PURPOSE: The aims of this study were to assess the intra-tester and inter-tester reliability and validity of hip range of motion using a smart phone in hip operative patients. METHODS: Twenty-five patients (eight total hip arthroplasty and seventeen femur neck fracture) participated in this study. The range of motions in active and passive hip flexion, abduction and external rotation were measured with a goniometer and smart phone over two times by two observers. The intra-tester and inter-tester reliability were evaluated using the intraclass correlation coefficient (ICC2,1). The validity was measured by Pearson's correlation coefficient RESULTS: The intra-observer reliability was good in all measured items (ICC>0.78). The inter-observer reliability was high with ICC (>0.90). All correlation coefficients of smart phone and goniometer was greater than 0.85 and showed a significant positive correlation (p<0.01). CONCLUSION: The range of motion measurement with a smart phone showed acceptable reliability. Therefore, it could be convenient and have economical benefits to measure the range of motion of the hip joint using a smart phone.
Objectives : This study was conducted for objective verification of effects of spiral taping therapy for the patients with motor disturbance. Methods : To verify whether spiral taping therapy is effective for treating motor disturbance from cervical sprain, 28 patients were randomly allocated into the control and experiment groups. Then 23 subjects who fulfilled the experiment requirements were measured for lateral rotation angle using the goniometer. Changes in rotation were observed and compared. Control group received acupuncture and herbal acupuncture treatment, whileas the experiment group received spiral taping therapy in addition. Results : Differences in age and the degree of motor disturbance were disregarded in comparison of the groups prior to rendering treatments. For the control group, significant changes were observed after the second treatment until the termination of treatment. For the experiment group, significant changes were observed after the first treatment until the termination of treatment. Difference between the groups was insignificant but experiment group with spiral taping therapy showed better results. Conclusion : Spiral taping therapy can be an effective complementary treatment method for treating neck motor disturbance. Further studies in the subject should be conducted to yield more concrete verification.
Background: This study examined the effects of the muscle energy technique and cervical traction after applying conservative physical therapy to patients with cervical radiculopathy. Methods: Twenty-six patients with cervical radiculopathy were randomized into two groups: the experimental group (n=13) and the control group (n=13). The two groups performed the muscle energy technique and for 4 weeks, 3 times a week, in 70-minute sessions. Pain, range of motion, function, and muscle performance were assessed using the visual analogue scale (VAS), cervical range of motion (CROM) goniometer, neck disability index (NDI), and stabilizer pressure biofeedback, before and after training. Results: After four weeks of therapy, the VAS (p<.001), NDI (p<.01), and the maximum strength of the deep neck flexion muscles significantly decreased (p<.01) and CROM significantly increased in both groups (p<.05). Conclusion: The muscle energy technique and therapeutic modalities such as cervical traction are effective in reducing VAS and NDI and increasing CROM and muscle performance in patients with cervical radiculopathy.
PURPOSE: The purpose of this study was to investigate the effect of modified motor developmental intervention on the SCM muscle thickness and the range of motion (ROM) of neck in subjects with congenital muscular torticollis. METHODS: Thirteen subjects who had congenital muscular torticollis were participated in this study and they were offered the modified motor developmental interventions for 8 weeks by pediatric physical therapist. SCM thickness of affected and non-affected side and ROM of neck rotation were collected between before and after intervention. SCM thickness was measured by using Ultrasound Unit (IU 22, Ultrasound system, Philips, Netherlands) and ROM of neck was measured by using the goniometer (Arthrodial protractor, North coast, USA). Data were represented as means ${\pm}SD$. Paired t-test was used to determine the effect of interventions on the SCM muscle thickness and neck ROM. RESULTS: SCM thickness of affected side was significantly decreased after the modified motor developmental interventions and that of non-affected side was generally increased but it was not significantly difference between pre and post interventions. ROM of neck rotation and lateral flexion was significantly increased after interventions. CONCLUSION: These results suggested that the modified motor developmental interventions could improve the recovery of SCM muscle thickness and neck ROM.

