사상체질인(四象體質人)의 병증(病證) 치료를 위한 한방음악치료(韓方音樂治療)에 관한 연구;장부대소(臟腑大小)와 호흡출납(呼吸出納)을 중심으로 (Oriental Medicine Music therapy for The treatment of Sasangin's disdases)

  • The biological and pathological states of lung, spleen, liver and kidneys are determined by nature and emotion of sorrow, anger, joy and pleasure, It is considered that the music, besides medicine or acupuncture, can be a way of treating as a therapeutics for the treatment of Sasangin's diseases. This study is to find out a way of music therapy for the treatment of Sasangin's diseases. The ways of music therapy in each constitution based on 'the great and small of Viscera and Bowels' and 'the exhale and inhale, the drawing in and sending out' are as follows: 1. In case of Soeumin, a skip going up melodic progression, dotted rhythm and the major triad harmonic progression are recommended to improve the Qi of warm Yang. 2. In case of Soyangin, in regular sequence down melodic progression, a sixteenth note and the minor triad harmonic progression are recommended to improve the Qi of cool Yin. 3. In case of Taeumin, in regular sequence going up melodic progression, a sixteenth note and the bright and active the major triad harmonic progression are recommended to improve the Qi of dispersing exhale. 4. In case of Taeyangin, a skip down melodic progression, a quarter note and a diminished triad harmonic progression are recommended to improve the Qi of gathering inhale.

음악요법이 위내시경 검사자의 불안과 불편감 감소에 미치는 효과 (The Effects of Music Therapy in the Reduction of Anxiety and Discomfort on Patients Undergoing Gastrofibroscopy)

  • Purpose: This study was to examine the effects of music therapy on the levels of anxiety and discomfort in patients undergoing gastrofibroscopy. Method: This study was equivalent control group pre-post test design. The subjects were 61 patients who visited H hospital internal medicine department for gastrofibroscopy ; 31 were randomly selected for the control group and the remaining 30, for the experimental group. Result: 1. The experimental group who used music therapy showed a significantly lower level of anxiety during gastrofibroscopy than the control group. 2. The experimental group revealed a lesser change in pulse rate between pre and post gastrofibroscopy than the control group. 3. There was a significant difference in the levels of subjective and objective discomfort during gastrofibroscopy between the two groups. Conclusion: The results suggested that music therapy may help relieve anxiety and discomfort for patients undergoing gastrofibroscopy. These results showed that the use of music aided in the reduction of anxiety and discomfort during the unpleasant diagnostic procedure and, testing the effectiveness of music therapy deserves further study in other hospital settings.

혈액암 환자(患者)에게 실행한 한방음악치료(韓方音樂治療) 요법이 백혈구 내(內) ANC(절대호중구수)수치 증가에 미치는 영향 (The effectiveness of ANC's number increasing by using Oriental Medicine Music Therapy which was applied to blood cancer patients)

  • The purpose of this research is to discover changes of WBC and ANC numbers before and after applying Oriental medicine therapy to blood cancer patients. After that, appropriate music therapy method was well planned and carried for patients. Demonstration and music were conducted according to Ohaeng theory. The oriental music therapy was conducted three hours everyday by listening to music and we made patients participate in playing the instrument for one hour in two times a week. The result was verified in three ways by checking and comparing numbers of WBC, ratio of ANC and New Trophil before and after the experiment. In addition to that verification, we analyzed patients' survey and their response after treatment. The result was that WBC and ANC were efficient as p=0.0419, p=0.0262 each and the ratio of New trophil was not efficient in p=0.999, but partially increased.

전신마취 하 비골골절 수술 환자에서 음악요법이 환자의 불안 및 활력징후에 미치는 영향 (Effect of Music Therapy on Anxiety and Vital Sign of Nasal Closed Reduction Under General Anesthesia)

  • Purpose: Most of the patients who underwent surgery feels variable kinds of fear or anxiety; an uncomfortable mood state that happens without specific object affects patient's satisfaction before and after the surgery. As music therapy is rather noninvasive method generally used in reducing patient's anxiety, the authors researched about the extent of anxiety with the change of vital sign before the operation while comparing with the cases of patients who took the music therapy at closed reduction under general anesthesia. Method: We divided the patients in 4 groups; A with the premedication (Midazolam, Dormicum$^{(R)}$) before the operation, B with the premedication and music therapy, C with only music therapy, D with no premedication or therapy. And we measured the vital signs after the arrival at the operation room, after induction and 20 minutes after the operation. Also we observed the changes of anxiety index with the STAI (State Trait Anxiety Inventory)-K (Korea)YZ 1 hour before and 8 hours after the surgery. Result: The group B showed the least changes in blood pressure as the group D showed the highest change. Both group C and A showed increase in blood pressure but the upswing in group A was lower than group C. At the change of pulse rate group B showed the lowest upswing also group D showing the highest. Group B showed quite a few upswing but lower than group D, but, at the same time, group A showed lower upswing when comparing two cases. After analysis of STAI-KYZ score, the anxiety relatively decreased in group B and C in comparison with group D. And the index of anxiety state of group A showed just as much to group D. Conclusion: The music therapy is better healthcare method compared to other therapies in reducing anxiety also with satisfying effect who underwent operation. The authors recommend music therapy assisted with use of premedication for better relief of anxiety.

