Future Direction of National Health Insurance (국민건강보험 발전방향)

  • It has been forty years since the implementation of National Health Insurance (NHI) in South Korea. Following the 1977 legislature mandating medical insurance for employees and dependents in firms with more than 500 employees, South Korea expanded its health insurance to urban residents in 1989. Resultantly, total expenses of the National Health Insurance Service (NHIS) have greatly increased from 4.5 billion won in 1977 to 50.89 trillion won in 2016. With multiple insurers merging into the NHI system in 2000, a single-payer healthcare system emerged, along with separation policy of prescribing and dispensing. Following such reform, an emerging financial crisis required injections from the National Health Promotion Fund. Forty years following the introduction of the NHI system, both praise and criticism have been drawn. In just 12 years, the NHI achieved the fastest health population coverage in the world. Current medical expenditure is not high relative to the rest of the Organization for Economic Cooperation and Development. The quality of acute care in Korea is one of the best in the world. There is no sign of delayed diagnosis and/or treatment for most diseases. However, the NHI has been under-insured, requiring high-levels of out-of-pocket money from patients and often causing catastrophic medical expenses. Furthermore, the current environmental circumstances of the NHI are threatening its sustainability. Low birth rate decline, as well as slow economic growth, will make sustainment of the current healthcare system difficult in the near future. An aging population will increase the amount of medical expenditure required, especially with the baby-boomer generation of those born between 1955 and 1965. Meanwhile, there is always the problem of unification for the Korean Peninsula, and what role the health insurance system will have to play when it occurs. In the presidential election, health insurance is a main issue; however, there is greater focus on expansion and expenditure than revenue. Many aspects of Korea's NHI system (1977) were modeled after the German (1883) and Japanese (1922) systems. Such systems were created during an era where infections disease control was most urgent and thus, in the current non-communicable disease (NCD) era, must be redesigned. The Korean system, which is already forty years old, must be redesigned completely. Although health insurance benefit expansion is necessary, financial measures, as well as moral hazard control measures, must also be considered. Ultimately, there are three aspects that we must consider when attempting redesign of the system. First, the health security system must be reformed. NHI and Medical Aid must be amalgamated into one system for increased effectiveness and efficiency of the system. Within the single insurer system of the NHI must be an internal market for maximum efficiency. The NHIS must be separated into regions so that regional organizers have greater responsibility over their actions. Although insurance must continue to be imposed nationally, risk-adjustment must be distributed regionally and assessed by different regional systems. Second, as a solution for the decreasing flow of insurance revenue, low premium level must be increased to an appropriate level. Likewise, the national reserve fund (No. 36, National Health Insurance Act) must be enlarged for re-unification preparation. Third, there must be revolutionary reform of benefit package. The current system built a focus on communicable diseases which is inappropriate in this NCD era. Medical benefits must not be one-time events but provide chronic disease management. Chronic care models, accountable care organization, patient-centered medical homes, and other systems that introduce various benefit packages for beneficiaries must be implemented. The reimbursement system of medical costs should be introduced to various systems for different types of care, as is the case with part C (Medicare Advantage Program) of America's Medicare system that substitutes part A and part B. Pay for performance must be expanded so that there is not only improvement in quality of care but also medical costs. Moreover, beneficiaries of the NHI system must be aware of the amount of their expenditure through a deductible payment system so that spending can be profiled and monitored. The Moon Jae-in Government has announced its plans to expand the NHI system; however, it is important that a discussion forum is created so that more accurate analysis of the NHI, its environments, and current status of health care system, can take place for reforming NHI.

  • Food is very important for human being. It is not easy for us to enjoy good life because the situation of food is not good in Korea. The self-sufficient food rate is very low and most of food which we take was produced by industrial agriculture. Food of this kind has some problems with respect to the health of consumer, agriculture, environment and community. In order to secure the true food which is safe and environmentally sound, producers, consumers and community have to work together. Producers must engage in environmental agriculture. They must produce the true food and need to know regional consumers. Consumers must pay attention to the regional agriculture and must participate the production of true food. Local government must open farmers' markets for direct marketing between farmers and consumers. NGOs should educate and organize the producers and consumers.

  • Management of Swimming pool is focused on Swimming pool samitation in relation with chlorination of swimming water and potable water, disposal of waste and excrement within the boundry of swimming pool that may be summerised as follows: 1. Chlorination of Swimming Water Residual chlorine must be kept within the range of $0.4\~0.6 mg/l$ and in case of chloramine should be $0.7\~1.0mg/l$ while swimming pool is in operation 2. Chlorination of potable Water Residual chlorine must be kept within the rangs of $0.2\~04 mg/l$ at all time 3. Disposal of litters must be kept in the water tight waste bin with fitted lid, and waste should not be overflow -out of the bin. When waste in landfilled, the sufficient amount of cover material should be used daily. 4. Disposal of excrement Toilet must be water-flush type. However, The establishment of pit latrine is unavoidable the excrement must be covered with lime or dirt so that excrement should not be exposed to air.