This study was designed to provide basic data for developing exercise program that helps correcting posture by knowing the effect of strengthening and elongation exercises of upper extremity muscle to forward head posture correction. In this study determined subjects whether they had forward head posture or not. On the basis of the New York state posture rating, if a subject's posture is match up with the normal standard posture, gives 5 points and if the posture is slightly get out of the normal standard posture, gives 3 points and if the posture is apparently get out of the standard, gives 1 points. When determining the forward head posture, if talus, humerus and outer ear center are on the same line, it is determined as normal and if outer ear center is off the line less than 1.0cm, it is a slight deformation and if outer ear center is off the line more than 1.0cm, it is a high deformation. In the study selected people who have more than 1 cm gap between two vertical lines start from outer ear center and acromion separately as subjects. Length between the ideal alignment line measured by using goniometer and temporal region showed statistically significant decrease as $2.36{\pm}1.07cm$ before the intervention and $1.06{\pm}0.88cm$ after the intervention. After 4 weeks of neck and chest extensor muscle exercise, the group who exercised both showed increase in range of neck joint motion and neck flexion of the forward head posture. Meanwhile the group who only exercised neck extensor muscle only and the group who only exercised chest extensor muscle didn't showed statistically significant result. That only the group who exercised both muscles showed significant result is the different with studies before. Because this study didn't target patient who had a lesion, couldn't compare effect of the conservative manner and exercise. However, this study provides the fact that the group who exercised both neck and chest muscle had more effect than the control group.
Background: This study aimed to investigate the effect of elastic band-resistive exercise using audio-visual medium on pain, proprioception, and motor function in adults with chronic neck and shoulder pain. Design: One group pretest-posttest follow-up experimental design. Method: Twenty adult women with neck and shoulder pain voluntarily participated in this study. Elastic band-resistive exercise using audio-visual medium including cervical flexion and extension, shoulder external rotation, and scapular retraction-protraction motions was conducted 5 times a week for 3 weeks. The Numerical Rating Scale, pressure threshold tool, CROM goniometer, and Image J software were used to assess subjective pain level, tenderness threshold (pain), joint position sense error (proprioception), joint range of motion, and postural alignment (motor function), respectively. Result:: The pain intensity and threshold and joint position sense error showed significant decreases after the intervention, whereas the joint range of motion angle revealed significant increases. The postural alignment including forward head posture and rounded shoulder revealed significant improvements after the intervention. Conclusions: Therefore, we suggest that elastic band-resistive exercise through audio-visual medium would be helpful in preventing and managing pain and physical dysfunction in individuals with chronic neck and shoulder pain, and then it would support the development of health management-related online education content.
Objective: The purpose of this study was to evaluate of various material of pillows by using biomechanical variables such as the cervical stability, head pressure distribution, and muscle activity. Method: Eight subjects participated in the experiment. Three different materials such as polyester sponge, memory foam and the buckwheat shell used for Korean traditional pillow were tested. Electro-goniometer, six channels of electromyography(EMG), ten channels of the head pressure sensors were used to measure the biomechanical responses. Surface electrodes were attached to the right/left semispinals capitis(RSC, LSC), the right/left sternocleidomastoid(RSM, LSM), the right/left upper trapezius(RUT, LUT). The cervical stability was evaluated by the angle deviated from the standing neck position. The head pressure distribution was evaluated by the pressure per unit area recorded on the sensors and the intensity of peak pressure. Electromyography(EMG) data were analyzed by using root mean square(RMS) and mean power frequency(MPF). Results: The buckwheat shell material showed a higher stability in the cervical spine then the other pillows during spine position. In terms of head pressure distribution, the memory form indicated the lowest pressure at supine position, buckwheat shell material indicated the lowest pressure during lying down to side, and polyester cushion recorded the highest pressure at all postures. Conclusion: The buckwheat shell material has a biomechanical advantage to maintain a healthy neck angle and reduce the pressure on the head, which means the buckwheat shell is a potential material for ergonomic pillow design. The pillow with memory form showed second best biomechanical performance in this study. Application: The shape of the buckwheat shell pillow and the characteristics of materials can be used to design the pillow preventing neck pain and cervical disk problems.

