Mycobacterium bovis 감염(感染)이 화학적(化學的)으로 유발(誘發)된 지연형(遲延型) 피부과민증(皮膚過敏症)에 미치는 영향(影響) (Chemically-Induced Delayed Cutaneous Hypersensitivity in Guinea Pigs and Rabbits Experimentally Infected with Mycobacterium bovis)

  • In order to evaluate the effects of the cell-mediated immunity of the animal in the chronic diseases the guinea pigs and rabbits were inoculated with Mycobacterium bovis. After 6 weeks these animals were sensitized and challenged with 2,4-dinitrochlorobenzene. The cutaneous reactions observed in these animal species were similar each other. Macroscopic and microscopic responses in the animal experimentally infected with M. bovis were markedly reduced compared to those in the control animals. The results indicated that the cell-mediated immunity of the animals was depressed by infection of M. bovis.

A Case of Pulmonary Mycobacterium kansasii Disease Complicated with Tension Pneumothorax

  • Pneumothorax is an extremely rare complication of non-tuberculous mycobacterial infection. A 52-year-old man presenting with difficulty breathing and chest pain was admitted to our hospital. A right-sided pneumothorax was observed on chest radiography and chest computed tomography showed multiple cavitating and non-cavitating nodules with consolidation in the upper to middle lung zones bilaterally. Serial sputum cultures were positive for Mycobacterium kansasii, and he was diagnosed with pulmonary M. kansasii disease complicated by tension pneumothorax. After initiation of treatment including decortications and pleurodesis, the patient made a full recovery. We herein describe this patient's course in detail and review the current relevant literature.

A rare case of bovine tuberculosis caused by Mycobacterium bovis in a domestic rabbit

  • A 12-month-old domestic rabbit died suddenly without specific clinical signs. Gross examination revealed irregular yellowish hepatic nodules with pus in the submandibular muscles, lungs, uterus, and small intestines. Histopathological examination of the liver showed granulomatous inflammation with acid-fast-positive bacteria. Mycobacterium bovis SB1040 was identified by polymerase chain reaction and spoligotyping, and Pasteurella multocida was isolated from the multiple lesions. This report demonstrates the pathological features of rare bovine tuberculosis (bTB) in a domestic rabbit, the first case in the Republic of Korea. To ensure public safety, we recommend routine monitoring of rabbits to control the incidence of bTB.

  • In order to supplement a diagnostic method for detection of infectious cattle to bovine tuberculosis, performed ELISA for detection of antibody to if bovis in serum and milk. The diagnostic efficacy of the established ELISA was compared with test of the tuberculin skin test for bovine tuberculosis. The positive corresponding rate of serum ELISA and tuberculin skin test showed 84.3%, milk ELISA and tuberculin skin test showed 75.0%, milk ELISA and serum ELISA showed 75.0% respectively. Comparison of the serum and milk to tuberculin antibody concentration in tuberculin positive cattle, the milk contained 1/100-1/150 concentration compared serum tuberculin concentration. The established ELISA was considered efficient for detection of antibodies to M bovis in serum and milk.

Heterogeneous Natures of the Microbial Steroid $9{\alpha}$-Hydroxylase in Nocardioforms

  • Steroid $9{\alpha}$-hydroxylase is an enzyme found in nocardioform microorganisms which can utilize steroids as a sole carbon source. After fractional centrifugation of the cell homogenates, the enzyme activity in Nocardia and Rhodococcus was found in cytoplasmic membrane fraction. On the contrary, Mycobacterium had its 9.alpha.-hydroxylation activity in cytosolic fraction. To characterize the enzyme in these microorganisms, several potential inhibitors of 9.alpha.-hydroxylase were tested and the cofactor requirement for the same enzyme was also examined. The inhibitory effect of ferrous ion chelators indicated involvement of iron containing proteins in the 9.alpha.-hydroxylase system. On the other hand, metyrapone, an inhibitor known to be specific for cytochrome P450 interfered with the enzyme in Mycobacterium, but didn't inhibit the enzyme activity in Nocardia and Rhodococcus. While the $9{\alpha}$-hydroxylase system in Nocardia and Rhodococcus required NADPH, NADH was required as an election donor in Mycobacterium.

