비골 골절 치료시 일차적 비중격 성형술 (Primary Septoplasty In The Treatment of Nasal Bone Fractures)

  • Purpose: Most nasal bone fractures involve the septum; either or both of the ethmoidal perpendicular plate(EPP) and quadrangular cartilage(QC). Unlocked tension from the underlying quadrangular cartilage and poorly reducted bony septum are obstacles to the successful reduction of nasal bone. So we compared the preliminary outcome of septoplasty as a primary treatment with the untreated septum in nasal bone fractures. Methods: We performed a retrospective study of 215 patients underwent reduction of nasal fracture between January 2002 and February 2008. We graded patients into four groups according to the amount of deviation and direction of force by CT. Our indication for septoplasty and combined procedures was the deviation of EPP or QC over 50% from the midline. We interviewed part of the patients by telephone regarding the subjective esthetic and functional outcomes. Results: Forty five of 215 patients (21 percent) underwent septoplasty and combined procedures (cartilage graft, etc) after the informed consent. Patients who underwent septoplasty significantly satisfied with the outcome of esthetic appearance and nasal patency compared with patients who underwent simple closed reduction despite of having septal deviation over 50 percent from the midline. (p < 0.05) Conclusion: Septal surgery and esthetic consideration shoud be made even in simple nasal reductions. And if CT scans reveal severe deviation of septum, septoplasty should be considered as a primary treatment.

반전된 U자형 실리콘 시트를 이용한 코뼈골절의 치료 (Treatment of Nasal Bone Fracture with Reverse U-Shaped Silicone Sheet)

  • Purpose: The majority of nasal bone fractures have been managed by routine procedure of closed reduction and intranasal packing. However, nasal packing can cause nasal injury, hypoxia and patient's discomfort. And, synechiae formation is a frequently occurred after the nasal surgery. Various methods are used to reduce the incidence of synechiae formation. The purpose of this study was to compare routine procedure and nasal packing with reverse "U" shaped silicone sheet with respect to postoperative nasal synechiae formation and final outcome. Methods: We analyzed the medical records of 100 patients with nasal bone fracture who were operated by closed reduction in the last one year. The silicone sheet was designed in reverse "U" fan - shape and inserted between the middle turbinate and the septal wall. Nasal packing was removed at two days after the operation, and silicone sheet was removed at 10 to 14 days. Results: Synechiae in the middle meatus developed in 2 of 75 patients. Although synechiae between the middle meatus and nasal septum occurred in patients, the patients did not complaint of any olfactory disturbance and nasal obstruction. Conclusion: The reverse "U" fan - shape silicone sheet caused less pain for patients and no significant differences in outcome were found. The results of this study suggest that insertion of silicone sheet between the middle meatus and septum can be a useful method in the prevention of intranasal synechiae formation. The reverse "U" silicone sheet is a good alternative for routine packing methods.

비중격에 발생한 신경섬유종의 내시경적 비내 수술 치험 1예 (A Neurofibroma arising from nasal septum)

  • Neurofibroma is a neurogenic tumor arising from schwann cells or peripheral tissues of nerve sheaths. Neurofibroma rarely occurs as a solitary lesion but mostly occurs as part of neurofibromatosis and reports of neurofibromas developed in the nasal cavity or paranasal sinus are very rare. We report on a case of neurofibroma of the nasal cavity treated by endoscopic surgery. Preoperative computed tomography(CT), and punch biopsy suggested that the tumor was benign neurogenic tumor cofined to right nasal cavity. The tumor was removed with endoscopic surgery completely, and confirmed as neurofibroma by histological and immunohistochemical examination. We discuss the clinical and pathological characters of neurofibroma arising in the nasal septum.