  • As for the purpose of this study, education facilities must be considered by a change of an education facilities equipment acid education system and aromatic character of an educational policy with a means to support to efficiently proceed education by study about a plan of the school education facilities which considered safety. That is, education facilities must become maintenance on design construction according to education action character and goal. This is not for a function of education to be decided according to education facilities, and education facilities must be decided so that adjustment can do an education function. The student center, a student are convenient, and an actor must do a plan of education facilities with an usable physical space contrariety, future directivity. As for the plan of school education facilities, facilities must be designed on functionality, an economy, flexibility, aesthetic appreciation anger, a foundation of stability for this so that objective of education realization is easy.

The trends and evaluations for Governmental system of redressing consumers' losses in Japan: comparison to Korea (일본의 소비자피해구제 행정체제 현황 및 평가: 한국과의 비교를 중심으로)

  • This study examined in detail the Japanese system of redressing consumers' losses and the overall consumer administrative system and compared them with the Korean counterpart to seek for a direction of reforming the systems. Several policy suggestions could be made as follows to activate the systems and improve the performances of the consumer administration in both Japan and Korea. First, in Japan, consumer center must be efficiently operated while private consumer groups, firms, consumer agencies and other organizations must actively join to actively redress consumers' losses and implement other consumer policies. Second, in Korea, regional consumer policies are weak so that they must learn Japanese well-developed regional consumer administration system by examining their current development, strengths, and weaknesses. Third, working conditions must be urgently improved, increasing the number of consumer counselors who are involved in consumer policies as well as to guarantee minimum level of their payment. On the other hand, the trend of hiring married and old employees being responsible for implementing consumer policies in Japan must be improved. Fourth, divisions of works in consumer policies among consumer groups as well as their specialization in Korea are necessary. Finally, as counseling of consumers through internet were not available in Japan, that type of counseling must introduce and become more popular in the future.

The humidifier disinfectant case and the legislative challenges of the 20th Congress

  • A number of absurdities surrounding the humidifier disinfectant (HD) incident may have occurred because 1) a judicial system operates on the underlying false assumption that the involved parties are equals in knowledge, information and resource mobilization capabilities, regardless of respective real status as company or individual; 2) there is a lack of a system that mandates a company to prevent and actively manage possible catastrophes; 3) the regulatory scheme makes companies believe that as long as they are complying with the existing regulations, they have satisfied all of their responsibilities. I believe that this issue is an opportunity to bring about changes in the judicial redress system, the system of internal management of manufacturers, and the regulatory system of the government. The following regulation amendments are needed to move towards the changes stated above. First, legislation relating to victim relief that is applicable to the HD incident must be established. Second, a risk management system must be formed within the manufacturing company and to this end an institutional environment for the system must be established within regulatory framework. Furthermore, legislation must be passed that could punish companies themselves that have caused severe damage to individuals because they had failed to take necessary actions to avoid foreseeable harm. Finally, the framework of regulation must be changed so that the company, who has the necessary information regarding the product and the component chemicals used in the product, must self-directed experiment and assessment of the safety of their own products.

A culture needs must development of event system, leisure information system. A traffic needs must development of automatic traffic geographical system, automatic operating system. An education needs must development of teleeducation system, knowledge studying support system.

  • This study has surveyed the problems raised through the standardization process of differentiation system and searched its solution, reaching the following conclusion. 1. The most serious problem is, most frequently occurred, an wilful application of differentiation system or constitutive classification for disease. 2. To overcome this wilfulness, we must make a definite understanding of the origin and its applicative limit of the theory. 3. And it must be preceded for a correct and objective differentiation to standardize in all choice of the differential diagnostics and its applicative condition. 4. To establish such a corresponding diagnostic scheme, it must be generally agreed on an individuality and inevitability of the suitability to differentiation principles. 5. And, though we can make a prescription through the frequency, degree, and clarified cause of the symptom, its flexibility of application must not be necessarily, ignored. 6. As this study for standardizing the disharmony between liver and pancreas(肝脾不調) shows, accordingly, we must begin by scrutinizing and defining what the chief cause, head, and inevitable symtoms are and discern them from its affinities, making necessarily a great deal attention to minute feelings and degree about its individual symptoms. On the ground of these recognition, we must go further to establish a general standardization and try to get a specific study series.

The control system of sludge amount inspection and discharge materials of outlet water and affiliated water-purification tank (오수/합병정화조의 배출물 제어시스템 연구)

  • The individual rotten water purification tank recently discharges wastewater and sewage through the outlet without purification ability. The outlet water and affiliated water purification tank with microorganism cultivator tank cultivates microorganism and then drops the value BOD, COD of sewage and discharges the quality of water into the outlet. The blower and water pump operating continuously prompts the waste of energy and deterioration of equipment. Each room of deposition tank, foaming tank, microorganism cultivator tank is equipment with the sludge detection senses so it can detect the density of each room. The power-drive plant of the blower and water pump which ate the system cultivating the microorganism must be made as fuzzy controlization (If the sludge amount of each room become higher, the rate of operation of blower and water pump must heighten, on the contrary, in case of row sludge amount, the total handling amount and microorganism handling amount of each room of control. Tank reducing the rate of operation must be DB. At present, the blower amount in proportion to the sludge and oxygen demanding amount has to control. Each mom must be checked outlet level of the outlet, also each room must flow backward discharge materials, and must operate feed-back control until we want to be come as a below value of BOD/COD(10PPM ; KS).