중년여성에게 실행한 한방음악치료 전후(前後) 자아존중감 및 분노와 우울증상 개선에 대한 1례(例) (The Case Study of Oriental Medicine Music Therapy's Effect on Improving Middle Aged Woman's Self-Esteem Inventory & Anger Expression and Depression)

  • Objectives: The study was implemented to observe how Oriental Medicine Music Therapy (Korean Medicine Music Therapy) influences on curing the symptoms of anger and depression. Methods: Study conducted an experiment with an age of 43 female who usually suffers from headache and dizziness, and frequently gets angry at her daughter. The Study was conducted for an hour, 1 times a week. In the present study, the whole program consisted of 7 sessions. Other than the Oriental Medicine Music Therapy, the patient had never been cured by other treatments. Results: 1. According to the result of STAXI analysis, the patient with anger scale of 36 (before treatment) became to have anger scale of 31 after the treatment, indicating that the therapy had positive effect on curing the anger symptom. 2. Moreover, the analysis result of BDI showed that the patient with depression scale of 17 (before treatment) turned out to be non-depression state, which he had the depression scale of 8. Regarding the results above, it was observed that the treatment cured the depression symptom. 3. Based on the result of SEI analysis, the patient with a SEI score of 139 (higher the SEI score, more the self-esteem one has) became to have score of 142 after the treatment. Conclusions: From the research, it was observed that Korean Medicine Music Therapy has positive influence on improving one's anger or depression control and self-esteem. Clinical trial is needed to verify statistical significance.

자폐아동의 즉흥음악치료에서 나타난 주요 음악요인 분석과 아동의 대상선정에 관한 연구 (Salient musical elements and children's choice of objects in improvisational music therapy for children with autism)

  • 즉흥음악치료는 자폐아동들의 사회적 상호작용을 증진시킨다고 알려져 있다. 본 연구는 즉흥음악치료에서 치료사-내담자간의 상호작용의 음악적 동시성 및 선택대상을 규명하는 것을 목적으로 한다. 또한 즉흥음악치료 상황에서 일어나는 상호작용의 핵심적 요인을 음악요소 분석을 통해 규명하고자 하였다. 본 연구는 악기를 주로 사용하는 즉흥음악치료를 장난감을 사용하는 놀이치료 상황과 비교했으며, 두 가지 치료를 무작위 배치하는 교차설계를 이용하였다 (n = 10). 특정 회기별 목표행동분석(1, 4, 8, 12회기)을 실시하고, 반복측정 분산분석을 활용하였다. 분석결과 치료사와 아동간의 음악적 동시성의 빈도와 기간이 즉흥음악치료에서 장난감 놀이치료 상황에 비해 현저히 높게 나타났다 (p < .001). 또한 리듬과 다이내믹이 치료사와 아동간의 음악적 동시성을 가능케 하는 주요 요소로 측정되었다. 즉흥음악치료에서 자폐아동들은 나팔과 각종 호루라기를 가장 자주 선택했고, 장난감 놀이 상황에서는 인형의 집을 제일 많이 선택했다. 본 연구는 학령 전 자폐아동과 일하는 음악치료 임상가들에게 유용한 정보를 제공한다.

음악치료에서의 개인적 역량: 교육과 임상적 훈련에 대한 도전 (Intrapersonal Competency in Music Therapy: Challenges for Education and Clinical Training)

  • 음악치료사의 자질과 능력 증진은 많은 연구자들과 학자들에 의해 연구되어온 분야이며 이를 위한 효과적인 교육 및 임상 전략들이 규명되고 개발되고 있다. 본 연구에서는 기존의 음악치료의 정의를 중심으로 음악치료사의 기본 자질을 규명하고 학술단체와 기관에서 정의하는 음악치료가 강조하는 다른 항목들을 비교분석함으로써 공통적으로 음악치료사가 갖추어야 하는 역량들을 규명하였다. 그 결과 본 연구에서는 음악치료사는 세 가지 역량을 갖추어야 하고 기존의 교육과정은 이러한 역량들을 강화할 수 있는 내용으로 구성되어야 하는데 이에는 학문적 역량, 실기역량, 그리고 개인적 역량이 포함된다. 여기서 개인적인 역량은 음악치료 교육과정에서 다루어야 하는 새로운 역량 분야로 규명되었다. 이는 음악치료가 지식기반 내용만을 다루는 학문이 아님으로 치료를 위한 음악중심의 개입을 제공할 수 있는 내적 성찰과 치료사적 통찰력이 기반이 된 역량을 갖추어야 하기 때문이다. 음악치료가 정의하는 학문과 전문성을 갖추기 위해서 음악치료사는 이 세 가지 역량을 갖추어야 하며 이러한 역량 강화를 위해서는 효율적인 교과내용들이 기존의 교육과정에 반영되어야 한다.