Computational approaches for molecular characterization and structure-based functional elucidation of a hypothetical protein from Mycobacterium tuberculosis

  • Adaptation of infections and hosts has resulted in several metabolic mechanisms adopted by intracellular pathogens to combat the defense responses and the lack of fuel during infection. Human tuberculosis caused by Mycobacterium tuberculosis (MTB) is the world's first cause of mortality tied to a single disease. This study aims to characterize and anticipate potential antigen characteristics for promising vaccine candidates for the hypothetical protein of MTB through computational strategies. The protein is associated with the catalyzation of dithiol oxidation and/or disulfide reduction because of the protein's anticipated disulfide oxidoreductase properties. This investigation analyzed the protein's physicochemical characteristics, protein-protein interactions, subcellular locations, anticipated active sites, secondary and tertiary structures, allergenicity, antigenicity, and toxicity properties. The protein has significant active amino acid residues with no allergenicity, elevated antigenicity, and no toxicity.

폐결핵 잔단에서 Amplified Mycobacterium Tuberculosis Direct Test의 임상적 유용성 (Clinical Utility of Amplified Mycobacterium Tuberculosis Direct Test in the Diagnosis of Pulmonary Tuberculosis)

  • 연구배경: 폐결핵을 확진하는 방법은 결핵균의 증명이며 결핵균 동정을 하는 고전적인 방법인 항산균 도말 검사와 결핵균 배양 검사에 대해 상호 보완적으로 폐결핵의 보다 신속한 진단을 하는데 있어서 MTD의 유용성을 알아보고자 하였다. 방 법: 1997년 11월부터 1998년 5월까지 부산대학교 병원에 내원하여 폐결핵이 의심된 187명의 환자로부터 얻은 호흡기검체 267건(객담 187건, 기관지 세척액 94건)에 대해 각각 항산균 도말, 결핵균 배양 검사, MTD를 시행한 결과를 임상 소견과 연관지어 비교 분석하였다. 결 과: 1) 객담 검체에서 항산균 도말 검사, 결핵균 배양 결과와 비교하였을 때 항산균 도말 양성인 경우 결핵균 배양 검사와 비교한 MTD의 민감도는 75.6%, 도말 음성인 경우 결핵균 배양 검사와 비교한 MTD의 민감도는 61.9%, 특이도는 85.6%였다. 2) 기관지 세척액에서 항산균 도말 검사, 결핵균 배양 결과와 비교하였을 때 항산균 도말 양성인 경우 결핵균 배양 검사와 비교한 MTD의 민감도는 89.5%, 특이도는 28.6%, 도말 음성인 경우 결핵균 배양 검사와 비교한 MTD의 민감도는 89.5%, 특이도는 67.3%였다. 3) 객담과 기관지 세척액을 합한 경우 항산균 도말검사, 결핵균 배양 결과와 비교하였을 때 항산균 도말 양성인 경우에서 결핵균 배양 검사와 비교한 MTD의 민감도는 79.7%, 특이도는 20.0%를 보였으며, 항산균 도말 음성인 경우 결핵균 배양 검 사와 비교한 MTD의 민감도는 75.0%, 특이도는 80.3%를 보였다. 4) 결핵균 배양 결과와 비하여 MTD의 결과가 일치하지 않는 경우 MTD의 민감도는 67.5%, 특이도는 84.3%였으나 불일치 해결후에 MTD의 민감도는 79.2%, 특이 도는 84.4%를 보여 향상된 결과를 보였다. 5) 항산균 도말 음성, 결핵균 배양 음성이며 MTD만 양성인 31예에서 임상 경과 추적 및 항결핵제 치료 반응 웅의 분석에서 14예에서만 최종 폐결핵으로 진단되었으며 이 경우 MTD의 민감도는 45.2%였으며 진성 양성을 확인할 수 있었다. 6) 항산균 도말 음성, MTD 음성에서 mycobacterium species로 자란 13 건의 검체에서 Mycobacterium tuberculosis와 nontuberculous mycobacteria를 분리하기 위해 accuprobe M.tuberculosis complex culture kit를 이용하여 11건에서 M.tuberculosis로 배양되었으며 2건(17.3%)에서 비결핵 항산균이 배양되었다. 결 론: 이상의 결과로 볼 때 MTD는 빠르고 정확한 결핵균 진단에 유용한 것으로 생각되며, 폐결핵 진단에 있어서 MTD는 항산균 도말 검사 및 배양 검사와 병행함으로써 유용성이 높으며, 고전적인 진단 방법을 대체하기 보다는 상호 보완적인 방법으로 판단된다.

