activity in both neoplastic and non-neoplastic human lung tissues using the c-$myc^{424-434}$ peptide (EQKLISEEDL) as a substrate. The activities measured for the neoplastic tissue were 600-750 U/mg protein while those for the control tissue was in the range of 650-800 U/mg. $K_m$ value for c-myc peptide was determined as $0.33{\mu}M$ in non-neoplastic tissue and $0.18{\mu}M$ in neoplastic tissue. In this study, we did not observe an increased activity in the neoplastic tissue when compared with the non-neoplastic lung tissue, but we recorded two times higher affinity for c-$myc^{424-434}$ in cancer tissue. Considering the metabolic position of c-$myc^{424-434}$, our results suggest that phosphorylation by CK2 may be important in dimerization and thus it might affect the regulation of c-myc in cancer tissues.

Analysis of premature death of Sprague-Dawley rats in carcinogenicity studies

  • To help the interpretation of causes of death, it is critical that the background incidence of factors contributing to death be recorded and archived. Information was gathered from the control groups of 19 rat carcinogenicity studies. All cases of death occurring within the 2-year period were reviewed. Out of 1124 males and 1084 females, 720 male (64.1%) and 689 female (63.6%) decedents were recorded. There was no difference in the probability of survival between two sexes. Analysis of factors contributing to death revealed that 400 males (48.7%) had neoplastic changes, 189 males (23.0%) had non-neoplastic lesions, and 232 males (28.3%) died from unknown causes. In females, these figures were 627 (76.4%), 62 (7.6%) and 132 (16.0%), for neoplastic, non-neoplastic and unknown findings, respectively. It could be suggested that the risk of death by non-neoplastic reasons was higher in the males than in the females, whereas females were more likely to be affected by tumours. In the neoplastic causes of death, pituitary tumours were the most common in both sexes, followed by mammary tumours in females, and haemopoietic tumours in males. In non-neoplastic cause of death, renal diseases were the most common in both sexes, followed by skin diseases and cardiovascular diseases in males, and skin diseases and poditis in males. A relatively large number of animals (28.3% in males and 16.0% in females) were found dead, without any significant clinical or histologically identifiable cause. Most of the animals with pituitary tumours were killed in extremis and the proportion of females (70.1%) being greater than males (46.8%). There were no case which died by accident, and also only minimal incidence which died by bleeding procedures.

Arginase-1 and P-glycoprotein are downregulated in canine hepatocellular carcinoma

  • Background: Hepatocellular carcinoma is the most common primary hepatic malignancy in humans and dogs. Several differentially expressed molecules have been studied and reported in human hepatocellular carcinoma and non-neoplastic liver lesions. However, studies on the features of canine hepatocellular carcinoma are limited, especially related to the differential characteristics of neoplastic and non-neoplastic lesions. Objectives: The study's objective was 1) to examine and evaluate the expression of arginase-1, P-glycoprotein, and cytokeratin 19 in canine liver tissues and 2) to investigate the differential features of hepatocellular carcinomas, liver tissue with non-neoplastic lesions, and paracancerous liver tissues in dogs. Methods: The expression levels of three markers underwent immunohistochemical analysis in 40 non-neoplastic liver tissues, 32 hepatocellular carcinoma tissues, and 11 paracancerous liver tissues. Scoring of each marker was performed semi-quantitatively. Results: Arginase-1 and P-glycoprotein were significantly downregulated in hepatocellular carcinoma, compared with hepatic tissues with non-neoplastic diseases (p < 0.001). Expression levels of arginase-1 and P-glycoprotein were also significantly lower in hepatocellular carcinoma than in paracancerous liver tissues (arginase-1, p = 0.0195; P-glycoprotein, p = 0.047). Few cytokeratin 19-positive hepatocytes were detected and only in one hepatocellular carcinoma and one cirrhotic liver sample. Conclusions: The results of this study suggest that downregulation of arginase-1 and P-glycoprotein is a feature of canine hepatocellular carcinoma; thus, those markers are potential candidates for use in differentiating hepatocellular carcinomas from non-neoplastic liver lesions in dogs.

Maxillary Ameloblastic Carcinoma in a Shih Tzu Dog

  • A 10-year-old castrated male Shih Tzu dog was submitted to a local animal hospital with a mass from gingiva to maxillofacial skeleton. Computed tomography revealed that strong invasion of the mass result in osteolysis in orbit and frontal bone. The excised mass was presented to the Pathology Department of the Veterinary Medicine, Jeju National University. Surgically excised mass was rubbery to firm in consistency. Histologically, the neoplastic mass was composed of irregular or interdigitating cords, islands or pseudo-glandular structures of stratified epithelial cells. These cords or islands showed typical palisading pattern of neoplastic epithelial cells to periphery without intercellular bridge (desmosome) and surrounded by eosinophilic immature collagenous matrix. Some area showed islands of well differentiated keratinizing squamous cell foci. Some lumen of glandular structures contained fibrin-like materials and RBC. These neoplastic cells showed marked invasive tendency to adjacent connective tissues and bony tissues, therefore solitary neoplastic cells were widely distributed throughout the surround connective tissue. The neoplastic cells showed positive reactions for pan-CK and CK14, weakly positive reaction for CK5/6. And the surrounding immature collagenous matrix was only labeled for vimentin.