음악요법이 수술환자의 수술전 불안에 미치는 효과 (The Effects of Music Therapy on the Preoperative Anxiety of Surgical Patients)

  • The purpose of this study was to elucidate the effects of music therapy on the preoperative anxiety of surgerial patients. The research design was a nonequivalent control, group non-synch-ronized design. The data were collected during the period from January 4 to March 17, 1999 at C-University hospital in Seoul. The subjects were sixty patients who had surgery under general anesthesia and had undergone laparotomy. They were assigned to two groups, thirty to the experimental group and thirty to the control group. They also did not have any complication, were alert enough to be interviewed and agreed willingly to participate in this study. The tool of Trait Anxiety Inventory was used to measure trait-anxiety on all patients and the Visual Analogue Scale(VAS) was used to measure state-anxiety on all patients. And systolic and diastolic blood pressure, pulse rate, and blood sugar levels were collected a the day before surgery and the preoperative period. The experimental group received music therapy with self-selected music tapes after choosing from a Music Preference Questionnaire, while the control group didn't receive music therapy. Data were analyzed by ${\chi}^2$-test, t-test, paired t-test. The results of this study are summarized as follows: 1. The preoperative state anxiety of the experimental group was re-markably lower than that of the control group. 2. Decreasing rate in the vital signs of the experimental group was remarkably lower than that of the control group. 3. The preoperative blood sugar of the experimental group was remarkably lower than that of the control group. According to these results, Music Therapy can be regarded as an effective nursing intervention that relieves preoperative anxiety of surgical patients and helper stabilize vital signs. From this study, the following recommendations can be made: 1. In order to decrease surgical patient's preoperative anxiety, I suggest the nursing intervention should go side by side with music therapy.

텍스트 마이닝 기반의 음악치료 관리 모델에 관한 연구 (A Study on the Music Therapy Management Model Based on Text Mining)

  • 음악치료는 장애아동 및 정신치료에 많은 효과를 보이고 있다. 오늘날의 음악치료 시스템은 구체적인 치료 시스템이 구축되어 있지 않은 상황으로, 음악 치료사들이 정확한 치료를 하기 위해 다양한 음악치료 사례들과 치료 이력 데이터들을 분석하고, 해당 환자 또는 내담자에게 가장 적합한 치료를 시행해야 하지만, 현실은 여러 가지 요인들로 인해 많은 어려움이 따르고 있다. 이를 해결하고자 본 논문에서는 기존 치료 데이터와 텍스트 마이닝 기술을 융합한 음악치료 지식관리 모델을 제안한다. 제안 모델을 활용하면 유사한 사례검색 및 환자에 관련된 구체적이고 확실한 데이터들을 기반으로 환자 또는 내담자로 하여금 정확하고 효과적인 치료가 가능하다. 이를 통해서 음악치료의 본래 목적과 그 효과를 최대로 이끌어 내는 효과를 기대할 수 있고 나아가 많은 환자들의 치료에 도움이 될 것으로 기대된다.

Effects of Musculoskeletal Disease Prevention and Treatment Customized Exercise Program according to Various Music Tempo on the Pain, Posture, and Function of Auto Parts Manufacturing Workers

  • Objective: The purpose of this study is to investigate the effect of exercise programs according to various music tempo on pain, posture, and function of automobile parts manufacturing workers. Design: Two-group pretest-posttest design. Methods: This study was an exercise program tailored to the prevention and treatment of musculoskeletal diseases were performed for a total of 4 weeks and 3 times a week for 60 minutes. The exercise program consisted of customized exercises according to the characteristics of the subject's musculoskeletal system. The subject provided music during the exercise program, and the group who applied the fast tempo music listened to the music of 120~140 bpm, and the group who applied the slow tempo music listened to the music of the low tempo of 60~80 bpm. Results: Significant differences were found in pain and physical function before and after the fast tempo music group and the slow tempo music group (p<0.01). However, there was a significant difference in posture characteristics only in the fast tempo music group (p<0.01). In terms of pain, physical function, and posture characteristics, the fast tempo music group showed significant improvement compared to the slow music tempo group (p<0.05). Conclusions: A customized exercise program using fast-tempo music and slow-tempo music to workers in the automobile parts manufacturing industry, significant differences in pain, physical function, and posture characteristics applied with fast-tempo music were found. In the future, applying an exercise program with a fast tempo is expected to help improve musculoskeletal disorders.
