Conservative and esthetic closure of maxillary midline diastema without creating "black triangle" using direct resin composite (Black triangle이 존재하지 않는 보존적이고 심미적인 상악 정중이개의 치료)

  • Anterior diastemas are common esthetic problems. One of the challenges in clinical esthetic dentistry is closing anterior diastemas without creating "black triangles" between the teeth. The success of a restorative treatment in anterior teeth depends on the esthetic integration between soft tissues and hard tissues. This report describes the successfully accomplished diastema closure case by producing the emergence profile with natural contours at the gingival-tooth interface and then generating of gingival recontouring process.

Effects of Head Posture and Occlusal Splint on Swallowing Movement (두부자세 및 교합장치에 따른 연하운동의 변화)

  • This study was performed to investigate the effects of head posture and occlusal splint on the vertical dimension in mandibular rest position and swallowing. Thirty health dental students ware selected lot this study and BioEGNⓡ(Bioresearch Inc., USA) was used for measuring interocclusal distance during rest - swallowing - rest - tapping movement. This swallowing movements were observed in both normal head posture(NHP) and forward head posture (FHP). Thickness of occlusal splint was about 2mm at posterior molar area and even tooth contact were achieved on light biting. The four mandibular positions at which interocclusal distance measured were swallowing position, after swallowing position in which interocclusal distance was maximum, rest position follows swallowing, and tapping position after rest. Changes of distance in each position were measured for three mandibular planes, that is, sagittal, frontal, and horizontal plane, respectively. The results obtained were as follows : 1. In normal head posture, the mandible was raised 1.03mm without splint, and 0.77mm with splint on swallowing, and there was no significant difference between the two. In horizontal plane, however, mandible was displaced more anteriorly in both swallowing position and tapping position with splint. 2. In forward head posture, the mandible was less raised with splint on swallowing, but features in horizontal plane were almost same as those in normal head posture. 3. In natural dentition, significant difference between NHP and FHP were observed in horizontal plane trajectory for swallowing and tapping position. But the difference for same positions were observed in frontal trajectory with splint. 4. Total amount of mandibular movement of two groups classified with sagittal interocclusal distance of swallowing position generally showed significant difference between the higher and the lower height group in head posture without splint. 5. Correlationship among total amount of mandibular movement for three mandibular planes were observed between sagittal plane and horizontal plane, and between sagittal plane and frontal plane in head posture without splint.

Relationship between the number of functional teeth and Geriatric Oral Health Assessment Index(GOHAI) in elderly (노인의 기능 치아 수와 구강건강평가(GOHAI)와의 관련성)

  • Objectives: The purpose of the study was to investigate the association between remaining functional teeth and oral health-related quality of life using Geriatric Oral Health Assessment Index(GOHAI) in the elderly. Methods: A self-reported questionnaire was completed by 237 female elderly in Daegu from May to September, 2015. The questionnaire consisted of general characteristics of the subjects and Geriatric Oral Health Assessment Index(GOHAI). A total of 216 female elderly data were analyzed for oral health status and their GOHAI score. Data were analyzed using SPSS 23.0 version. Logistic regression analysis showed the association of functional teeth and GOHAI score. Results: Number of functional teeth can affect oral health related quality of life in the elderly. When functional teeth is small, the odds ratio(OR) was 2.119(95% CI: 1.215-3.696). After adjusting age, residence type and education, OR was 1.900(95% CI: 1.060-3.399). Policies to preserve natural tooth should be recommended for increasing quality of life in general. Conclusions: This study showed significant association between number of functional teeth and the GOHAI score of the elderly women.