Function was measured with the neck disability index (NDI). Cervical range of motion (ROM) was measured with a cervical range of motion (CROM) goniometer. Results: After four weeks of therapy, VAS (p<.05) and NDI (p<.05) significantly decreased, and ROM significantly increased in both groups (p<.05). There were also significant differences between the two groups for these three measures (p<.05). Conclusion: The muscle energy technique and cervical traction are more effective than cervical traction alone in reducing VAS and NDI and increasing ROM in patients with cervical radiculopathy.
이 연구는 만성 경부통을 가진 민간 경비원들을 대상으로 경부 안정화 운동을 실시하여, 목통증, 경부장애지수, 관절가동범위, 근육통증, 재위치감각 변화에 미치는 영향을 비교 분석 하고자 실시하였다. 연구 대상자는 경기 지역 민간경비업체에 종사하는 직원 중 만성 경부통을 호소하는 21명을 대상으로 경부 안정화 운동그룹, 자세교정 운동그룹으로 무선 할당하여, 주3회, 8주간 운동을 실시하였다. 실험 전 후 설문지를 이용하여 목통증과 경부장애지수를 조사하였으며, 관절가동범위, 근육통증, 재위치감각 검사를 실시하여 실험 전 후 변화된 값을 비교 측정하여 다음과 같은 결과를 얻었다. 통증강도와 경부장애지수는 경부 안정화 운동그룹이 실험 후 유의하게 감소하였으며(p<0.05), 관절가동범위, 근육통증, 재위치감각은 경부 안정화 운동그룹에서 실험 후 유의한 향상이 있었으나, 자세교정 운동그룹은 유의한 차이가 없는 것으로 나타났으며(p>0.05), 그룹 간 비교에서도 경부 안정화 운동그룹이 통증, 경부장애지수, 관절가동범위, 근육통증, 재위치감각에서 유의한 향상이 있었다(p<0.05). 본 연구 결과를 볼 때, 만성 경부통을 호소하는 민간 경비원들을 대상으로 경장근과 다열근과 같은 심부근육 강화에 초점을 둔 경부안정화 운동프로그램이 일반적인 자세교정 운동프로그램보다 통증 및 목 기능개선에 효과적인 것으로 나타났다. 따라서 경부 안정화 운동이 만성 경부통을 가진 민간 경비원들의 목 통증 감소와 기능 회복에 효과적이라고 할 수 있겠다.
Purpose: Forward head posture (FHP) is known to cause pain, limit range of motion, and reduce quality of life. Joint mobilization is commonly used to correct FHP. However, no study has compared cervical, thoracic, and combined cervical and thoracic joint mobilization for FHP. The aim of this study was to investigate and compare the effects of each mobilization technique on range of motion in the sagittal plane and pain in patients with FHP. Methods: Forty-five patients were recruited and randomly divided into three groups: the mobilization group (CM; n = 15), the cervical and thoracic mobilization group (CTM; n = 15), and the thoracic mobilization group (TM; n = 15). Each intervention was performed in sets of three and repeated six times. Range of motion and pain were assessed pre- and post-intervention. The cervical range of motion was evaluated using a goniometer, and pain was evaluated using a visual analogue scale and pain thresholds of the suboccipital and upper trapezius muscles. Results: All groups showed an increase in range of motion post-intervention, but the increase in the CTM group was significantly greater than in the CM and TM groups (p < 0.05). Pain measured using the visual analogue scale decreased in all groups, but the decreases in the CM and CTM groups were significantly greater than in the TM group (p < 0.05). The pain thresholds of the suboccipital and upper trapezius muscles increased in all groups, but the increase in the CTM group was significantly greater than in the CM and TM groups (p < 0.05). Conclusion: Overall, our findings suggest that CTM may be more effective than CM or TM for improving cervical range of motion in the sagittal plane and pain in patients with FHP.
