Purpose: The subcutaneous pocket graft of the thumb tip amputation across or proximal to the lunula is chosen in case of impossible microvascular anastomosis and in patient who strongly desired to preserve the thumb tip after failed replantation. Materials and Methods: Two patients who underwent a subcutaneous pocket graft for a thumb tip reconstruction between August 2008 and November 2009 were reviewed retrospectively. They were all males with a mean age at the time of surgery of 48 years and had sustained complete thumb tip amputations across or proximal to the lunula. In one case, the microsurgical replantation was not feasible and the other one revealed arterial insufficiency at the 7th day after microsurgical replantation. Results: Authors had experienced 2 cases of flaps which survived completely. The results of sensibility was good, the range of motion at interphalangeal joint and tip to tip pinch was acceptable and color mismatch and loss of thumb finger nail was unacceptable after more than 1 year follow up with conventional successful thumb tip replantation. Conclusion: The subcutaneous pocket graft could be chosen in thumb tip amputation in case of impossible microvascular anastomosis as well as who strongly desires to preserve thumb tip after failed replantation.
본 연구는 최근 화장품 유해성분에 관한 이슈를 끊임없이 제기되면서 네일 제품에서 발생할 수 있는 유해물질 인체 노출 저감을 위한 안전관리 방안 마련을 목적으로 국내외 문헌고찰을 통해 네일 제품의 안전 표시 기준 현황을 비교 분석하고, 네일 제품 내 유해물질 허용 기준치 초과 사례를 파악하였다. 연구 결과 국내 유통된 인조 손톱 접착제와 네일팁에서 각각 톨루엔 40.3배, 클로로포름 22.8배, 안티몬 15.4배 초과 검출되었다. 선진국은 다양한 정책연구 및 법제화를 통해 작업장 물질안전보건자료 제공 및 환기 장치 설치 등을 의무화하고 있으나 국내에서는 별도의 안전 기준이 없어 네일 제품을 통한 유해물질 인체 노출이 심각한 상황이다. 따라서 네일 제품 작업환경 종사자 소비자 각각의 특성에 맞게 세부 네일 유해물질 안전관리 규정이 우선적으로 정립되어야 하고, 이에 따른 다양한 교육 프로그램 개발 및 보급을 통해 유해물질 화장품에 대한 인지도를 제고한다면 국민의 질병예방 및 건강증진의 융복합적 차원에 기여할 수 있을 것이다.
최근 몇 년 동안 사람들의 고유한 생리적인 특징을 이용한 생체 인식은 새로운 학문으로서 연구 및 개발이 활발하게 진행되고 있다. 지금까지, 오로지 지문 인식만이 다른 생체 인식에 비해 확인과 식별 시스템들이 더 정교하고, 비싼 취득 인터페이스들과 인식 과정을 필요로 하기 때문에 온라인 보안 검사를 위하여 한정된 성공을 보았다. Hand-Geometry는 생체 인식의 확인 그리고 취득의 편리 때문에 식별 그리고 확인을 위하여 사용되고 있다. 그러므로, 본 논문은 이러한 특징을 가지는 손의 기하학적인 Hand-Geometry 인식 시스템을 제안하고자 한다. 해부학적인 관점에서, 인간의 손은 길이, 폭, 두께, 기하학적인 모양, 손바닥의 모양, 그리고 손가락들의 기하학적인 모양까지 특성으로 나타내어질 수 있다. 본 논문에서 제안한 Hand-Geometry 인식 시스템은 30개의 특징 데이터를 가진다. 그러나 특징 데이터 가운데 사용자의 Hand-Geometry의 특징에 따라 길이 데이터가 변하는 것을 실험적으로 발견하였다. 따라서 이와 같은 가변적인 길이 데이터를 안정화시키기 위하여 본 논문에서는 길이 데이터의 기준점을 손톱 아래 점으로 정하고, GA를 적용하여 보다 안정된 특징점을 추출하였다.
This study improves the dimensional suitability of nail tip products by reviewing the type of fingernail as seen among adult women in Korea. Subjects were 147 adult women ranging from age 20 to 40. Direct fingernails measurements were made using digital Vernier calipers and a curvature gauge. Data on various fingernail shapes were collected through indirect measurements using photography to reveal the shape characteristics of each fingernail types. In this sense, data were analyzed by statistical methods with the use of factor analysis. There were various factors considered to classify extracted fingernail types such as five factors (for the thumb), four factors (index, middle, and little fingers), and three factors (for the ring finger). The cluster analysis resulted in three types. Type 1 is a 'Square' shape characterized with wide fingernails, low height, flat, and low curvature of the cuticle line. Type 2 is 'U-round' shape characterized with narrow fingernails, high height, convex, and the highest curvature of the cuticle line. Type 3 is a 'Round' shape in which the fingernails are wide, high in height, convex, and characterized with a medium curvature of the cuticle line. The results are useful for companies that develop the free edge shape of fingernails for consumer products. The measured values of fingernail can be applied to the shape and lake setting of the nail tip product, depending on type of fingernail.