Influence of denture wearing on a mini-mental state examination(MMSE-K) in the elderly (노인들의 의치 사용이 MMSE-K 점수에 미치는 영향)

  • Objectives: The purpose of the study was to investigate the association between denture wearing and dementia using a Mini-Mental State Examination (MMSE-K) in the community dwelling elderly. Methods: The study subjects were 184 community dwelling elderly using Korean version of mini-mental state examination(MMSE-K). The variables included the general characteristics of the subjects, denture earing, number of mastication tooth, subjective mastication ability, and MMSE(dementia). Denture wearing was divided into two groups of the natural dentition including the fixed prosthesis and removable denture including the complete and/or partial denture. The data were analyzed by ${\chi}^2$ test, t-test, and binary logistic regression using SPSS version 22.0 program. Logistic regression was used to analyze the association of MMSE-K score and denture wearing, and 95% confidence interval was calculated. The subjective mastication ability was measured by Likert 5 points scale. Dementia was measured by MMSE-K. Results: There was a significant association between MMSE-K score and denture wearing. The odds ratio(OR) of denture waering was 6.01(95% CI: 2.824-12.784). After adjusting the age, gender, residence and education, OR was 5.53(95% CI: 2.364-12.935). Conclusions: This study showed a significant association between MMSE-K score and denture wearing in the Korean elderly.

Clinical study on the food impaction between implant prostheses and adjacent teeth (임플란트 보철물과 인접치 사이의 식편압입에 관한 임상적 연구)

  • Purpose: The purpose of this study was to investigate the food impaction between implant prostheses and adjacent natural teeth. Materials and methods: For this study, 51 patients with food impaction were selected and investigated mobility, tightness of contact area, gingival index, plaque index, attachment loss, alveolar bone level, proximal caries, marginal ridge distance and occlusal relationships. Results: Food impaction was found in the upper teeth (60.7%) more than the lower teeth (39.2%). Food impaction was occurred on mesial side of implant prostheses (86.2%) more than distal side (13.7%). Food impaction was mostly found in loose or open contact area (94.2%). Food impaction was frequent on stepped relationship between implant and adjacent teeth. Conclusion: Treatment plan should include proper adjacent and antagonistic occlusal plane and occlusal surface, to prevent food impaction, and the plan should include less adjacent tooth mobility with proper tightness between implant prostheses and adjacent teeth.

A Study on Oral Health Behavior of the Industrial Workers (산업체 근로자의 구강보건행태에 관한 조사 연구)

  • This study was conducted to provide basic data for improvement of oral health of workers, by investigating oral health behaviors of industrial workers. About sixty percent(60.9%) of subjects experienced in having dental examinations and 54.3% of them didn't receive follow-up treatments after the dental examinations. Also, those who brushed teeth twice per week(58.6%) and averagely, earned 2~3 million won of monthly income(p<.01), showed a higher frequency of tooth brushing. Experienced workers(p<.01) were more interested in oral health. Those who were engaged in other jobs and had less than 10~20 years of experience(p<.05), had lots of experience in staying away from work. More aged group recognized that oral health education is necessary(p<.05). The population of oral examination and health instruction may have a favorable impact on maintenance of oral health status and improvement in quality of life.

IN VITRO EVALUATION OF PERIOTEST VALUES UNDER VARIOUS CONDITIONS OF PROSTHESES (보철물 조건에 따른 Periotest수치의 실험적 평가)

  • Periotest(Siemens, Germany) has been used to test mobility of the implants clinically, however the effects of target materials and connection methods on the PTVs(Periotest Values) have not been evaluated. Periotest has been regarded as a reliable and objective tool to test implant and natural teeth mobility clinically, however this instrument showed different PTVs under various test conditions. This in vitro study was designed to compare PTVs of different veneering materials and prosthodontic designs (single and bridge restorations). To compare the effects of veneering materials on PTVs, 1 mm thickness of five different testing materials (porcelain, type III gold alloy, pure titanium, composite resin, acrylic resin) were placed on the resin block. Three full length of 13 mm Mark II implant fixtures were embedded into autopolymerizing resin block to fabricate single and bridge restorations. To evaluate effects of the connection method in single restorations, PTVs of screw retained(UCLA type) and cementation type(Cera-One system) were compared. Finally, to test reliability of PTVs of the final restorations, screw retained three unit short span PFM bridges were fabricated on the standard and Estheti-Cone abutments. All testing components were tightened with torque controller and PTVs of all specimens were measured 15 times for statistical analysis with SAS program. Following conclusions were made within the limit of this in vitro study. 1. PTVs of type III gold alloy, grade II titanium, composite resin veneering materials showed no significant differences, however acrylic resin and porcelain showed significant differences (P<0.05). 2. Single tooth restorations showed consistent PTVs as long as proper torque force was applied. 3. PTVs of bridge type prostheses was inconsistent regardless of abutment types. 4. PTVs of the prostheses showed higher scores and standard deviations than those of abutments regardless types of connection (P<0.05).