정상 개에서 컴퓨터단층촬영을 이용한 비중격 변위 평가 (Computed Tomographic Evaluation of the Nasal Septum Deviation in Clinically Normal Dogs)

  • 본 실험은 임상적으로 건강한 시츄 견 36 두, 비글 견 16 두에서 컴퓨터단층촬영을 이용하여 비중격의 해부학적 차이와 편위 정도를 평가하고자 실시되었다. 비중격의 편위 정도는 비중격 편위의 각도 및 거리 측정에 의해 평가되었다. 비중격 편위의 각도 및 거리는 비글 견보다 시쮸 견에서 유의적으로 높은 수치를 나타냈다. 시츄, 비글견 두 품종에서 체중 및 나이는 비중격 편위의 각도 및 거리와 모두 유의적인 관계는 관찰되지 않았다. 이 결과에 기초하여 비중격의 편위는 임상적으로 건강한 개에서도 관찰 가능하며, 특히 시츄 견에서 쉽게 관찰되는 것으로 생각된다.

Correlations between anatomical variations of the nasal cavity and ethmoidal sinuses on cone-beam computed tomography scans

  • Purpose: Anatomical variations of the external nasal wall are highly important, since they play a role in obstruction or drainage of the ostiomeatal complex and ventilation and can consequently elevate the risk of pathological sinus conditions. This study aimed to assess anatomical variations of the nasal cavity and ethmoidal sinuses and their correlations on cone-beam computed tomography (CBCT) scans. Materials and Methods: This cross-sectional study evaluated CBCT scans of 250 patients, including 107 males and 143 females, to determine the prevalence of anatomical variations of the nasal cavity and ethmoidal sinuses. All images were taken using a New Tom 3G scanner. Data were analyzed using the chi-square test, Kruskal-Wallis test, and the Mann-Whitney test. Results: The most common anatomical variations were found to be nasal septal deviation (90.4%), agger nasi air cell (53.6%), superior orbital cell(47.6%), pneumatized nasal septum(40%), and Onodi air cell(37.2%). Correlations were found between nasal septal deviation and the presence of a pneumatized nasal septum, nasal spur, and Haller cell. No significant associations were noted between the age or sex of patients and the presence of anatomical variations (P>0.05). Conclusion: Radiologists and surgeons must pay close attention to the anatomical variations of the sinonasal region in the preoperative assessment to prevent perioperative complications.

Common Sites, Etiology, and Solutions of Persistent Septal Deviation in Revision Septoplasty

  • Objectives. To investigate the common causes of persistent septal deviation in revision septoplasty and to report the surgical techniques and results to correct them. Methods. A total of 100 consecutive patients (86 males) who had revision septoplasty due to persistent septal deviation from 2008 and 2014 were included in the study. Their mean age was 35.6 years and the mean follow-up duration was 9.1 months. Presenting symptoms, sites of persistent septal deviation, techniques used to correct the deviation, and surgical results were reviewed. Results. The mean interval between primary and revision surgery was 6.2 years. Forty-eight patients received revision septoplasty and 52 received revision septoplasty combined with rhinoplasty. Nasal obstruction was the most presenting symptom in almost all patients. The most common site of persistent septal deviation was middle septum (58%) followed by caudal septum (31%). Correcting techniques included further chondrotomy and excision of deviated portion in 76% and caudal batten graft in 39%. Rhinoscopic and endoscopic exams showed straight septum in 97% and 92 patients had subjective symptom improvement postoperatively. Conclusion. Middle septum and caudal septum were common sites of persistent deviation. Proper chondrotomy with excision of deviated middle septum and correction of the caudal deviation with batten graft are key maneuvers to treat persistent deviation.

Open Reduction of an Isolated Anterior Nasal Spine Fracture: A Case Report and Review of the Literature

  • A 14-year-old girl had a midfacial trauma event caused by hitting against an opening door and experienced discomfort and swelling of the columella and upper lip. Physical examination revealed mild tenderness on light palpation without any discomfort with upper lip movement. A computed tomography scan of the maxillofacial bones with three-dimensional reconstruction showed a fracture of the anterior nasal spine with obvious leftward displacement, mild-deviation of the caudal aspect of the nasal septum, and no sign of nasal bone fracture. Open reduction and internal fixation was performed with regard to aesthetic and functional concerns, including nasal septum deviation. The postoperative course was uneventful, and healing proceeded normally without complications. Herein, we emphasize the importance of differential diagnosis of isolated anterior nasal spine fractures in patients with midfacial trauma and clinicians' strategic decision-making in treatment modalities.