저산소와 NO 조건에서 Mycobacterium smegmatis의 adenylyl cyclase 유전자 발현 (Expression of Adenylyl Cyclase Genes in Mycobacterium smegmatis under Hypoxic and Nitric Oxide Conditions)

  • 결핵균인 Mycobacterium tuberculosis H37Rv에서는 16개의 adenylyl cyclase (AC) 유전자가, 비병원성인 M. smegmatis에서는 10개의 AC 유전자가 발견되었다. M. smegmatis에서 발견된 10개의 AC 유전자들 중 MSMEG_0228, MSMEG_3243, MSMEG_3780, MSMEG_4279, MSMEG_4477, MSMEG_6154 유전자들의 발현은 저산소 조건에서 유도되었다. 반면에 M. smegmatis가 nitric oxide (NO)에 노출이 되면 MSMEG_3780과 MSMEG_4279 유전자의 발현이 유도되었다. 유산소 조건에서 키운 M. smegmatis와 비교할 때, 저산소 조건에서 성장한 M. smegmatis 세포 내부와 성장 배지 내의 cAMP 양은 각각 450배와 9.8배 증가하였다. M. smegamtis가 NO에 노출이 되면 세포 내의 cAMP 농도는 5.8배 증가하였다. DevSR two-component system은 M. smegmatis가 저산소 조건이나 NO에 노출되었을 때 많은 유전자의 발현을 유도하는 주요 조절계로 알려져 있는데, 저산소 조건에서 발현이 유도된 6개의 AC 유전자의 발현이 M. smegmatis devR mutant에서도 wild type에서와 마찬가지로 저산소 조건에서 유도됨이 관찰되었다. 이 결과는 저산소 조건에서 발현이 유도되는 6개의 AC 유전자들이 DevSR two-component system에 의해 조절되지 않음을 나타내고 있다. 저산소 조건에서 발현이 가장 많이 유도된 MSMEG_3780 유전자의 발현조절에 관련이 있는 조절자를 발굴하기 위해 ligand fishing 분석을 수행하였고, 후보 조절자로서 MSMEG_5136을 발굴하였다.

전신성 홍반성 낭창 환자에서 발생한 Mycobacterium abscessus에 의한 폐렴 1예 (A Case of Mycobacterium Abscessus Pneumonia in a Patient with Systemic Lupus Erythematosus)

  • 전신성 홍반성 낭창으로 3년 전부터 스테로이드를 복용 중이던 40세의 여자 환자가 발열과 기침을 주소로 내원하였다. 환자의 증상과 방사선학적 소견은 경험적인 항균제 투여에 반응이 없었으며 개흉 폐생검의 결과 항산균을 관찰할 수 있었고 객담 항산균 배양 검사에서는 신속 성장균을 발견하여 생화학적 검사와 분자 생물학적 검사로 Mycobacterium abscessus로 확인하였다. 감수성 검사 결과에 따라 imipenem과 amikacin을 정주 투여하여 환자의 증상과 방사선 소견을 모두 호전시킬 수 있었다.

Comparison of One-Tube Nested-PCR and PCR-Reverse Blot Hybridization Assays for Discrimination of Mycobacterium tuberculosis and Nontuberculous Mycobacterial Infection in FFPE tissues

  • Currently, molecular diagnostic assays based on nucleic acid amplification tests have been shown to effectively detect mycobacterial infections in various types of specimen, however, variable sensitivity was shown in FFPE samples according to the kind of commercial kit used. The present study therefore used automated PCR-reverse blot hybridization assay (REBA) system, REBA Myco-ID HybREAD 480®, for the rapid identification of Mycobacterium species in various types of human tissue and compared the conventional one-tube nested-PCR assay for detecting Mycobacterium tuberculosis (MTB). In conventional nested-PCR tests, 25 samples (48%) were MTB positive and 27 samples (52%) were negative. In contrast, when conducted PCR-REBA assay, 11 samples (21%) were MTB positive, 20 samples (39%) were NTM positive, 8 samples (15%) were MTB-NTM double positive, and 13 samples (25%) were negative. To determine the accuracy and reliability of the two molecular diagnostic tests, the one-tube nested-PCR and PCR-REBA assays, were compared with histopathological diagnosis in discordant samples. When conducted nested-PCR assay, 10 samples (59%) were MTB positive and seven samples (41%) were negative. In contrast, when conducted PCR-REBA test, three samples (17%) were MTB positive, 10 samples (59%) were NTM positive and four samples (24%) were negative. In conclusion, the automated PCR-REBA system proved useful to identify Mycobacterium species more rapidly and with higher sensitivity and specificity than the conventional molecular assay, one-tube nested-PCR; it might therefore be the most suitable tool for identifying Mycobacterium species in various types of human tissue for precise and accurate diagnosis of mycobacterial infection.