A morphometric analysis of maxillary central incisor on the basis of facial appearance in Korea

  • PURPOSE. We aimed to identify a more esthetic width-to-length ratio by analyzing maxillary central incisor of Korean adult population. Information regarding tooth ratio in the Korean population may be useful to clinicians when restoring anterior teeth. MATERIALS AND METHODS. A total of 40 men and 40 women were included in this study. The NON-MEDIA group included 40 non-celebrities, who were healthy and 25 to 39 years old, with normal dentofacial appearance. The MEDIA group consisted of 40 celebrities selected only on the basis of their soft-tissue facial appearance. The facial photographs of 40 celebrities were collected from the Internet websites. The width and length of the maxillary central incisor were measured using Adobe $Photoshop^{(R)}$ 7.0, a software for analysis, measurement and edition of photographs. Data were analyzed statistically using the Independent t-test at 5% statistical significance level. RESULTS. The mean ratio for the MEDIA group was 0.77, whereas that of the NON-MEDIA group was 0.88. The difference between the two groups was statistically significant. No significant gender differences were found in the width-to-length ratio in MEDIA group. In NON-MEDIA group, however, there were significant differences between female and male. CONCLUSION. After analyzing maxillary anterior teeth of Korean adults, our results were in accordance with the general Western standards of esthetics. A dentist restoring the natural look and beauty of teeth will have to consider those standards.

Addition of Hydroxyapatite to Toothpaste and Its Effect to Dentin Remineralization

  • In this study, hydroxyapatite (HAp) was incorporated into toothpaste and its effect on the remineralization and restoration of dental enamel was evaluated. Different sets of toothpaste were incorporated with HAp levels of 0%, 5%, 10 %, and 15 %. The filler particles of the resulting toothpaste samples were observed via SEM and XRD and compared with compositions of several commercially available toothpastes, showing that the HAp was successfully incorporated into the toothpaste samples. Different sets of human enamel were inflicted with lesions and then treated with the different fabricated toothpaste samples for five minutes three times a day for seven days. During the treatment, the teeth were subjected to demineralization and remineralization cycles to simulate the effect of natural saliva. The surface of the enamel samples were observed using SEM before and after one week of treatment, showing the formation of HAp layers on the surfaces of the enamel samples. The effect of the toothpaste on the lesions was observed using an inverted light microscope and the lesion depth was found to decrease as the concentration of HAp in the toothpaste used increased. HAp was successfully incorporated in the toothpaste and its presence was found to lessen lesion depths and improve tooth remineralization.

A full-mouth rehabilitation using zirconia all-ceramic crowns: a case report (지르코니아 전부 도재관을 이용한 구강 완전 회복 증례)

  • As patients' esthetic expectations increase, there is an increase in demand for cosmetic restorations of anterior and posterior teeth that resemble the natural tooth morphology and color. An example of high-strength all-ceramic systems is zirconia with CAD/CAM application. This case report describes a full-mouth rehabilitation using zirconia all-ceramic crowns supported by upper and lower implants and by a minimal increase in the occlusal vertical dimension in a patient with severe abrasion due to loss of posterior teeth.