Purpose: We reviewed 66 cases of the glomus tumors in the hands. The purpose of this study was to review the characteristic signs and symptoms of glomus tumors and the surgical outcomes. We made hypothesis that (1) our guideline for surgical excision are reliable and (2) our surgical technique is efficient and less harmful to the nail root. Materials and Methods: The clinical diagnosis was made with occasional radiological studies and surgical excision was performed according to our indications. Patients with electrical shock-like pain by touching (the essential symptom) and at least 2 of 4 clinical signs and symptoms (cold hypersensitivity, paroxysmal pain that radiated proximally, blue discoloration, nail deformity for dorsal tumor or a palpable nodule for pulp tumor) underwent surgical excision. Results: The duration of symptoms of the patients was about 5.3 years. No difference of prevalence was found in both hands, but the thumb and the middle finger were more affected than the other fingers. The mean follow up period was 2.4 years and none of the patients complained the same characteristic pain at the last visit. Conclusion: To the best of our knowledge, this report is the largest case series about glomus tumors in the finger tip. The clinical signs and symptoms are important to make a diagnosis of glomus tumors, and surgical excision is the treatment of choice.

Background Glomus tumors were first described by Wood in 1812 as painful subcutaneous tubercles. It is an uncommon benign neoplasm involving the glomus body, an apparatus that involves in thermoregulation of cutaneous microvasculature. Glomus tumor constitutes 1%-5% of all hand tumors. It usually occurs at the subungual region and more commonly in aged women. Its classical clinical triad consists of pain, tenderness and temperature intolerance, especially cold sensitivity. This study reviews 15 cases of glomus tumor which were analyzed according to its anatomic location, surgical approach and histologic findings. Methods Fifteen patients with subungual glomus tumors of the hand operated on between January 2006 and March 2013, were retrospectively reviewed. Patients were evaluated preoperatively with standard physical examination including ice cube test and Love's test. Diagnostic imaging consisted of ultrasonography, computed tomography, and magnetic resonance imaging. All procedures were performed with tourniquet control under local anesthesia. Eleven patients underwent excision using the transungual approach, 3 patients using the volar approach and 1 patient using the lateral subperiosteal approach. Results Total of 15 cases were reviewed. 11 tumors were located in the nail bed, 3 in the volar pulp and 1 in the radial aspect of the finger tip. After complete excision, patients remained asymptomatic in the immediate postoperative period. In the long term follow up, patients exhibited excellent cosmetic results with no recurrence. Conclusions Accurate diagnosis should be made by physical, radiologic and pathologic examinations. Preoperative localization and complete extirpation is essential in preventing recurrence and subsequent nail deformity.

Purpose: This study aims to compare patients in whom fixation failure occurred via cut-out (CO) or cut-through (CT) in order to determine patient factors and radiographic parameters that may be predictive of each mechanism. Materials and Methods: This retrospective cohort study includes 18 patients with intertrochanteric (IT) hip fractures (AO/OTA classification 31A1.3) who underwent treatment using a single lag screw design intramedullary nail in whom fixation failure occurred within one year. All patients were reviewed for demographics and radiographic parameters including tip-to-apex distance (TAD), posteromedial calcar continuity, neck-shaft angle, lateral wall thickness, and others. Patients were grouped into cohorts based on the mechanism of failure, either lag screw CO or CT, and a comparison was performed. Results: No differences in demographics, injury details, fracture classifications, or radiographic parameters were observed between CO/CT cohorts. Of note, a similar rate of post-reduction TAD>25 mm (P=0.936) was observed between groups. A higher rate of DEXA (dual energy X-ray absorptiometry) confirmed osteoporosis (25.0% vs. 60.0%) was observed in the CT group, but without significance. Conclusion: The mechanism of CT failure during intramedullary nail fixation of an IT fracture did not show an association with clinical data including patient demographics, reduction accuracy, or radiographic parameters. As reported in previous biomechanical studies, the main predictive factor for patients in whom early failure might occur via the CT effect mechanism may be related to bone quality; however, conduct of larger studies will be required in order to determine whether there is a difference in bone quality.