인삼(人蔘) 추출액(抽出液)이 SO2 Gas에 폭로(暴露)된 새앙쥐 호흡기상피(呼吸器上皮)의 섬모(纖毛)변화에 미치는 영향(影響)에 관한 연구(硏究) (Studies on the Effects of Ginseng Extract for Mucociliary Change in Mice Nasal Septum Epithelia Exposured to Sulfur Dioxide gas)

  • In order to investigate the effect of Panax ginseng extract and it's degree in mucociliary change of mice nasal septum epithelia exposured to sulfur dioxide, 96 ICR male mice were used. They were at first divided the 4th week, the 8th week or the 16th week groups according to the age after birth and 6 hour or 12 hour groups according to the $SO_2$ gas exposured hour in a day, and at control, 50mg, 100mg and 200mg injection groups according to the dosage of the freeze-drying powder of the ginseng extract which was injected into the mouse peritoneal cavities in the condition of the solution solved with physiological saline solution. Each subgroups which were divided finaly included 4 male mice. The histological tissue sections for observation were made from nasal septum, posterior nasal orfice and trachea. The results obtained by experiments were summarized as followings. 1. The loss of the nasal mucosa epithelial cilia of the mouse exponsure to the $SO_2$ gas after ginseng extract injection was apparently diminish eompare to those exposured only $SO_2$ gas without pretreatment of ginseng extract (p<0.01). 2. The inhibition effect for the loss of nasal mucocilia according to the ginseng extract dosages not found in this research (p>0.05). 3. There were differences in the loss of nasal mucosa cilia according to the $SO_2$ gas exposure time between the control group and ginseng extract pretreatment group (p<0.01). 4. According to the increase of the postnatal time, there were remarkable differences between the control group and the ginseng extract pretreatment groups in the loss of nasal mucosa cilia (p<0.01). 5. Ciliary changes of the posterior nasal orifice and trachea according to the $SO_2$ gas exposure time, mice age and ginseng dosages, were not dearly observed in this light microscopical observation.

A New Approach to Objective Evaluation of the Success of Nasal Septum Perforation

  • Background Perforations in the nasal septum (NSP) give rise not only to disintegration of the septum anatomy but also impairment in normal nasal physiology. The successes of these surgical techniques are usually equated to anatomical closure of the perforation. The goal of this study is to evaluate the subjective and objective results of our surgical technique for septal perforation surgery. Methods All NSPs in the six patients were closed by inferior turbinate flap. The Nasal Obstruction Symptom Evaluation (NOSE) instrument was used to evaluate the preoperative and postoperative subjective sensation of nasal obstruction. Measurement of preoperative and postoperative nasal airway resistance was performed using active anterior rhinomanometry which is an objective test. Wilcoxson signed rank test and Spearman correlation test were used to analyze correlation between NOSE scores and rhinomanometric measurements. Results The full closure of the septal perforations was noted in 100% of patients. The total NOSE score was 14 preoperatively and one postoperatively. The improvement in NOSE scores was statistically significant ($P{\leq}0.002$). The mean preoperative total resistance (ResT150) value was $0.13Pa/cm^3s^{-1}$, which is below the normal range ($0.16-0.31Pa/cm^3s^{-1}$), while the mean postoperative ResT150 value was $0.27Pa/cm^3s^{-1}$. The correlation between the improvement in NOSE scores and improvements in ResT150 values was statistically significant. Conclusions Surgical approaches should aim to solve both the anatomical and physiological problems of NSP. The application of subjective and objective tests in the postoperative period will help surgeons assess the applied techniques.

비중격에 발생한 연골육종 (Chondrosarcoma of Nasal Septum)

  • Chondrosarcomas are uncommon tumors. Approximately 5-10% are located in the head and neck. Nasal septal chondrosarcoma is exceedingly rare, with an extensive review of the world literature revealing only 50 reported cases. Most occur in middle-aged man. Surgical resection is the definitive treatment of choice. Radiation and chemotherapy are reserved for residual or recurrent diseases and palliation. We present a case of nasal septal chondrosarcoma treated with craniofacial resection and postoperative radiation therapy.