최근, 갑작스런 집중호우로 인하여 자연 및 인공사면의 붕괴가 종종 발생하고 있다. 이를 예방하기 위한 여러 가지 보강공 법들이 개발되어 적용되고 있다. 그 중에서도 쏘일네일링 공법은 시공성과 경제성 등의 이유로 그 활용이 증대되고 있다. 쏘일네일링 공법은 다른 보강공법들과 함께 사용하는 경우도 있으며, 또한 강재와 시멘트그라우팅과의 일종의 복합 구조체로서 지반과 그라우팅사이의 마찰력을 활용하는 공법이다. 본 연구에서는 선단확장형 쏘일네일링 공법을 개발하였으며, 이는 네일 끝단에 앵커체가 부착되어 있다. 이 앵커체는 네일링 작업 중 네일 끝단에서 확개되면서 지반에 정착되게 된다. 본 연구에서는 네일링 시공 후 그라우팅을 하지 않은 상태와 그라우팅 후 양생이 된 경우 각각에 대해 현장인발저항시험을 실시하였다. 시험결과로부터 선단확장형 쏘일네일의 선단쐐기력을 분석하였으며, 거동특성을 종합적으로 고찰하였다.
This study was conducted to evaluate and compare the apical leakage in the following retrofilling techniques after apical resection; No apical cavity preparation and no retrofilling(control group), Amalgam(group I) or silver glass ionomer cement(group II) retrofilling after apical cavity preparation with mini contra-angle and bur, Amalgam(group III) or silver glass ionomer cement(group N) retrofilling after apical cavity preparation with ultrasonic micro endo tip. Extracted ninety upper anterior and lower canine teeth were fixed in skull simulators and root canals were prepared with step-back method and obturated with gutta-percha and zinc oxide eugenol sealer. Obturated roots were resected 2mm from apical ends and apical cavities of 1mm width and 2mm depth were prepared and retrofilled by above mentioned methods. After application of nail varnish on all surface except resected surface, apical 1/3 of the roots were placed in 1% methylene blue solution for 3 days. After longutudinal sectioning to expose central parts of filled materials, depths of penetrated dye were measured by measuring microscope and were analyzed statistically. The results were as follows. 1. Having no relation with instruments used in apical cavity preparation, amalgam retrofilling groups(group I and II) showed less apical leakage which was not significant statistically than no retrofilling group(control group) (P<0.05), but silver glass ionomer cement retrofilling gruoups(group II and IV) showed significantly less apical leakage than no retrofilling group(control group) (P<0.01). 2. In the groups retrofilled with the same material, the apical leakage in cavities prepared with ultrasonic micro endo tip (group III and IV) was less than that in cavities prepared with mini contra-angle and bur(group I and II), but not significant statistically(P>0.05). 3. When apical cavities were prepared with same instrument. the egroups retrofilled with silver glass ionomer cement(group II and IV) showed significantly less apical leakage than the groups retrofilled with amalgam(group I and III)(<0.01).
본 논문에서는 쏘일네일링 공법, 앵커 공법, 또한 이 두공법의 장점을 결합한 하이브리드 공법의 하중전이 메커니즘에 대한 연구를 수행하였다. 하이브리드 공법은 철근과 PC강연선을 보강재로 하며, 프리스트레스를 가함으로써 능동적인 주면마찰력 양상을 유도하고자 한다. 본 논문에서는 주면마찰력 산정 이론과 하중전이 이론을 바탕으로 각 공법의 하중전이 메커니즘을 파악하였다. 또한 현장인발시험을 실시하여 측정된 계측결과의 분석 및 하중전이 이론과의 비교 검토를 통해서 하이브리드 공법의 하중전이 메커니즘을 규명하였다. 쏘일네일링 공법은 인발거동 시 전면에서부터 선단으로 인장력이 전이되는 양상을 보이는 반면, 앵커 공법은 내하체가 위치한 선단에서 프리스트레스를 가함에 따라 선단에서 전면으로 압축력이 전이된다. 이 두 공법을 결합시킨 하이브리드 공법은 프리스트레스를 가할 때에는 앵커공법과 같이 선단에서 압축력이 발생하지만 동시인발 시에는 선단에서는 압축력이 전면에서는 인장력이 발생하게 된다. 따라서 하이브리드 공법의 하중전이 메커니즘은 쏘일네일링의 거동과 앵커의 거동이 모두 나타난다.
